Abstract

La invención se refiere en general a sistemas y
dispositivos relacionados con implantes protésicos, incluyendo un
dispositivo para impedir que las superficies de articulación
protésicas se separen unas de otras, y más específicamente a un
dispositivo para impedir que la cabeza de vástago de un implante se
desplace en relación con un componente protésico.
Los implantes artificiales, incluyendo de

articulación de la cadera humana actúa mecánicamente como una
articulación de rótula esférica, en la que la cabeza del fémur con
forma de esfera está situada dentro del acetábulo de la pelvis con
forma de cavidad. Diversas lesiones y enfermedades degenerativas
pueden requerir la sustitución de la totalidad o una parte de la
cadera usando materiales sintéticos. Los componentes protésicos se
fabrican generalmente o bien con metales, materiales cerámicos o
bien con plásticos.
La hemiartroplastia y la artroplastia total de
cadera son dos procedimientos que se conocen bien en la industria
médica para la sustitución de la totalidad o parte de la cadera de
un paciente. Una artroplastia total de cadera sustituye tanto el
componente femoral como la superficie acetabular de la
articulación, de modo que se requieren tanto una prótesis femoral
como una prótesis acetabular. Una prótesis acetabular convencional
puede incluir una copa, una copa y un revestimiento, o en algunos
casos sólo un revestimiento, todos los cuales pueden formarse con
diferentes formas y tamaños. Generalmente, se usan una copa
metálica y un revestimiento polimérico. Sin embargo, el
revestimiento puede fabricarse de una variedad de materiales,
incluyendo polietileno, polietileno de peso molecular ultra alto y
materiales cerámicos. La copa es normalmente de forma generalmente
semiesférica y se caracteriza por una superficie convexa externa y
una superficie cóncava interna que está adaptada para alojar un
revestimiento de copa. El revestimiento encaja dentro de la copa y
tiene una superficie cóncava y convexa. El revestimiento de copa es
el elemento de soporte en el conjunto de componente acetabular. La
superficie convexa del revestimiento corresponde a la superficie
cóncava interna de la copa o acetábulo, y la superficie cóncava del
revestimiento aloja la cabeza de un componente femoral. Una copa
acetabular puede incluir una superficie interna sumamente pulida
con el fin de disminuir el desgaste.
La superficie cóncava del revestimiento, o
superficie cóncava interna, se caracteriza por rasgos relativos a
un eje a través del centro de la superficie cóncava. Este eje puede
estar alineado o no con el eje central de la cubierta. En un
revestimiento típico, la superficie cóncava tiene una geometría
semiesférica y también se denomina como el diámetro interno. En
tales revestimientos, la geometría se caracteriza por rasgos que
son concéntricos a un eje que discurre a través del centro del
diámetro interno.
Puede fijarse una prótesis acetabular en el
acetábulo escariado de un paciente. Una prótesis de este tipo puede
incluir una copa (o un conjunto de copa y revestimiento) que se
fija o bien colocando tornillos a través de aberturas en la copa o
bien sujetando la copa con cemento. En algunos casos, sólo se
cementa un revestimiento en un paciente debido a una escasa
densidad ósea. En otros casos, puede ajustarse a presión una copa
que tiene una superficie porosa en el interior de la superficie
acetabular escariada.
Una prótesis femoral usada en la artroplastia
total de cadera incluye generalmente una cabeza esférica o casi
esférica unida a un vástago alargado con un cuello que conecta la
cabeza y el vástago. En uso, el vástago alargado está situado en el
conducto intramedular del fémur y la cabeza esférica o casi
esférica está articulada en relación con el componente acetabular.
Las prótesis femorales usadas en los procedimientos de artroplastia
total de cadera pueden diferenciarse o no de una endoprótesis usada
en una hemiartroplastia, descrita a continuación. Sin embargo, la
cabeza femoral de cada prótesis tipo es generalmente de un tamaño y
forma convencionales. Pueden proporcionarse diversas copas,
revestimientos, cubiertas, vástagos y otros componentes en cada
artroplastia tipo para formar prótesis modulares para restablecer
la función de la articulación de la cadera.
Hemiartroplastia se refiere a sustituir parte de
una articulación de cadera, tal como sustituir un componente
femoral de modo que una prótesis femoral esté articulada contra el
tejido corporal natural en el acetábulo del paciente. Una prótesis
femoral implantada durante una hemiartroplastia se denomina
generalmente como endoprótesis. Generalmente, una endoprótesis
incluye un vástago, una cabeza, y puede incluir componentes
adicionales tales como cubiertas y revestimientos. Los diseños de
endoprótesis actuales incluyen diseños (1) monobloque; (2) de dos
componentes; (3) de tres componentes; y (4) de cinco componentes.
