Abstract

La prothèse d'épaule selon l'invention se caractérise essentiellement par le fait qu'elle comporte une patte oblongue 1 d'axe 2 apte à être implantée dans le canal médullaire d'un humérus 3, une tête de rotation 4 apte à coopérer avec la cavité glénoïdale 5 de l'omoplate 6, et des moyens 7 pour lier la tête de rotation 4 avec la patte 1, la tête de rotation 4 étant constituée par une rotule 20, une calotte 21 comportant un logement 22 en creux de forme complémentaire de la rotule mâle hémisphérique 20 de façon que la calotte 21 soit apte à s'orienter par rapport à la rotule et une surface de frottement 23 apte à coopérer avec la cavité glénoïdale 5 de l'omoplate, et des moyens 24 pour bloquer la calotte 21 par rapport à la rotule 20.

PROTHESE D'EPAULE
La présente invention concerne les prothèses d'épaule.
La plupart des prothèses d'épaule sont conçues comme les prothèses de hanche. Elles sont en définitive difficiles à implanter et, en outre, ne redonnent pas toujours, après leur implantation, le mouvement anatomique originel du bras par rapport à l'épaule. Tel est le cas par exemple de celle décrite dans le EP-A-1 048274.
La présente invention a essentiellement pour but de réaliser une prothèse d'épaule qui soit plus facile à implanter que les prothèses d'épaule de l'art antérieur et qui soit en outre plus facile à conformer pour être le plus proche possible du mouvement anatomique originel du bras par rapport à l'épaule du patient sur laquelle elle a été implantée.
Plus précisément, la présente invention a pour objet une prothèse d'épaule comportant une patte oblongue définie suivant un premier axe et apte à être implantée dans le canai médullaire de l'humérus, une tête de rotation apte à coopérer avec la cavité glénoïdale de l'omoplate et des moyens pour lier ladite tête de rotation avec ladite patte, caractérisée par le fait que ladite tête de rotation est constituée par : une rotule mâle hémisphérique, une calotte comportant un logement en creux de forme complémentaire de la rotule mâle hémisphérique de façon que ladite calotte enfichée dans ledit logement soit apte à s'orienter par rapport à la rotule mâle hémisphérique, et une surface de frottement apte à coopérer avec la cavité glénoïdale de l'omoplate, et . des moyens pour bloquer dans une position déterminée ladite calotte par rapport à la rotule mâle hémisphérique.

Les figures 3 à 5 représentent, dans une représentation normalisée en dessin industriel, la partie calotte de la prothèse d'épaule selon les figures 1 et 2, respectivement dans trois vues de face, de gauche et de dessus, la coupe partielle selon la figure 4 étant référencée A-A sur la figure 3, la coupe selon la figure 5 étant référencée B-B sur la figure 3.
Il est bien précisé que, sur les figures, les mêmes références désignent les mêmes éléments, quelle que soit la figure sur laquelle elles apparaissent et quelle que soit la forme de représentation de ces éléments. De même, si des éléments ne sont pas spécifiquement référencés sur l'une des figures, leurs références peuvent être aisément retrouvées en se reportant à une autre figure.
Le Demandeur tient aussi à préciser que les figures représentent un mode de réalisation préférentiel de l'objet selon l'invention, mais qu'il peut exister d'autres modes de réalisation qui répondent à la définition de cette invention.
Il précise en outre que, lorsque, selon la définition de l'invention, l'objet de l'invention comporte"au moins un" élément ayant une fonction donnée, le mode de réalisation décrit peut comporter plusieurs de ces éléments.
Il précise aussi que, si le mode de réalisation de l'objet selon l'invention tel qu'illustré comporte plusieurs éléments de fonction identique et que si, dans la description, il n'est pas spécifié que l'objet selon cette invention doit obligatoirement comporter un nombre particulier de ces éléments, l'objet de l'invention pourra être défini comme comportant"au moins un"de ces éléments.
La prothèse d'épaule dont un mode de réalisation est illustré sur les figures 1 à 5, comporte une patte oblongue 1 définie suivant un premier axe 2 et apte à être implantée dans le canal médullaire de l'humérus 3, une tête de rotation 4 apte à coopérer avec la cavité glénoïdale 5 de l'omoplate 6 et des moyens 7 pour lier la tête de rotation 4 avec la patte 1.
Selon une caractéristique de l'invention, la tête de rotation 4 est constituée par une rotule mâle hémisphérique 20, une calotte 21 comportant un logement en creux 22 de forme complémentaire de la rotule mâle hémisphérique 20 de façon que, lorsque la rotule mâle hémisphérique est enfichée dans le logement 22, la calotte soit apte à s'orienter par rapport à cette rotule et une surface de frottement
23 apte à coopérer avec la cavité glénoïdale 5 de l'omoplate, et des moyens 24 pour bloquer, dans une position déterminée, la position de la calotte 21 par rapport à la rotule mâle hémisphérique 20.

