Abstract

Prothèse de genou comprenant d'une part un élément méniscal (3) comportant deux cavités d'appui sphériques (31, 32), et d'autre part un implant fémoral (1) à deux condyles (10, 11) dont les faces d'appui (14, 15) sont des segments sphériques de centres (A, B) distincts et de rayon (R) égal à celui des cavités sphériques (31, 32), lesdits centres (A, B) étant, quelle que soit la taille dudit implant fémoral (1), écartés de la même distance, permettant que l'élément méniscal (3) soit, à l'exception de l'épaisseur, de dimensions constantes quelle que soit la taille de l'implant fémoral (1).

Description

La présente invention a pour objet une de prothèse de genou.
Les prothèses du genou connues à ce jour comportent toutes un implant fémoral comprenant deux condyles et une trochlée, et un implant tibial dont le plateau est recouvert d'au moins un élément méniscal, mobile ou non, sur lequel les condyles prennent un appui glissant.
En vue de pouvoir adapter une prothèse à 1'anatomie d'un patient il est nécessaire de disposer d'implants fémoraux et tibiaux de tailles différentes.
De plus, afin de respecter les tensions ligamentaires, il est indispensable de disposer d'éléments méniscaux d'épaisseurs différentes, ce qui multiplie d'autant le nombre de pièces nécessaires pour une intervention.
La présente invention a pour but de remédier à cet inconvénient en proposant une prothèse de genou qui se caractérise essentiellement en ce qu'elle comprend d'une part un élément méniscal comportant deux cavités d'appui sphériques, et d'autre part un implant fémoral à deux condyles dont les faces d'appui sont des segments sphériques de centres distincts et de rayon égal à celui des cavités sphériques, lesdits centres étant écartés, quelle que soit la taille dudit implant fémoral, de la même distance, permettant que l'élément méniscal soit, à l'exception de l'épaisseur, de dimensions constantes quelle que soit la taille de l'implant fémoral.
Une prothèse selon l'invention permet, tout en conservant une parfaite congruence entre les condyles et l'élément méniscal, de diminuer considérablement le nombre de pièces nécessaires à une intervention chirurgicale, puisque ne nécessitant que des implants fémoraux de dimensions différentes et des éléments méniscaux qui ne diffèrent qu'en épaisseur.
Les avantages et les caractéristiques de la présente invention ressortiront plus clairement de la description qui suit et qui se rapporte au dessin annexé, lequel en représente un mode de réalisation non limitatif.
Dans le dessin annexé
- la figure 1 représente une vue de face d'une de prothèse selon l'invention, sur laquelle on a représenté plusieurs implants fémoraux de dimensions différentes, en position de flexion à 90".
- la figure 2 représente une vue de profil de la même prothèse dans la même position.
Sur ces figures on peut voir qu'une prothèse selon 1'invention comporte un implant fémoral bicondylien 1, une embase tibiale 2 à quille 20, et un élément méniscal ou plateau 3.
L'implant fémoral 1 comporte deux condyles 10 et 11 séparés par une échancrure inter-condylienne 12, et prolongés par une trochlée 13.
Les faces d'appui 14 et 15 des condyles, respectivement 10 et 11, ont une certaine courbure dans le sens longitudinal et une autre dans le sens transversal, ces courbures étant de rayon constant et identique R, en sorte que les faces d'appui 14 et 15 sont des segments sphériques de centres respectivement A et B.
L'élément méniscal ou plateau 3 comporte dans sa face supérieure 30 deux cavités sphériques 31 et 32 de même rayon
R, destinées à recevoir en appui glissant les condyles, respectivement 10 et 11.
Il en résulte une parfaite congruence des condyles 10 et 11 avec l'élément méniscal 3, quel que soit l'angle de flexion.
il y a lieu de noter qu'afin de permettre un déplacement axial de l'implant fémoral par rapport à l'implant tibial, l'élément méniscal peut être mobile en glissement sur ce dernier, notamment en pivotement.
On a représenté sur les figures 1 et 2 quatre implants fémoraux 1 de tailles croissantes.
Cette différence de taille se traduit par d'une part un élargissement de la trochlée 13 et des condyles 10 et 11 par des prolongements latéraux extérieurs, respectivement 17 et 18, et d'autre part une ouverture interne 16 plus grande entre les condyles 10 et 11 et la trochlée 13, tandis que la distance qui sépare les centres A et B des segments sphériques que forment les faces d'appui 14 et 15, reste identique quelle que soit la taille de 1' implant fémoral 1, ce qui permet d'utiliser le même élément méniscal 3.
A titre d'exemple, la pose d'une prothèse actuelle nécessite pour un patient d'une taille donnée, 4 implants fémoraux de dimensions différentes, un implant tibial et des éléments méniscaux aux dimensions correspondantes et dans 5 épaisseurs différentes, soit au total 25 pièces, alors qu'une prothèse selon l'invention n'en nécessite que 10, soit 4 implants fémoraux, un implant tibial et 5 éléments méniscaux ne différant qu'en épaisseur.

Claims (2)

  1. REVENDICATIONS
    Prothèse de genou caractérisée en ce qu'elle comprend d'une part un élément méniscal (3) comportant deux cavités d'appui sphériques (31, 32), et d'autre part un implant fémoral (1) à deux condyles (10, 11) dont les faces d'appui (14, 15) sont des segments sphériques de centres (A, B) distincts et de rayon (R) égal à celui des cavités sphériques (31, 32), lesdits centres (A, B) étant, quelle que soit la taille dudit implant fémoral (1), écartés de la même distance, permettant que l'élément méniscal (3) soit, à l'exception de l'épaisseur, de dimensions constantes quelle que soit la taille de l'implant fémoral (1).
  2. 2) Prothèse selon la revendication 1 caractérisée en ce que l'élément méniscal (3) est mobile en déplacement sur l'implant tibial (2) fixé à l'os.
