Abstract

Prothèse de genou unicondylienne du type dit "à glissement". Elle comporte d'une part un condyle métallique (1) en forme de patin courbe de la face interne (10) duquel fait saillie un tenon (11) prolongée d'une tige filetée (12) sur laquelle est adaptable par vissage un manchon (3), de dimensions choisies en fonction de celles du trou d'ancrage pratiqué dans la matière osseuse, et en ce que la face interne (10) dans la région postérieure (14) comporte un aileron (15) de calage en rotation; et d'autre part une embase tibiale, destinée à être implantée avec une inclinaison postérieure de 3 deg., comportant inférieurement des plots d'ancrage, inclinés dans le sens antéro-postérieur, dont les bases sont situées dans la région antérieure de ladite embase, et de section transversale présentant globalement la forme d'un triangle à faces courbes convexes.

Description

La présente invention a pour objet une prothèse de genou unicondylienne du type dit à glissement.
Une prothèse de genou unicondylienne est destinée au re-surfaçage d'un condyle lésé, alors que l'autre condyle est sain.
Une telle prothèse ne nécessite pour sa mise en place qu'une petite résection osseuse, ce qui permet si nécessaire une intervention ultérieure sur l'autre condyle au moyen d'une prothèse semblable, ou au moyen d'une prothèse totale bicondylienne.
Les prothèses de ce type comportent toutes une pièce métallique en forme de patin courbe solidarisée au condyle et associée à un plateau en matière plastique, généralement du polyéthylène, monté sur une embase métallique fixée au tibia. La pièce métallique est généralement solidarisée au condyle au moyen d'au moins un tenon enfoncé et cimenté dans un trou d'ancrage préalablement percé axialement dans le condyle.
Pour obtenir un bon ancrage il est nécessaire que le trou soit percé dans un os de bonne qualité, or pour des patients d'un certain âge il est nécessaire d'aléser une, ou plusieurs fois ledit trou, ce qui nécessite que le chirurgien dispose de condyles munis de tenons de dimensions variées, sachant que la taille des condyles est également variable.
La présente invention a pour but de remédier à cet inconvénient en proposant une prothèse unicondylienne permettant, outre de réduire le nombre de pièces nécessaires à une intervention, d'offrir un choix plus étendu de modes de fixation.
Une prothèse unicondylienne selon l'invention se caractérise essentiellement en ce qu'elle comporte un condyle métallique en forme de patin courbe de la face interne duquel fait saillie une tige filetée sur laquelle est adaptable un manchon de dimensions choisies en fonction de celles du trou d'ancrage pratiqué dans la matière osseuse, et en ce que ladite face interne comporte un aileron de calage en rotation.
Selon une autre caractéristique de la prothèse selon l'invention les bords latéraux postérieurs de la région antérieure du condyle sont tranchants.
Selon une caractéristique additionnelle de la prothèse selon l'invention la tige filetée est vissée dans un alésage taraudé pratiqué dans le condyle.
Selon une autre caractéristique additionnelle de la prothèse selon l'invention, la face interne de la région postérieure du condyle est plane et en contre-dépouille pour former avec le manchon un moyen supplémentaire de rétention.
Selon une autre caractéristique additionnelle de la prothèse selon l'invention l'embase tibiale comporte inférieurement des plots d'ancrage, inclinés dans le sens antéro-postérieur, dont les bases sont situées dans la région antérieure de ladite embase, et dont la section transversale présente globalement la forme d'un triangle à faces courbes convexes.
Dans un premier mode de réalisation de la prothèse selon l'invention le manchon est métallique et sa fixation dans le trou d'ancrage est réalisée au moyen d'un ciment.
Dans un second mode de réalisation, préférentiel de la prothèse selon l'invention le manchon, réalisé en une matière déformable, présente extérieurement une succession d'ailettes annulaires coaxiales dont le diamètre de est supérieur à celui du trou d'ancrage pratiqué dans la matière osseuse.
Dans ce mode de réalisation, les ailettes assurent une rétention suffisante jusqu'à la repousse osseuse, laquelle se réalise entre les ailettes et consolide l'ancrage.
Le chirurgien choisit le type de fixation le mieux approprié à la qualité de l'os et au trou d'ancrage qu'il a pratiqué, sans obligation d'avoir à sa disposition un important stock de prothèses.
Les avantages et les caractéristiques de la présente invention ressortiront plus clairement de la description qui suit et qui se rapporte au dessin annexé, lequel en représente un mode de réalisation non limitatif.

