Abstract

La présente invention concerne une prothèse d'acétabule, utilisable sans ciment et comportant des moyens d'ancrage à couronne. La prothèse selon l'invention comporte une calotte sensiblement hémisphérique munie sur sa périmétrie d'une série d'ailettes disposées de manière uniforme sur toute la surface de ladite calotte et ayant la forme de coin dont les sommets sont tournés vers la partie supérieure de la calotte. Un élément couvrant en polyéthylène est appliqué par enclenchement sur la surface concave de la calotte 2.

Description

PROTHESE D'ACETABULE UTILISABLE SANS CIMENT
La présente invention concerne une prothèse d'acétabule, utilisable sans ciment, et comportant des moyens d'ancrage à couronne.
Il est bien connu que pour appliquer une prothèse de hanche, il est nécessaire au préalable de placer dans l'acétabule une autre prothèse conçue pour former un logement approprié pour la tige fémorale.
Les prothèses connues ne permettent pas toujours une application convenable de la prothèse d'acétabule dans le bassin, notamment en ce qui concerne la fixation sur la zone osseuse, qui bien souvent est réalisée sans ciment, ce qui limite la durée dans le temps de l'ancrage ainsi réalisé.
Un autre problème fréquemment rencontré réside dans la stabilité de la fixation de la prothèse, au regard de son déplacement dans la direction axiale, ou par rotation autour de son axe.
La présente invention a pour but d'éliminer les inconvénients mentionnés ci-dessus en réalisant une prothèse d'acétabule pouvant être utilisée sans ciment et appliquée directement dans la zone osseuse, pour assurer un ancrage précis et stable dans le temps.
Dans ce but, la présente invention se propose de réaliser une prothèse d'acétabule pouvant être appliquée d'une manière à la fois simple et facile tout en assurant une bonne prise au niveau des zones osseuses de grande épaisseur, et dont l'usage serait tout à fait fiable tout en pouvant être réalisé au moyen de matériaux couramment disponibles dans le commerce, le tout sous un coût très réduit.
Les buts mentionnés ci-dessus sont atteints par une prothèse d'acétabule caractérisée en ce qu'elle comprend une calotte sensiblement hémisphé rique portant sur son périmètre extérieur une série d'ailettes disposées de manière uniforme sur la surface convexe de la demi-sphère et ayant la forme de coins dont les sommets seraient tournés vers le sommet de la calotte, et en ce qu'un revêtement recouvre la surface concave de ladite calotte.
D'autres caractéristiques de la présente invention vont à présent être décrites en référence à un mode de réalisation préférentiel, donné à titre non limitatif, d'une prothèse d'acétabule selon la présente invention, et en référence au dessin annexé, dans lequel - la figure 1 est une vue schématique en élévation de la prothèse

