Abstract
Dispositif de maintien d'un écartement normal entre les vertèbres, destiné au remplacement de vertèbres manquantes et comprenant deux pièces (1, 2) pouvant être déplacées axialement l'une par rapport à l'autre et pouvant être bloquées en position, caractérisé en ce que les deux pièces (1, 2) sont emboîtables l'une dans l'autre et comprennent au niveau de leurs faces mutuellement en regard des formes complémentaires ou conjuguées qui coopèrent de manière à pouvoir déplacer lesdites pièces l'une par rapport à l'autre en distraction et à éviter un déplacement en compression de l'une par rapport à l'autre.
Description
DISPOSITIF DE MAINTIEN D'UN ECAT T NORMAL ENTRE LES
ERTEHS ET DESTINE AU REMPULACMENT DE VERTEMBES
La présente Invention a pour objet un dispositif de maintien d'un écartement normal entre les vertèbres, destiné au remplacement de vertèbres manquantes et comprenant des pièces pouvant être déplacées axialement l'une par rapport à l'autre et
pouvant être bloquées.
Il est connu qu'un évidement entre les vertèbres de la colonne vertébrale provenant d'une blessure ou de l'extraction d'une tumeur peut être remplacé par un os autologue (implant) pris à partir d'une autre partie du système osseux, par une opération alloplastique et/ou par un dispositif mécanique destiné
au maintien d'un écartement normal entre deux vertèbres.
Même dans le cas de l'utilisation des matières autologues ou alloplastiques, il est nécessaire de supporter les parties autour de l'évidement, étant donné que l'incorporation des implants autologues prend une longue période de temps et il n'est pas possible de les
charger tout de suite.
Il est connu d'utiliser des cylindres en tissu réticulé de titane qui entourent d'une part la matière implantée entre les vertèbres et supportent d'autre part les vertèbres voisines. Cependant, ces cylindres en tissu de titane doivent être utilisés ensemble avec des vis vertébrales et des tiges de blocage, leur utilisation est
donc assez compliquée et prend beaucoup de temps.
Il est connu également de remplacer l'évidement par un dispositif destiné au maintien d'un écartement entre les vertèbres, à longueur variable, pourvu des griffes. Cependant, la longueur de ce dispositif doit être réglée de chaque côté par une fixation à vis, ce qui est bien difficile au cours de
l'intervention chirurgicale.
La présente Invention vise donc un dispositif de maintien d'un écartement normal entre les vertèbres, pour le remplacement des vertèbres manquantes, de structure très simple, permettant son implantation rapidement, simplement et en grande sécurité, même dans
les conditions opératoires.
Pour atteindre ce but, l'invention propose un dispositif de maintien d'un écartement normal entre les vertèbres, destiné au remplacement de vertèbres manquantes et comprenant deux pièces pouvant être déplacées axialement l'une par rapport à l'autre et pouvant être bloquées en position, caractérisé en ce que les deux pièces sont emboîtables l'une dans l'autre et comprennent au niveau de leurs faces mutuellement en regard des formes complémentaires ou conjuguées qui coopèrent de manière à pouvoir déplacer lesdites pièces l'une par rapport à l'autre en distraction et à éviter un déplacement en compression de l'une par rapport à
l' autre.
Selon un mode de réalisation, les deux pièces respectivement extérieure et intérieure sont de forme cylindrique, l'enveloppe interne de la pièce extérieure comporte des rainures en dents de scie et l'enveloppe externe de la pièce intérieure comporte des ailettes en saillie qui ont une forme complémentaire ou conjuguée de
celle des rainures.
Selon une autre caractéristique de l'invention, l'enveloppe interne de la pièce extérieure comporte au moins une nervure axiale, et en ce que la paroi de la pièce intérieure comporte au moins une fente axiale dans laquelle la nervure est destinée à se
positionner pour bloquer en position les deux pièces.
