Abstract

The present invention concerns a catheter intended to be introduced into a body channel, comprising a tube 4 for guiding an energy-conducting fibre, such as, in particular, an optical fibre, the said tube having, in the vicinity of its distal end, at least one radially deformable structure intended to centre the distal end of the said fibre in the said channel. According to the invention, the said radially deformable structure 6 consists of a bundle of filaments 7 surrounding the said fibre 4 whose controlled variation of curvature transmits the radial deformation of the said structure 6, and the distal end of the said bundle of filaments 7 is designed to form a window 8 facing the distal end 4a of the said fibre 4. The invention applies to the examination and/or the destruction or the reduction of obstacles within a body channel, in particular a blood vessel.

Description

The present invention relates to a catheter for insertion into a body canal.

More particularly, although not exclusively, the catheter of the invention allows the examination and / or destruction or reduction of obstacles within a body channel, for example a blood vessel, using an optical fiber.

The use of laser radiation to locally destroy, for example atheromas, presents difriculties because of these risks of deterioration of the wall of the body channel if the laser shot is badly centered, these difficulties increasing considerably for small diameter channels, such as blood vessels.

It was therefore thought, in a catheter comprising a tube for guiding an optical fiber, to provide the distal end of said tube with an inflatable balloon ensuring the centering of the fiber in the body channel. However, in the case of a blood vessel, the balloon once inflated blocks the circulation of blood, risking damage to the tissues downstream of said balloon. In addition, such balionets are difficult to use in small blood vessels
The present invention aims to avoid these drawbacks, and relates to a catheter allowing intervention in body channels, in particular blood vessels, of small diameter, without the risk of damaging and / or closing them.

To this end, the catheter intended to be introduced into a body channel, of the type comprising a guide tube of an energy-conducting fiber, such as in particular an optical fiber, said tube having, in the vicinity of its distal end, at the at least one radially deformable structure intended to center the distal end of said fiber in said channel is remarkable, according to the invention, in that said radially deformable structure is constituted by a bundle of wires surrounding said fiber, the controlled variation of which curvature translates the radial deformation of said structure, and in that the distal end of said bundle of wires is designed to provide a window facing the distal end of said fiber.

Thus, after introduction of the catheter into the body channel in the undeformed state of said structure, the latter, at the location of the obstacle to be destroyed, can be deformed so that the wires bear on the wall of the body channel thereby centering the fiber in said channel.

Advantageously, means for distributing the pressure of the wires on the internal wall of said channel are provided, which allow this to reduce the risks of damage to the wall of the channel. Said pressure distribution means can be constituted by an elastic sleeve sheathing said wires.

Preferably, the distal ends of said wires are joined by a guide ring, at least partially crossed by the distal end of said fiber, constituting said window.

According to another characteristic of the invention, the catheter comprises a cable extending from the proximal end of the guide tube to said ring, inside said tube and controlling the deformation of said structure.

According to yet another characteristic of the invention, said wires, helically wound with contiguous turns, constitute the guide tube of said fiber. In this case, at least some of said wires may be hollow and pass through said ring.

According to a variant of the invention, said guide tube can be surrounded by a sheath extending from the proximal end of said tube in the vicinity of said structure.

Preferably, said ring includes means for clamping and / or bonding said fiber.

By the way, said ring can have an elongated body whose rounded end is rounded.

Advantageously, the distal end of the fiber ladIte is maintained in position by a narrowing of the central channel of the said ring.

In addition, several of said longitudinally spaced radially deformable structures may be provided in the vicinity of the distal end of said guide tube.

Reference will be made, first of all, more particularly to FIGS. 1 to ~.

The catheter, intended to be introduced into a body channel 2, for example a blood vessel, comprises a guide tube 3 of an energy-conducting fiber 4, such as in particular an optical fiber, guide tube 3 of which only the distal end part 5, introduced into channel 2, is shown in the drawing. The tube 3 has, in the vicinity of its distal end, a radially deformable structure 6 intended to center the distal end 4a of the fiber 14 in the channel 2.

