Abstract
Un implante (10) para columna vertebral que comprende: un cuerpo que tiene una superficie (16) superior, una superficie (18) inferior, una porción (12) anterior, una porción (14) posterior, un par de paredes laterales (20) sustancialmente planas que se extiende en paralelo entre las superficies superior e inferior, y una abertura (24) central que se extiende a través de las mismas entre las superficies superior e inferior para facilitar el crecimiento interno óseo, a través de las mismas, caracterizado porque la superficie (16) superior y la superficie (18) inferior no son paralelas de modo que uno de los pares de paredes laterales tiene una altura mayor que la otra pared lateral para formar un cuerpo en forma de cuña, configurado para inserción lateral entre cuerpos vertebrales de la columna vertebral de un paciente, comprendiendo la porción (12) anterior un borde delantero formado por paneles que se extiende desde las superficies superior e inferior con un ángulo relativo entre sí y convergiendo en una boquilla afilada (22) para permitir la separación concomitante de tejido blando durante la inserción; y una o más aberturas (26) está dentro de la porción (14) posterior del cuerpo para recibir al menos un elemento (40) de fijación, en el que al menos una de las aberturas (26) está configurada para permitir una cantidad predeterminada de nutación por un elemento de fijación (40).

De manera aún más particular, la presente divulgación se relaciona con un implante de fusión para columna vertebral configurado para inserción lateral.
Antecedentes
La integridad de la columna vertebral, incluidos sus subcomponentes, como los cuerpos vertebrales y los discos intervertebrales, que son partes estructurales bien conocidas del cuerpo que forman la columna vertebral, son clave para la salud del paciente. Estas partes se pueden aplastar o dañar como resultado de un trauma o lesión, o se pueden dañar por enfermedad (por ejemplo, por tumor, enfermedad autoinmune), o como resultado del desgaste a lo largo del tiempo o la degeneración que se causa por el proceso de envejecimiento normal.
En muchos casos, una o más partes estructurales dañadas del cuerpo se pueden reparar o reemplazar con una prótesis o implante. Por ejemplo, específico para la columna vertebral, un método de reparación es eliminar la vértebra dañada (en su totalidad o en parte) y/o el disco dañado (en su totalidad o en parte) y reemplazarlo con un implante o prótesis. En algunos casos, es necesario estabilizar una región vertebral debilitada o dañada al reducir o inhibir la movilidad en el área para evitar una mayor progresión del daño y/o reducir o aliviar el dolor que se causa por el daño o lesión. En otros casos, es deseable unir las vértebras dañadas y/o inducir la curación de las vértebras. En consecuencia, un implante o prótesis se puede configurar para facilitar la fusión entre dos vértebras adyacentes. El implante o prótesis se puede colocar sin medios de fijación o fijar en posición entre partes estructurales adyacentes del cuerpo (por ejemplo, cuerpos vertebrales adyacentes).
Típicamente, un implante o prótesis se asegura directamente a una estructura ósea por medios mecánicos o biológicos. Una forma de reparación de la columna vertebral consiste en unir un implante de fusión o prótesis a cuerpos vertebrales adyacentes al usar un elemento de fijación, tal como un tornillo. La mayoría de los implantes y sus medios de fijación se configuran para proporcionar una fijación rígida inmediata del implante al sitio de implantación. Desafortunadamente, después de la implantación, los implantes tienden a hundirse o asentarse en el ambiente circundante a medida que el peso del paciente se ejerce sobre el implante. En algunos casos, este hundimiento puede provocar que los medios de fijación se fijen rígidamente, suelten, desplacen o dañen potencialmente uno o más de los cuerpos vertebrales.
Se pueden usar varias técnicas quirúrgicas conocidas para implantar una prótesis para columna vertebral. La idoneidad de cualquier técnica particular puede depender de la cantidad de acceso disponible para el sitio del implante. Por ejemplo, un cirujano puede elegir una vía de entrada particular dependiendo del tamaño del paciente o la condición de la columna vertebral del paciente, como en el caso en que se presente un tumor, tejido cicatricial u otro obstáculo. Otras veces, puede ser deseable minimizar la intrusión en la musculatura del paciente y el tejido ligamentoso asociado. En algunos pacientes que han tenido cirugías previas, los implantes o los elementos de fijación ya se pueden haber insertado en la columna vertebral del paciente y, como tal, una vía de introducción del implante puede ser tenida en cuenta para estas condiciones previas existentes.
