Cet implant comprend une armature (10) destinée à être fixée à l'os et un cotyle à cimenter pouvant être mis en place sur cette armature (10), l'armature (10) présentant un anneau (20) délimitant une cavité (15) de réception du cotyle à cimenter et une ou plusieurs pattes (16) permettant sa fixation à l'os.Selon l'invention, l'armature (10) présente des extensions de parois (21) solidaires dudit anneau (20), faisant saillie du côté opposé à l'ouverture de la cavité (15) que délimite cet anneau (20) et convergeant les unes vers les autres jusqu'à se rejoindre et former ainsi un fond, ces extensions de parois (21) comprenant des plots (12) faisant saillie de leurs faces internes, espacés angulairement les uns des autres, dont la hauteur correspond à l'épaisseur requise de ciment entre ladite armature (10) et le cotyle à cimenter.Notamment destiné à être utilisé lorsque l'os du bassin délimitant la cavité cotyloïdienne est détérioré, en particulier en cas de "reprise ".

La presente invention concerne un implant cotylodien de prothese de hanche, notamment destine a etre utilise lorsque l'os du bassin delimitant la cavite cotylodienne est deteriore, en particulier en cas de
"reprise", c'est-a-dire de remplacement d'un implant anterieurement pose.
S Un implant cotylodien existent, adapte a une telle indication, comprend une piece de base formant une armature, cite ci-apres "armature", destinee 3 etre fixce a l'os et un cotyle a cimenter pouvant etre mis en place sur cette armature; I'armature presente un anneau delimitant une cavite de reception du cotyle a cimenter et est solidaire d'une ou plusieurs pastes permettant sa fixation a l'os; le cotyle a cimenter delimite une cavite destinee a recevoir la tete spherique de
['implant femoral de la prothese de hanche.
En pratique, des greffons vent mis en place dans la cavite cotylodienne si cela est necessaire puis ['armature est fixee a l'os au moyen de vis, apres quoi du ciment est depose dans la cavite, au travers de l'anneau. Le ciment flue dans la cavite delimitee par l'anneau et le cotyle 3 cimenter est mis en place dans cette cavite de maniere a faire fluer le ciment entre la paroi de l'anneau et la paroi du cotyle a cimenter,
ce qui assure la fixation de ce cotyle a cimenter a l'os.
Cet implant a pour inconvenient de ne pas garantir qu'une epaisseur de ciment uniforme soit obtenue entre l'anneau et le cotyle a cimenter, alors qu'une telle epaisseur uniforme est essentielle a la resistance de l'ancrage du cotyle a cimenter aux contraintes repetees que
subit ['implant.
11 existe egalement le risque que le cotyle a cimenter vienne en contact de la paroi de l'anneau, ce qui peut induire un frottement des deux pieces l'une contre l'autre lors des micro-mouvements de ces pieces sous ltexercice des contraintes que subit ['implant. Ce contact peut egalement induire un phenomene d'oxydo-reduction si le cotyle a cimenter comprend une piece metallique et que le metal constituent cette piece est de nature differente du metal constituent ['armature. Ces frottement et cette oxydo reduction conduisent a un relarguage peu souhaitable de particules de
metal ou d'ions metalliques dans l'organisme.
En outre, la position de ['armature par rapport a l'os peut amener a certaines restrictions dans le choix de la position du cotyle a cimenter. Ainsi, il peut ne pas etre possible de placer ce cotyle dans la position la plus avantageuse, qui est celle permettant un debattement
optimal de ['articulation et reduisant le risque de luxation de celle-ci.
La presente invention vise a remedier a ['ensemble de ces inconvenients. L'implant qu'elle concerne comprend, de maniere connue en soi, une piece de base formant une armature, cite ci-apres "armature", destinee 3 etre fixee a ltos, et un cotyle a cimenter pouvant etre mis en place sur cette armature, I'armature presentant un anneau delimitant une cavite de reception du cotyle a cimenter et une ou plusieurs pastes permettant sa fixation a l'os, et le cotyle a cimenter delimitant une cavite destinee a
recevoir ia tete spherique de ['implant femoral de la prothese de hanche.
Selon ['invention, I'armature presente des extensions de parois solidaires audit anneau, faisant saillie du cate oppose a l'ouverture de la cavite que delimite cet anneau et convergeant les unes vers les autres jusqu'a se rejoindre et former ainsi un fond, ces extensions comprenant des plots faisant saillie de leurs faces internee, espaces angulairement les uns des autres, dont la hauteur correspond a l'epaisseur requise de ciment
entre ladite armature et le cotyle a cimenter.
