Abstract

L'invention concerne des ligaments avec un système de vis à os utilisées comme moyen de fixation d'une extrémité du ligament dans l'os et mises en place à l'aide d'une clé creuse. Ces ligaments et leurs vis sont destinés à des opérations du genou ou de la clavicule et ne diffèrent que par leurs dimensions et leur résistance.

Description

La présente invention consiste en deux ligaments prothétiques avec système de vis à os, capables de remplacer, pour l'un les ligaments croisés du genou, l'autre de réparer la luxation acromio claviculaire.
- Genou : la rupture la plus fréquente est celle du ligament croisé antérieur (85 % des cas), la rupture du ligament croisé posterieur plus rare (15 % des cas).
Les techniques habituelles de remplacement prothetique du ligament croisé du genou peuvent se résumer en deux chapitres.
La présente invention consiste donc sur le plan réalisation a) Ligament de genou.
Une vis à os (planche 2/4 repère 1) de diamètre extérieur ne dépassant pas 10.5mm avec un pas extérieur dit "d'artillerie" c'est à dire anti recul. Le corps de cette vis est alésé intérieurement suivant un profil cylindro conique qui passe de 5.5 pour le petit diamètre à 6.8mm pour le grand diamètre. La face interne peut être obstruée par un bouchon conique , ou ne pas être obstruée par le bouchon, dans ce cas les brins du ligament dépassent l'extrémité de la vis de lOmm environ (planche 2/4, repère 2).
L'autre face, ou externe possède 2 encoches en bout, diamétralement opposées, cela pour permettre, grâce à une clé creuse
appropriée (planche 3/4, repère 1), le vissage de la vis dans l'os. Pour mettre les encoches de la vis en regard des tenons de la clé, il est évident que le ligament doit passer à l'inté- rieur de la clé creuse (planche 3/4, repère 1).
Le ligament (planche 2/4, repère 1 et 2) est composé de 5 brins
séparés tissés selon un procédé particulier, procédé donnant une élasticité optimale pour des efforts normaux d'utilisation courante. Sa résistance à la rupture est de 260 da N - 5 da N.
Afin de limiter les effets de contact entre l'extrémité arrondie de la vis et les brins de fibre, une gaine de protection
est mise en place sur une longueur d'environ 8mm, la matière
de cette gaine est en visco élastique appartenant à la famille des polyuréthannes. Son but, non seulement limite les effets
de contact, mais élimine éventuellement des vibrations parasites dûes aux contraintes subies par les brins.
Les 5 brins sont fixés à l'intérieur du corps de la vis suivant
un procédé de serrage par vis/cheville; l'ensemble résiste à des tractions de 160 da N, ce qui est bien supérieur à la resistance d'arrachement de la vis dans l'os lui-même qui est voisine de 100 da N.
L'extrémité du ligament opposée à la vis est constituée par une partie souple réunissant les 5 brins; la longueur de cette gaine est de 12 cm; elle se termine en "queue de rat", c'est à dire que le diamètre extérieur de la gaine au ras des brins est à cet endroit maximum pour diminuer progressivement et régulièrement.
A un centimètre de l'extrémité la plus fine 4 fils de traction sont mis en place et reliés entre eux, la longueur de ces brins réunis est de 15 cm, longueur suffisante pour passer dans la clé creuse et tirer le ligament jusqu'au contact clé/vis.
b) Luxation acromio claviculaire.
Une vis à os (planche 4/4) de diamètre extérieur ne dépassant pas 6 mm, avec un pas extérieur dit d'artillerie, c'est à dire anti-recul. La conception interne de la vis est identique à celle du genou. Par contre le ligament lui-même est composé de 2 brins séparés, sa résistance à la rupture est de 150 da N + 5 da N.
L'extrémité du ligament opposée à la vis est identique à celle du 5 brins avec bien sur des diamètres de gaine adaptés aux 2 brins.
Il est par ailleurs indiqué que; la longueur maximum du système de vis à os sera de 20 mm dans le cas d'un ligament de genou et de 1-8 mm dans le cas d'un ligament d'épaule, que l'extrémité externe de la vis à os présente un arrondi entre le petit diamètre interne et le diamètre externe de-la vis.

Claims (9)

REVENDICATIONS
1 - Ligament prothétique caractérisé en ce qu'il comprend un système de
vis à os pour sa fixation et en ce qu'il comporte une gaine en ma
tèrevisoeélastique mise en place entre les brins formant le ligament
et le corps interne de la vis à os de longueur d'environ 8mm, cela
afin d'éviter les effets de contact métal/fibre.
2 -Ligament selon la revendication 1, caractérisé en ce que l'extrémité
externe de la vis à os présente un arrondi entre le petit diamètre
interne et le diamètre externe de la vis.
3 -Ligament selon 11 une des revendications précédentes caractérisé en
ce que l'extrémité interne de la vis à os reçoit un bouchon conique.
4 -Ligament selon l'une des revendications 1 et 2 caractérisé en ce que
l'extrémité interne de la vis à os reste libre, laissant dépasser les
brins du ligament vers l'extérieur.
5 -Ligament selon l'une des revendications 1 à 4, caractérisé en ce que
le système à vis présente un pas extérieur dit "d'artillerie" avec un
diamètre extérieur ne dépassant pas 10.5mm et une longueur maximum
de 20mm.
6 -Ligament selon la revendication 5, caractérisé en ce qu'il comprend 5
brins séparés, reliés entre eux dans la vis et offrant une résistance
à la rupture de 260+ 5 da.N 7 -Ligament selon la revendication 6 caractérisé en ce qu'il s'agit d'un
ligament de genou.
8 -Ligament selon l'une des revendications 1 à 4, caractérisé en ce que
le diamètre de la vis à os présente un pas extérieur dit "d'artillerie"
avec un diamètre extérieur ne dépassant pas 6mm et une longueur maximum
de 18mm.
9 -Ligament selon la revendication 8 caractérisé en ce qu'il comprend
2 brins offrant une résistance à la rupture de 150 +da N.
10 - Ligament selon la revendication 9 caractérisé en ce qu'il s'agit d'un
ligament d'épaule.
