Abstract

La présente invention a pour objet une tige fémorale de prothèse de hanche aisément extractible. Il est constitué par la combinaison d'une tige fémorale (1) scellée, formée d'une partie distale (2) conique, d'une partie proximale (3) fortement évasée pourvue de rainures (7) longitudinales parallèles augmentant la surface de scellement et d'un col (4) portant un emmanchement tronconique (5) recevant la tête d'articulation, ladite partie proximale comportant un perçage (8) borgne fileté ouvert vers le haut, dont l'axe (9) est voisin de celui de la prothèse, et destiné à la fixation d'un extracteur, le col comportant en outre, sous l'emmanchement tronconique, du côté interne, un trou de désimpaction (10). Elle concerne le domaine de la chirurgie orthopédique osseuse de la hanche.

Description

TIGE FÉMORALE DE PROTHESE DE HANCHE
AISÉMENT EXTRACTIBLE
La présente invention a pour objet une tige fémorale de prothèse de hanche aisément extractible.
Elle concerne le domaine de la chirurgie orthopédique osseuse de la hanche.
Dans de très nombreux cas, la tige fémorale d'une prothèse de la hanche doit être remplacée à une ou plusieurs reprises chez un patient. Cette tige est soit scellée dans le fémur au moyen d'un ciment chirurgical, soit enfoncée à force.
Le chirurgien est donc souvent amené à retirer une ancienne prothèse avant de pouvoir en mettre en place une nouvelle. Cette opération présente fréquemment de grandes difficultés dues au fait que la seule prise utilisable est le tronc de cône portant la sphère d'articulation. Or ce tronc de cône est fortement déporté par rapport à l'axe du fémur, et les violentes tractions qu'il faut exercer pour extraire la tige fémorale à remplacer entraînent un risque important de provoquer des liaisons de l'os généralement fragilisé par la ou les opérations antérieures, sans compter le traumatisme initial ayant nécessité ces interventions.
La conception d'une tige fémorale nécessite de touver un compromis satisfaisant entre des exigences contradictoires. En effet, en raison des efforts considérables auxquels elle est soumise, la prothèse doit impérativement être fixée très rigidement, mais elle doit également pouvoir être retirée sans causer de dommages à l'os qui la reçoit. Cette seconde considération milite en faveur d'une fixation sans ciment, mais cette technique est souvent la cause de douleurs post-opératoires dues à des contraintes permanentes ponctuelles exercées sur l'os en raison de l'impossibilité de fabriquer une tige fémorale épousant exactement l'intérieur du fémur.
Le dispositif selon la présente invention se propose d'apporter un remède à ce problème en pemettant une extraction plus facile de la prothèse fémorale tout en diminuant fortement les risques de dégradation de la matière osseuse.
Il est constitué par la combinaison d'une tige fémorale scellée, formée d'une partie distale conique, d'une partie proximale fortement évasée pourvue de rainures longitudinales parallèles augmentant la surface de scellement et d'un col portant un emmanchement tronconique recevant la tête d'articulation, ladite partie proximale comportant un perçage borgne fileté ouvert vers le haut, dont l'axe est voisin de celui de la prothèse, et destiné à la fixation d'un extracteur, le col comportant en outre, sous l'emmanchement tronconique, du côté interne, un trou de désimpaction.
Sur les dessins annexés, donnés à titre d'exemple non limitatif d'une des formes de réalisation de l'objet de l'invention:
la figure 1 est une vue de face de la tige fémorale de la prothèse,
la figure 2 est une vue latérale externe, suivant la flèche A de la figure 1,
et la figure 3 est une coupe transversale de la partie proximale, suivant les flèches B-B de la figure 1.
Le dispositif, figures 1 à 3, est constitué d'une tige 1 s'insérant dans le fémur et comportant une partie distale 2 sensiblement conique et une partie proximale 3 surmontée d'un col 4 portant un élément tronconique 5 sur lequel se fixe la sphère d'articulation, et dont l'axe fait un angle de 45 degrés avec celui de la partie basse conique 2.
La partie proximale est de section rectangulaire et forme un trapèze inversé avec côté interne 6 concave se prolongeant jusqu'au col 4 afin de remplir au mieux la partie supérieure évasée du fémur.
Elle comporte sur ses grandes faces antérieure et postérieure des rainures 7 longitudinales parallèles, et sa surface supérieure comporte un perçage 8 borgne fileté ouvert vers le haut, dont l'axe est voisin de l'axe 9 de la partie distale 2 et parallèle à ce dernier. Cette disposition permet la fixation d'un extracteur qui pourra agir dans la direction dudit axe 9 et permettra de retirer la prothèse sans exercer d'efforts latéraux sur le fémur, cette opération étant facilitée par le fait que les rainures 7 sont également parallèles à l'axe de la partie distale, ainsi que par la présence d'un trou de désimpaction 10 situé sur la face interne du col 4, sous l'élément tronconique 5.
La partie distale 2 comporte, dans le plan vertical antéro-postérieur une extrémité inférieure 11 d'environ 4 centimètres de longueur avec un angle plus ouvert, compris entre 15 et 200 pour éviter de toucher les corticales externes du fût fémoral.
La tige fémorale 1 est exécutée en huit tailles à progression homothétique qui, grâce à la conception générale de l'invention, permettent de couvrir tous les besoins.
Le positionnement des divers éléments constitutifs donne à l'objet de l'invention un maximum d'effets utiles qui n'avaient pas été, à ce jour, obtenus par des dispositifs similaires.

REVENDICATIONS
10. Tige fémorale de prothèse de hanche aisément extractible, destinée au domaine de la chirurgie orthopédique osseuse de la hanche,
caractérisée par la combinaison d'une tige fémorale (1) scellée, formée d'une partie distale (2) sensiblement cônique, d'une partie proximale (3) de section rectangulaire fortement évasée pourvue, sur ses grandes faces antérieure et postérieure, de rainures (7) longitudinales parallèles augmentant la surface de scellement et d'un col (4) portant un élément tronconique (5) recevant la tête d'articulation, ladite partie proximale comportant un perçage (8) borgne fileté ouvert vers le haut, dont l'axe est voisin de l'axe (9) de la partie distale (2) et destiné à la fixation d'un extracteur, le col (4) comportant, sous l'élément tronconique (5), du côté interne, un trou de désimpaction (10).
20. Tige fémorale selon la revendication 1, se caractérisant par le fait que les rainures (7) sont parallèles à l'axe (9) de la partie distale (2).
30. Tige fémorale selon l'une quelconque des revendications précédentes, se caractérisant par le fait que la partie distale (2) comporte, dans le plan vertical antéro-postérieur une extrémité inférieure (11) d'environ 4 centimètres de longueur avec un angle plus ouvert, compris entre 15 et 200.
40. Tige fémorale selon l'une quelconque des revendications précédentes, se caractérisant par le fait que la tige fémorale (1) est exécutée en huit tailles à progression homothétique.