Si on se réfère à la figure 1 on peut voir une prothèse unicondylienne selon l'invention, elle comprend un condyle 1 réalisé en métal et un plateau tibial 2 réalisé en matière plastique du type polyéthylène destiné à être solidarisé sur une embase tibiale métallique qui sera décrite plus loin.
Le condyle 1 a la forme d'un patin courbe et comporte à sa face postérieure 10 un tenon 11 prolongé d'une tige filetée 12 sur laquelle est vissé un manchon 3 destiné à être introduit dans un trou d'ancrage pratiqué dans la matière osseuse.
Il est à noter que dans une variante, la tige filetée 12 est solidarisée au tenon 11 par vissage, celui-ci comportant à cet effet un alésage taraudé.
Le manchon 3 est réalisé dans une matière déformable, il comporte extérieurement une succession d'ailettes annulaires 30, d'un diamètre supérieur à celui du trou d'ancrage pratiqué dans la matière osseuse, en sorte qu'après impaction, elles assurent une rétention suffisante jusqu'à la repousse osseuse.
La face interne 13 de la région postérieure 14 du condyle 1 comporte un aileron 15 destiné au calage en rotation du condyle 1.
Le calage est également assuré par les bords postérieurs 16 de la région antérieure 17 du condyle 1, qui sont tranchants afin de permettre une pénétration dans la matière osseuse.
La face interne 13 de la région postérieure 14 est plane et est en contre-dépouille, c'est-à-dire qu'elle n'est pas parallèle à l'axe du tenon 11, faisant avec celuici un angle compris entre 3 et 5 , en sorte que cela permet de réaliser un pincement de la matière osseuse, la possible déformation transversale du manchon 3, et plus particulièrement celle des ailettes 30, rendant réalisable la mise en place du condyle 1.
I1 est à noter que dans le cas d'un manchon métallique destiné à être fixé au moyen de ciment, c'est la compression de ce dernier qui permet la mise en place du condyle.
Si on se réfère maintenant à la figure 2 on peut voir que le manchon 3 comporte intérieurement un alésage taraudé 31, et que les ailettes 30 peuvent être de profiles différents.
Les différents profils des ailettes 30 sont choisis selon la qualité de l'os.
Les ailettes 301 et 302 sont de section triangulaire et peu épaisses, ce qui leur confère une certaine souplesse en extrémité, elles comportent chacune, du côté de l'introduction du manchon 3, une face inclinée, alors que l'autre face est pratiquement perpendiculaire à l'axe du manchon 3, afin respectivement de faciliter l'introduction et d'offrir une résistance à l'enlèvement.
Les ailettes 301 et 302 sont plus adaptées à une implantation dans un os d'excellente qualité, celui d'un sujet jeune par exemple.
Les ailettes 303, 304 et 305, sont massives et peu effilées, ce qui leur donne plus de rigidité, elles sont réservées à un os de moins bonne qualité qui nécessite un accrochage plus ferme.
Si on se réfère maintenant aux figures 3, 4 et 5 on peut voir une embase tibiale 4 implantable sans ciment, destinée à recevoir supérieurement le plateau tibial 2 représenté à la figure 1.
L'embase tibiale 4 comporte une face inférieure 40, présentant des aspérités 41 destinées à accroître l'accrochage, d'où font saillie trois plots d'ancrage 42, inclinés dans le sens antéro-postérieur, comportant chacun trois faces latérales 43 courbes convexes et une extrémité 44 aiguë, et dont les bases 45 sont situées dans la région antérieure du plateau, l'ensemble de ces éléments facilitant la mise en place sans ciment de l'embase 4.
Il est à noter que lors de la pose de l'embase 4, il convient de respecter une inclinaison postérieure d'environ 3 , ce qui permet avantageusement de réduire la résection osseuse et de diminuer les contraintes de cisaillement entre l'embase 4 et l'os.

Claims (8)

REVENDICATIONS
1) Prothèse de genou unicondylienne du type dit
à glissement, caractérisée en ce qu'elle comporte d'une part un condyle métallique (1) en forme de patin courbe de la face interne (10) duquel fait saillie un tenon (11) prolongée d'une tige filetée (12) sur laquelle est adaptable par vissage un manchon (3), de dimensions choisies en fonction de celles du trou d'ancrage pratiqué dans la matière osseuse, et en ce que ladite face interne (10) dans la région postérieure (14) comporte un aileron (15) de calage en rotation; et d'autre part une embase tibiale (4), destinée à être implantée avec une inclinaison postérieure de 3 , comportant inférieurement des plots d'ancrage (42), inclinés dans le sens antéro-postérieur, dont les bases (43) sont situées dans la région antérieure de ladite embase (4), et de section transversale présentant globalement la forme d'un triangle à faces courbes convexes.
2) Prothèse selon la revendication 1 caractérisée en ce que les bords latéraux postérieurs (16) de la région antérieure (17) du condyle sont tranchants.
3) Prothèse selon la revendication 1 ou la revendication 2 caractérisée en ce que la tige filetée (12) est vissée dans un alésage taraudé pratiqué dans le condyle (1).
4) Prothèse selon l'une quelconque des revendications précédentes caractérisée en ce que la face interne (13) de la région postérieure (14) du condyle est plane et en contre-dépouille pour former avec le manchon (3) un moyen supplémentaire de rétention.
5) Prothèse selon l'une quelconque des revendications précédentes caractérisée en ce que le manchon est métallique et sa fixation dans le trou d'ancrage est réalisée au moyen d'un ciment.
6) Prothèse selon l'une quelconque des revendications de 1 à 4 caractérisée en ce que le manchon (3), réalisé en une matière déformable, présente extérieurement une succession d'ailettes annulaires (30) coaxiales, le diamètre de celles-ci étant supérieur à celui du trou d'ancrage pratiqué dans la matière osseuse.
7) Prothèse selon la revendication 6 caractérisée en ce que les ailettes (301, 302) sont de section triangulaire et peu épaisses, de manière à leur conférer une certaine souplesse en extrémité, elles comportent chacune, du côté de l'introduction du manchon (3), une face inclinée alors que l'autre face est pratiquement perpendiculaire à l'axe dudit manchon (3).
8) Prothèse selon la revendication 6 caractérisée en ce que les ailettes (303, 304, 305) sont massives et peu effilées, de manière à leur conférer une certaine rigidité.
