Abstract

L'invention a pour objet une prothèse de hanche. La prothèse de hanche est du type comportant une coupole métallique (1), qui est susceptible d'être fixée par des vis (25) dans le bassin d'un patient, laquelle coupole (1) reçoit un noyau (4) en matière plastique, dont une partie au moins de la face externe épouse une partie au moins de la face interne de ladite coupole (1), caractérisée par le fait que ledit noyau (4) comporte une cavité, qui peut recevoir la tête d'une tige qui peut être fixée au fémur dudit patient. L'invention s'applique aux prothèses de hanche.

Description

L'invention a pour objet une prothèse de hanche.
Les procédés et dispositifs de prothèse de hanche ont fait l'objet de nombreuses recherches et sont décrits dans de nombreux documents.
Le brevet FR. 2.406.433 décrit un dispositif de prothèse de hanche et son procédé d'implantation sans ciment, contrairement aux prothèses connues avant ce document ; selon ce document, la prothèse comporte une coquille métallique acétabulaire, prévue pour être scellée dans une cavité de la hanche, la coquille comportant, sur sa face extérieure, une surface poreuse et irrégulière pour la fixation de la coquille par pénétration intime de l'os dans cette surface, laquelle prothèse comporte également des boulons permettant de fixer la coquille pendant la période de pénétration de l'os dans la surface poreuse. La prothèse comporte également une garniture, fixée sur la coquille, qui reçoit la tête du fémur et une tête fémorale métallique munie d'une cavité ayant une surface poreuse ou irrégulière également, laquelle tête fémorale métallique peut être fixée au fémur par une tige.
Ce procédé de fixation décrit dans ce brevet est maintenant bien connu sous le terme d'hydroxylapatite.
Le brevet FR. 2.578.162 décrit une cupule pour prothèse de hanche, qui comporte des pattes radiales, qui sont liées de manière amovible et réglables en position angulaire au bord périphérique de la cupule ; selon ce document, la cupule comporte une gorge présentant une section transversale en contre dépouille et l'extrémité des pattes liées â la cupule comportent une section de profil complémentaire à celui de la gorge.
Le brevet FR. 2.626.168 décrit une prothèse qui comporte une tige de longueur appropriée, grâce à un collet interchangeable, qui s'insère entre la tige et le cotyle, qui peut être cimenté ou ancré dans l'acétabule.
Selon ce document, le cotyle comporte une partie en forme de demi-sphère en polyéthylène, logée dans une coquille de forme identique en titane, laquelle coquille comporte des ergots le long de plans parallèles à la base de la coquille.
La demande de brevet EP 353.171 décrit une prothèse qui comporte une cupule de soutien, destinée à être insérée dans l'os de l'articulation, qui comporte sur sa face extérieure, une cheville expansible et une pluralité de fentes d'expansion, dont une au moins est prolongée jusqu'au pôle de la cupule, et qui comporte à la base de sa face interne, un filetage qui permet la fixation de la cupule de frottement.
Les prothèses de hanche connues présentent encore de nombreux inconvénients.
En effet, l'ancrage initial de la coupole métallique dans la cavité osseuse n' est pas facile à réaliser par des vis qui plaquent ladite coquille dans ladite cavité ; par ailleurs, l'orientation des vis qui pénètrent dans l'os ne peut pas être maintenue strictement coaxiale et perpendiculaire avec la portée de vis prévue dans cette coupole ; par ailleurs, ces vis ne suffisent pas toujours à assurer une bonne fixation initiale, et la présence d'ergots le long de plans parallèles à la base de la coupole tel que décrit dans FR. 2.626.168 ne facilite pas cette fixation initiale.
De plus, le problème posé au chirurgien lors d'une opération de prothèse de hanche est l'impossibilité de pouvoir contrôler le bon contact de la face extérieure de la coquille avec la cavité osseuse.
Un autre problème important qui n'est pas résolu est le blocage du noyau en matière plastique à l'intérieur de la coquille, notamment le blocage en rotation, ainsi que le blocage empêchant le noyau de ressortir de la coquille après y avoir été introduit.
Enfin, l'inconvénient majeur des prothèses de hanche actuelles est celui des luxations : en effet, la tête (généralement sphérique) de la tige qui est fixée sur le fémur est simplement engagée dans la cavité ménagée dans le noyau en matière plastique ; cela fait qu'en cas de mouvement mal contrôlé du patient, la tête de la prothèse peut sortir de la cavité, ce qui entraîne une très forte douleur pour le patient.