Por lo tanto, es deseable proporcionar un implante que se pueda insertar fácilmente en concordancia con una vía o abordaje específicos. Por ejemplo, en ciertas situaciones, es deseable proporcionar un implante para columna vertebral que se pueda insertar usando un abordaje lateral. Además, es deseable proporcionar un implante que se configure para reducir las fuerzas de inserción. Además, es deseable proporcionar un implante y elementos de fijación asociados que puedan dar cuenta del hundimiento que se produce con el implante después de la implantación, a la vez que proporciona también una fijación rígida.
Aunque la siguiente discusión se centra en implantes o prótesis para columna vertebral, se apreciará que muchos de los principios se pueden aplicar igualmente a otras partes estructurales del cuerpo dentro de un cuerpo humano o animal.
El documento WO 2008/102174 A2 divulga implantes para columna vertebral modulares que tienen componentes que se interconectan entre sí para formar un único implante, en particular implantes que se seccionan a lo largo de un plano longitudinal. Se divulgan implantes que incluyen canales para la asociación interfragmentaria con un tornillo óseo alargado y que permiten la variabilidad angular del tornillo con respecto al canal.

Con referencia ahora a la Figura 1, se muestra un implante 10 para columna vertebral de la presente divulgación. El implante 10 para columna vertebral se puede implantar en el espacio 6 intervertebral entre los cuerpos 2, 4 vertebrales y fijarse a los cuerpos 2, 4 vertebrales con tornillos 40 de fijación. El implante 10 para columna vertebral se puede emplear en las regiones lumbar o torácica. Alternativamente, el implante 10 para columna vertebral se puede emplear en la región cervical de la columna, de manera similar a la que se describe para el implante cervical de la Solicitud de Patente de Estados Unidos N° 11/938.476 presentada el 12 de noviembre de 2007, y se titula "Implantes y prótesis ortopédicas". Se puede proporcionar una versión cervical siempre y cuando se dimensione y configure adecuadamente, y el abordaje quirúrgico tenga en cuenta este diseño específico.
Como se muestra en las Figuras 2A-2D, el implante 10 para columna vertebral puede incluir porciones 12, 14 anterior y posterior y superficies 16, 18 superior e inferior que se perfilan para corresponder con el perfil de cualquier material óseo al que se van a asegurar. Un par de paredes 20 laterales se extiende entre las superficies 16, 18 superior e inferior y se conecta a las porciones 12, 14 anterior y posterior. El implante 10 para columna vertebral puede incluir una abertura central o lumen 24 que se extiende entre las superficies 16, 18 superior e inferior para facilitar el crecimiento o la fusión ósea entre los segmentos óseos adyacentes, tales como los cuerpos 2, 4 vertebrales. Por ejemplo, la porción 12 anterior puede ser una punta, una punta en forma de bala o boquilla 22. La geometría única del implante 10 que incluye esta boquilla 22 afilada soporta fuerzas de inserción reducidas, y puede ayudar también a separar el tejido durante la inserción. Esto puede ser útil, por ejemplo, cuando hay cicatrices u otras obstrucciones en el sitio de implantación, o donde hay estenosis y/o alguna otra anomalía anatómica, como cuando las placas terminales han crecido juntas.

La porción posterior o extremo 14 trasero del implante 10 para columna vertebral incluye orificios 26 para recibir elementos de fijación, tales como tornillos 40 óseos. En la realización que se muestra, el implante 10 para columna vertebral incluye dos orificios 26 para tornillo, uno que se extiende de manera superior y otro que se extiende de manera inferior (véase en particular la Figura 2D). Sin embargo, un experto en la materia apreciará que el implante 10 puede comprender cualquier número de orificios en cualquier ubicación en el implante 10. Por ejemplo, en lugar de tener un orificio superior y un orificio inferior en el implante 10, como se muestra en los dibujos, el implante puede tener dos orificios superiores, o se puede adaptar para tener dos orificios inferiores. Como se discutió anteriormente, el implante 10 se puede configurar con cualquier cantidad de orificios sin apartarse del espíritu de la divulgación. Además, el implante 10 puede incluir perforaciones 50 para recibir características como un marcador radiológico u otro marcador 70 de formación de imágenes, como se muestra en la Figura 7. El marcador 70 de formación de imágenes puede tener forma de varilla, por ejemplo, para su inserción en las perforaciones 50.
