Abstract
Description
The present invention relates to a device intended to improve the positioning of the patient on a surgical operating table, in particular for operations to be performed in the lumbar region.
It is known that, for most surgical operations relating to the spine, the patient must be anesthetized in a position where he is practically on all fours. For this, there are special operating tables having a form called "in staircase ". In fact, these special tables are very expensive and are only found in a small number of very specialized hospitals. On the other hand, when the surgeon operates in an establishment which is not specialized only in this type of operations, he only has a flat and rigid table, of which only the overall inclination is adjustable. to anesthetize the patient on all fours on such a flat table, it is therefore necessary to wedge him, with more or less makeshift means which have various drawbacks.
First of all, the patient's position remains poorly defined, and he is not rigorously immobilized during the intervention. On the other hand, we note that at least part of his abdomen is more or less compressed by cushions, which has the effect of increasing the blood pressure in his vena cava, so that the wound then practices by the surgeon in the vicinity of the spine is bleeding excessively, which compromises the proper conduct of the intervention. Finally, the patient being often led to occupy false positions during anesthesia it is common that upon waking, he suffers from various pains, in particular cervical. Finally, such positioning of the patient deprives the surgeon of support points, which bothers him during the operation.
The object of the present invention is to avoid these drawbacks, by providing a positioning device, easily adaptable to a standard operating table, to improve both the quality of the operation and the comfort of the surgeon as well. during the intervention, only from the patient upon awakening
A positioning device according to the invention, intended to be adapted on a surgical operating table of the standard type, is characterized in that it comprises - on the one hand, a rigid bridge in the form of a lower sole
support on the table, with, on each side, a
adaptable spindle in the exis quick clamp mechanism
both sides of the table, this sole being
surmounted by a U-shaped profile seat, intended for
fit under the patient's buttocks, and on both sides
to immobilize his pelvis; - on the other hand, at least one block of foam forming a cushion,
the upper part of which is intended to support the weight
from the patient's chest, over a corresponding length
at the length of its sternum, whereas on the contrary, the
upper part of this foam block is largely interchangeable
creates, in the whole area corresponding to the abdomen of the pa
is holding
Thanks to this arrangement, it is certain that, when the patient is in a position on all fours, his abdomen hangs completely, without being in any way compressed, which is essential to avoid any bleeding in the wound that the surgeon will practice in the vicinity of the column. breast worm
According to another characteristic of one invention, the bridge is constituted by a welded and braced tubular structure, terminated by two uprights which, behind the seat, and against it, are open upwards, which makes it possible to engage, removably, two pins secured to the underside of the front edge of a tablet. Thus, during the operation, the surgeon has against the patient's buttocks, a tablet on which he can lean, or place his instruments.
According to another characteristic of the invention, the device furthermore comprises a block of foam forming a cushion, the upper face of which has a notch, capable of receiving the patient's forehead, to serve as a support, which Thus positively maintains the patient's head in a natural position, for the duration of the operation. Thanks to this arrangement, it can be seen that upon awakening, the patient does not suffer from neck pain, as is too often the case with known devices.
According to another characteristic of one invention, the tubular bridge has no cross member below the seat, which allows to accommodate between the two uprights which carry the latter, a thick transverse cushion, placed on the operating table, to serve as support for the patient's neck, without his toes abutting against the operating table. This provision is important, to also contribute to removing pains from which the patient could suffer when waking up.
The accompanying drawing, given by way of nonlimiting example, will allow a better understanding of the characteristics of the invention.

Figure 6 represents the immobilized patient, ready to be anesthetized for the operation.
A standard type surgical operating table is shown in the drawings, which includes
in known manner, a tray 1 carried by a base 2
with respect to which its inclination is adjustable. This table 1, 2 comprises various usual accessories, not shown, among which, on each side of the plate 1, a quick-tightening mechanism 3, capable of receiving and holding in place a spindle which is just adapted there by introduction from top to low
In the case of the present invention, two pins 4 are provided to fit each in the corresponding mechanism 3.
