Abstract

Cette prothèse est du type comprenant un élément fémoral fixé à l'extrémité inférieur du fémur, un élément tibial fixé à l'extrémité supérieure du tibia et un plateau intermédiaire en matériau favorisant le glissement. Selon l'invention, la face (4) de l'élément tibial (1) recevant le plateau intermédiaire (2) comprend, d'une part, un rebord périphérique (5) faisant saillie vers le haut et, d'autre part, un plot médian (6), situé du côté postérieur de l'élément tibial (1), ayant une forme triangulaire allant s'élargissant en direction du côté postérieur de la prothèse et ayant une section transversale en forme de queue d'aronde à bords arrondis, tandis que le plateau intermédiaire (2) comprend une creusure périphérique (7) et un évidement (8), correspondant respectivement au rebord périphérique (5) et au plot (6) précités, sur lesquels ils sont destinés à être emboîtés.

Description

La présente invention concerne une prothèse de genou.
Une prothèse de genou comprend, de manière classique, un élément fémoral fixé à l'extrémité inférieure du fémur, un élément tibial fixé à l'extrémité supérieure du tibia et un plateau intermédiaire en matériau favorisant le glissement, tel qu'en polyéthylène.
L'élément tibial et l'élément fémoral comprennent chacun une tige médullaire d'ancrage dans l'os et une surface d'appui.
Cette surface d'appui est plane en ce qui concerne l'élément tibial, et reproduit les condyles fémoraux en ce qui concerne l'élément fémoral.
Le plateau intermédiaire est monté sur la surface d'appui de l'élément tibial, et sa face supérieure sert de surface d'appui et de glissement aux parties de l'élément fémoral qui reproduisent les condyles osseux.
Un implant rotulien peut compléter la prothèse.
Les ligaments croisés de l'articulation du genou, notamment le ligament croisé antérieur, sont fréquemment détériorés, voire même sectionnés, et n'assurent que très imparfaitement ou même plus du tout leur rôle de limitation du mouvement des os de l'articulation.
Les contraintes répétées que subit la liaison élément tibial-plateau intermédiaire sont, dans ce cas, importantes et peuvent conduire à une usure génératrice d'un jeu indésirable entre ces deux pièces.
Par ailleurs, un plateau intermédiaire doit rester relativement facile et rapide à interchanger, afin de permettre au praticien de déterminer le plateau dont l'épaisseur convient le mieux à l'articulation traitée, en fonction de l'état des ligaments et de l'importance des résections osseuses opérées.
Dans les prothèses existantes, la solidité de la liaison élément tibial-plateau intermédiaire est souvent obtenue au détriment de la facilité d'interchangement du plateau intermédiaire, et inversement.
La présente invention vise à remédier à cet inconvénient, en fournissant une prothèse assurant un parfait maintien du plateau sur l'élément tibial sans pour autant affecter l'interchangeabilité facile et rapide de ce plateau intermédiaire.
A cette fin, la face de l'élément tibial recevant le plateau intermédiaire comprend, d'une part, un rebord périphérique faisant saillie vers le haut et, d'autre part, un plot médian, situé du côté postérieur de l'élément tibial, ayant une forme triangulaire allant s'élargissant en direction du côté postérieur de la prothèse et ayant une section transversale en forme de queue d'aronde à bords arrondis, tandis que le plateau intermédiaire comprend une creusure périphérique et un évidement, correspondant respectivement au rebord périphérique et au plot précités, sur lesquels ils sont destinés à être emboîtés.
Le plateau intermédiaire est destiné à être engagé sur l'élément tibial par coulissement dans le sens antéro-postérieur. Le plot, grâce à sa forme triangulaire, permet de centrer le plateau par rapport à l'élément tibial et de le guider vers sa position de montage. Au moment où le bord antérieur du plateau intermédiaire arrive à l'aplomb du bord antérieur de l'élément tibial, le plateau intermédiaire est pressé vers le bas de manière à ce qu'il vienne s'encliqueter sur le plot pour amener ladite creusure en position d'emboîtement sur ledit rebord périphérique.
Dans cette position de montage, le plateau intermédiaire est parfaitement maintenu sur l'élément tibial, dans toutes les directions, et ce notamment lorsque s'exercent sur lui des contraintes importantes en pivotement. La déformabilité limitée du matériau constituant le plateau élimine tout risque de soulèvement de l'avant de ce dernier et d'échappement par l'avant.
Le bord antérieur du plateau et/ou de l'élément tibial comprend avantageusement un ou plusieurs évidements pour l'engagement de l'extrémité d'un instrument, cet instrument étant destiné à permettre de soulever l'avant du plateau. Par ce soulèvement, le plateau peut être libéré d'autour du plot, et donc être retiré de l'élément tibial.
Ainsi, grâce à l'invention, un parfait maintien en position du plateau est obtenu sans que l'interchangeabilité facile et rapide du plateau ne soit affectée par les moyens de maintien de ce plateau sur l'élément tibial.
L'invention vise également à procurer un élément tibial modulable en fonction des besoins.
A cet effet, le plateau tibial comprend un bossage médian taraudé, faisant saillie de sa face inférieure, auquel peuvent être fixées par vissage des quilles d'ancrage interchangeables, de forme ou de longueur différentes, et notamment des quilles pourvues d'ailettes de stabilisation venant prendre appui dans l'os ou dans le ciment de scellement.
Pour son parfait maintien en pivotement par rapport à l'os, l'élément tibial comprend avantageusement deux plots latéraux d'ancrage, faisant saillie de sa face inférieure, pouvant être engagés dans des cavités aménagées dans l'os pour les recevoir, et être fixés à l'os par cimentation ou par vis d'ancrage engagées au travers d'eux.
En outre, l'élément tibial peut comprendre deux trous antérieurs pouvant recevoir des vis d'ancrage supplémentaires, si besoin est.
Pour sa bonne compréhension, l'invention va être à nouveau décrite ci-après en référence au dessin schématique annexé représentant, à titre d'exemple non limitatif, une forme de réalisation préférée de la prothèse de genou qu'elle concerne.