Una endoprótesis monobloque es una estructura de una sola pieza que
incluye un vástago y una cabeza femoral. La polaridad se refiere al
número de superficies de articulación que contiene una prótesis.
Una endoprótesis monobloque tiene una superficie de articulación
entre la cabeza y el acetábulo natural del paciente y, por tanto,
se denomina como monopolar.
Una endoprótesis de dos componentes incluye un
componente femoral y una cubierta. El componente femoral puede
incluir una cabeza y vástago modulares. Un diseño de dos
componentes puede ser bipolar, de modo que la cabeza esté
articulada en relación con la cubierta y la cubierta esté
articulada en relación con el acetábulo. Una endoprótesis de tres
componentes incluye un componente femoral, un revestimiento y una
cubierta. De manera similar a un diseño de dos componentes, el
componente femoral puede incluir una cabeza y vástago modulares.
Una endoprótesis de tres componentes puede ser o bien: bipolar, en
la que el revestimiento se fija en la cubierta; o tripolar, en la
que la cabeza está articulada en relación con el revestimiento, el
revestimiento está articulado en relación con la cubierta y la
cubierta está articulada en relación con el acetábulo. Una
endoprótesis de cinco componentes incluye un componente femoral
(que puede incluir una cabeza y vástago modulares), un primer
revestimiento, una primera cubierta, un segundo revestimiento y una
segunda cubierta. Ambos del primer y el segundo revestimientos se
fijan dentro de cada una de la primera y la segunda cubiertas. Por
lo tanto, este diseño es un diseño tripolar: la segunda cubierta
está articulada libremente con respecto al acetábulo, la cabeza
modular del componente femoral está articulada con respecto al
primer revestimiento y la primera cubierta está articulada en
relación con el segundo revestimiento. Por tanto, las endoprótesis
pueden describirse tanto con respecto al número de componentes como
con respecto al número de superficies de articulación instaladas en
un paciente. Algunos diseños actuales también pueden incluir un
dispositivo mecánico, tal como un anillo de fijación, para
restringir la cabeza femoral, descrito adicionalmente a
continuación.
Las endoprótesis, así como las prótesis totales
de cadera, también pueden describirse como prótesis restringidas y
no restringidas. Las prótesis no restringidas se basan en la fuerza
descendente del cuerpo a través de la articulación y la tensión
creada por el tejido blando, incluyendo los músculos, ligamentos y
tendones, para retener la prótesis en su posición implantada. Otras
prótesis incluyen mecanismos para impedir el desplazamiento de los
componentes, tales como la cabeza de vástago de implante.
Normalmente, estas prótesis tienen mecanismos de restricción que dan
como resultado una menor amplitud de movimientos de la articulación
de la cadera, y generalmente se denominan como componentes
"restringidos".
Un ejemplo de un mecanismo de restricción es una
cubierta o revestimiento que tiene una cobertura mayor que la
semiesférica alrededor de la cabeza, de tal manera que la cabeza
queda restringida dentro del diámetro interno, impidiendo por
tanto, la subluxación y el desplazamiento. A diferencia de los
revestimientos habituales, los revestimientos restringidos emplean
una parte elevada, extendida sobre un segmento de la periferia del
diámetro interno del revestimiento con el fin de aumentar la
cobertura de la cabeza femoral y, por tanto reducir la probabilidad
de desplazamiento y ayudar en la reducción de la cabeza si se
produce subluxación. Aunque el uso de componentes restringidos
generalmente no es deseable debido a la disminución de la amplitud
de movimientos resultante, el uso de componentes restringidos puede
ser beneficioso en casos de poca estabilidad con el fin de evitar

1996, págs. 80-86. Sin embargo, los componentes
restringidos tienen una reducción en la zona de movimiento para
entrar en contacto en la dirección del segmento de reborde elevado,
por tanto, hay una pérdida sustancial de la amplitud de movimientos
global en comparación con un revestimiento habitual. Una cabeza de
vástago de implante restringida por una cubierta o revestimiento
puede desplazarse si el componente femoral gira más allá de la
amplitud de movimientos permitida por el conjunto. Puede producirse
desplazamiento porque el borde o reborde del revestimiento o la
cubierta que retiene la cabeza de vástago de implante actúa como un
punto de apoyo alrededor del que pivota el componente femoral,
haciendo así que la cabeza de Vástago de implante se desplace de su
posición dentro del revestimiento o la cubierta de la prótesis. El
desplazamiento de una prótesis de cadera es doloroso y a menudo
requiere intervención médica.