De façon très avantageuse, afin que la surface de frottement 23 soit la plus congruente possible de la cavité glénoïdale 5 de l'omoplate 6, cette surface de frottement 23 présente la forme d'une portion de tore qui, de préférence est définie sur une surface engendrée par la rotation, autour d'un deuxième axe 25, d'une courbe convexe formée d'au moins trois première, deuxième et troisième sections circulaires consécutives 26,27, 28 respectivement de rayon Ri, Rset Rs, R2 étant inférieur à Ri et à R3, avec de préférence les rayons R1 et R3 égaux.
Pour répondre à la majorité des cas qui peuvent se présenter aux praticiens, le Demandeur a fait réaliser une prothèse d'épaule dans laquelle les valeurs des rayons Ri, R2 et R3 qui donnent de bons résultats sont respectivement égales à 22,5 mm, 17,5 mm, et 22,5 mm, les centres des trois première, deuxième et troisième sections circulaires consécutives 26,27, 28 étant répartis et positionnés comme illustré sur la figure 4.
De façon à optimiser la coopération de la calotte 21 avec la cavité glénoïdale 5 de l'omoplate 6, comme plus particulièrement visible sur la figure 5, le centre 29 du logement 22 n'est pas situé dans le plan contenant le deuxième axe 25 et le pôle 70 de la courbe convexe situé, dans le mode de réalisation mentionné ci-dessus, à une distance du deuxième axe 25, référencée R4 sur cette figure 5, égale à 22,5 mm.
Dans une réalisation préférentielle, les moyens 7 pour lier la tête de rotation 4 avec la patte 1 sont constitués par un emboîtement du type mâlefemelle tronconique 30, la partie tronconique femelle 31 étant réalisée dans la rotule mâle hémisphérique 20 et la partie mâle 32 étant solidaire de la patte 1.
Dans le but de favoriser l'implantation de la prothèse, il est avantageux que la patte 1 soit constituée par une tête de patte proximale 40 comportant une première percée cylindrique de révolution 47 définie selon le premier axe 2 défini ci-avant, la tête étant apte à être implantée dans la partie proximale du canal médullaire de l'humérus.
Cette patte 1 comporte en outre une queue de patte distale 41 comportant deux parties 42,43, une première partie d'implantation 42 apte à être implantée dans la partie distale 44 du canal médullaire de l'humérus 3, et une seconde partie d'implantation 43 constituée par une tige 46 solidaire de la première partie d'implantation en formant avec cette dernière une surface d'épaulement 48. Cette tige 46 est montée dans la première percée cylindrique de révolution 47 de façon