Los orificios 26 proporcionan un recorrido a través del cual se pueden insertar medios de fijación (por ejemplo, elementos de fijación tales como tornillos 40 óseos) para asegurar el implante 10 a cuerpos 2, 4 vertebrales superior e inferior respectivos. Los orificios 26 se pueden configurar para acomodarse a una variedad de medios de fijación, tales como tornillos, pasadores, grapas o cualquier otro dispositivo de sujeción adecuado. En una realización, los tornillos 40 de fijación pueden ser de autorroscado y/o autoperforación y pueden ser de tipo de tornillo óseo, tales como los que los expertos en la técnica conocen bien. En algunas realizaciones, la porción 42 de cabeza de los tornillos 40 de fijación se extiende dentro de un cuerpo 44 alargado que termina en una boquilla 46. Mientras que el implante 10 se muestra con tornillos 40 que se dimensionan y conformados para fijación ósea unicortical, se contempla que se puede emplear también, con la presente divulgación, tornillos óseos que se dimensionan y conforman para la fijación ósea bicortical.
Los orificios 26 del implante 10 para columna vertebral se pueden configurar para permitir una cantidad predeterminada de alternancia de tornillo (es decir, inclinación angular) y permitir un efecto de retardo cuando el tornillo 40 de fijación se inserta y reside dentro del orificio o lumen 26. En otras palabras, los orificios 26 permiten un cierto grado de nutación por el tornillo 40 y, por lo tanto, los tornillos 40 pueden alternar de una posición a una o más posiciones diferentes, por ejemplo, durante el hundimiento. Como se representa en la Figura 5, los orificios 26 se pueden configurar con un rango de movimiento cónico (es decir, distancia angular) de aproximadamente 25 a aproximadamente 35 grados, aunque se contempla que sea posible un rango incluso mayor, tal como 20 a 40 grados, o 15 a 45 grados. En una realización, el rango es de aproximadamente 22 a 28 grados. Se cree también que la alternancia de tornillo predeterminada (que se permite por la distancia entre el lumen, o el orificio 26 y el tornillo 40) promueve el bloqueo del tornillo 40 al implante 10 después del hundimiento posterior a la implantación. Alternativamente, los orificios 26 del implante 10 se pueden configurar con poca o ninguna distancia para lograr la fijación rígida, por ejemplo, cuando el implante 10 se va a implantar en el hueso esclerótico.
Como se muestra en la Figura 2C, el implante 10 para columna vertebral puede tener superficies 16, 18 superior e inferior no paralelas para formar un implante 10 en forma de cuña. Sin embargo, un experto en la técnica apreciará que el implante 10 para columna vertebral se puede proveer también de superficies 16, 18 superior e inferior paralelas. El implante 10 para columna vertebral puede tener cualquier forma o tamaño que se adecúa para permitir su uso en condiciones de lordosis o cifosis. Por ejemplo, en un ejemplo, el implante 10 para columna vertebral puede tener un perfil de lordosis de 12 grados desde una vista anteroposterior (A-P).
En algunas situaciones, después de la inserción en el cuerpo vertebral, el elemento de fijación o tornillo 40 se puede aflojar y/o retroceder, es decir, retirarse del cuerpo vertebral. La consecuencia de retroceder o aflojar incluye una fusión inadecuada o incompleta, pérdida de estabilidad, riesgo potencial para el paciente y una cirugía de revisión por separado, costosa y frecuentemente dolorosa. La Figura 3 muestra un tornillo 40 de fijación de la presente divulgación, junto con un elemento antirretroceso para evitar tales problemas. Como se ilustra, el elemento antirretroceso comprende un anillo 60 dividido que actúa como un anillo de bloqueo. La Figura 4 muestra el anillo 60 dividido que reside entre la cabeza 42 del tornillo 40 de fijación y el orificio 26 del implante 10 para columna vertebral.