El documento FR2785525 da a conocer un mecanismo
de restricción que comprende una cubierta que es más que
semiesférica. La cubierta tiene un orificio circular y una cavidad
interna que puede albergar una cabeza. La cabeza se basa en una
esfera, teniendo la cabeza una parte plana de tal manera que la
cabeza tiene un diámetro mínimo y uno máximo. El diámetro mínimo de
la cabeza es menor que el diámetro del orificio en la cubierta, lo
que significa que cuando se alinea apropiadamente la cabeza, la
cabeza puede ajustarse a través del orificio en la cubierta y
albergarse en la cavidad. El diámetro máximo de la cabeza es mayor
que el diámetro del orificio en la cubierta, lo que significa que
cuando se alinea apropiadamente la cabeza dentro de la cubierta, la
cabeza no puede ajustarse a través del orificio y, por lo tanto, se
restringe en la cavidad.
Se conocen en la industria endoprótesis
bipolares de tres componentes que incluyen revestimientos de
polietileno, y tienen al menos tres problemas clínicos importantes.
En primer lugar, la mayor parte de la articulación se produce entre
el revestimiento y la cubierta, y no es raro no obtener casi
movimiento relativo entre la cubierta y el acetábulo. En segundo
lugar, hay a menudo una considerable cantidad de partículas de
desgaste de polietileno generadas a partir del dispositivo debido a
la carga de fatiga del revestimiento. Finalmente, hay un limite
inferior para el tamaño de la cubierta debido a la necesidad de
incorporar un tamaño de cabeza habitual y un revestimiento de
espesor apropiado. Las soluciones actuales a estos problemas
incluyen un diseño que tiene una cubierta cerámica, una cabeza
cerámica y un anillo de retención de polietileno, que bloquea la
cabeza en la cubierta. Tales diseños conducen frecuentemente al
desgaste del polietileno y tienen un montaje complejo. Otra
solución ha sido el uso de un dispositivo monobloque unipolar, que
no requiere un revestimiento pero que da como resultado un
desgaste excesivo del acetábulo.
Por tanto, existe la necesidad de un componente
protésico que pueda retener una cabeza de vástago de implante para
impedir que se desplace mientras proporciona una mayor amplitud de
movimientos que la que permiten las prótesis restringidas
convencionales. También existe la necesidad de un componente
protésico que pueda retener una cabeza de implante para impedir que
se desplace mientras se elimina la necesidad de una superficie de
soporte interna, o revestimiento.
A continuación se expone un breve sumario de
sistemas según la invención que tratan los problemas anteriores y
proporciona beneficios y ventajas según los fines de la presente
invención tal como se realiza y describe ampliamente en el presente
documento. Una prótesis según una realización de esta invención
proporciona una prótesis restringida con un aumento de la amplitud
de movimientos con respecto a las prótesis restringidas actuales.
Según una realización de esta invención, una prótesis restringida
impide el desplazamiento de un componente femoral. Una prótesis de
esta invención puede usarse con seres humanos y animales y puede
usarse con prótesis de cadera convencionales incluyendo las
endoprótesis y prótesis usadas en la artroplastia total de
cadera.
Un sistema de contención según una realización
de esta invención incluye un elemento estructural de implante que
tiene una superficie externa generalmente esférica y que tiene una
cavidad y un orificio adaptados para alojar una cabeza de vástago
de implante u otro componente protésico. El orificio de la cavidad
del elemento estructural de implante incluye un reborde que tiene
una banda para retener la cabeza de vástago de implante dentro de
la cavidad e impedir que se desplace. La banda comprende sólo una
parte del reborde que forma el orificio. Como resultado, el
componente femoral puede moverse a través de una mayor amplitud de
movimientos que la amplitud de movimientos proporcionada por un
componente femoral acoplado a una prótesis restringida
convencional. En una realización, una parte de una superficie
externa de la cabeza de vástago de implante está configurada para
corresponder a la banda sobre la cubierta. Una cabeza de vástago de
implante según una realización de esta invención incluye una
abertura adaptada para alojar un componente de vástago femoral.
Un sistema de contención según una realización
de esta invención se monta alineando, en primer lugar, la
superficie de la cabeza de vástago de implante configurada para
corresponder a la banda del elemento estructural de implante en una
primera orientación de modo que corresponda a la banda. La cabeza
de vástago de implante se inserta entonces en el elemento
estructural de implante y se hace girar hasta que la abertura de la
cabeza es visible a través del orificio en el elemento estructural
de implante. La cabeza de vástago de implante no puede orientarse
en la primera orientación en la que la superficie de la cabeza se
corresponde con la banda mientras que el componente de vástago
femoral está acoplado a la cabeza de vástago de implante. Por lo
tanto, la cabeza de vástago de implante no puede retirarse del
elemento estructural de implante a menos que se retire en primer
lugar el vástago femoral. Como resultado, el componente femoral no
puede desplazarse mientras se encuentra dentro de un paciente.
