Abstract

Dispositif pour changer l'antéversion et l'angle d'une prothèse de hanche. Il est constitué par une tête avec un axe de révolution YY' désaxé d'un angle alpha par rapport à l'axe de révolution XX'. Ce dispositif est particulièrement destiné à changer de façon rapide l'orientation et l'angulation de la partie supérieure des prothèses de hanche.

Description

DESCRIPTION
La présente invention concerne une tête fémorale prothétique classique, composée d'un segment (ou partie) de sphère A sphère diminuée d'un segment B) (fig.1)
Elle est creusée, conne c'est maintenant habituel dans ce type de matériel, à partir de la tranche de section (plan 7i), d'une cavité tronconique, borgne, en vue de son assemblage, par emboitenent mâle-femelle (type cône morse), avec la tige cervicale également tronconique d'une prothèse fémorale (fig.2-3).
La particularité de cette invention est que l'axe de révolution W' de ce cOne est désaxé d'un angle alpha par rapport à l'axe de révolution XX' de la tête prothétique ; le sonnet P de cet angle alpha est le point d'intersection de l'axe de révolution XX' de la tête avec le plan n donc avec l'embase de la tête prothétique. L'axe YY' s'écarte progressivement de XX' au fur et à mesure de son entrée dans la tete prothétique, au lieu d'être confondu avec lui (fig. 2-3).
Cette invention s'applique aussi bien aux têtes prothétiques fémorales de prothèse totale, qu'aux prothèses de remPlacement des tetes fémorales, dites prothèses céphaliques, pour fracture du col.
L'assemblage entre la partie céphalique et la partie cervicale d'une prothèse fémorale, qu'elle soit de type "prothèse totale" ou de type "prothèse céphalique" pour fracture du col, peut se faire par emboitenent d'un tronc de cône mâle dans un tronc de cOne femelle.
L'angle de pente de ce tronc de cône peut varier, en théorie de O à 90 ; en fait, ne sont retenues, en général, que des valeurs entre 3 et 100 pour des raisons de tenue du matériel (environ 40 dans nos schémas pour exemple).
Pour ce qui concerne les têtes fémorales prothétiques, elles sont dans la très grande majorité des cas, constituées par une partie d'une sphère coupée par un plan 11'. déterminant ainsi deux segments sphériques inégaux A et B (de volumes variables). Une tete fémorale prothétique est composée presque toujours du segment de sphère le plus grand des deux, dont la taille, variable, est déter- minée par les hauteurs h et h' du plan de coupe ir sur l'axe de la sphère (fig. 4).
La surface circulaire "de coupe", constituant l'embase de la tête est de situation donc variable sur l'axe XX'. Il est entendu que plus le segment réséqué à la sphère est petit, plus la stabilité de l'assemblage par cône sera en théorie solide, car plus grande sera la surface de contact (ceci restant vrai pour chaque valeur de pente du cône sans préjuger ici de l'intérêt de telle valeur, par rapport à telle autre, de cet angle de pente, qui sera retenue pour une meilleure tenue).
L'invention consiste à permettre une infinité de variations de la position de la tête céphalique par rapport au col, dans tous les plans de l'espace. L'axe de révolution YY' du cône d'emboitage femelle étant incliné d'un angle alpha sur l'axe de révolution XX' de la tête, il génère un cône de sommet P et d'angle alpha, lors du déplacement de la tête autour du col (les deux axes se croisant au point P).
Cet angle alpha peut etre variable, mais en orthopédie humaine, au niveau de la hanche, il se situe le plus favorablement entre 100 et 200 ; il est limité de toute façon par les possibilités théoriques, géométriques, d'un tronc de cône oblique dans une sphère, et surtout par l'intérêt bio-mécanique que l'on recherche en clinique humaine (et par extension, vétérinaire éventuellement).
Au niveau du plan de coupe %, la section de la tête, et la section du tronc de cône oblique, restent tout à fait concentriques et perpendiculaires à cet axe ; en effet, les centres de l'ellipse, résultant de la section légèrement oblique du tronc de cône oblique (et que l'on peut assimiler à un cercle) et celui de la section (circulaire) de la tête, peuvent être confondus. Le cône oblique d'assemblage se dirige ensuite, selon l'angle alpha, pouvant être déterminé au besoin selon l'anatomie particulière du sujet traité, vers la périphérie de la tête prothétique, s'éloignant donc, de la valeur de l'angle alpha, de l'axe médiateur de cette tete prothétique.
Précédemment les têtes fémorales prothétiques étaient munies d'une cavité tronconique axiale ne permettant aucun déport angulaire quelle que soit la position de la bille sphérique.

Claims (3)

REVENDICATIONS
1- Tête fémorale prothétique formée d'une calotte sphérique et creusée d'une cavité tronconique borgne, caractérisée en ce que l'axe de révolution YY' du cône est désaxé d'un angle alpha par rapport à l'axe de révolution de la calotte sphérique elle-même, cette désaxation permettant de donner à la tête fémorale une antéversion droite ou gauche sur une même tige fémorale.
2- Tête fémorale prothétique selon la revendication 1, caractérisée en ce qu'elle est positionnée, pour rattraper un problème de lon gueur, soit en valgus, soit en varus maximaux, cette valeur maximale correspondant à l'angle alpha, par ailleurs, toutes les positions intermédiaires entre valgus, varus, antéversion ou rétroversion étant possibles selon les besoins de l'intervention.
3- Application de la tête fémorale prothétique selon la revendication 1 ou 2xp athologie humaine ou vétérinaire.