que la surface d'épaulement 48 vienne buter contre le bord de la première percée 47 définie sur une première extrémité 49 de la tête de patte proximale 40.
De plus, la patte 1 comporte un embout 50 avec lequel est solidaire la partie mâle 32 de l'emboîtement mâle-femelle tronconique 30, cette partie mâle étant définie selon un troisième axe 51 et l'embout étant apte à venir en appui sur la seconde extrémité 52 de la tête de patte proximale 40 opposée à la première extrémité 49. L'embout 50 comporte une seconde percée cylindrique de révolution 53 définie selon un quatrième axe 54 dans laquelle s'enfiche l'extrémité libre 55 de la tige 46, et des moyens 56 pour le solidariser avec la tige 46 en emprisonnant la tête de patte proximale 40 entre cet embout et la surface d'épaulement 48.
Ces moyens 56 sont, comme illustré sur la figure 1, constitués par exemple par une vis à tête d'épaulement dont la tige filetée se visse dans l'extrémité libre 55 de la tige 46 conformée en écrou.
De façon avantageuse, la prothèse est conformée de façon que les troisième 51 et premier 2 axes fassent entre eux un angle d'environ cent trente cinq degrés.
Dans une réalisation possible qui donne de bons résultats, les moyens 24 pour bloquer dans une position déterminée la calotte 21 par rapport à la rotule mâle hémisphérique 20, sont constitués par au moins deux orifices filetés 60,61 réalisés dans la calotte 21 suivant deux cinquième 62 et sixième 63 axes non confondus mais concourant sensiblement au centre 29 du logement 22, et deux vis 64,65 montées respectivement dans les deux orifices filetés 60,61 aptes à venir en appui contre la rotule 20. Avantageusement pour bien assujettir les vis contre la rotule 20, au moins l'une des deux vis 64,65 est constituée par une vis dont l'extrémité 66 apte à venir au contact de la rotule mâle hémisphérique 20 est creuse.
La prothèse d'épaule décrite ci-dessus, s'utilise de la façon suivante :
Il est tout d'abord précisé que cette prothèse d'épaule est plus particulièrement utilisée dans le cas d'une fracture de l'humérus, quand notamment la cavité glénoïdale 5 de l'omoplate 6 est encore en bon état.
Dans ce cas, le chirurgien qui veut implanter une telle prothèse commence à l'assembler comme illustré sur la figure 1, mais sans que la calotte 21 soit bloquée par rapport à la rotule 20 et que la vis des moyens 56 soit vissée à fond

Après avoir réséqué la tête proximale de l'humérus 3 et préparé le canal médullaire, le chirurgien introduit la patte 1 dans ce canal et oriente la tête de patte proximale 40 par rapport à la queue de patte distale 41 pour respecter l'hélitorsion de la diaphyse humérale.
Ensuite, il oriente tout à la fois l'embout 50 par rapport la tête de patte proximale 40 et la calotte 21 par rapport à la rotule 20 de façon à parfaitement positionner la surface de frottement 23 par rapport à la cavité glénoïdale 5 de l'omoplate 6 pour respecter la rétroversion de la tête humérale originelle.
Quand il a trouvé toutes les positions optimales relatives des différents éléments constitutifs de la prothèse définis ci-dessus, il bloque la calotte 21 sur la rotule 20 au moyen des vis 64 et 65, ainsi que l'ensemble constitué par la tête de patte proximale 40, la queue de patte distale 41 et l'embout 50 grâce aux moyens de fixation 56.
Il effectue ensuite les opérations nécessaires à l'implantation définitive de la prothèse d'épaule qui sont de son domaine et qui ne seront pas décrites ici car elles sont connues en elles-mêmes et, de plus, n'entrent pas dans le champ de protection de la présente invention.

Claims (10)