The radially deformable structure 6 is constituted by a bundle of wires 7 surrounding the fiber 4, the controlled variation of curvature of which translates the radial deformation of the structure 6. In the example shown, corme can best be seen in FIG. 3, the bundle comprises eight wires 7, this number of course not being limiting, a minimum of three angularly spaced wires of 1200 being however necessary to form the structure 6. A first end 7a of each wire 7 is secured to the distal end 5 of the guide tube 3, optionally via the medullary of a ring of the same diameter integral with the latter, while the second end 7b of each wire 7 is secured to a guide ring 8, traversed at least partially by the distal end 4a of the fiber 4.

In order to distribute the pressure of the wires 7 on the internal wall of the channel 2, thus avoiding any risk of damage to the latter, an elastic sleeve 9 advantageously sheaths the wires 7. The sleeve 9 can be made of latex, silicone or any other similar material, biologically tolerated by the organism.

Par ailleurs, un câble it, s'étendant de l'extrémité proximale (non représentée' du tube de guidage 3 à la bague 8 à laquelle il est solidarisé, est destine à commander la déformation de la structure 6, par manipulation dudit câble, à son extrémlte proximale, par l'opérateur qui lui fait effectuer des mouvements longitudInaux ou de torsion. ainsi, par exemple, une traction sur le câble 10 permettra de passer de l'état non déformé de la structure 6 permettant l'introduction du cathéter dans le canal corporel, montré sur la figure 1, à l'état radialement déformé de celle-ci, montre sur la figure 2, dans lequel la structure 6 assure le centrage de la fibre 4 dans le canal 2.Furthermore, a cable it, extending from the proximal end (not shown) of the guide tube 3 to the ring 8 to which it is secured, is intended to control the deformation of the structure 6, by manipulation of said cable, at its proximal extremlte, by the operator who makes it perform longitudinal or torsional movements. thus, for example, traction on the cable 10 will make it possible to pass from the undeformed state of the structure 6 allowing the catheter to be introduced in the body channel, shown in FIG. 1, in the radially deformed state thereof, shown in FIG. 2, in which the structure 6 ensures the centering of the fiber 4 in the channel 2.

It will be noted that, to avoid obstruction of the body channel, in particular in the case of a blood vessel, the sleeve 9 can be perforated thereby leaving passages for the fluid conveyed in the channel, in particular blood.

Cet agencement permet de mieux épouser les courbes des canaux corporels.FIG. 4 illustrates a variant of the catheter of the invention, in which several radially supported, flexible structures 6 (two are shown), longitudinally spaced are provided in the vicinity of the distal end 5 of the guide tube 3. Each of these structures 6 is of course similar to the corresponding structure in FIGS. 1 to 3. This arrangement makes it possible to better match the curves of the body channels.

In the variant illustrated by FIGS. 5 and 6, a sheath 11 surrounds the guide tube 3, thereby facilitating the insertion of the latter, with the radially deformable structure 6, into the body channel 2 (FIG. 5). Once the catheter is in place at the site of the lesion to be treated, the sheath can be partially or completely removed (Figure 6), thereby freeing up the distal end 5 of the tube 3 and the structure 6.

In this case, it has an elongated body 12 whose free end 13 is rounded so as to avoid any risk of injury to the body canal 2. In addition ', the distal end 4a of the fiber is held in position by a narrowing 14 of the central channel 15 of the ring 8. Furthermore, clamping and / or bonding means 16 of the fiber can also be provided in the channel 15.

FIG. 7 also illustrates the possibility that at least some of the wires 7 are hollow and pass through the ring 8. These hollow wires can thus lead, from the outside to the location of the lesion to be treated, fluids or energy. One or two sons can also be used for observation or analysis.

In accordance with FIG. 8, the guide tube 3 can be constituted by the helical winding with contiguous turns of the wires 7 constituting the radially deformable structure 6. It will be noted that, in FIG. C, it has been considered that the structure 6 was made by three sons 7.