L'armature ne comprend ainsi pas uniquement un anneau delimitant iadite cavite de reception du cotyle a cimenter mais presente en outre des extensions de parois solidaires de cet anneau; ces extensions comprennent des plots formant des zones d'appui ponctuel du cotyle a cimenter, qui assurent ltexistence d'une epaisseur de ciment sensiblement uniforme entre ['armature et le cotyle. Ces extensions de parois delimitent de larges ouvertures entre elles et ne genent done pas la mise en place
puis le fluage du ciment.
De preference, le cotyle a cimenter comprend une coque exterieure metallique et un insert en materiau favorisant le glissement, notamment en polyethylene a haute densite, qui est recu dans cette coque et qui delimite ladite cavite de reception de la tete spherique de ['implant
femoral de la prothese de hanche.
Les plots precites permettent l'utilisation d'un tel cotyle en deux peces. La coque peut presenter, sur son cote externe, une conformation et/ou un revetement de surface qui favorise sa prise d'appui
contre le ciment.
Avantageusement, la face interne de la coque presente une forme sensiblement hemispherique et la face exterieure de ['insert presente egalement une forme sensiblement hemispherique, de rayon legerement inferieur a celui generant ladite face interne de la coque, cet insert pouvant etre recu dans la coque avec possibilite de debattement multidirectionnel
par rapport a celle-ci.
Ce debattement permet un debattement optimal de ['articulation, evite de solliciter l'ancrage de la coque dans les positions de
debattement maximal et reduit le risque de luxation de ['articulation.
De preference, le cotyle a cimenter presente une forme exterieure hemispherique et les plots vent au nombre de trots,
regulierement repartis sur une meme portion de surface spherique.
Ces plots permettent de positionner le cotyle a cimenter selon toutes orientations angulaires possibles par rapport a ['armature, de sorte que le positionnement de cette derriere par rapport a 1'os n'induit pas de restrictions dans le choix de la position du cotyle par rapport a cette armature. Ces plots permettent egalement d'utiliser plusieurs tailles de cotyles avec une meme armature, offrant au praticien une latitude de choix augmentee. Lesdites extensions de parois vent dans ce cas avantageusement conformees de maniere a delimiter, avec l'anneau, une cavite de forme hemispherique, et le cotyle a cimenter presente une forme hemispherique. Lorsque le cotyle comprend une piece exterieure en materiau metallique, les plots vent en un materieu n'induisant pas d'oxydo-reduction avec ce materiau. Si ['armature est elle-meme en materiau metallique, les plots peuvent alors etre rapportes sur lesdites extensions de parois. Ces derrieres peuvent notamment comprendre des trous taraudes, et les plots comprennent alors des portions filetees pouvant etre vissees dans ces trous, ou des portions auto-filetables pouvant etre vissees en force dans
ces trous.
Lesdites extensions de parois peuvent comprendre des picots faisant salilie de leurs faces exterieures, qui permettent, a titre
complementaire, de stabiliser ['armature par rapport au ciment.
La zone de jonction de ces extensions de parois comprend avantageusement un trou taraude permettant le montage de cette armature sur un ancillaire de mise en place de cette piece dans la cavite cotylo''dienne. Pour sa bonne comprehension, I'invention est 3 nouveau decrite ci-dessous en reference au dessin schematique annexe, representant, a3 titre d'exemple non limitatif, une forme de realisation preferee de ['implant
cotylo'dien qu'elle concerne.
La figure 1 en est une vue en perspective eclatee, avec une partie de l'os du bassin dans loquel il est destine a etre mis en place; la figure 2 en est une vue en coupe longitudinale apres mise en place, avec la portion correspondante de l'os du bassin, I' implant femoral cooperant avec ['implant cotylodien selon ['invention etant egalement represente en partie; la figure 3 est une vue en perspective d'une piece cite "armature" que comprend cet implant cotylodien; la figure 4 est une vue en perspective de cette armature sous un autre angle, avec des plots que comprend ['implant cotylo''dien, et la figure 5 est une vue en perspective de ['implant, avec
arrachement partiel.
La figure 1 represente un bassin 1 dont la cavite cotylo''dienne 2 doit etre equipee d'un implant cotylo''dien 3. Comme cela appara^'t plus particulierement sur la figure 2, I'os du bassin 1 delimitant la cavite 2 est relativement deteriore, de sorte que des greffons 4 dolvent etre mis en place pour restaurer cet os autant que faire se peut avant de mettre
['implant 3 en place.
Ce dernier fait partie d'une prothese de hanche comprenant, outre luimeme, un implant femoral 5. De maniere bien connue en soi, cet implant 5 comprend une tige medullaire 6 et un col 7 recevant une tete spherique 8, cette tete 8 pouvant etre engagee, avec debattement multidirectionnel, dans une cavite que forme ['implant cotylo'dien 3. Dans I'exemple represente, la tige 6 presente des picots 9 favorisant son
ancrage au femur.