Un rasgo de un sistema de contención según una
realización de esta invención es la eliminación del riesgo de
desplazamiento de un componente femoral.
Otro rasgo de un sistema de contención según una
realización de esta invención es un componente protésico
restringido que proporciona un aumento de la amplitud de
movimientos.
Aún otro rasgo de un sistema de contención según
una realización de esta invención es que un sistema de contención
es económico de fabricar e incluye pocas piezas.
Otro rasgo de un sistema de contención según una
realización de esta invención es la eliminación de la necesidad de
un revestimiento de polietileno, lo que permite el uso de una
cabeza de mayor diámetro con un aumento resultante en la amplitud
de movimientos y la eliminación de la posibilidad de desgaste del
polietileno.
Aún otro rasgo de un sistema de contención según
una realización de esta invención es un sistema de contención que
es fácil de montar y que no requiere instrumentos adicionales para
el montaje.
Según la presente invención, se proporciona un
implante, que comprende:
(a) un primer elemento estructural de implante
adaptado para alojar la cabeza de un vástago de implante, que
comprende:
una superficie externa y un reborde; y
una superficie interna que forma una cavidad que
tiene una forma generalmente esférica, incluyendo adicionalmente la
superficie interna una banda, extendiéndose la banda alrededor de
sólo una parte del reborde del elemento estructural de implante,
incluyendo una superficie interna que es una continuación de la
superficie interna de forma generalmente esférica de la cavidad del
elemento estructural de implante y conformada para corresponder
generalmente a una superficie externa de la cabeza de vástago de
implante, adaptada la banda para permitir que la cabeza de vástago
de implante se inserte en la cavidad del elemento estructural
cuando la cabeza de vástago de implante está orientada en una
primera orientación y para restringir la cabeza de vástago de

que comprende:
situar un elemento estructural de implante
adaptado para alojar una cabeza de vástago de implante,
comprendiendo el elemento estructural de implante: una superficie
externa y un reborde; y una superficie interna que forma una
cavidad que tiene una forma generalmente esférica, incluyendo
adicionalmente la superficie interna una banda, extendiéndose la
banda alrededor de sólo una parte del reborde del elemento
estructural de implante, incluyendo una superficie interna que es
una continuación de la superficie interna de forma generalmente
esférica de la cavidad del elemento estructural de implante y

Los sistemas y dispositivos según esta invención
intentan proporcionar un sistema de contención para impedir que
las superficies de articulación protésicas se separen entre sí
mientras se permite un aumento de la amplitud de movimientos con
respecto a prótesis restringidas convencionales. Una realización de
un sistema de contención según esta invención intenta proporcionar
un componente protésico de cadera para restringir una cabeza de
vástago de implante de un componente femoral mientras proporciona
un aumento de la amplitud de movimientos. Generalmente, un sistema
de contención según una realización de esta invención, incluye un
elemento estructural de implante que tiene una cavidad y un
orificio para alojar una cabeza de vástago de implante. El orificio
incluye un reborde y una banda a lo largo de una parte del reborde.
El sistema de contención también incluye una cabeza de vástago de
implante que tiene una superficie de acción conjunta con una forma
que se corresponde con la forma de la banda. La cabeza de vástago
de implante está adaptada de modo que sólo puede insertarse en y
retirarse del elemento estructural de implante cuando la superficie
de acción conjunta de la cabeza está alineada con la banda.
Un sistema de contención según una realización
de esta invención se monta alineando la superficie de acción
conjunta de la cabeza de vástago de implante con la banda del
elemento estructural de implante e insertando la cabeza de vástago
de implante en el elemento estructural de implante. Entonces se
hace girar la cabeza de vástago de implante y el componente de
vástago femoral está unido. La cabeza de vástago de implante sólo
puede retirarse del elemento estructural de implante si se retira
en primer lugar el vástago de la cabeza, permitiendo que la
superficie de acción conjunta de la cabeza de vástago de implante
se oriente con la banda. De esta manera, la cabeza queda
restringida en el elemento estructural de implante siempre que
estén montados el vástago y la cabeza.
