Abstract

La présente invention a pour objet un composant fémoral de prothèses de hanche destiné à être implanté avec ou sans ciment comprenant une partie formant tige 2 adaptée pour être introduite et éventuellement scellée dans la cavité médullaire du fémur, comportant deux faces frontales 4 et au moins une partie formant joue 5 adaptée pour épouser la cavité médullaire. Selon l'invention, chaque face frontale 4 destinée à être associée à une partie formant joue 5 comporte au moins une nervure 6 destinée à coopérer avec une ou plusieurs rainures 7 de forme conjuguée prévue sur la face d'appui de la joue 5 formant les moyens de guidage de la joue sur ladite face frontale 4. Application : chirurgie orthopédique.

Description

Composant fémoral pour prothèses de hanche.
La présente invention concerne généralement les prothèses de hanche, et a essentiellement pour objet un composant fémoral destiné à être implanté avec ou sans ciment dans la cavité médullaire du fémur.
L'invention trouve notamment application dans le domaine de la chirurgie orthopédique.
On rappelle qu'un composant fémoral de prothèse de hanche comporte généralement une partie formant tige surmontée à sa partie supérieure d'une partie formant col réalisée par exemple à l'aide d'un cône morse usiné susceptible de recevoir - soit une cupule blindée ou une pièce céphalique destinée à être
insérée directement dans le cotyle anatomique, pour le traitement
des fractures du col du fémur - soit une tête sphérique, métallique ou céramique, destinée à
s'articuler sur un cotyle artificiel, dans le traitement de
l'arthrose.
Il existe actuellement de nombreux types de tiges que l'on peut classer en deux grands groupes selon l'épaisseur de l'âme de la tige - Les composants fémoraux dont l'âme est de faible épaisseur
présentent l'intérêt de pouvoir etre orientés facilement, et
éventuellement d'être antéversés, mais ils présentent
l'inconvénient de remplir faiblement l'évasement métaphysaire,
et leur implantation nécessite par conséquent l'utilisation de
grandes quantités de ciment dont les inconvénients sont bien
connus.
- Les composants fémoraux dont l'âme est d'épaisseur moyenne ou de
forte épaisseur présentent l'intérêt de permettre une diminution
de la quantité de ciment à utiliser pour leur implantation, mais
souffre de l'inconvénient de limiter les possibilités de
positionnements dans le fémur et de réglage en antéversion, ce
qui rend plus difficile le travail du chirurgien.
Des recherches ont donc été réalisées pour mettre au point un composant fémoral d'une nouvelle conception qui présente les avantages combinés des composants traditionnels des deux groupes définis précédemment, sans en avoir les inconvénients. Ainsi, la demande de brevet français n085 04548 décrit un composant fémoral de prothèse de hanche dont la tige comporte deux faces frontales, de préférence sensiblement planes s'étendant en position d'implantation au moins au niveau de la région du trochanter associées chacune à une partie formant joue ou cale adaptée pour épouser la cavité médullaire et susceptible d'être montée de façon guidée sur lesdites faces frontales de la tige.
A cet effet, les faces frontales et les cales sont pourvues de moyens de guidage complémentaires constitués par une rainure en queue d'aronde dans chaque face frontale et, pour chaque cale un plot à tête conique en saillie sur la face d'appui.
Cependant, un tel composant fémoral présente les inconvénients suivants - L'usinage en creux des queues d'aronde nécessite une âme
d'épaisseur suffisante pour ne pas affaiblir la résistance
mécanique de l'ensemble - l'utilisation d'un plot de guidage permet une liberté en rotation
des cales, mais celles-ci ne peuvent être fixées de façon sûre ; - les possibilités de positionnements dans l'espace de ces cales
sont limitées.
La présente invention a pour but de résoudre le nouveau problème technique consistant en la fourniture d'un composant fémoral de prothèse de hanche du type précité, dont l'âme puisse être d'épaisseur réduite, dont les joues ou cales puissent être fixées dans des positions différentes, permettant ainsi au chirurgien, à partir d'une âme de dimensions données, de composer une prothèse de dimensions finales variables.
La solution, conforme à la présente invention, pour résoudre le problème technique posé, consiste en un composant fémoral de prothèse de hanche destiné à titre implanté avec ou sans ciment et comprenant une partie formant tige, adaptée pour être introduite et éventuellement scellée dans la cavité médullaire du fémur, comportant deux faces frontales de préférence sensiblemnt planes, s'étendant en position d'implantation, au moins au niveau de la région du trochanter ; et au moins une partie fo nuant joue adaptée pour épouser la cavité médullaire et susceptible d'être montée de façon guidée sur l'une desdites faces frontales de la tige, caractérisé en ce que chaque face frontale destinée à être associée à une partie formant joue comporte au moins une nervure, par exemple en forme de queue d'aronde, destinée à coopérer avec une ou plusieurs rainures de forme conjuguée prévues sur la face d'appui de ladite joue et formant moyen de guidage de la joue sur la face frontale.
Selon une caractéristique particulière de l'invention, chaque partie formant joue précitée peut comporter un nombre de rainures supérieur au nombre de nervures de la face frontale associée de la tige. Cette conformation permet un positionnement variable selon une première direction contenue dans le plan frontal.
Selon une autre caractéristique particulière de l'invention, chaque nervure précitée présente une épaisseur variable sur au moins une partie de sa longueur. Cette conformation permet de régler le positionnement de la partie formant joue selon un axe perpendiculaire au plan frontal.
Selon encore une autre caractéristique, un composant fémoral selon l'invention comporte des moyens de fixation de ladite partie formant joue en position relative prédéterminée sur ladite tige. Cette conformation permet un réglage du positionnement selon une seconde direction contenue dans le plan frontal, sensiblement perpendiculaire à la première direction précitée.
