Abstract

Prothèse de hanche dont la surface (20) de chacun des flancs (21) de la partie métaphysaire (2) de l'implant fémoral est creusée de cavités (22) présentant chacune une paroi supérieure (23), deux parois latérales (24, 25) triangulaires, et un fond (26), parallèle à l'axe longitudinal de l'implant, joignant les bases desdites parois (23, 24, 25) à ladite surface (20).

Description

La présente Invention a pour objet une prothèse de hanche et plus particulièrement la partie métaphysaire de l'implant fémoral d'une telle prothèse.
Une prothèse de hanche est en général constituée de deux parties, une pièce cotyloidienne destinée à remplacer le cotyle du bassin, et une pièce fémorale destinée à remplacer la partie supérieure du fémur.
La pièce fémorale comporte notamment une partie diaphysaire et une partie métaphysaire surmontée d'un col et d'une rotule destinée à être introduite dans le cotyle prothétique.
La partie diaphysaire est constituée d'une tige cylindrique destinée à assurer le guiaage dans le canal médullaire du fémur, tandis que la partie métaphysaire est évasée vers le haut pour assurer une bonne répartition des efforts de poussée dus au poids du corps du patient.
La solidarisation de l'implant fémoral au fémur est réalisée au moyen d'un ciment pour la partie diaphysaire, généralement complétée, pour la partie métaphysaire, d'un traitement de surface permettant une fixation biologique.
La surface de la partie métaphysaire peut ainsi être poreuse ou recouverte d'une matière du type hydroxyapatite, en vue de favoriser l'ostéogenèse.
Si une telle prothèse permet une bonne fixation elle n'en présente pas moins des inconvénients lorsqu'il est nécessaire de réaliser une reprise, c'est-à-dire de l'enlever pour la remplacer.
En effet, alors que les efforts principaux sont exercés du haut vers le bas, l'ancrage de l'implant à la matière osseuse agit dans les deux sens, à l'enfoncement et à l'arrachement, ce qui ne facilite pas son extraction.
La présente invention a pour but de remédier à cet inconvénient en proposant une prothèse de hanche et plus particulièrement la partie métaphysaire de celle-ci, permettant une extraction aisée en cas de reprise tout étant apte à supporter les efforts de poussée dus au poids du corps du patient.
La prothèse objet de la présente invention se caractérise essentiellement en ce que la partie métaphysaire évasée comporte sur ces flancs des cavités creusées dans la surface desdits flancs, présentant chacune une paroi supérieure, sensiblement perpendiculaire à l'axe longitudinal de l'implant, deux parois latérales triangulaires, et un fond, parallèle audit axe longitudinal joignant les bases desdites parois à ladite surface.
Lorsque l'implant est en place, la matière osseuse prolifère dans les cavités dont la forme interdit l'enfoncement sans entraver une éventuelle l'extraction dudit implant.
Selon une caractéristique asditionnelle du dispositif selon l'invention, les cavités sont disposées en rangées et en colonnes, et elles sont débouchantes dans le sens vertical, c'est-à-ire que pour deux cavités adjacentes dans une même colonne, l'une débouche dans l'autre, elles ont ainsi en commun une arête correspondant pour la cavité la plus basse au bord supérieur de sa paroi supérieure et pour la cavité la plus haute au bord inférieur de son fond.
Les avantages et les caractéristiques de la présente invention ressortiront plus clairement de la description qui suit et qui se rapporte au dessin annexé, lequel en représente un mode de réalisation non limitatif.
Dans le dessin annexé :

Si on se réfère à la figure 1 on peut voir une prothèse de hanche et plus précisément un implant fémoral comportant une partie diaphysaire 1 et une partie métaphysaire 2 destinée à recevoir supérieurement un col non représenté.
Les flancs 20 de la partie métaphysaire 2, dont un seul est visible sur la figure, comportent chacun à leur surface 21 des cavités 22 disposées en rangées et en colonnes.
Les cavités 22 comportent chacune une paroi supérieure 23 en arc de cercle, deux parois latérales triangulaires 24 et 25, et un fond 26 joignant les bases des parois 23, 24 et 25 à la surface 21 du flanc 20.
Les fonds 26 sont parallèles à l'axe longitudinal de la prothèse, leur inclinaison relative étant due à l'évasement de la partie métaphysaire 2.
Lorsque la prothèse est implantée dans le fémur, les parois supérieures 23 des cavités 22 prennent appui sur la matière osseuse qui se développe dans les cavités 22, et une éventuelle extraction de l'implant est facilitée par la verticalité des fonds 26, qui sont parallèles à l'axe d'extraction.
Si on se réfère maintenant à la figure 3 on peut voir que selon une variante, les cavités 22 sont débouchantes, c'est-à-dire pour deux cavités adjacentes 22 d'une même colonne, la cavité 22 supérieure débouche dans la cavité 22 inférieure.
I1 va de soi que la présente invention ne saurait être limitée à la description qui précède de deux de ses modes de réalisation, susceptibles de subir un certain nombre de modifications sans pour autant sortir du cadre de l'invention.

REVENDICATIONS
1) Prothèse de hanche caractérisée en ce que la surface (20) de chacun flancs (21) de la partie métaphysaire (2) de l'implant fémoral est creusée de cavités (22) présentant chacune une paroi supérieure (23), deux parois latérales (24, 25) triangulaires, et un fond (26), parallèle à l'axe longitudinal dudit implant, joignant les bases desdites parois (23, 24, 25) à ladite surface (20).
2) Prothèse selon la revendication 1 caractérisée en ce que les parois supérieures (23) des cavités (22) sont en forme d'arc de cercle.
3) Prothèse selon la revendication 1 ou la revendication 2 caractérisée en ce que les cavités (22) sont disposées en rangées et en colonnes, et elles sont débouchantes dans le sens vertical, c'est-à-dire que pour deux cavités (22) adjacentes dans une même colonne, l'une débouche dans l'autre, elles ont ainsi en commun une arête correspondant, pour la cavité (22) la plus basse au bord supérieur de sa paroi supérieure (23), et pour la cavité (22) la plus haute, au bord inférieur de son fond (26).
