Abstract
Un inhibidor de arginasa para el uso en la terapia profiláctica de mantenimiento de un paciente asmático o alérgico reduciendo la sensibilidad de las vías respiratorias a un alérgeno inhalado y protegiendo contra el desarrollo de una reacción asmática precoz (EAR) y reacción asmática tardía (LAR) inducidas por el alérgeno, en el que el inhibidor de arginasa se administra mediante inhalación tópica.

Análisis de datos
La sensibilidad al alérgeno inhalado se expresó como la cantidad total (mg) de alérgeno nebulizado para obtener la obstrucción de las vías respiratorias, que es una función del tiempo de nebulización (s), la dosis de alérgeno en el nebulizador (mg/ml) y el caudal del aerosol (ml/min).
Las magnitudes de EAR y LAR se expresaron como el área bajo la curva tiempo-respuesta de Ppl (ABC) entre 0 y 5 h (EAR) y entre 8 y 24 h (LAR) después de la provocación con alérgeno. Ppl se expresó como el cambio en porcentaje desde el valor basal, y el ABC se calculó mediante la integración trapezoidal a lo largo de periodos de tiempo discretos de 5 min (Santing et al. J. Allergy Clin. Immunol. 1994, 93:1021-1030).
Todos los datos se expresan como medias ± EEM. Se determinó la significación estadística de las diferencias mediante el uso de una prueba t de Student bilateral para datos emparejados, y la significación se aceptó cuando P<0,05.
Productos químicos
Se obtuvo dihidrocloruro de histamina, ovoalbúmina (grado III), hidróxido de aluminio, hidrocloruro de L-arginina e hidrocloruro de D-arginina de Sigma Chemical Co. La solución salina se adquirió de Braun (Países Bajos). El ácido 2(S)-amino-6-boronohexanoico lo proporcionó Organon (Oss, Países Bajos).
RESULTADOS
Protocolo de inversión
La Fig. 3 muestra que la ovoalbúmina induce una AHR significativa después de la EAR y LAR, tal como se indica mediante los valores de PC100 significativamente disminuidos para histamina después de estas reacciones. La inhalación de solución salina no afectó a la reactividad basal respecto de la AHR inducida por histamina ni por alérgeno después de la EAR y LAR.
La inhalación del inhibidor de arginasa ABH no tuvo efecto sobre la reactividad basal de las vías respiratorias, pero invirtió la AHR inducida por alérgeno después de la EAR, tal como se indica mediante el valor de PC100 significativamente incrementado en comparación con la medida de control tras esta reacción.
Además, se observó una tendencia hacia una reducción de la AHR después de la LAR, mientras ya no hubo una AHR significativa después de la inhalación de ABH en comparación con la reactividad basal (Fig. 3).
La Fig. 4 demuestra que ABH reduce la AHR inducida por alérgeno, expresada como la proporción de PC100 antes/después de la exposición, de 4,77 ± 0,56 veces a 2,04 ± 0,34 veces (P<0,001) después de la EAR y de 1,95 ± 0,23 veces a 1,56 ± 0,47 veces (P<0,10) después de la LAR.
Como ABH, la inhalación de L-arginina no afectó a la reactividad basal de las vías respiratorias hacia histamina (Fig. 5). De manera notable, la AHR después de la EAR y después de la LAR se invirtió en un grado similar al obtenido con ABH (Figs. 5 y 6).
Como con la solución salina, la inhalación del enantiómero D biológicamente inactivo de arginina no afectó en absoluto a la reactividad basal de las vías respiratorias y la AHR inducida por alérgeno (Fig. 7).
Protocolo de protección
De manera interesante, el pretratamiento con ABH 25 mM 0,5 h antes de la exposición al alérgeno provocó una protección significativa hacia la AHR después de la EAR en comparación con el tratamiento de control con solución salina (Fig. 8). Además, una inhalación adicional de ABH 25 mM 8 h después de la exposición al alérgeno previno casi completamente la AHR después de la LAR. El pretratamiento con solución salina no afectó a la AHR después de la EAR o LAR (Fig. 8).
La Fig. 9 muestra que ABH redujo significativamente la AHR inducida por el alérgeno después de la EAR de 6,33 ± 1,30 veces (control de solución salina, semana 1) a 3,05 ± 0,51 veces (P<0,05) y de 2,08 ± 0,31 veces a 1,41 ± 0,25 veces (P<0,005) después de la LAR.
De manera notable, tras el pretratamiento con ABH en la semana 2, fue necesario un pretratamiento con una concentración 32,8 veces mayor de ovoalbúmina (1,31 ± 0,69 mg) para inducir la obstrucción de las vías respiratorias en comparación con el tratamiento con solución salina de los mismos animales en la semana 1 (0,04 ± 0,01 mg; P<0,01), lo que indica que ABH disminuye considerablemente la sensibilidad al alérgeno. No se observó un incremento significativo de la dosis de ovoalbúmina para los animales tratados con solución salina (Fig. 10).
Para la apreciación completa del efecto de ABH a una carga de alérgeno normalmente obstructiva, se llevaron a cabo provocaciones con dosis iguales de alérgeno en los animales tratados con solución salina (expuestos a la obstrucción) y tratados con ABH.