El implante 10 para columna vertebral y sus componentes se pueden formar de cualquier material adecuado de grado médico, tales como metales biocompatibles como acero inoxidable, titanio, aleaciones de titanio, etc. o un plástico de grado médico tal como la polieteretercetona (PEEK) u otro material radiotransparente, polietileno ultra de alto peso molecular (UHMWPE), etc. Si así se desea, el implante 10 se puede formar también de un material biorreabsorbible. El material biorreabsorbible puede ser preferentemente osteoconductivo u osteoinductivo (o ambos).
En un método de ejemplo de inserción del implante 10 para columna vertebral, el cirujano prepara el sitio de implantación retirando parte del material discal del espacio 6 discal entre dos vértebras 2, 4 adyacentes. Debido a que el implante 10 para columna vertebral se configura para inserción lateral (es decir, de manera opuesta a la inserción en la línea media), se necesita eliminar menos material discal para acomodar el implante 10, que tiene un perfil delgado. Esto proporciona el beneficio de preservar más de la anatomía natural del paciente y, más específicamente, preserva los tejidos blandos y el ligamento que rodea el sitio. El implante 10 para columna

vertebral se puede proporcionar al cirujano con los tornillos 40 preajustados, o por separado, según se desee. Después de que el cirujano coloca el implante 10 en la ubicación que se desea, tal como la región cervical de la columna vertebral de un paciente, el cirujano puede apretar los tornillos 40 en el tejido óseo circundante, asegurando así el implante 10.
Como se indicó, el implante 10 se puede configurar para permitir una cantidad predeterminada de alternancia de tornillo y permitir un efecto de retardo cuando el tornillo 40 de fijación se inserta y reside dentro del orificio o lumen 26. Al apretar, se puede observar el efecto de retardo mediante el cual el implante 10 saca tejido óseo hacia sí mismo, lo que puede promover una mejor fusión.
Como se indicó adicionalmente, la alternancia de tornillo predeterminada promueve bloqueo del tornillo 40 con el implante 10 después del hundimiento posterior a la implantación. Por ejemplo, después de la cirugía, el movimiento natural del paciente dará como resultado el asentamiento y el hundimiento del tejido óseo y el implante 10 in situ. Se cree que, durante este proceso, el peso que se ejerce sobre el implante 10 hace que los tornillos 40 de fijación se alternen y se bloqueen eventualmente contra una o más superficies de los orificios 26 del implante 10.
Algunos médicos prefieren permitir cierto grado de movimiento entre el implante y el cuerpo vertebral adyacente después de la implantación. En ese caso, las cabezas 42 de tornillo se pueden proveer, en su lado inferior, de contornos que permiten a los tornillos 40 alternar con respecto a la abertura 26 contorneada del implante 10. Otros médicos pueden preferir un implante más rígido que se bloquee firmemente con el cuerpo vertebral adyacente. Las realizaciones del implante 10 permiten cualquier preferencia.
En la versión que se fija rígidamente, los tornillos 40 se pueden proporcionar sin el contorno en su cara inferior (es decir, un lado inferior relativamente plano) mientras que la abertura 26 del implante 10 no incluiría tampoco un contorno. Por lo tanto, cuando se aseguran juntos, los tornillos 40 y el implante 10 forman una construcción que se bloquea rígidamente. Cuando se desea una fijación rígida (es decir, sin alternar), la parte inferior de los tornillos 40 puede incluir también características de superficies para proporcionar una fijación segura al implante 10.
Mientras se describen una alternancia y una versión que se fija rígidamente del implante 10 y los tornillos 40, se entiende que se puede lograr una combinación de alternancia y fijación rígida en un único implante 10. Por ejemplo, es posible proporcionar un implante 10 que permite alternar uno o más tornillos 40, mientras que permite también la fijación rígida del otro de los tornillos 40.
Se apreciará también que el posicionamiento angular de los diversos orificios, como se describió anteriormente, permite que el implante 10 presente sea de un tamaño relativamente pequeño y, por lo tanto, insertable desde un acceso lateral dentro de los espacios intervertebrales de la columna vertebral. Por lo tanto, se apreciará que el posicionamiento angular de los orificios es importante para la operación efectiva del implante 10 y la capacidad de "apilar" implantes en procedimientos multinivel adyacentes sin que los medios de fijación interfieran entre sí, lo que puede ser de gran importancia en algunas situaciones.
