Abstract

Le dispositif est composé de 3 pièces (A, B, et C) dont les pièces A et B sont utilisées pour fixer le grand trochanter après mise en place d'une prothèse totale de hanche. La pièce A est une agrafe à 3 ou 4 branches (i), caracténsée par une douille (3) de longueur variable implantée au centre de sa face inférieure. La pièce B est une vis dont une extrémité (5) est munie d'un filetage vissé dans le logement (6) situé au dos de la queue de la pièce fémorale près de la base du col. L'autre extrémité de la pièce B est une tête (4) qui est plus large que le canal (2) de la douille (3) de la pièce A. En vissant la vis B passée dans la douille (3) de la pièce A, on exerce un serrage du grand trochanter sans toutefois provoquer d'extraction de la pièce fémorale grâce à une longueur adéquate de la douille (3) dont l'extrémité inférieure est au contact du dos de la pièce fémorale de la prothèse. La pièce C est une pièce munie à une extrémité d'un filetage (7) pour être vissé dans le logement (6) de la prothèse et à son autre extrémité d'une pointe (8). Cette pièce sert à positionner le grand trochanter en l'abaissant sur la pointe (8), elle sert de guide pour la mise en place de l'agrafe A et permet la mesure exacte de la longueur de la vis B de la douille (3) de l'agrafe A.

Description

DISPOSITIF D'OSTEOSYNTHESE DU GRAND TROCHANTER FIXE SUR LA
PIECE FEMORALE D'UNE PROTHESE TOTALE DE HANCHE
L'ostéotomie du grand trochanter pour l'abord chirurgical de la hanche en vue d'implanter une prothèse totale est reconnue la voie d'abord la plus sécurisée mais aussi la plus controversée du fait des faibles moyens d'ostéosynthèse utilisés, du délai tardif de l'appui, et des risques de démontage qui pourrait compromettre le résultat final. Le présent dispositif (fig. 1) est un mode d'ostéosynthèse par agrafe (A) dont la particularité est d'être fixée par le moyen d'une vis (B) au dos de la pièce fémorale après sa mise en place dans la diaphyse

Le dispositif de base est composé de deux pièces auxquelles est adjointe une 3ème pièce (C) qui sert de guide à la mise en place successive des pièces A, puis B. * La pièce A (fig. 2) est une agrafe métallique à 3 ou 4 branches (1) suffisamment souples pour disposer d'un effet ressort et dont l'extrémité est munie d'un bec pour l'ancrage à la surface du grand trochanter. Le centre de cette agrafe est occupé par un orifice (2) prolongé à la face inférieure de l'agrafe (A) d'une douille (3), dont la longueur est variable pour chaque pièce afin qu'une fois posée, l'extrémité de cette douille soit au contact du dos de la pièce fémorale. La pièce (B) est une vis munie d'une tête (4) dont la caractéristique est d'être plus large que le diamètre d'ouverture de l'orifice (2) de la pièce A. Cet artifice permet par vissage d'exercer un effet de serrage sur l'agrafe A, sans toutefois risquer de déchausser la pièce fémorale grâce au contact de la douille (3) sur
la pièce fémorale.
L'extrémité de la vis B est un filetage (5) vissé dans le logement (6) muni également d'un filetage et ménagé au dos de la pièce
fémorale (fig. 3) à proximité de la base du col.

La pièce C (Fig.4) du système est le guide de mise en place du dispositif. C'est une tige munie à une extrémité (7) d'un filetage identique à celui de la vis B pour être également vissé dans le logement 6 de la pièce fémorale. L'autre extrémité de la pièce C est une pointe (8) utilisée pour perforer le grand trochanter lors de son positionnement sur la coupe d'ostéotomie du fémur. La pièce C est munie d'un étalonnage centimétrique (9) pour mesurer l'épaisseur du grand trochanter. C'est cette mesure qui donnera à la fois, la longueur adéquate de la vis (B) et celle de la douille(3) de l'agrafe A. io La pièce C sert donc à perforer le grand trochanter (fig. 5 et 6), elle permet de déterminer la longueur des pièces A et B à utiliser et elle sert de guide pour la mise en place de la pièce A. En effet, l'agrafe (A) est placée en introduisant l'orifice inférieur de la douille (3) sur la
pointe (8) de la pièce C (fig.7).
1s Après que l'agrafe A soit en place, la pièce C qui a servi de guide est dévissée de son logement (6) et remplacée par la vis B de longueur adéquate. En utilisant une agrafe (A) dont l'extrémité de la douille (3) vient bien en contact avec le dos de la pièce fémorale, on évite le risque de déchaussement de celle-ci, au cours du vissage de la vis B (fig.8 et
9) dans le logement de la pièce fémorale.

Claims (4)

REVENDICATION
1) Dispositif d'ostéosynthèse du grand trochanter fixé sur le dos de la pièce fémorale d'une prothèse de hanche après qu'elle soit mise en place dans le fémur. Le dispositif est composé de 2 pièces A et B auxquelles est adjointe une pièce C qui sert de guide de mise en place des pièces A et B. - La pièce A est une agrafe à 3 ou 4 branches (1) souples et munies d'un bec à leurs extrémités. La face inférieure de l'agrafe est munie en son centre d'une douille (3) de longueur variable et dont l'orifice d'entrée (2) au centre de la face o0 supérieure permet le passage des pièces B et C. - La pièce B est une vis dont une extrémité est occupée par une tête (4) dont la largeur dépasse le diamètre de l'orifice (2) de l'agrafe (A). L'autre extrémité (5) est munie d'un filetage pour être vissé dans le logement (6) ménagé au dos de la pièce

2) Dispositif selon la revendication 1, caractérisé en ce que la pièce fémorale de la prothèse, dispose sur la convexité de la queue, proche de son col, d'un logement (6) muni d'un filetage visant à recevoir successivement l'extrémité filetée (7) de la pièce C, puis (5) de la vis B.
3) Dispositif selon les revendications 1 et 2, caractérisé en ce que le
système dispose d'une pièce supplémentaire C qui sert de guide à la mise en place successive des pièces A et B. Cette pièce est une tige dont l'extrémité (7) est munie d'un filetage identique à celui (5) de la vis B et dont l'autre extrémité (8) est conformée en
pointe pour traverser l'épaisseur du grand trochanter.
4) Dispositif selon la revendication 3, caractérisé en ce que la pièce C dispose sur sa longueur d'un étalonnage centimétrique (9) pour permettre le choix de la longueur de la vis B et de la douille (3) de la pièce A.
) Dispositif selon une quelconque des revendications 1,2,3, et 4,
caractérisé en ce qu'il comprend une série d'agrafes A qui disposent, chacune, d'une douille (3) de longueur variable afin que l'extrémité de celle-ci soit bien en contact avec le dos de la prothèse fémorale pour éviter tout risque d'extraction de celle-ci, au cours de la mise en place de la vis B.