L'invention sera mieux comprise, et d'autres buts, caractéristiques et avantages de celle-ci apparaîtront plus clairement à la lecture de la description explicative qui va suivre, faite en référence aux dessins schématiques annexés donnés uniquement à titre d'exemples non limitatifs illustrant plusieurs modes de réalisation actuellement préférés de l'invention et dans lesquels - La figure 1 est une vue de principe en perspective d'une partie d'un composant fémoral å l'état non assemblé conforme à un premier mode de réalisation de l'invention comportant un élément formant joue - la figure 2 est une vue de côté d'un composant fémoral semblable
o elui représenté à la figure 1, mais comportant deux éléments
formant joue - la figure 3 est une vue semblable à la figure 2 d'un composant
fémoral conforme à un second mode de réalisation de l'invention
et - la figure 4 est une vue en perspective montrant une variante de
réalisation d'un élément formant joue.
Un composant fémoral de prothèse de hanche 1 conforme à l'invention comporte de façon connue en soi une partie formant tige 2 ayant à son extrémité inférieure non représentée une forme éfilée adaptée pour permettre une implantation dans la cavité médullaire du fémur, ladite tige étant surmontée à sa partie supérieure d'une partie formant col 3 réalisée dans l'exemple représenté sous forme d'un cône morse usiné dont l'utilité a été rappelée précédemment.
La tige 2 comporte une âme limitée parrallèlement au plan frontal par deux faces sensiblement planes 4 s'étendant, en position d'implantation, au moins au niveau de la région du trochanter c'est-à-dire à l'extrémité supérieure du fémur.
Le composant fémoral 1 selon l'invention comporte en outre au moins une partie formant joue 5 adaptée à s'accoler à la tige 2 suivant l'une des faces frontales 4 par une face d'appui complémentaire.
Chaque face frontale 4 destinée à être associée à une partie formant joue 5 comporte au moins une nervure en saillie 6, par exemple en forme de queue d'aronde, destinée à coopérer avec une ou plusieurs rainures 7 de forme conjuguée prévues sur la face d'appui de la joue 5, permettant ainsi un montage guidé de la joue 5 sur la tige 2.
Dans l'exemple représenté à la figure 1 seule la face frontale avant est associée à une joue 5, mais il est bien évident qu'un composant fémoral selon l'invention peut comporter deux joues montées de part et d'autre de la tige 2 sur chacune des faces frontales 4 (figures 2 et 3).
De même, chaque face frontale peut être pourvue de une ou plusieurs, (par exemple deux) nervures. Dans le cas où a nombre de nervures est supérieur ou égal à deux, celles-ci sont réparties de façon espacée parallèlement sur la face frontale 4, suivant une direction X X' contenue dans le plan frontal, dont l'orientation sera choisie de façon prédéterminée.
Selon un premier aspect de l'invention, il est possible d'obtenir différents positionnements de chaque joue 5 sur sa face frontale 4 associée, en prévoyant sur ladite joue un nombre de rainures supérieur d'au moins une unité au nombre de nervures de la face frontale associée. (Fig.1). On obtient ainsi un réglage du positionnement de la joue 5 selon un axe Y Y' contenue dans le plan frontal, et dont la direction est sensiblement perpendiculaire à la direction de l'axe X X' défini précédemment.
Dans l'exemple représenté à la figure 1, la joue 5 comporte trois rainures, tandis que la face frontale associée 4 comporte deux nervures, ce qui permet d'obtenir deux positions de la joue 5 suivant l'axe Y Y'.
L'épaisseur de chaque nervure 6 peut être constante sur toute sa longueur. (Figures 1 et 2).
Cependant, selon une autre caractéristique particulière de l'invention, chaque nervure 6 peut être réalisée d'épaisseur variable sur au moins une partie de sa longueur. Dans l'exemple représenté à la figure 3, l'épaisseur de la nervure 6 diminue de façon sensiblement linéaire de sa partie supérieure à sa partie inférieure. Bien entendu, dans ce cas, la forme de chaque rainure des joues 5 sera prévue en conséquence (profondeur variable).
Cette conformation particulière donne une possibilité de réglage selon un axe Z Z' perpendiculaire au plan frontal.
Comme on l'a vu, un composant fémoral selon l'invention peut comporter indifféremment une ou deux joues 5. Il en résulte pour les chirurgiens la possibilité de faire varie l'épaisseur apparente de la prothèse finale, possibilité augmentée par le fait que les joues peuvent être d'épaisseur différente. On peut ainsi concevoir une prothèse symétrique d'épaisseur souhaitée au moment de l'intervention, ou une prothèse d'épaisseur réglable de façon dissymétrique par rapport au plan frontal.
Les différentes possibilités de réglage du positionnement des joues dans le plan frontal suivant l'axe X X' etlou Y Y' pourront être utilisées en combinaison avec des possibilités de réglage d'une ou deux joues 5 selon l'axe Z Z'.
Selon un aspect complémentaire de l'invention, on pourra prévoir des moyens de fixation de la partie formant joue 5 en une position relative prédéterminée sur la tige 2. A cet effet, comme le montre la figure 4, chaque joue 5 pourra être associée à au moins une vis à extrémité plate 9 susceptible d'être vissée dans un trou fileté 8 prévu sur la partie formant joue 5 et susceptible de prendre appui sur la tige 2 en immobilisant ainsi la joue 5 sur cette tige dans une position prédéterminée choisie par le chirurgien.
Selon l'invention, chaque partie formant joue 5 peut être revêtue d'un dépôt d'une substance ou de nature à favoriser la repoussse osseuse, d'une façon connue en soi.
La mise en place du composant fémoral conforme à l'invention se déduit de façon aisée de sa structure qui vient d'être décrite.
L'avantage essentiel de ce composant fémoral consiste en ce qu'il permet pour le chirurgien, à partir d'une âme de dimensions données, de composer une prothèse de dimensions finales variables, le positionnement de chaque joue pouvant être réglé à volonté suivant chacun des axes X X', Y Y', Z Z'.
Bien entendu l'invention n'est pas limitée aux modes de réalisation donnés précédemment. On pourra par exemple prévoir pour les nervures et rainures des formes quelconques complémentaires sans sortir du cadre de l'invention. De même, la fixation ou l'immobilisation de chaque joue 5 sur la tige 2 peut être obtenue par d'autres dispositifs que celui spécifiquement décrit en référence à la figure 4.