Como se ilustra, el implante 10 para columna vertebral puede tener cualquier variedad de características superficiales, tales como características antimigración y de unión al hueso. Por ejemplo, se contempla que el implante pueda tener hilos, dientes, púas, rugosidades superficiales, etc. para ayudar en la unión al hueso. Además, se pueden emplear agentes biológicos tales como factores de crecimiento óseo, por ejemplo, para mejorar la unión al hueso. Las Figuras 6A y 6B ilustran realizaciones a modo de ejemplo de un implante 10 para columna vertebral de la presente divulgación que tiene dientes 30 en las superficies 16, 18 superior e inferior para evitar la migración después de la implantación y mejorar la fijación al tejido óseo.
Otras realizaciones de la divulgación serán evidentes para los expertos en la técnica a partir de la consideración de la especificación y la práctica de la divulgación que se proporciona en este documento. Se pretende que la especificación y los ejemplos se consideren solo como ejemplos.
Claims (13)

Un implante (10) para columna vertebral que comprende:un cuerpo que tiene una superficie (16) superior, una superficie (18) inferior, una porción (12) anterior, una porción (14) posterior, un par de paredes laterales (20) sustancialmente planas que se extiende en paralelo entre las superficies superior e inferior, y una abertura (24) central que se extiende a través de las mismas entre las superficies superior e inferior para facilitar el crecimiento interno óseo, a través de las mismas, caracterizado porque la superficie (16) superior y la superficie (18) inferior no son paralelas de modo que uno de los pares de paredes laterales tiene una altura mayor que la otra pared lateral para formar un cuerpo en forma de cuña, configurado para inserción lateral entre cuerpos vertebrales de la columna vertebral de un paciente, comprendiendo la porción (12) anterior un borde delantero formado por paneles que se extiende desde las superficies superior e inferior con un ángulo relativo entre sí y convergiendo en una boquilla afilada (22) para permitir la separación concomitante de tejido blando durante la inserción; yuna o más aberturas (26) está dentro de la porción (14) posterior del cuerpo para recibir al menos un elemento (40) de fijación, en el que al menos una de las aberturas (26) está configurada para permitir una cantidad predeterminada de nutación por un elemento de fijación (40).
- 2. El implante para columna vertebral de la reivindicación 1, que comprende además una o más perforaciones (50) para recibir un marcador (70) de formación de imágenes.
- 3. El implante para columna vertebral de la reivindicación 2, en el que el marcador (70) de formación de imágenes tiene forma de varilla.
- 4. El implante para columna vertebral de la reivindicación 1, en el que al menos una de las aberturas (26) está configurada para permitir aproximadamente 25 a aproximadamente 35 grados de nutación por el elemento de fijación (40).
- 5. El implante para columna vertebral de la reivindicación 1, en el que al menos una de las aberturas (26) está configurada para permitir aproximadamente 20 a aproximadamente 40 grados de nutación por el elemento de fijación (40).
- 6. El implante para columna vertebral de la reivindicación 1, en el que al menos una de las aberturas (26) está configurada para permitir aproximadamente 15 a aproximadamente 45 grados de nutación por el elemento de fijación (40).
- 7. El implante para columna vertebral de la reivindicación 1, en el que al menos una de las aberturas (26) está configurada para permitir aproximadamente 22 a aproximadamente 28 grados de nutación por el elemento de fijación (40).
- 8. El implante para columna vertebral de la reivindicación 1, en el que una o más aberturas (26) comprenden una abertura superior que se extiende de manera superior y una abertura inferior que se extiende de manera inferior.
- 9. El implante para columna vertebral de la reivindicación 8, en el que la abertura superior está configurada para permitir aproximadamente 15 a aproximadamente 45 grados de nutación por un primer elemento de fijación y la abertura inferior está configurada para fijarse rígidamente a un segundo elemento de fijación.

El implante para columna vertebral de la reivindicación 8, en el que la abertura inferior está configurada para permitir aproximadamente 15 a aproximadamente 45 grados de nutación por un primer elemento de fijación y la abertura superior está configurada para fijarse rígidamente a un segundo elemento de fijación.
- 11. El implante para columna vertebral de la reivindicación 1, que está configurado además para tener un perfil de lordosis.
- 12. El implante para columna vertebral de la reivindicación 1, que incluye además características antimigración.
- 13. El implante para columna vertebral de la reivindicación 12, en el que las características antimigración incluyen dientes (30) en las superficies superior e inferior del cuerpo.