REVENDICATIONS
1. Prothèse d'épaule comportant une patte oblongue (1) définie suivant un premier axe (2) et apte à être implantée dans le canal médullaire de l'humérus (3), une tête de rotation (4) apte à coopérer avec la cavité glénoïdale (5) de l'omoplate (6) et des moyens (7) pour lier ladite tête de rotation (4) avec ladite patte (1), caractérisée par le fait que ladite tête de rotation (4) est constituée par : * une rotule mâle hémisphérique (20), . une calotte (21) comportant un logement en creux (22) de forme complémentaire de la rotule mâle hémisphérique (20) de façon que ladite calotte enfichée dans ledit logement (21) soit apte à s'orienter par rapport à la rotule mâle hémisphérique, et une surface de frottement (23) apte à coopérer avec la cavité glénoïdale (5) de l'omoplate, et 'des moyens (24) pour bloquer dans une position déterminée ladite calotte (21) par rapport à la rotule mâle hémisphérique (20).
2. Prothèse d'épaule selon la revendication 1, caractérisée par le fait que ladite surface de frottement (23) présente la forme d'une portion de tore.
3. Prothèse d'épaule selon la revendication 2, caractérisée par le fait que ladite surface de frottement (30) en forme de portion de tore est définie sur une surface engendrée par la rotation, autour d'un deuxième axe (25), d'une courbe convexe formée d'au moins trois première, deuxième et troisième sections circulaires consécutives (26,27, 28) respectivement de rayon Ri, R2 et R3, R2 étant inférieur à Ri et à R3.
4. Prothèse d'épaule selon la revendication 3, caractérisée par le fait que les rayons Ri et R3 sont sensiblement égaux.
5. Prothèse d'épaule selon l'une des revendications 3 et 4, caractérisée par le fait que le centre (29) dudit logement (22) n'est pas dans le plan contenant le deuxième axe (25) et le pôle (70) de la courbe convexe.

Prothèse d'épaule selon l'une des revendications 1 à 5, caractérisée par le fait que les moyens (7) pour lier la tête de rotation (4) avec la patte (1) sont constitués par un emboîtement mâle-femelle tronconique (30), la partie tronconique femelle (31) étant réalisée dans la rotule mâle hémisphérique et la partie mâle (32) étant solidaire de ladite patte.
7. Prothèse d'épaule selon la revendication 6, caractérisée par le fait que la patte (1) est constituée par : 'une tête de patte proximale (40) comportant une première percée cylindrique de révolution (47) définie selon ledit premier axe (2), ladite tête étant apte à être implantée dans la partie proximale du canal médullaire de l'humérus, 'une queue de patte distale (41) comportant deux parties (42,43), * une première partie d'implantation (42) apte à être implantée dans la partie distale (44) du canal médullaire de l'humérus (3), et * une seconde partie d'implantation (43) constituée par une tige (46) solidaire de la première partie d'implantation en formant avec cette dernière une surface d'épaulement (48), ladite tige (46) étant montée dans la première percée cylindrique de révolution (47) de façon que la surface d'épaulement (48) vienne buter contre le bord de ladite première percée (47) définie sur une première extrémité (49) de la tête de patte proximale (40), 'un embout (50) avec lequel est solidaire la partie mâle (32) dudit emboîtement mâle-femelle tronconique (30), cette partie mâle étant définie selon un troisième axe (51), ledit embout étant apte à venir en appui sur la seconde extrémité (52) de la tête de patte proximale (40) opposée à la première extrémité (49) et comportant une seconde percée cylindrique de révolution (53) définie selon un quatrième axe (54) dans laquelle s'enfiche l'extrémité libre (55) de ladite tige (46), et 'des moyens (56) pour solidariser ledit embout (50) avec ladite tige (46).
8. Prothèse d'épaule selon la revendication 7, caractérisée par le fait que les troisième (51) et premier (2) axes font entre eux un angle d'environ cent trente cinq degrés.

Prothèse d'épaule selon l'une des revendications 1 à 8, caractérisée par le fait que les moyens (24) pour bloquer dans une position déterminée ladite calotte (21) par rapport à la rotule mâle hémisphérique (20) sont constitués par : 'au moins deux orifices filetés (60,61) réalisés dans ladite calotte (21) suivant deux cinquième (62) et sixième (63) axes non confondus mais concourant sensiblement au centre (29) dudit logement (22), et . deux vis (64,65) montées respectivement dans les deux orifices filetés (60,61).
10. Prothèse d'épaule selon la revendication 9, caractérisée par le fait qu'au moins l'une des deux vis (64,65) est constituée par une vis dont l'extrémité (66) apte à venir au contact de la rotule mâle hémisphérique (20) est creuse.
