Abstract
Description
The subject of the present invention is a universal autostatic surgical retractor composed of a spacing control mechanism, rigid spacing arms in the transverse direction and malleable in the vertical plane and sheaths threadable on said spacing arms. . The retractor can be used in various applications of orthopedic surgery or in other procedures regardless of the shape of the operating field incision, long and narrow, or short and wide and / or deep.
The intervention difficulties for which the modular retractor was designed are encountered in interventions on the spine, hip, shoulder, etc ... by the posterior or anterior route, on muscular or otherwise lean patients.
The autostatic retractor must generally meet very precise specifications:
* Spread to the width necessary for the intervention without having to develop too much effort on his sleeve and without making it bend under the effort,
* Stay in the precise place where it was positioned without having to be tied up on the spot or
to be stowed with an additional accessory to avoid slipping,
* Do not be too bulky inside the incision or outside the operating field, do not interfere with the operating gesture,
* Being able to spread in certain cases over great lengths without obstructing the opening,
* Being able to spread deep in certain areas of the incision and on the surface in other areas of the same incision,
To meet these criteria, it was necessary until now to invest in many models of spacers, which is expensive and clutters the instrumentation tables, already loaded with the instruments necessary for the operation proper.
Various situations can arise in the case of an intervention on the spine:
* Autostatic hinged retractors are usually used to spread the operating field, either by 2, by 3 or 4 units for long incisions such as scoliosis. In this case, the opening and access of the implants to the operating area are obstructed.
* Some spacers are more specifically designed to develop a large spreading power, to the detriment of space or weight. Their valves are too large, their racks protrude too much, and despite their power, they do not always hold in place.
* Others are more discreet in shapes and dimensions but on the other hand are unstable, fragile and very not very rigid.
* Certain conceptions become momentarily stable thanks to claws on the valves which, on the other hand, generate post-operative necrosis of the soft parts on which the support was taken.
With regard to the case of retractors to be used in hip interventions, there are very few models adapted to the depth of certain approaches and the necessary spreading power.
The invention aims to provide a spacer which overcomes the aforementioned drawbacks, that is to say which is both rigid to withstand the transverse stresses of spacing and malleable to adapt to the vertical anatomical contours of the incision, modular to adapt to all dimensions of orthopedic incisions and stable that is to say that does not slip outside the incision and this, by means simple to handle in the room operation, efficient and inexpensive.
To this end, the invention relates to an autostatic retractor characterized in that it comprises:
* At least two spacer arms rigid in the direction transverse to the incision, but malleable in the vertical plane, modular according to their length, their curvature, their shape and general external functions.
* A spreading and holding mechanism for the spreading arms whose overall transverse dimension does not exceed that of the spreader whatever its opening, so that it can be buried in a deep incision without interfering, mechanism can be fully or partially buried under the sub-skin to be kept in the necessary place by it and not be able to come out of itself, which makes the retractor very stable.
* An assortment of flexible sheaths, each fulfilling various functions in the incision.
The spacer arm according to the invention is composed of simple elements of various types which overlap one another, a cable passing through their center, mechanical means for securing said cable at the ends and mechanical means of final tensioning of said cable in the factory.
The transverse rigidity in bending is obtained by the compression of all the consecutive elements thanks to the tensioning of the cable passing in their center.
The transverse rigidity obtained by the cable thus stretched does not prevent the relative mobility of the elements together in a vertical plane, that is to say samaleability.
The malleability, allowing the surgeon to make the retractor follow the anatomical contours, is obtained by manipulating the elements by hand to make them take a given shape. These remain in the desired position thanks to the effect of three concepts acting simultaneously:
* The compression of the elements between them,
* The elements are compressed on particular surfaces formed on their ends, so-called lift surfaces through which the cable necessarily passes,
* The interposition of elements of various shapes, such that the length of the cable always remains identical, whatever the relative orientation of the elements between them, that is to say the shape given to the arm of the retractor.
