Abstract
Description
Injectable lipoprotein emulsions, their preparation process and their therapeutic applications.
The present invention relates to injectable lipoproteic emulsions, an original process for the preparation of these emulsions, and their applications for treating pathological conditions in relation in particular to nutritional problems.
We know in this area the role of certain lipids, amino acids and carbohydrates.
It is known to use venous infusion of formulations of nutrients containing in particular such elements.
Such treatments are generally of long duration, and the injectable nature of the formulations by venous imposes strict requirements which are known.
These requirements are not all resolved at present, either because of the products used or of the injectable formulations which one is capable of producing, or because of the known preparation processes which do not make it possible to achieve all the characteristics which would be desirable or required.
There are thus encountered in practice significant or even serious problems, which are well known.
Examples include - the fact that each lipid, amino acid and carbohydrate solute
finds separate:
emulsified soybean oil,
or glucose or lactose solution,
or solution of amino acids at low concentration,
they can be injected simultaneously, but do not mix - the fact that lipid emulsions, to remain stable, must
between kept cold; they are not autoclavable - the fact that amino acids cause headache and
liver problems; they do not allow the maintenance of power
liver lipoprotector - the fact that lipids form larger diameter droplets
laughs at 10 microns and sometimes causes capil obstructions
milk by agglutination; - the fact that the veins do not tolerate repeated infusions of
glucose which must be injected frequently, due to its consumption
immediate and total donation by the body, thus not allowing
energy coverage extended over time.
These problems have been posed for many years with greater or lesser acuity depending on the patient's condition, the duration of treatment, etc., but, despite the interest there would be in solving them, research work n have so far been unsuccessful.
The invention makes it possible to propose lipoprotein formulations injectable by venous route, not presenting the drawbacks mentioned above.
- possibilité d'incorporer toutes sortes de lipides permettant de résoudre des problèmes énergétiques ou plastiques (phospholipides et acides gras indispensables) ;
- la présentation de l'ensemble lipides-proteines se fait sous la forme de lipoprotéines analogues aux lipoproteines naturelles ;;
- apport de glucides simples ou polymérisés
- les conditions physicochimiques du produit permettent un équilibre biologique du milieu analogue à celui du plasma (isotonicité, pH = 7,35, pouvoir tampon) ;
- apport calorique suffisant pour couvrir en une injection les besoins d'une journée ;
Thus, in particular, the invention leads to formulations whose main characteristics are the following
- possibility of incorporating into the solute, at the time of manufacture, either liposoluble substances or water-soluble substances, the whole constituting a complete and physiological solute
- possibility of incorporating all kinds of lipids to solve energy or plastic problems (phospholipids and essential fatty acids);
- plastic intake of tolerated non-sensitizing proteins, usable by the body without causing disorders as the amino acids would have done
- The presentation of the lipid-protein package is in the form of lipoproteins analogous to natural lipoproteins;
- intake of simple or polymerized carbohydrates
- the physicochemical conditions of the product allow a biological equilibrium of the environment similar to that of plasma (isotonicity, pH = 7.35, buffering capacity);
- sufficient caloric intake to cover the needs of a day in one injection;
- the solute is autoclavable and can be stored at room temperature
- lack of awareness of the proteins injected.
The practitioner will not fail to be surprised by the number of serious problems solved simultaneously and by the fact that the proposed solute is autoclavable and can be stored at room temperature.
It was known in fact that a passage in the autoclave causes in particular, in current formulations, the hydrolysis of lipids.
We also knew that there was no question of keeping products of the type considered out of a refrigerated cabinet.
The preparation process according to the invention, which is original and will be described below, makes it possible to obtain the results which have been indicated, as well as other results which have not been possible to date, such as in particular particles of diameter less than 0.1 / u, and the incorporation from the manufacturing stage of many useful ingredients, according to several possible variants.
The Applicant has therefore developed compositions of the above type, having certain specific characteristics.
The presence of sodium bicarbonate at approximately 2.26 g / l is essential.
It is imperative to respect a content of 0.40 g / l of potassium chloride (except for a specific formulation: this rate can be exceeded for applications in the event of cardiac disorders, for example 1.8 g / l).
The protein level can also be between 55 and 65 g / l.
A casein level of 60 g / l will preferably be chosen.
On the other hand, greater latitude can be granted with regard to energetic substances: carbohydrates and lipids, up to a maximum of 40 g / l of lipids. The recommended sugar levels are 5 to 10% for example, and the trace element levels lower than 10 / co.
- Vitamins and hormones, liposoluble and water-soluble: usual contents known to those skilled in the art.
