Abstract

Dispositif pour l'ancrage chirurgical d'un tendon au niveau osseux.L'invention concerne un implant original de forme hélicoïdale, pouvant être mise par voie arthroscopique ou chirurgicale permettant une triple fixation du tendon par effet tire-bouchon, par compression dans le tunnel et par accrochage au niveau d'une spire. Le dispositif permet en outre la réalisation d'une greffe osseuse en force.Ce dispositif selon l'invention est particulièrement destiné à la reconstruction du ligament croisé antérieur du genou (ligamentoplastie) en utilisant les tendons droits interne - demi-tendineux (DIDT) ou le tendon rotulien et/ou un ligament synthétique.

Description

La présente invention est relative à un dispositif de fixation
chirurgicale d'un ligament à
l'os ou d'un fragment osseux à l'os.
Il est plus particulièrement destiné à la fixation d'une plastie ligamentaire du ligament croisé antérieur du genou à l'aide des tendons du droit interne - demi-tendineux ou du tendon rotulien.
Il existe plusieurs modes de fixation à ce jour.
1). Les vis d'interférences qui offrent une tenue moyenne au niveau de la zone métaphysaire de l'os.
2). Les vis corticales ou les agrafes qui nécessitent une contre-incision 1 0 supplémentaire.
3). Le système Transfix qui nécessite également une contre-incision et des manoeuvres intra-articulaires sous arthroscopie.
Aucun de ces systèmes ne permet une greffe osseuse en force sous arthroscopie.
La présente invention permet de remédier à ces inconvénients. Il est constitué d'une pièce 1 5 métallique chirurgicale ou biorésorbable rigide en forme hélicodale et comportant deux diamètres différents (1, 2), reliés par un toron intermédiaire (3). Le nombre de spires peut être variable dans les deux parties (fig. 2) de même que le diamètre des spires. L'embase est aplatie et comporte au moins deux perforations permettant l'adaptation d'un ancillaire de vissage (Fig. 4).
En coupe transversale, toutes les formes géométriques peuvent être envisagées à partir de la forme cylindrique de base.
Une possibilité envisagée non limitative est un aplatissement sur deux faces (Fig. 5) afin de limiter le recul.
La longueur de l'implant est variable en fonction de chaque fémur pour lequel il est destiné (Fig. 6). Un deuxième implant métallique ou biorésorbable de forme hélicodale comporte un diamètre unique avec une embase élargie et aplatie également perforée de deux ou trois trous pour l'adaptation de l'ancillaire. Le sommet de cette pièce (Fig. 3) est coiffé d'un "chapeau" en matière résorbable afin de ne pas accrocher le tendon lors du vissage. Cet implant est destiné au tibia (Fig. 6).
La structure creuse de l'implant permet la réalisation d'une greffe osseuse en force englobant le tendon, avec un contact maximum. L'embout du petit diamètre permet une fixation avec effet tire-bouchon au niveau de l'os donnant un ancrage d'emblée solide (Fig.1). Cette structure creuse permet également l'accrochage du tendon au niveau d'une spire permettant une meilleure rétention. Cette structure creuse de l'implant permet également la mise en place du dispositif le long d'une broche sans contre-incision.

Claims (2)

REVENDICATIONS
1. Dispositif de fixation chirurgicale d'un ligament à l'os ou d'un fragment osseux à l'os, caractérisé en ce qu'il est constitué d'une pièce en forme hélicodale.
2. Dispositif de fixation chirurgicale selon la revendication 1 caractérisé en ce qu'il est composé d'une pièce hélicodale comportant deux diamètres différents (1,2) avec un toron intermédiaire (3).