El sistema 10 de contención de la presente
invención se ilustra como una endoprótesis bipolar de dos
componentes en las figuras 1 y 2. Este sistema 10 de contención
incluye un elemento 14 estructural de implante, tal como una
cubierta, adaptado para alojar una cabeza 12 de vástago de
implante, que está adaptada para acoplarse al componente 13 de
vástago femoral, formando la cabeza de vástago de implante y el
componente de vástago femoral un componente femoral cuando están
montados. Tal como se muestra en las figuras 1 y 2, el elemento 14
estructural de implante está adaptado para encajar dentro del
acetábulo 11 y está adaptado para atrapar y retener la cabeza 12 de
vástago de implante. El elemento 14 estructural de implante incluye
una cavidad 16 que tiene una forma generalmente esférica y que está
formada por una superficie 18 interna y un orificio 20. El orificio
20 incluye un reborde 36 que tiene una banda 22, que reduce el
tamaño del orificio 20 de modo que el orificio 20 forma una forma
de D, tal como se muestra en la figura 5. En una realización
alternativa, mostrada en la figura 6, la banda 22 incluye un borde
31 curvo, que permite que el componente de vástago femoral gire a
través de una amplitud de movimientos ligeramente mayor. En otras
realizaciones, la banda 22 tiene otras formas adecuadas.
La cabeza 12 de vástago de implante puede
extenderse o no más allá del orificio 20 del elemento 14
estructural de implante. Por ejemplo, tal como se muestra en las
figuras 1-2 y 10-11, la cabeza 12
de vástago de implante se extiende más allá del orificio 20 del
elemento 14 estructural de implante, pero la cabeza 12 y el
elemento 14 también pueden adaptarse de modo que la cabeza 12 no se
extienda más allá del orificio 20. Por ejemplo, tal como se muestra
en las figuras 3-4 y 7-9, el
elemento 14 estructural de implante está adaptado para alojar la
cabeza 12 de vástago de implante de tal manera que la cabeza 12 no
se extiende más allá del orificio 20 del elemento 14 estructural de
implante, pero también puede adaptarse de modo que la cabeza 12 sí
se extienda más allá del orificio 20. En una realización, el plano
de la banda 22 está en ángulo recto con respecto al reborde 36; sin
embargo, en una realización alternativa, el plano de la banda 22
está en cualquier ángulo adecuado en relación con el reborde. La
descripción del sistema 10 de contención de dos componentes es
generalmente aplicable a todas las realizaciones mostradas en las
figuras 1-11, siendo la única diferencia la
situación de la cabeza de vástago de implante en relación con el
elemento estructural de implante, tal como se describió
anteriormente.
Tal como se muestra en la figura 7, una primera
línea 30 tangente y una segunda línea 32 tangente no son paralelas.
En su lugar, la primera línea 30 tangente está en un ángulo \alpha
que es inferior a 90 grados en relación con la línea 34 de
referencia, que es paralela a la segunda línea 32 tangente. La
banda 22 está formada como una continuación de la superficie 18
interna y mantiene el mismo radio de curvatura que otras partes de
la superficie 18 interna. Por tanto, la banda 22 se forma
extendiendo la superficie 18 interna más allá de la línea 34 de
referencia para formar un ángulo \alpha, tal como se muestra en
la figura 7. En esta configuración, la banda 22 disminuye el
diámetro a través del orificio 20 y no crea una superficie que
probablemente produzca un desgaste innecesario en la cabeza 12 de
vástago de implante.
La cabeza 12 de vástago de implante es de forma
generalmente esférica e incluye una superficie 24 de acción
conjunta que permite que la cabeza 12 de vástago de implante se
inserte dentro de la cavidad 16 del elemento 14 estructural de
implante. La superficie 24 puede ser plana, tal como se muestra en
las figuras 1-4, o puede formarse con formas
alternativas que corresponden a la forma de la banda. Tal como se
muestra en la figura 7, la superficie 24 de acción conjunta está
situada sobre la cabeza 12 de vástago de implante de modo que un
diámetro 15 de la cabeza 12 de vástago de implante tomado
perpendicular a la superficie 24 de acción conjunta y que discurre
a través del centro de la cabeza 12 de vástago de implante es
ligeramente menor que la distancia 17 dentro del orificio 20. Esta
configuración permite que la cabeza 12 de vástago de implante se
inserte en la cavidad 16 alineando la superficie 24 de acción
conjunta con la banda 22. La orientación de la superficie 24 de
acción conjunta de esta manera permite que la cabeza 12 de vástago
de implante se inserte en la cavidad 16 pasada la banda 22, tal
como se muestra en la figura 8.
Tras haberse insertado completamente la cabeza
12 de vástago de implante en la cavidad 16 del elemento 14
estructural de implante, se hace girar la cabeza 12 de modo que la
cabeza 12 quede retenida en la cavidad 16. específicamente, tras
hacerse girar la cabeza 12 de vástago de implante, la cabeza 12 no
puede retirarse del elemento 14 estructural de implante porque el
diámetro 19 de la cabeza 12 de vástago de implante, mostrada en la
figura 7, es mayor que la distancia 17 del orificio 20, y la
superficie 24 de acción conjunta ya no está orientada para
corresponderse con la banda 22, tal como se muestra en la figura 9.
