Abstract

Active compsn. for the treatment of psoriasis, partic. of the scalp, comprises: (a) an antiseptic and/or (b) an antiseborrhoeic fraction consisting of at least one tar arising from distn. of organic matter (living or fossil, vegetable or animal); (c) an agent active against pityriasis comprising at least one 1-hydroxy-2-pyridone, pref. pitoctone olamine; and (d) a keratolytic agent, pref. salicylic acid. USE/ADVANTAGE - The compsn. is useful for the treatment of psoriasis, partic. of the scalp, by topical application, or as a shampoo. Topical treatments of prior art are difficult to apply to the scalp because of the hair, require care in use because of possible side-effects and may have an unpleasant odour as a result of the tar used. The claimed compsn. does not have these disadvantages and does not require medical supervision in use. The use of the agent (c) allows a large redn. in the tar-based antiseborrhoeic cpd. Use of a 1-hydroxy-2-pyridone allows use of a reduced amount of an optional selenium cpd., which reduces toxicity problems.

Active composition for the treatment of psoriasis,
including scalp psoriasis
The present invention relates to the treatment of psoriasis, and more particularly, although not exclusively, psoriasis of the scalp. To this end, the invention relates to active compositions with respect to psoriasis, whether these are considered as medicaments for external use, or as dermo-cosmetic preparations.

 Psoriasis is a frequent dermatosis, of unknown etiology, with chronic evolution, characterized by more or less extensive, well circumscribed red spots, covered with dry, profuse and brittle scales. The corresponding lesions are located mainly on the elbows, knees, scalp, but can also invade the whole body.

 Regarding scalp psoriasis, it is estimated that 0.5 to 1% of the population is affected by this condition, which is particularly unpleasant and very difficult to treat due to the presence of hair.

 Very often, scalp psoriasis causes thick scales and erythematous plaques.

Like any other psoriasis, that of the scalp is defined histologically by hyperkeratosis, locally parakeratotic, regular acanthosis with lengthening of the dermal papillae. PAS positive inclusions are visible in the epidermis of the scalp, and micro-abscesses are also found in the stratum corneum, as well as a spongiform aspect of the latter, possibly with exocytosis of neutrophilic polynuclear cells.

 Psoriasis of the scalp should be distinguished, as for its diagnosis, from the various forms of pityriasis, or pelicular states of the scalp, and from seborrheic dermatitis.

 Different local treatments have already been proposed for psoriasis of the scalp, but are difficult to apply because of the existence or presence of the hair. In particular, various topical preparations are available, and most of them include salicylic acid combined with different reducing agents, such as coal tar, anthralin, and / or contain different dermo-corticoids.

 All of these products are delicate to use, because many side effects can appear, among which one can note atrophy of the skin, stretch marks, delayed healing of wounds, different phenomena of skin dessication, folliculitis, acneiform eruptions. , or even a depigmentation of the scalp.

 The object of the present invention is to remedy the aforementioned drawbacks.

 More specifically, the subject of the invention is an active composition for the treatment of psoriasis and preferably psoriasis of the scalp, for dermo-cosmetic or pharmaceutical use, without any particular side effects and therefore not requiring medical control during its use.

 First of all, the presence of the active agent in the isolated state against pityriasis makes it possible to reduce to a large extent the proportion of the heavy antiseborrheic fraction, based on tar, and possibly the weight proportion of the selenium compound. This makes it possible to limit the drawbacks linked to the possible toxicity of these products.

 Then the composition according to the invention avoids any drawback linked to the unpleasant and permanent odor of the heavy tar-based fraction, often inconvenient for the patient and his entourage.

 Finally, various clinical trials carried out according to the invention have shown an improvement in the condition of the patients, there in particular where traditional products did not bring any substantial result.

 From an active composition according to the invention, any drug suitable for external use can be formulated, with an excipient suitable for topical application, and optionally an additive promoting the systemic penetration of the active principle, in this case the identified composition. like before.

 With regard to the treatment of scalp psoriasis, the medicament will be presented and formulated in the form of a shampoo, the excipient then comprising at least one surfactant.

 And from the same active composition, different dermo-cosmetic preparations can be formulated, favoring the treatment of psoriasis, in particular scalp psoriasis, comprising as active material a composition as defined above, as well as a suitable vehicle to a topical application. Such preparations may be creams, ointments, lotions, ointments, gels, and other alcoholic solutions, customary in the field of cosmetics.

 With regard to scalp psoriasis, of course the preferred form for the aforementioned preparation will be a shampoo, the vehicle of which will comprise at least one surfactant and one viscosity agent.

 A dermo-cosmetic shampoo, comprising an active composition, as defined above, was the subject of a multicenter study, in hospital consultation, with respect to scalp psoriasis, from 16 subjects of 18 to 64 years old, in four hospitals.

 All subjects initially presented with erythema, dander, infiltration, and most with pruritus in the scalp.

 The treatment lasted eight weeks, and the shampoo according to the invention was used by each patient every other day, for the first fifteen days, then every other day for the remainder of the treatment.

 The effectiveness of such a shampoo has proven to be important and rapid, since there is, on the one hand, a significant improvement with respect to the four diagnostic signs of psoriasis, in 80% of the cases, and complete whitening. , in 40% of cases.

 The tolerance of such a shampoo was perfect in 100% of the cases, no adverse reaction having been observed.

The qualities of such a shampoo have been particularly appreciated by the users, since it has been found
- no persistence of shampoo odor on the hair, in 98% of cases,
- an absence of reaction hyperseborrhea, in 50% of cases,
- as well as a styling and volumizing effect in 71% of the cases.