Selon encore une autre caractéristique de l'invention, une extrémité de la pièce extérieure est fermée par un fond, dont la face externe est pourvue de griffes d'accrochage, et une extrémité de la pièce intérieure est également fermée par un fond, dont la face
externe est pourvue de griffes d'accrochage.
En variante, les deux extrémités de la pièce extérieure sont ouvertes, et un bouchon est rapporté sur l'une des deux extrémités de la pièce extérieure, ce bouchon étant équipé de griffes d'accrochage ou d'un pied de support et, de manière semblable, les deux extrémités de la pièce intérieure sont ouvertes, et un bouchon est rapporté sur l'une des deux extrémités de la pièce intérieure, ce bouchon étant équipé de griffes
d'accrochage ou d'un pied de support.
L'invention pourra être encore mieux comprise
à l'aide de la description d'un mode de réalisation
illustré par des dessins annexés sur lesquels: - la figure 1 est une vue en coupe axiale de la pièce extérieure du dispositif selon l'invention, la figure 2 est une vue en coupe axiale de la pièce intérieure du dispositif selon l'invention, et - la figure 3 est une vue en coupe axiale des
deux pièces assemblées du dispositif selon l'invention.
Dans le mode de réalisation illustré sur les figures, le dispositif selon l'invention comprend une pièce extérieure 1 et une pièce intérieure 2 qui sont formées à partir de deux cylindres 3 et 4, respectivement. L'enveloppe externe de la pièce extérieure 1 est cylindrique et son enveloppe interne, également cylindrique, comporte des rainures 5 globalement circulaires et usinées dans la paroi de l'enveloppe. Ces rainures 5 sont en forme de harpons ou dents de scie pour constituer des moyens de blocage dans la direction axiale indiquée par la flèche F. A une extrémité, le cylindre 3 est fermé par un fond 6, dont la face externe est pourvue de griffes 7. Deux rainures axiales 8 se faisant face sont usinées dans l'enveloppe externe du cylindre 3 pour former deux nervures 8a en
saillie à l'intérieur du cylindre 3.
Une extrémité de la cavité interne du cylindre 4 de la pièce intérieure 2 est également fermée par un
fond 9, dont la face externe est pourvue de griffes 10.
Vers son autre extrémité, l'enveloppe externe du cylindre 4 comporte des ailettes circulaires 11 en saillie. Ces ailettes 11 sont en forme de harpons ou dents de scie qui sont complémentaires de celles des rainures 5 de la pièce extérieure 1 pour pouvoir s'emboîter les unes dans les autres. La paroi du cylindre 4 de la pièce intérieure 2 est plus mince que celle du cylindre 3 de la pièce extérieure 1, et cette paroi comporte des fentes axiales 12 qui s'étendent sur presque toute la longueur de la paroi et débouchent à une extrémité de celle-ci. Une fois les deux pièces extérieure 1 et intérieure 2 emboîtées l'une dans l'autre (figure 3), l'élasticité procurée par la paroi du cylindre 4 de la pièce intérieure 2 et par les fentes 12 permet de distraire la pièce intérieure 2 dans la direction F, les ailettes 11 glissant sur les
rainures 5 pour s'emboîter dans les rainures suivantes.
Ainsi, les pièces extérieure 1 et intérieure 2 forment un dispositif à longueur variable et réglable, qui permet de maintenir un écartement entre deux vertèbres. Ce dispositif est bloqué contre la compression mais est susceptible d'être distracté par l'intermédiaire du
déplacement de la pièce intérieure 2.
En alignant les nervures 8a de la pièce extérieure 1 sur des fentes 12 de la pièce intérieure 2 et en les calant, on peut complètement bloquer le dispositif dans la position désirée sans avoir recours à
des moyens de fixation complémentaires.