L'objectif de la présente invention est de remédier à ces inconvénients en utilisant une prothèse de hanche avec un col fixe.
La prothèse de hanche est du type comportant une coupole métallique, qui est susceptible d'être fixée par des vis dans le bassin d'un patient, laquelle coupole reçoit un noyau en matière plastique, dont une partie au moins de la face externe épouse une partie au moins de la face interne de ladite coupole, ledit noyau comportant une cavité, qui peut recevoir la tete d'une tige qui peut être fixee au fémur dudit patient.
Ladite coupole comporte une partie en forme de calotte hémisphérique limitée par deux plans parallèles P2, P4, de rayon intérieur R1 et de centre C1, qui est prolongée par une partie sensiblement tubulaire, dont l'axe est perpendiculaire auxdits plans, et qui est limitée par l'un desdits plans et par un plan
P1, parallèle auxdits plans et qui contient ledit centre de ladite calotte hémisphérique et ladite face extérieure dudit noyau comporte une partie sensiblement hémisphérique de centre C2 et de rayon R2, sensiblement égal audit rayon Ri, laquelle partie hémisphérique dudit noyau est limitée par deux plans parallèles
P6, P7, et est prolongée par une partie cylindrique, dont le plus grand diamètre extérieur est sensiblement inférieur au diamètre intérieur de ladite partie tubulaire de ladite coupole, lequel diamètre intérieur a une valeur sensiblement inférieure au double dudit rayon R1 et ladite coupole comporte des moyens pour empêcher les mouvements relatifs dudit noyau par rapport à ladite coupole.
Lesdits moyens pour empêcher les mouvements relatifs dudit noyau par rapport à ladite coupole comportent au moins deux languettes, qui s'étendent selon une ligne sensiblement parallèle auxdits plans P1, P2, lesquelles languettes sont fixées par une première extrémité à ladite partie tubulaire de ladite coupole, et dont la deuxième extrémité est libre, lesdites languettes étant légèrement proéminentes à l'intérieur de ladite partie tubulaire de ladite coupole, de sorte qu'elles permettent l'introduction dudit noyau à l'intérieur de ladite coupole, moyennant une légère déformation locale de ladite partie cylindrique dudit noyau en regard desdites extrémités libres desdites languettes, et de sorte que lesdites languettes empêchent ensuite la rotation relative dudit noyau par rapport à ladite coupole ; l'épaisseur de ladite partie tubulaire de ladite coupole peut avantageusement être inférieure à 1 'épaisseur de ladite partie hémisphérique de ladite coupole.
Ladite partie cylindrique dudit noyau comporte au moins une première partie cylindrique, qui prolonge ladite partie hémisphérique, et dont le diamètre extérieur est égal audit diamètre extérieur de ladite partie cylindrique dudit noyau, laquelle première partie cylindrique est prolongée par une deuxième partie cylindrique coaxiale avec ladite première partie cylindrique, laquelle deuxième partie cylindrique a un diamètre extérieur, sensiblement inférieur audit diamètre extérieur de ladite partie cylindrique dudit noyau, laquelle deuxième partie cylindrique est prolongée par au moins une troisième partie cylindrique, coaxiale avec ladite deuxième partie cylindrique, et dont le diamètre extérieur est sensiblement supérieur audit diamètre intérieur de ladite partie tubulaire de ladite coupole, de sorte que ladite deuxième partie cylindrique, qui est située entre lesdites première et troisième parties cylindriques, constitue une gorge dans laquelle lesdites languettes viennent se loger et empêchent ainsi que ledit noyau puisse sortir de ladite coupole.
Prothèse de hanche du type comportant une coupole métallique, qui est susceptible d'être fixée par des vis dans le bassin d'un patient, laquelle coupole reçoit un noyau en matière plastique, dont une partie au moins de la face externe épouse une partie au moins de la face interne de ladite coupole ; ledit noyau comporte une cavité , qui peut recevoir la tête d'une tige qui peut être fixée au fémur dudit patient ; ladite tige est pourvue d'un cône fixe, dit cône "morse" qui reçoit les têtes prothétiques qui entrent dans le noyau , l'angle cervico-diaphysaire augmente selon la taille de la tige.