Claims (6)

REVENDICATIONS
1. Composant fémoral de prothèses de hanche destiné à être implanté avec ou sans ciment et comprenant une partie formant tige, adaptée pour être introduite et éventuellement scellée dans la cavité médullaire du fémur, comportant deux faces frontales, de préférence sensiblement planes, s'étendant en position d'implantation au moins au niveau de la région du trochanter ; et au moins une partie formant joue adaptée pour épouser la cavité médullaire et susceptible d'être montée de façon guidée sur l'une desdites faces frontales de la tige, caracterisé en ce que chaque face frontale (4) destinée à être associée à une partie formant joue (5) comporte au moins une nervure (6), par exemple en forme de queue d'aronde, destinée à coopérer avec une ou plusieurs rainures (7) de forme conjuguée prévue sur la face d'appui de ladite joue (5) et formant des moyens de guidage de la joue sur ladite face frontale.
2. Composant fémoral selon la revendication 1, caractérisé en ce que chaque partie formant joue (5) précitée comporte un nombre de rainures (7) supérieur au nombre de nervures (6) de la face frontale (4) associée de la tige.
3. Composant fémoral selon la revendication 1 ou 2, caractérisé en ce que chaque nervure (6) précitée présente une epaisseur variable sur au moins une partie de sa longueur.
4. Composant fémoral selon l'une des revendications 1 à 3, caractérisé en ce qu'il comporte en outre des moyens de fixation de ladite partie formant joue (5) en position relative prédéterminée sur la tige (2), constitués par exemple d'une vis (9) à tête plate susceptible d'être vissée dans un trou fileté (8) prévu sur ladite partie formant joue (5) et de prendre appui sur ladite tige (2).
5. Composant fémoral selon la revendication 3, caractérisé en ce que l'épaisseur de la nervure précitée (6) diminue de la partie supérieure de la tige (2) vers sa partie inférieure.
6. Composant fémoral selon l'une des revendications 1 à 5, caractérisé en ce que chaque partie formant joue (5) est revêtue d'un dépôt d'une substance ou de nature à favoriser la repousse osseuse