Comme le montrent les figures, I' implant cotylo''dien 3 comprend une armature 10 destinee a etre f ixee a l'os, un cotyle a cimenter 1 1 pouvant etre mis en place sur cette armature 10 et trots plots 12. L'armature 10 est en un materiau metallique tel que de l'acier, du titane, un alliage incluant du titane ou un alliage chrome-cobalt. Wile presente une partie centrale formant une cavite 15 sensiblement hemispherique, de reception du cotyle 1 l, et deux pastes 16 permettant sa S fixation a l'os, faisant saillie lateralement de ladite partie centrale selon des
directions diametralement opposees.
La cavite 15 est delimitee par un anneau 20 et par trots extensions de parois 21 solidaires de cet anneau 20, faisant saillie du cote oppose 3 I'ouverture de la cavite 15. Ces extensions de parois 21 vent disposes a 120 les unes des autres et convergent les unes vers les autres
du cote oppose a l'anneau 20 jusqu'3 se rejoindre et former ainsi un fond.
Chaque extension de paroi 21 presente un trou taraude 22, dont l'axe forme, avec le plan de i'ouverture de la cavite 15, un angle de
l'ordre de 45 .
Ledit fond comprend un trou taraude 25 permettant le montage de ['armature 10 sur un ancillaire de mise en place de cette piece dans la
cavite cotylodienne 2.
Les pastes 16 ont des conformations adaptees a l'anatomie des regions osseuses voisines de la cavite cotylodienne 2 et vent percees de trous pour la reception de vis d'ancrage 26 de ['armature 10 a l'os. Ces pastes 16 peuvent avoir une epaisseur telle qutelles puissent etre deformees, notamment cambrees, si cela s'avere necessaire pour les
adapter a l'anatomie specifique du patient.
Le cotyle 11 presente une forme exterieure hemispherique et est forme par une coque exterieure 30 et par un insert 31 rec,u dans cette
coque 30.
La coque 30 est en un materiau metallique tel que de l'acier, du titane, un alliage incluant du titane ou un alliage chrome-cobalt. Wile presente, sur son cote externe, des saillies 32 qui favorisent sa prise d'appui contre un ciment permettant sa fixation au bassin 1, comme decrit plus loin. Sa face interne presente une forme sensiblement hemispherique
et est lisse.
L'insert 31 est en un materiau favorisant le glissement, notamment en polyethylene a haute densite, et delimite ladite cavite de reception de la tete 8 de ['implant 5. Sa face exterieure presente une forme sensiblement hemispherique, de rayon legerement inferieur a celui generant ladite face interne de la coque 30, de sorte que ['insert 31 peut etre recu dans la coque 30 avec possibilite de debattement multidirectionnel par
rapport a celle-ci.
Chaque plot 12 est en polyethylene a haute densite. II presente une partie filetee permettant son vissage dans un trou 22 et une partie cylindrique elargie ayant une hauteur qui correspond a l'epaisseur requise
de ciment entre ladite armature 10 et la coque 30, a savoir environ 2 mm.
En pratique, des greffons 4 vent mis en place dans la cavite cotylodienne 2 si cela est necessaire puis ['armature 10 est engagee dans cette cavite 2, 3 I ' aide de l' ancillaire sur lequel cette piece est montee grace a son trou 25, et est fixee a ltos 1 au moyen des vis 26. Du ciment est ensuite injecte dans le fond de la cavite 2, au travers des larges ouvertures delimitees par les extensions de parois 21, et est depose en exces dans la cavite 15, puis la coque 30 est mise en place dans cette cavite 15 jusqu'a prendre appoi contre les plots 12, ce qui fait fluer le ciment entre ['armature 10 et cette coque 30. Apres la prise du ciment, I'insert 31, mis en place sur la tete 8, est engage dans la cavite de la
coque 30.
Les plots 12 formant des zones d'appui ponctuel de la coque 30, qui assurent ['existence d'une epaisseur de ciment sensiblement uniforme entre ['armature 10 et la coque 30, et permettent, tent que le ciment n'est pas solidifie, de positionner la coque 30 selon toutes orientations angulaires possibles par rapport a ['armature 10. Le positionnement de cette derriere par rapport a I'os n'induit pas de restrictions dans le choix de la position de la coque 30 par rapport a cette
armature 10.
En outre, la possibilite d'utiliser un cotyle a debattement multidirectionnel de ['insert 31 par rapport 3 la coque 30 permet un debattement optimal de ['articulation, evite de solliciter l'ancrage de la coque 30 dans les positions de debattement maximal et reduit le risque de
luxation de ['articulation.