Prothèse de hanche dont l'angle entre l'axe longitudinal du corps et l'axe longitudinal du cône ou du col fixe est compris entre 135 et 1420.
Une variation dudit angle est, bien sur, possible par l'utilisation des cônes de différentes tailles ou cols, cette variation pouvant aller de 1300 à 145 selon le cône utilisé.
L'épaisseur de la coupôle métallique est supérieure à l'épaisseur des têtes des vis et ce, de manière à éviter tout frottement avec le noyau en matière plastique.
Sur la face extérieure de la partie hémisphérique de la coupôle métallique sont prévues des protubérances qui ont une forme de dent et dont la forme en coupe forme un arc de cercle.
Les dessins ci-joints sont donnés à titre d'exemples indicatifs et non limitatifs. Ils représentent un mode de réalisation préféré selon l'invention. Ils permettront de comprendre aisément l'invention.

La coupole métallique 1 selon l'invention est réalisée dans un alliage de titane par exemple ; ladite coupole 1 comporte une partie en forme de calotte hémisphérique 2, qui est limitée par deux plans P2 et P4 sensiblement parallèles.
Ladite calotte hémisphérique 2 a un rayon intérieur R1 et un centre C1. Ladite calotte hémisphérique 2 est prolongée par une partie sensiblement tubulaire 3, dont l'axe longitudinal est perpendiculaire auxdits plans P2, P4, et ladite partie sensiblement tubulaire 3 est limitée par ledit plan P2 et par un plan P1 parallèle audit plan P2, lequel plan P1 contient ledit centre C1 de ladite calotte hémisphérique 2.
Ladite coupole métallique 1 comporte des moyens pour empêcher les mouvements relatifs du noyau 4 par rapport à ladite coupole 2, qui consistent en quatre languettes 5 qui s'étendent selon une ligne sensiblement parallèle auxdits plans P1 et P2 ; lesdites languettes 4 sont fixées à ladite partie tubulaire 3 de ladite coupole métallique 1 ; l'épaisseur de ladite partie tubulaire 3 de ladite calotte 2 est sensiblement inférieure à l'épaisseur de ladite partie hémisphérique de ladite calotte 2. Les languettes 5 comportent des fenêtres 6 avec une ouverture 7 du type baïonnette.
Ladite partie 2 en forme de calotte hémisphérique de ladite coupole est traversée par au moins un orifice 8. D'autres orifices 9, 10, 11 sont prévus pour recevoir les vis permettant la fixation de la coupole 2 dans la cavité osseuse du bassin du patient.
Sur la face extérieure 12 de la partie hémisphérique de ladite coupole, des protubérances 13 sont prévues, qui ont une forme de dent. Les protubérances 13 améliorent la fixation initiale de la coupole 1 sur le bassin dudit patient pour permettre au chirurgien un vissage aisé.
Ladite coupole 1 comporte également, dans sa partie hémisphérique, un alésage 14 ayant pour axe ledit axe zzl et ladite coupole comporte une fenêtre 15 qui débouche dans ledit alésage 14 et qui s'étend sensiblement selon un rayon de ladite calotte hémisphérique.
Ledit alésage 14 est principalement destiné à permettre l'ancrage et le positionnement initial de ladite coupole métallique 1 dans ladite cavité osseuse.
Sur la figure 2, qui est une vue en plan de la coupole 1 métallique, on voit que ladite coupole métallique comprend six orifices 8, 9, 10, 11 et cinq protubérances 13 en forme de dents, qui sont situées à proximité desdits orifices 8, 9, 10, 11.
On voit également que ladite fenêtre 15 débouche dans ledit alésage 14 et que ladite fenêtre 15 a une largeur qui permet un contrôle visuel de la cavité osseuse du bassin en cours d'opération.
Sur les figures 3 et 4, est représenté le noyau en matière plastique 4 selon l'invention. La face arrière dudit noyau 4 comporte une face plane 16, une face 17, une face 18 et une face 19. Ladite face 16 s'étend angulairement dans le plan de la figure selon un angle voisin de 180 .