III-III de la figure 2.
Les figures représentent l'élément tibial 1 et le plateau intermédiaire 2 d'une prothèse totale de genou.
Cette prothèse comprend, de manière classique, un élément fémoral (non représenté) fixé à l'extrémité inférieure du fémur, l'élément tibial 1, qui est destiné à être fixé à l'extrémité supérieure du tibia, et le plateau intermédiaire 2 qui est en matériau favorisant le glissement, notamment en polyéthylène.
L'élément tibial 1 comprend, comme l'élément fémoral, une tige médullaire 3 d'ancrage dans l'os et une surface d'appui 4. Cette surface d'appui 4 est plane en ce qui concerne l'élément tibial, et reproduit les condyles fémoraux en ce qui concerne l'élément fémoral.
Le plateau intermédiaire 2 est monté sur la surface d'appui 4 de l'élément tibial, et sa face supérieure sert de surface d'appui et de glissement aux parties de l'élément fémoral qui reproduisent les condyles osseux.
Ainsi que le montrent les figures
- la face 4 de l'élément tibial 1 recevant le plateau intermédiaire 2 comprend, d'une part, un rebord périphérique 5 faisant saillie vers le haut et, d'autre part, un plot médian 6, situé sur le côté postérieur de l'élément tibial 1, ce plot 6 ayant une forme triangulaire allant s'élargissant en direction du côté postérieur de la prothèse et ayant une section transversale en forme de queue d'aronde à bords arrondis et
- le plateau intermédiaire 2 comprend une creusure 7 périphérique et un évidement 8, correspondant respectivement au rebord périphérique 5 et au plot 6 précités, sur lesquels ils sont destinés à être emboîtés.
De plus, les bords antérieurs du plateau 2 et de l'élément tibial 1 comprennent des évidements antérieurs 10 permettant l'engagement de l'extrémité d'un instrument (non représenté), pouvant prendre appui sur le plateau 2 d'une part et sur l'élément tibial 1 d'autre part.
En outre, le plateau tibial 12 de l'élément tibial 1 comprend un bossage médian taraudé 13, faisant saillie de sa face inférieure, auquel peuvent être fixées par vissage des quilles d'ancrage interchangeables 14,15, de forme ou de longueur différentes.
La quille 14 représentée à la figure 1 est destinée à équiper un élément tibial 1 devant être fixé à l'os au moyen d'un ciment de scellement. Elle présente une forme longiligne et comprend plusieurs méplats 16 lui permettant de prendre appui dans le ciment, ainsi qu'un téton fileté 17 lui permettant d'être vissée dans l'alésage taraudé du bossage 13.
La quille 15 représentée aux figures 2 et 3 est destinée à équiper un élément tibial 1 devant être fixé à l'os par repousse osseuse. Elle présente trois ailettes 18a,18b en saillie, à savoir une ailette antérieure 18a, située sensiblement dans le plan médian de l'articulation, et deux ailettes latérales 18b, légèrement orientées vers le côté postérieur de la prothèse. Cette quille 15 est percée d'un trou qui est lamé à son extrémité supérieure, ce lamage permettant l'engagement de la quille 15 sur le plot 13, et ce trou permettant l'engagement d'une vis 19 de fixation, pouvant être vissée dans l'alésage taraudé du plot 13.
Le plateau tibial 12 comprend également deux plots latéraux d'ancrage 20, faisant saillie de sa face inférieure, pouvant être engagés dans des cavités aménagées dans l'os pour les recevoir et être fixés à l'os par des vis d'ancrage engagées au travers d'eux, ainsi que deux trous antérieurs 21, pouvant recevoir des vis d'ancrage supplémentaires, si besoin est.
Le plateau intermédiaire 2 est destiné à être engagé sur l'élément tibial 1 par coulissement dans le sens antéro-postérieur. Le plot 6, grâce à sa forme triangulaire, permet de centrer le plateau 2 par rapport à l'élément tibial 1 et de le guider vers sa position de montage. Au moment où le bord antérieur du plateau intermédiaire 2 arrive à l'aplomb du bord antérieur de l'élément tibial 1, le plateau intermédiaire 2 est pressé vers le bas de manière à ce qu'il vienne s'encliqueter sur le plot 6 pour amener ladite creusure 7 en position d'emboîtement sur ledit rebord périphérique 5.
Dans cette position de montage, le plateau intermédiaire 2 est parfaitement maintenu sur l'élément tibial 1, dans toutes les directions, et ce notamment lorsque s'exercent sur lui des contraintes importantes en pivotement. La déformabilité limitée du matériau constituant le plateau 2 élimine tout risque de soulèvement de l'avant de ce dernier et d'échappement par l'avant.
L'instrument précité dont l'extrémité peut être engagée dans les évidements 10 est destiné à permettre de soulever l'avant du plateau 2, en venant prendre appui, d'une part, contre l'élément 1 et, d'autre part, contre le plateau 2, et en pivotant à la manière d'un levier. Par ce soulèvement, le plateau 2 peut être libéré d'autour du plot 6, et donc être retiré de l'élément tibial 1.
Les quilles 14,15 interchangeables permettent de modifier l'élément tibial 1 en fonction des besoins, et notamment en fonction de l'état des os à traiter, et de l'implantation de la prothèse avec cimentation ou avec repousse osseuse.
Ainsi, grâce à l'invention, un parfait maintien du plateau 2 sur l'élément tibial 1 est assuré, sans pour autant affecter l'interchangeabilité facile et rapide du plateau 2, et l'élément tibial 1 est modulable.