The modularity is characterized by a separation of the functions "Spacing mechanism", "articulated arm for maintaining the spacing" and surface accessories "of the spacer.
*Diverses fonctions peuvent etre remplies par les bras d'écartement grâce à la gaine enfilée par dessus.* Various lengths of spacer arms can be mounted on the mechanism,
* Different shapes varying between S, U and straightness can be given to the spacer arm,
* Various functions can be fulfilled by the spacer arms thanks to the sheath threaded from above.
The spacing mechanism has concepts that give it a universal use character. Indeed, it can be introduced into the incision without any protruding rack interfering, thus one and the same version of the retractor can adapt to a deep or on the contrary superficial incision, one and the same version can be set to a width ranging from simple to double at least.
It is composed on the one hand of a central box comprising means for maneuvering the opening width of the spacer arms controlled by screwing unscrewing using a key and on the other hand of two arm supports angle-shaped spacing one of whose branches slides on and operates in said central box and the other supports the spacing arms.
The spreading mechanism also has concepts that promote the general stability of the instrument, that is to say its ability to remain in the desired initial position, despite the accoups outward movements which can be communicated to it by the surrounding soft tissue or other surgical instruments used.
For this, the support arms that it supports are positioned under the case, with a recess between them and their support, recess which will be occupied by the under skin or the soft parts stretched axially at the incision. Thus the arms, and consequently the retractor, cannot go back to the surface.
An exemplary embodiment will now be described with reference to the appended drawings, in which:
* Figure 1 is a perspective view of the universal autostatic spacer according to the invention, as well as an operating key (28) of the spacing.
* Figures 2 and 3 are elevational views of a variant of the elements constituting the arms. They are of at least two types (4) and (5) assembled alternately, in the case of recligne configuration (fig.2) and case of tilting of one element with respect to the next (fig.3) thus creating a configuration locally curved.
* Figures 4,5 and 6 are simplified diagrams for the sake of understanding and represent the various possible forms that the spacer arms can take, thanks to the alternating assembly of simple elements of two types (4 ) and (5). One of the types is characterized by the ooopaabion of a rib (13) at one of its ends with a groove (14) on one of the ends of the next element. One has ends whose faces are parallel to the same plane (9A) at one of its ends and to a plane (9B) at its second end, the two planes being parallel to each other. the element has ends with at least two facets (1ou) and (lOB) of planar or arbitrary shape and inclined at any angle a between them.
* Figures 4,5 and 6 also show the interest of the alternation of the simple elements (4) and (5) described above associated with a forced passage point in the geometric center (12) for the cable (7) , made by drilling in the mass: The stretched cable passing through the center of the elements does not undergo any noticeable elongation during the shaping manipulations of the arm, since the distance (11) separating the plane of symmetry of an element and the center of the next element is not changed.
* Figure 7 is a partially sectional view of a spacer arm and representing on the one hand the tensioning (23) and securing (22) mechanisms of the cable (7) and on the other hand some examples of flexible sheaths (8) lining the spacer arms, which has devices on its surface and giving the retractor various complementary functions such as for example a protective lip (19) from the incision or protuberances (21) additional lashing or of the perforated tubular type (29).
* Figure 8 is a partially sectional view of the retractor buried in an incision (25) in the fully closed state. The sub-skin (24) of the incision is shown here in section: It stretches over the spacer arms (3A) and (3B), which contributes to the stability of the retractor (1).
* Figure 9 is a top view of the universal and modular retractor in the open position, showing the external positioning given to the arm (3A) and (3B) by means of supports (30) since they are located at beyond the outer surface (26) of the branch (17) of the arm supports (1 SA) and (15B) on the one hand and representing on the other hand the length (26) of each branch (l6) which is less to that (27) of the housing (18). The independent central box (18) has at least one recess (32) at each end and the two spacer arm supports (1 SA) and (15B) in the form of an angle have one of the branches of the cooperating angle with the said recesses (32) on either side in the transverse opening-closing movement of the spacer. All of these provisions allow the retractor to be fully buried in deep incisions, in the open or closed position.