Concrètement, l'implantation du dispositif peut se faire de la manière suivante: on dépose le dispositif en position complètement emboîtée, à la place35 de l'évidement, entre les vertèbres les plus proches et on tire sur les pièces à l'aide de pinces jusqu'à la position désirée des vertèbres voisines. L'implantation ne comprend donc que deux opérations simples qui sont faciles à faire, avec une grande sécurité, même dans les conditions d'une intervention chirurgicale. Si nécessaire, les pièces peuvent être calées de la manière mentionnée précédemment par coopération des nervures 8a
et des fentes 12.
Le dispositif selon la présente invention permet de simplifier d'une manière extraordinaire l'opération du remplacement d'un évidement, par le fait qu'on n'a pas besoin de faire une opération particulière pour bloquer les pièces pouvant être déplacées l'une par rapport à l'autre, et que la distance, une fois réglée, ne peut plus être modifiée par un faux mouvement ou bien
pour d'autres raisons.
Bien entendu, l'exemple illustré ne sert que pour l'illustration de l'invention et n'a aucun effet limitatif sur la portée de l'invention. Il est évident, que le dispositif peut être fabriqué avec de nombreuses variantes, par exemple, en utilisant d'autres éléments de soutien, par exemple des pieds de support à la place des griffes. Les cylindres ou tubes constituant les pièces peuvent être ouverts aux deux extrémités et, dans ce cas, les surfaces ou les griffes de soutien peuvent être
appliquées comme un bouchon à l'une des extrémités des pièces.
hEVkwICATIONS 1. Dispositif de maintien d'un écartement normal entre les vertèbres, destiné au remplacement de vertèbres manquantes et comprenant deux pièces (1, 2) pouvant être déplacées axialement l'une par rapport à l'autre et pouvant être bloquées en position, caractérisé en ce que les deux pièces (1, 2) sont emboîtables l'une dans l'autre et comprennent au niveau de leurs faces mutuellement en regard des formes complémentaires ou conjuguées qui coopèrent de manière à pouvoir déplacer lesdites pièces l'une par rapport à l'autre en distraction et à éviter un déplacement en compression de
l'une par rapport à l'autre.
2. Dispositif de maintien selon la revendication 1, caractérisé en ce que les deux pièces respectivement extérieure (1) et intérieure (2) sont de forme cylindrique, en ce que l'enveloppe interne de la pièce extérieure (1) comporte des rainures (5) en dents de scie, et en ce que l'enveloppe externe de la pièce intérieure (2) comporte des ailettes (11) en saillie qui ont une forme complémentaire ou conjuguée de celle des
rainures (5).

Dispositif selon la revendication 2 ou 3, caractérisé en ce que l'enveloppe interne de la pièce extérieure (1) comporte au moins une nervure axiale (8a), et en ce que la paroi de la pièce intérieure (2) comporte au moins une fente axiale (12) dans laquelle la nervure (8a) est destinée à se positionner pour bloquer en
position les deux pièces (1, 2).
4. Dispositif de maintien selon la revendication 2 ou 3, caractérisé en ce que l'extrémité de la pièce extérieure (1) est fermée par un fond (6), dont la face externe est pourvue de griffes d'accrochage (7).
5. Dispositif de maintien selon l'une quelconque des revendications 2 à 4, caractérisé en ce
qu'une extrémité de la pièce intérieure (2) est fermée par un fond (9), dont la face externe est pourvue de
griffes d'accrochage (10).
6. Dispositif de maintien selon la revendication 2, caractérisé en ce que les deux extrémités de la pièce extérieure (1) sont ouvertes, et en ce qu'un bouchon est rapporté sur l'une des deux extrémités de la pièce extérieure (1), ce bouchon étant équipé de griffes d'accrochage (7) ou d'un pied de
support.
7. Dispositif de maintien selon la revendication 2, 3 ou 6, caractérisé en ce que les deux extrémités de la pièce intérieure (2) sont ouvertes, et en ce qu'un bouchon est rapporté sur l'une des deux extrémités de la pièce intérieure (2), ce bouchon étant équipé de griffes d'accrochage (10) ou d'un pied de support.