Entonces se acopla un componente 13 de vástago femoral a la cabeza
12, impidiendo la alineación de la superficie 24 de acción conjunta
con la banda 22 de modo que la cabeza 12 queda retenida en el
elemento 14 estructural de implante. Una vez que se ha acoplado el
componente 13 de vástago femoral con la cabeza 12, no puede
retirarse la cabeza 12 de la cubierta 14 hasta que se haya retirado
el componente 13 de vástago femoral porque el vástago 13 impide que
la cabeza 12 gire de modo que la superficie 24 de acción conjunta
se alinea con la banda 22.
Tal como se muestra en las figuras
7-9, la cabeza 12 de vástago de implante incluye
una cavidad 26, que está adaptada para alojar el componente 13 de
vástago femoral y que es generalmente cilíndrica. En una
realización alternativa, la cavidad 26 puede ser cónica, cúbica o
de cualquier otra forma adecuada. En otras realizaciones
alternativas, la cavidad 26 puede incluir roscas, lengüetas,
anillos o cualquier otro conector mecánico adecuado para acoplar la
cabeza 12 al vástago 13. Aún en otra realización, puede usarse
adhesivo o cemento para acoplar la cabeza 12 al componente 13 de
vástago femoral.
En una realización, el orificio 28 de la cavidad
26 de la cabeza 12 de vástago de implante está situada sobre la
superficie 24 de acción conjunta. En esta realización, el orificio
28 está situado para alojar el vástago 13 sólo tras haberse hecho
girar la cabeza 12 de vástago de implante para exponer el orificio
28, tal como se muestra en la figura 9. La unión de vástago 13 a la
cabeza 12 de vástago de implante impide que la cabeza 12 de vástago
de implante gire de modo que la cabeza 12 de vástago de implante
puede retirarse de cavidad 16, tal como se muestra en la figura 10.
Específicamente, tras haberse unido la cabeza 12 de vástago de
implante al vástago 13, el vástago 13 entra en contacto con el
reborde 36 del orificio 20 e impide que la cabeza 12 de vástago de
implante gire más, tal como se muestra en la figura 11. Por tanto,
el reborde 36 del orificio 20 define la amplitud de movimientos del
componente 13 de vástago femoral. En otra realización, el orificio
28 está situado dentro de la cabeza 12 en una ubicación distinta a
dentro de la superficie 24. Por ejemplo, el orificio 28 puede estar
ubicado adyacente a la superficie 24 o en otra ubicación sobre la
cabeza 12. Durante su uso, la banda 22 está situada preferiblemente
en una posición superior, tal como se muestra en la figura 2, con
el fin de maximizar la amplitud de movimientos y minimizar la
posibilidad de subluxación.
En una realización según esta invención, se
proporciona excentricidad fuera del eje. En una realización, se
proporciona excentricidad negativa, forzando el giro del elemento
estructural de implante en relación con el acetábulo. En otra
realización, se proporciona excentricidad positiva, dando como
resultado un aumento de la amplitud de movimientos del componente
femoral. Por ejemplo, tal como se muestra en las figuras
7-9, el centro 37 de rotación de la cabeza 12 de
vástago de implante no está situado en la misma ubicación que el
centro 39 de la cubierta. El centro 37 de rotación de la cabeza 12
de vástago de implante está situado más cerca del orificio 20 que
el centro 39 de la cubierta. Como resultado, el componente femoral
está menos restringido que si tanto el centro 37 de rotación como
el centro 39 de la cubierta estuviesen ubicados en la misma
posición.
En una realización de esta invención, la cabeza
12 de vástago de implante es de cerámica y el elemento 14
estructural de implante es una cubierta cerámica. Esta realización
no incluye polietileno en ninguna forma, reduciendo las partículas
de desgaste. En una realización, la cabeza 12 de vástago de
implante es mayor que una cabeza convencional, permitiendo un
aumento de la amplitud de movimientos que resulta del aumento del
diámetro del conjunto. En una realización, se reduce el diámetro de
entrada de la cubierta para ser ligeramente menor que una
semiesfera completa, aumentando adicionalmente la amplitud de
movimientos.
Se ilustra una realización alternativa de un
sistema de contención según esta invención como una endoprótesis
tripolar de tres componentes en la figura 12. El sistema 41 de
contención es muy similar a la realización de dos componentes
descrita anteriormente, con el mecanismo de retención ubicado
dentro del revestimiento 38. Por ejemplo, el sistema 41 de
contención incluye una cubierta 46 que tiene una cavidad 40 para
alojar el revestimiento 38. La cubierta 46 está adaptada para
alojarse en el acetábulo 43. El revestimiento 38 está adaptado para
alojarse en la cavidad 40 de la cubierta 46 e incluye una cavidad
47 adaptada para alojar una cabeza 48 de vástago de implante. La
cavidad 47 del revestimiento 38 está definida por una superficie 42
interna y un orificio 44. La superficie 42 interna puede formarse a
partir de cualquier procedimiento convencional. Tal como se
describió en la realización de endoprótesis de dos componentes
anterior, una banda 50 está situada sobre el reborde del
revestimiento 38 para atrapar una cabeza 48 de vástago de implante
e impedir el desplazamiento de una cabeza 48 de vástago de implante
mientras el vástago 45 está acoplado a la cabeza 48.
