Abstract

L'invention a rapport à la chirurgie orthopédique dans laquelle une prothèse est fixée par du ciment chirurgical dans l'os. L'obturateur selon l'invention évite la pénétration du ciment plus profondément que nécessaire, grâce à sa consistance semi-rigide obtenue du fait de sa conception. Il présente en effet, une fente (5) entre chaque ailette qui lui donne sonélasticité premettant ainsi de trouver le maintien en position dans le canal médullaire et l'étanchéité adéquate. Il possède sur son fond une cuvette tronconique (6) qui autorise une préhension rapide sur le matériel ancillaire autotaraudeur. Le matériau est à base de poly (alpha-hydroxy-acides).

Description

La présente invention concerne un obturateur intramédullaire biorésorbable semirigide à ailettes.
Cette invention a rapport à la chirurgie orthopédique dans laquelle une prothèse est fixée par du ciment chirurgical dans l'os du patient pour remplacer une articulation défectueuse. Un bouchon ou obturateur (1) est fréquemment placé au fond de la cavité osseuse (2) avant d'injecter le ciment (3) puis de placer la prothèse (4) (Fig 1).
Cet obturateur est destiné d'une part, à éviter que le ciment chirurgical utilisé pour fixer la prothèse ne pénètre plus profondément que nécessaire dans le canal médullaire de l'os, d'autre part, à rendre ce ciment plus homogène au niveau de la queue de la prothèse, donc de meilleure qualité mécanique.
La présente invention concerne un obturateur biorésorbable intramédullaire semirigide à ailettes, notamment d'un canal médullaire fémoral.
Actuellement les obturateurs ne donnent pas entièrement satisfaction, ils sont soit en polyéthylène et peuvent poser des problèmes d'extraction en cas de reprise de la prothèse, soit biodégradables mais:
- complèfemenf rigide, dès lors l'adaptation morphologique est mauvaise et l'on renconfre des
problèmes d'éfanchéité.
- trop mou et l'obturateur ne tient pas sa position initiale sous la pression du ciment lors de la pose
ultérieure de la prothèse.
Les obturateurs à ailettes traditonnels généralement en polyéthylène (Fig 2) ont une plage d'utilisation pour chaque taille d'obturateur déterminée par la différence entre le diamètre maximum, au sommet des ailettes et le diamètre minimum à la base des ailettes. Si l'étanchéité et la tenue de tels obturateurs sont satisfaisantes à leur diamètre minimum, en revanche, à leur diamètre maximum il n'y a plus de resserement des ailettes sur le fut fémoral, il s'en suit un mauvais maintien sous la pression du ciment lors de la pose ultérieure de la prothèse et des fuites de ciment entre les ailettes.
L'obturateur à ailettes selon l'invention entièrement biorésorbable est de par sa conception de consistance semi-rigide et permet le maintien et l'étanchéité adéquate (Fig 3).
Pour être efficace aussi bien dans la résistance à la pression du ciment que dans l'étanchéité, I'invention comporte selon une première caractéristique, une fente (5), entre chaque ailette, permettant d'avoir pour diamètre maximum de prise la base des ailettes et pour diamètre minimum de prise le bas de cette fente.
Cette fente est suffisament étroite pour ne pas laisser passer le ciment et suffisament large pour permettre le resserement des parois.
Lors d'une utilisation d'une taille d'obturateur, selon l'invention, à son diamètre supérieur les ailettes exercent déjà une pression sur le fut fémoral.
De plus ia souplesse accrue par l'adjonction de la fente autorise une augmentation de l'angle du cône formé par l'obturateur, chaque ailette peut prendre alors un angle différent de celui des autres et l'adaptation morphologique est améliorée (Fig 4).
L'obturateur selon l'invention comporte selon une seconde caractéristique, une cuvette tronconique sur son fond (6) pour fixer l'obturateur sur le matériel ancillaire nécessaire à sa pose dans le canal intramédullaire.
L'extrémité autotaraudeuse du matériel ancillaire a un diamètre compris entre le diamètre supérieur et inférieur de la cuvette tronconique.
Le chirurgien centre donc facilement l'extrémité autotaraudeuse dans la cuvette et en effectuant un quart de tour l'obturateur tient sur le matériel ancillaire, d'où une manipulation plus rapide qu'avec un système autotaraudeur, vis et trou.
Lors de l'intervention le chirurgien détermine la taille de l'obturateur à l'aide de gabarits.
L'invention peut être fabriquée par injection et est adaptée pour les polymères de la classe des poly (a - hydroxy - acides), matériaux résorbables, dans le corps humain.

Claims (3)

REVENDICATIONS
1) Obturateur intramédullaire biorésorbable à ailettes d'un canal osseux, caractérisé
en ce qu'il est de consistance semi-rigide du fait de sa conception et qu'il est pourvu
d'un système de fixation sur le matériel ancillaire autotaraudeur, en forme de
cuvette tronconique.
2) Obturateur conforme à la revendication 1 caractérisé en ce que son élasticité est
obtenue sar une fente entre chaque ailette.
3) Obturateur conforme à la revendication I et 2 caractérisé en ce que le matériau est à base de polymère de la classe des poly (o: - hydroxy - acides).