Claims (5)

REVENDICATIONS
1 - Prothèse de genou, du type comprenant un élément fémoral fixé à l'extrémité inférieur du fémur, un élément tibial fixé à l'extrémité supérieure du tibia et un plateau intermédiaire en matériau favorisant le glissement, caractérisée en ce que la face (4) de l'élément tibial (1) recevant le plateau intermédiaire (2) comprend, d'une part, un rebord périphérique (5) faisant saillie vers le haut et, d'autre part, un plot médian (6), situé du côté postérieur de l'élément tibial (1), ayant une forme triangulaire allant s'élargissant en direction du côté postérieur de la prothèse et ayant une section transversale en forme de queue d'aronde à bords arrondis, tandis que le plateau intermédiaire (2) comprend une creusure périphérique (7) et un évidement (8), correspondant respectivement au rebord périphérique (5) et au plot (6) précités, sur lesquels ils sont destinés à être emboîtés.
2 - Prothèse selon la revendication 1, caractérisée en ce que le bord antérieur du plateau (2) et/ou de l'élément tibial (1) comprend un ou plusieurs évidements (10) pour l'engagement de l'extrémité d'un instrument, cet instrument étant destiné à permettre de soulever l'avant du plateau (2).
3 - Prothèse selon la revendication 1 ou la revendication 2, caractérisée en ce que le plateau tibial (12) comprend un bossage médian taraudé (13), faisant saillie de sa face inférieure, auquel peuvent être fixées par vissage des quilles d'ancrage interchangeables (14,15), de forme ou de longueur différentes, et notamment des quilles (15) pourvues d'ailettes de stabilisation (18a,18b) venant prendre appui dans l'os ou dans le ciment de scellement.
4 - Prothèse selon l'une des revendications 1 à 3, caractérisée en ce que l'élément tibial (1) comprend deux plots latéraux d'ancrage (20), faisant saillie de sa face inférieure, pouvant être engagés dans des cavités aménagées dans l'os pour les recevoir, et être fixés à l'os par cimentation ou par vis d'ancrage engagées au travers d'eux.
5 - Prothèse selon l'une des revendications 1 à 4, caractérisée en ce que l'élément tibial (1) comprend deux trous antérieurs (21) pouvant recevoir des vis d'ancrage supplémentaires, si besoin est.