Se ilustra aún otra realización alternativa de
un sistema de contención (no reivindicado) como una endoprótesis
de cinco componentes en la figura 13. De manera similar a la
realización de la figura 12, el mecanismo de retención del sistema
72 de contención está ubicado sobre un primer revestimiento 74, que
está adaptado para alojar una cabeza 76 de vástago de implante. La
primera cubierta 78 está adaptada para alojar el primer
revestimiento 74, mientras que el segundo revestimiento 80 está
adaptado para alojar la primera cubierta 78. El segundo
revestimiento 80 está alojado en una segunda cubierta 82, que está
alojada en el acetábulo 84. El primer revestimiento 74 incluye una
banda 86 y atrapa y retiene la cabeza 76 tal como se describió en
las realizaciones anteriores.
En una realización alternativa, mostrada en las
figuras 14-17, un sistema de contención incluye una
cubierta 54 que tiene dos bandas 56, 57, cada una situada en lados
opuestos del orificio 58 de la cubierta 54. Una cabeza 60 de
vástago de implante incluye dos superficies 62, 63 de acción
conjunta que se corresponden con bandas 56, 57 de modo que puede
insertarse la cabeza 60 de vástago de implante en la cavidad 64 de
la cubierta 54. En esta realización, el sistema de contención
funciona de la misma manera que las realizaciones que tienen una
sola banda. Por ejemplo, una cabeza 60 de vástago de implante está
situada de modo que las superficies 62, 63 se corresponden con
bandas las 56, 57 tal como se muestra en las figuras 14 y 15.
Entonces se inserta la cabeza 60 de vástago de implante en la
cubierta 54, tal como se muestra en la figura 16. Se hace girar
entonces la cabeza 60 de vástago de implante hasta que la cavidad
66 de la cabeza 60 puede alojar un componente 70 de vástago femoral
a través del orificio 68, tal como se muestra en la figura 17.
Otras realizaciones alternativas de un sistema de contención según
esta invención pueden incluir cubiertas o revestimientos que tengan
cualquier número apropiado de bandas que pueden configurarse tal
como se muestra o en configuraciones alternativas, tales como tres,
cuatro o cinco bandas y que tengan cabezas correspondientes a las
bandas.
Los componentes de las diversas prótesis
descritas pueden fabricarse a partir de metal, tal como acero
inoxidable y titanio, cerámica, plástico, tal como polietileno, o
cualquier otro material adecuado. En una realización, todos los
componentes de una endoprótesis bipolar de dos componentes son de
metal. En una realización alternativa, todos los componentes de una
endoprótesis bipolar de dos componentes son cerámicos. En otra
realización, la cubierta y la cabeza de vástago de implante de una
endoprótesis de tres componentes son o bien de metal o bien de
cerámica, mientras que el revestimiento es de polietileno.
Las realizaciones de un sistema de contención
tal como se describió anteriormente se refieren todas a
endoprótesis usadas en hemiartroplastia. Pueden incorporarse
realizaciones similares de un sistema de contención según esta
invención en una prótesis usada en artroplastia total de cadera,
usando una banda para restringir un componente de cabeza femoral.
Pueden utilizarse realizaciones alternativas de un sistema de
contención según esta invención para atrapar y retener otros
componentes protésicos. Por ejemplo, puede incorporarse una banda
en un segundo revestimiento de una endoprótesis de cinco
componentes con el fin de atrapar y retener el conjunto de la
primera cubierta y el primer revestimiento. Adicionalmente, también
pueden usarse realizaciones de un sistema de contención según esta
invención para atrapar componentes de otras prótesis ortopédicas,
tales como prótesis de hombro y pequeñas prótesis de articulación
de los dedos. Por ejemplo, una prótesis de hombro restringida puede
incluir una banda para atrapar y retener la cabeza humeral.