connexion entre la calotte et l'élément couvrant.
En se référant aux figures annexées, la prothèse d'acétabule selon la présente invention, désignée sous le chiffre de référence 1, comporte une calotte 2 sensiblement hémisphérique, d'épaisseur non constante.
L'épaisseur de la calotte 2 va en augmentant à partir du sommet vers le bord.
Un orifice passant 3 est aménagé au sommet de la calotte 2.
Sur tout le pourtour de la surface convexe de ladite calotte, on dispose d'une série d'ailettes 10 en forme de coins. Chaque ailette 10 comporte deux surfaces inclinées 11 formant le coin. Les ailettes 10 sont disposées de manière à converger vers le sommet de la calotte.
Selon cette disposition les ailettes présentent une épaisseur qui, dans la direction radiale, va en augmentant vers le sommet alors que leur largeur dans une direction tangentielle va en croissant vers le bord de la calotte.
Les ailettes 10 sont conçues pour être engagées par forcement dans le logement osseux, pour ainsi définir un engagement par insertion associé à un effet de compression osseuse qui dépend directement de la forme en coin desdites ailettes.
Les ailettes ainsi disposées agissent pour l'essentiel sur la bordure de la zone d'acétabule, c'est-à-dire là où l'os est très compact et peut donc de ce fait supporter les efforts exercés.
Sur la surface concave de la calotte 2, on peut appliquer un élément couvrant 20. Cet élément couvrant comporte également une surface sensiblement hémisphérique, avec une partie saillante 21 sur sa circonférence, laquelle peut être insérée par enclenchement dans une gorge 22 formée en correspondance dans la surface concave de la calotte 2.
En outre, l'élément couvrant 20 comporte un bord 23 conçu pour s'engager par enclenchement dans le bord correspondant de la calotte 2.
D'une manière avantageuse, la calotte 2 est réalisée en titane et peut comporter sur sa surface extérieure un revêtement en hydroxyapatite, ce qui permet de réaliser un ancrage osseux dans la zone de l'acétabule d'une manière tout à fait fiable.
L'élément couvrant est d'une manière avantageuse réalisé en polyéthylène, ce qui permet d'obvenir des surfaces de contact réciproque très précises, et d'un point de vue pratique auto-lubrifiante.
La description qui précède permet de remarquer que la forme de la calotte permet de former un accouplement mécanique tout à fait précis sans qu'il soit nécessaire d'utiliser du ciment, ceci grâce à l'insertion parfaite des ailettes dans la zone osseuse de l'acétabule présentant.une résistance mécanique plus élevée. En outre, la mise en place par enclenchement de l'élément couvrant la surface concave de la calotte autorise un accouplement plus rapide et plus facile.
La prothèse décrite ci-dessus pourra recevoir de nombreuses variantes et modifications évidentes pour l'homme de l'art sans que l'on sorte du cadre de l'invention.
En particulier, les détails techniques pourront être remplacés par d'autres éléments équivalents. De même, les matériaux utilisés dès lors qu'ils sont compatibles avec l'utilisation envisagée, de même que les dimensions et formes décrites pourront être quelconques selon les exigences.

Claims (7)

  1. Revendications.
    I. Prothèse d'acétabule utilisable sans ciment caractérisée en ce qu'elle comporte une calotte (2) sensiblement hémisphérique présentant sur sa périphérie une série d'ailettes (10) disposées de manière uniforme sur la surface convexe de ladite calotte, lesdites ailettes ayant la forme de coin dont le sommet est tourné vers le sommet de la calotte (2), un élément couvrant (20) étant appliqué par enclenchement sur la surface concave de ladite calotte.
  2. 2. Prothèse d'acétabule selon la revendication 1, caractérisée en ce que la calotte (2) comporte une épaisseur variable, allant en s'accroissant de manière continue à partir du sommet de la calotte (2) vers son bord.
  3. 3. Prothèse d'acétabule selon l'une des revendications 1 ou 2, caractérisée en ce que les ailettes (10) comportent deux surfaces inclinées convergent vers le sommet pour former un coin, lesdites surfaces inclinées étant délimitées par une paroi frontale s'étendant le long d'une surface cylindrique ayant pour base le corps périphérique de la calotte (2).
  4. 4. Prothèse d'acétabule selon l'une quelconque des revendications précédentes, caractérisée en ce que la calotte (2) est réalisée en titane et en ce que sa surface convexe reçoit un revêtement en hydroxyapatite.
  5. 5. Prothèse d'acétabule selon l'une quelconque des revendications précédentes, caractérisée en ce que l'élément couvrant (20) a une forme sensiblement hémisphérique, et comporte une partie saillante pouvant être associée par enclenchement avec une gorge annulaire ménagée en correspondance sur la surface concave de la calotte (2), ledit élément couvrant comportant un bord à bourrelet conçu pour s'engager avec le bord de ladite calotte.
  6. 6. Prothèse d'acétabule selon l'une quelconque des revendications précédentes, caractérisée en ce que l'élément couvrant (20) est réalisé en polyéthylène.
  7. 7. Prothèse d'acétabule selon l'une quelconque des revendications précédentes caractérisée en ce que les ailettes (10) peuvent être insérées sous compression osseuse dans la zone périmétrale de l'acétabule où
    l'épaisseur de l'os est la plus élevée.