Lesdites faces 17 et 18 délimitant un promontoire 19 s'étendent angulairement selon un angle voisin de 180 . Un chanfrein 20 et prévu pour raccorder ladite surface 16 à la face intérieure 21 dudit noyau 4 en matière plastique délimitant ladite cavité 22 qui peut recevoir la tête d'une tige qui peut être fixée au fémur du patient ; ladite tige est pourvue d'un cône fixe, dit cône "morse", qui reçoit les têtes prophétiques et entre dans le noyau en plastique.
Des trous taraudés 23 sont prévus dans ladite partie de la face arrière plane 16 dudit noyau 4 ; lesdits trous taraudés 23 sont en effet utiles au chirurgien pour positionner angulairement avec précision ledit noyau dans ladite coupole métallique, après que ledit noyau y ait été introduit.
Lors de l'implantation de ladite prothèse dans l'os du bassin du patient, on commence par implanter, par impaction, ladite coupole métallique 1 dans ledit os 24 dont l'ancrage est facilité par lesdites protubérances en forme de dents 13 ; ledit ancrage peut être réalisé grâce à une tige filetée passant dans ledit alésage 14.
Les vis 25 peuvent ensuite être vissées dans ledit os dudit bassin et, grâce à la face sphérique de leurs têtes, s'orienteur dans ladite portée sphérique desdits orifices 8, 9, 10, 11 de ladite coupole métallique 1, de façon à induire le minimum de contrainte mécanique et donc de façon à limiter les douleurs résultant de l'opération.
Grâce à 1 invention, et à ladite grande largeur de ladite fenêtre 15, le chirurgien peut vérifier que ladite coupole métallique 1 épouse parfaitement la cavité réalisée dans l'os 24 du bassin ; avantageusement, la surface extérieure de ladite coupole métallique est revêtue d'un hydroxyapatique de calcium et a subi un traitement de sablage, de façon à faciliter la liaison intime avec ladite cavité osseuse.
On peut alors introduire ledit noyau 4 dans ladite coupole métallique 1, qui est bloqué en rotation et ne peut ensuite s'échapper grâce auxdites languettes 5, qui sont légèrement proéminentes à l'intérieur de ladite partie tubulaire de ladite coupole, de sorte qu'elles permettent l'introduction du noyau 4 à l'intérieur de ladite coupole, moyennant une légère déformation locale de ladite partie cylindrique dudit noyau 4 en regard desdites extrémités libres desdites languettes 5.
La tête prothétique ne peut sortir de la cavité 21 prévue dans ledit noyau, grâce à la présence selon l'invention de la surface prévue à l'intérieur dudit noyau, au niveau notamment dudit promontoire 19, jouant le rôle de butée anti-luxation.

Claims (5)

REVENDICATIONS
1. Prothèse de hanche du type comportant une coupole métallique (1), qui est susceptible d'être fixée par des vis (25) dans le bassin d'un patient, laquelle coupole (1) reçoit un noyau (4) en matière plastique, dont une partie au moins de la face externe épouse une partie au moins de la face interne de ladite coupole (1), caractérisée par le fait
que ledit noyau (4) comporte une cavité (22), qui peut recevoir la tête d'une tige qui peut être fixée au fémur dudit patient, ladite tige est pourvue d'un cône fixe, dit cône "morse" qui reçoit les têtes prothétiques qui entrent dans le noyau (4), l'angle cervico-diaphysaire augmente selon la taille de la tige.
2. Prothèse de hanche selon la revendication 1 caractérisée par le fait
que l'angle entre l'axe longitudinal du corps et l'axe longitudinal du cône ou du col fixe est compris entre 135 et 1420.
3. Prothèse de hanche selon la revendication 1, caractérisée par le fait
qu'une variation de cet angle est, bien sur, possible par l'utilisation de cônes ou cols de taille différente ou cols, cette variation pouvant aller de 1300 à 145 selon le cône utilisé.
4. Prothèse de hanche selon la revendication 1, caractérisée par le fait
que l'épaisseur de la coupôle métallique (1) est supérieure à l'épaisseur des têtes des vis (25) et ce, de manière à éviter tout frottement avec le noyau (4) en matière plastique.
5. Prothèse de hanche selon la revendication I caractérisée par le fait
que sur la face extérieure (12) de la partie hémisphérique de la coupôle métallique (1) sont prévues des prétubérances qui ont une forme de dent et dont la forme en coupe forme un arc de cercle.