Each vertical spindle 4 is integral with a horizontal arm 5 in which it is adjustable by sliding in a transverse direction relative to the plate 1 of the table. After adjusting a spindle 4 by telescoping in its arm 5, it is immobilized in the desired position by tightening screws 6
Each arm 5 being tubular, it is provided with a second locking screw 7, making it possible to immobilize it by its opposite end, on a transverse post 8 which is provided with an easel 9. This bridge 9 is constituted by a welded and braced tubular structure, comprising in particular two uprights 10, and two lower spars 11. The base of each upright 10 is connected to the front of a spar 11, and to the corresponding tenon 8, these three elements then being oriented in the direction of the three axes of a tri-rectangle trihedron
Spacers 12 ensure the rigidity of the assembly of each upright 10 with its spar 11 while a rear cross member 13 joins the rear of the two spars ll
The width 14 of the easel thus produced, that is to say the spacing of the two longitudinal members 11, is slightly less than the width 15 of the table of the operating table 1, 2
Against the front of the easel 9, and near the upper part of the uprights 10, is fixed a seat 16, formed by a rigid plate folded along a U-shaped profile. This plate thus defines a bottom 17 directly fixed against the uprights 10, and two lateral wings 18, projecting towards the front of the easel 9
Note in FIG. 1, that the gerons lanyards 11, the crosspiece 13, and the tenons 8 constitute a sole intended to be placed on the plate 1 of the operating table, while no crosspiece exists between the two let's hold 8
The tubular uprights 10 are open at their top, which allows them to receive removably, two pins 19, integral with the front edge of a rigid shelf 20, below which it protrudes
Three cushions ~, namely a central cushion 21 and two material cushions 22, are provided for vehir apply against the inner faces of the panels 17 and 18, as will be seen later.
The device according to the invention also comprises two foam blocks 23 and 24, the anatomical shapes of which are well defined.
The first block 23 has at its upper part, a wide notch 25, open towards the rear, under the location which, after fitting, will correspond to the upper part of the abdomen 26 of the patient 27
On the other hand, in front of the notch 25, and over a length 28 corresponding to the length of the patient's sternum 27, the first foam block 23 has a bearing face 29, provided to support the patient's chest 27
The second block of foam 24 comprises, on the rear part of its upper face, a notch 30, also open, towards the rear, to receive the forehead 31 of the patient 27. The height and dimensions of the cond block 24 are provided so as to wedge the patient's head 27, while keeping it in a natural position, that is to say in the extension of the body, as illustrated in FIG. 6
Finally, to facilitate the use of the first block 23, there are provided on its upper face and at the front, two lateral notches 34, provided to allow the arms 32 of the patient to pass
The operation is as follows:
To prepare the operating table 2, we begin by tilting its tray 1 to the orientation illustrated in Figures 5 and 6. The sole 11, 13, of the easel 9 is then placed on the lower part of the table, the pins 4 being engaged and tightened in the quick-tightening mechanisms 3. The shelf 20 is put in place, the pins 19 of which are engaged in the upper part of the uprights 10
On the upper part of the tray 1, the first block 23 is successively placed, then the second block 24
The patient 27 is then installed in the four-legged position illustrated in FIG. 6. For this, the seat 16 being placed under his buttocks, the cushions 21 and 22 are put in place, to wedge the patient between the two lateral wings 18
Furthermore, its forehead 31 is supported in the notch 30 of the second block 24, which maintains its head in a natural position, in the extension of the body this arrangement is important, because, upon awakening, it will prevent the patient from suffering from neck pain .
Finally, the first block 23 supports, by its support face 29, the weight of the thorax, while the abdomen 26 hangs entirely, including in the space of the notch 25. As previously indicated, this feature is important, to avoid any bleeding when the surgeon incises the patient's back 27, in the lumbar region.
Furthermore, pn places under the neck of the patient's feet 27, a transverse cushion 33, sufficiently high, to prevent the toes t5. are curled up against tray 1 of the table. This provision is also important to avoid pain on waking.
The patient being thus positioned (FIG. 6), his arms 32 pass into the notches 34 on either side of the first block of foam 23. He is then anesthetized, and the surgeon begins his work. For this, it is found that the device according to the invention makes available to it the tablet 20, which is convenient for it, both to support itself, or to store its instruments.
Of course, each block 23 and 24 can be produced in two pieces, 36, 37 and 38, 39, the lower parts 37 and 39 being interchangeable, to allow the height to be adjusted more finely.