II apparaft de ce qui precede que I' invention apporte une amelioration determinante a la technique anterieure, en fournissant un implant cotylodien particulierement adapte a un bassin dont l'os delimitant la cavite cotyloidienne est deteriore, comme cela est frequemment le cas d'une "reprise", c'est-a-dire du remplacement d'un implant anterieurement pose. 11 va de sol que 1'invention n'est pas limitee a la forme de realisation decrite ci dessus a titre d'exemple mais qu'elle en embrasse au contraire toutes les variantes de realization couvertes par les
revendications ci-annexees.

Claims (9)

REVENDICATIONS
1 - Implant cotylodien de prothese de hanche, comprenant une piece de base ( 10) formant une armature, cite ci-apres "armature", destinee a etre fixee a l'os, et un cotyle a cimenter (11) pouvant etre mis en place sur cette armature (10), I'armature (10) presentant un anneau (20) delimitant une cavite (15) de reception du cotyle a cimenter (11) et une ou plusieurs pastes (16) permettant sa fixation a ltos, et le cotyle a cimenter (1 1) delimitant une cavite destinee a recevoir la tete spherique (8) de ['implant femoral (5) de la prothese de hanche; implant (3) caracterise en ce que ['armature (10) presente des extensions de parois (21) solidaires audit anneau (20), faisant saillie du cote oppose a ltouverture de la cavite (15) que delimite cet anneau (20) et convergeant les unes vers les autres jusqu'a se rejoindre et former ainsi un fond, ces extensions de parois (21) comprenant des plots ( 1 2) faisant saillie de leurs faces internee, espaces angulairement les uns des autres, dont la hauteur correspond a ltepaisseur requise de ciment entre ladite
armature (10) et le cotyle a cimenter (1 1).
2 - Implant selon la revendication 1, caracterise en ce que le cotyle a cimenter (11) comprend une coque exterieure (30) metallique et un insert (31) en materiau favorisant le glissement, notamment en polyethylene a haute densite, qui est recu dans cette coque (30) et qui delimite ladite cavite de reception de la tete spherique (8) de ['implant
femoral (5) de la prothese de hanche.
3 - Implant selon la revendication 2, caracterise en ce que la coque (30) presente, sur son cote externe, une conformation et/ou un
revetement de surface qui favorise sa prise d'appui contre le ciment.
4- Implant selon la revendication 2 ou la revendication 3, caracterise en ce que la face interne de la coque (30) presente une forme sensiblement hemispherique et en ce que la face exterieure de ['insert (31) presente egalement une forme sensiblement hemispherique, de rayon legerement inferieur a celui generant ladite face interne de la coque (30), cet insert (31) pouvant etre recu dans la coque (301 avec possibiiite de
debattement multidirectionnel par rapport a celle-ci.
5- Implant selon l'une des revendications 1 a 4, caracterise en
ce que le cotyle a cimenter ( 1 1) presente une forme exterieure hemispherique et en ce que les plots ( 12) vent au nombre de trots,
regulierement repartis sur une meme portion de surface spherique.
6 - Implant selon la revendication 1 a 5, caracterise en ce que lesdites extensions de parois (21) vent conformees de maniere a delimiter, S avec l'anneau (20), une cavite (15) de forme hemispherique, et en ce que
le cotyle a cimenter ( 1 1) presente une forme hemispherique.
7 - Implant selon l'une des revendications 1 a 6, caracterise en
ce que, lorsque le cotyle a cimenter (11) comprend une coque exterieure (30) en materiau metallique, les plots (12) vent en un materiau n'induisant
pas d'oxydo-reduction avec ce materiau.
8 - Implant selon l'une des revendications 1 a 7, caracterise en
ce que les plots (12) vent rapportes sur lesdites extensions de parois (21).
9 - Implant selon la revendication 8, caracterise en ce que les extensions de parois (21) comprennent des trous taraudes (22) et en ce que les plots (12) comprennent des portions filetees pouvant etre vissees dans ces trous (22), ou des portions auto-filetables pouvant etre vissees en
force dans ces trous (22).
- Implant selon l'une des revendications 1 a 9, caracterise en
ce que lesdites extensions de parois comprennent des picots faisant saillie
de leurs faces exterieures.
1 1 - Implant selon l'une des revendications 1 a 10, caracterise
en ce que la zone de jonction desdites extensions de parois (21) comprend un trou taraude (25) permettant le montage de ['armature ( 10) sur un
ancillaire de mise en place de cette piece dans la cavite cotylodienne (2) .
