Abstract
Description
Instrument demicroprélèvement du sang.Blood micro-collection instrument.
In French patent 81 11 074, an instrument for micro-sampling of the blood in order to carry out micro-hemocultures is described. This instrument includes a tubular body which continues into a capillary tube.
From the body leaves a bar ending in a point.
To take the sample, first prick the skin with the tip of the bar, then, by simply moving the instrument, bring the capillary tube to the pricked area, before the blood has was able to spread out and without letting go of the heel of the newborn in whom the bite was done, to drop a lancet and grab a pipette, as was the case so far. This instrument, which is simple and quick to handle and manufacture, and which can be of the type to be discarded after use, greatly reduces the danger of contamination of the micro sample of blood from the sample.
Although this instrument has already become in common use in analysis laboratories, it is criticized for not being able to demonstrate the presence of anaerobic strict bacteria which are orinin in a percentage which is not insignificant neonatal septicemia (between 5 and 15%). It is known, of course, to take blood anaerobically using a plunger syringe. But the manipulation of the instrument requires both hands, one being no longer free to hold the heel of the newborn.
The need to allow air to escape through the end of the capillary tube connected to the tubular body in the micro-sampling instrument described in French patent 81 11 074, so that the blood can rise by capillary action, appears incompatible with a blood sample under zeaerobic conditions which require that the sanq not be in contact with air.
This is however what the invention achieves, by an instrument which nevertheless retains the simplicity and the ease of handling and manufacture of that described in French patent 81 il 074.
The blood micro-sampling instrument according to the invention comprises a tubular body which continues into a capillary tube and a bar which starts from the body and which is terminated by a point. A ball, with a diameter less than that of the body but greater than that of the capillary tube, is freely engaged in the body.
The micro-sampling is carried out by pricking the skin using the point, then by putting the free end of the capillary tube in contact with blood which bead at the place of the skin which has been pricked, while the tubular body is at a level lower than that of the tube, and therefore that the ball is distant from this end, so that air can escape to allow blood to enter by capillarity into the capillary tube, then, once that enough blood has entered the capillary tube, inverting the instrument so that the ball comes to close the end of the capillary tube turned towards the body, so that the blood is no longer in contact with l air, the lower end of the tube being closed by the cap of the anaerobic blood culture bottle during the operation of filling the bottle with blood contained in the capillary tube. Thanks to the fact that the ball is freely mounted in the body , it is possible by the simple orientation of the instrument , which can be performed with one hand, to establish communication between the capillary tube and the tubular body, to allow air to escape during capillary sampling, and to interrupt this communication then by a simple wrist movement.
According to an improvement, a grid is fixed to the body on the side of the end thereof which is distant from the capillary tube. The mesh of the grid is smaller than the diameter of the ball, so that it cannot exit the tube, but that air can pass freely.
In the accompanying drawing, given only by way of example
FIG. 1 is a sectional view of the instrument according to the invention in the blood collection position, and
Figure 2 is a sectional view of this instrument, while the ball closes the capillary tube in an airtight manner.
The instrument comprises a tubular body 1, made of glass, open at one end, but blocked by a cotton pad 2 which is engaged therein. The tubular body continues into a capillary tube 3. The whole of the body 1 and of the tube 3 constitute a conventional Pasteur pipette.
From the body leaves a strip 4 made of solid glass.
The bar 4 is welded at 5 to the outer wall of the body l. The bar comprises a section 6, which starts from the spot 5 of welding and which is followed, along an elbow, by a section 7 substantially parallel to the tube 3 and going in the same direction as the latter, from the body l.
At the end of the bar 4, the latter opens out into a tubular opening 8 for receiving a lancet 9 with a tip 10. The lancet body 9 has the same length as the opening 8. The tip 10 and the end of the tube 3 are substantially in the same plane.
In the tubular body 1 can freely move a ball 11 of plastic or steel having a diameter less than that of the body 1. The diameter of the ball 11 is greater than that of the tube 3. The connection between the body 1 and the tube 3 is effected by a conical part which can form a seat for the ball 11.
On the side of the end of the body 1 where the cotton pad 2 is located, a grid 12 is forcibly mounted, the mesh size of which is less than the diameter of the ball 11.
To take the blood sample, the instrument is placed in the position shown in FIG. 1, in which the tube 3 is at a level higher than the body 1, and therefore in which the ball is moved away from its seat.
L'aspiration du sang par capillarité peut ainsi s'effectuer, puisque l'air peut être refoulé du tube 3 à l'extérieur, en passant par le corps 1.Aspiration of blood by capillary action can thus be carried out, since the air can be discharged from the tube 3 outside, passing through the body 1.
As soon as the sample is taken, the instrument is turned over by a simple movement of the wrist to put it in the position shown in FIG. 2, in which the ball 11 is applied to its seat in an airtight manner, and thus protects the blood drawn in the tube 3 from any contact with air.
The blood contained in the capillary tube is then aspirated by the vacuum of the anaerobic blood culture flask. The ball prevents the penetration of air from the pipette body into the capillary tube, and therefore into the anaerobic blood culture bottle; it therefore allows a strict anaerobic culture.
The following analyzes illustrate the invention. Three chickens are injected with 1 ml of a culture containing 109
Bacteroldes fragilis which is a strict anaerobic germ.
30 minutes, 6G minutes et 120 minutes après l'injection de cet inoculum bactérien, on effectue chaque fois et sur chaque poulet, 9 prélèvements, les trois premiers par un prélèvement veineux classique, à l'aide d'une seringue à piston, les trois suivants à l'aide d'un instrument de prélèvement suivant le brevet français 81 il 074, et les trois derniers à l'aide d'un instrument suivant l'invention.30 minutes, 6G minutes and 120 minutes after the injection of this bacterial inoculum, 9 samples are taken each time and from each chicken, the first three by a standard venous sample, using a plunger syringe, the three following using a sampling instrument according to French patent 81 il 074, and the last three using an instrument according to the invention.
Chaque prélèvement est de 100 iil. Each sample is 100 iil.
The samples are introduced respectively into anaerobic culture flasks. The incubation lasts 48 hours and is carried out at 370C in the oven. At the end of the oven, we look to see if a culture has developed. If so, it is then verified, by a conventional technique, that the germ which has sprouted is indeed Bacteroides fragilis.
All venous samples were positive for Bacteroides fragilis. No sampling carried out using the instrument described in French patent 81 11 074 has made it possible to demonstrate the sampling of Bacteroïdes fragilis. This presence was noted in eight pre-remittances out of the nine made using the instrument according to the invention.
The poor results of the instrument of French patent 81 11 074 are explained by the penetration of air simultaneously with that of the blood during the aspiration of the latter by the vacuum of the anaerobic blood culture flask. This air penetration destroys strict anaerobic germs and prevents their cultivation. On the other hand, the ball which prevents any penetration of air into the anaerobic blood culture bottle, allows the culture of anaerobic germs, such as Bacteroides fragilis.