Un método de uso de una forma de estructura
según esta invención es tal como sigue. La cabeza 12 de vástago de
implante, mostrada en la figura 7, está situada para insertarse en
la cubierta 14 orientando la cabeza 12 de vástago de implante en
una primera orientación de modo que la superficie 24 de acción
conjunta se corresponde con la banda 22. Tal como se muestra en la
figura 8, la cabeza 12 de vástago de implante se inserta en la
cubierta 14 de modo que la superficie exterior de la cabeza 12 de
vástago de implante entra en contacto con la superficie 18 interior
de la cubierta 14. La cabeza 12 de vástago de implante se pone
entonces en una segunda orientación haciendo girar la cabeza 12 de
modo que se expone la cavidad 26 y en una posición para alojar el
componente 13 de vástago femoral. Tras haberse acoplado el
componente 13 de vástago femoral a la cabeza 12 de vástago de
implante, la cabeza 12 no puede retirarse de la cubierta 14 a menos
que se retire en primer lugar el vástago 13 porque el vástago 13
impide que se oriente la cabeza 12 en una primera orientación en la
que la superficie 24 de acción conjunta se corresponde con la banda
22. Entonces se implanta quirúrgicamente la endoprótesis, de modo
que se implanta el vástago del componente femoral en el fémur de un
paciente. Luego, se implanta la endoprótesis en el acetábulo de una
pelvis.
Se proporciona lo anterior con fines de
ilustrar, explicar y describir realizaciones de esta invención.
Resultarán evidentes modificaciones y adaptaciones de estas
realizaciones para los expertos en la técnica y pueden realizarse
sin apartarse del alcance de esta invención o las siguientes
reivindicaciones.
Claims (19)
1. Implante, que comprende:
(a) un primer elemento (14) estructural de
implante adaptado para alojar la cabeza (12) de vástago (13) de un
implante, que comprende:
- una superficie externa y un reborde (36); y
- una superficie (18) interna que forma una cavidad (16) que tiene una forma generalmente esférica, incluyendo adicionalmente la superficie interna una banda (22), extendiéndose la banda (22) alrededor de sólo una parte del reborde (36) del elemento (14) estructural de implante, incluyendo una superficie interna que es una continuación de la superficie (18) interna de forma generalmente esférica de la cavidad (16) del elemento estructural de implante y conformada para corresponder generalmente a una superficie externa de la cabeza de vástago de implante,
- la banda (22) adaptada para permitir que la cabeza (12) de vástago de implante se inserte en la cavidad (16) del elemento estructural cuando la cabeza (12) de vástago de implante está orientada en una primera orientación y restringir la cabeza (12) de vástago de implante dentro de la cavidad (16) cuando la cabeza (12) de vástago de implante está orientada en una segunda orientación y está unida al vástago (13);
(b) una cabeza (12) de vástago de implante, que
comprende:
- un cuerpo generalmente esférico que tiene una superficie (24) configurada para corresponder a la banda (22) que permite que la cabeza (12) de vástago de implante se inserte en el elemento (14) estructural de implante cuando la cabeza (12) de vástago de implante está en la primera orientación; Y
- una cavidad (26) para alojar un vástago (13) de implante; y
(c) un vástago (13) de implante adaptado para
acoplarse a la cabeza (12) de vástago de implante, en el que la
cabeza (12) de vástago de implante puede estar articulada dentro
del primer elemento (14) estructural de implante pero no puede
retirarse del primer elemento (14) estructural de implante una vez
que está unido al vástago (13).
2. Implante según la reivindicación 1, en el que
la superficie (18) interna de la banda (22) está conformada para
actuar conjuntamente con la cabeza (12) de vástago de implante ya
que la cabeza de vástago de implante está articulada en relación
con el primer elemento (14) estructural de implante.
3. Implante según cualquier reivindicación
anterior, en el que la banda (22) forma un orificio con forma
generalmente de D.
4. Implante según la reivindicación 1 ó 2, en el
que la banda (22) se compone de un borde (31) curvo.
5. Implante según cualquier reivindicación
anterior, en el que la banda (22) del primer elemento (14)
estructural de implante comprende una superficie interna que tiene
un radio de curvatura igual a un radio de curvatura que forma la
superficie (18) interna del primer elemento (14) estructural de
implante.
6. Implante según cualquier reivindicación
anterior, en el que la banda (22) puede situarse en una posición
superior dentro de un paciente.
7. Implante según cualquier reivindicación
anterior, en el que un punto (37) central de la cabeza (12) de
vástago de implante no está en la misma posición que un punto (39)
central del primer elemento (14) estructural de implante cuando la
cabeza (12) de vástago de implante está situada dentro del primer
elemento (14) estructural de implante.
8. Implante según cualquier reivindicación
anterior, en el que la cavidad (26) en la cabeza (12) de vástago de
implante comprende un orificio situado sobre la superficie de la
cabeza (12) de vástago de implante que está configurado para
corresponder a la banda (22).
9. Implante según cualquier reivindicación
anterior, que comprende además una copa (46) acetabular adaptada
para acoplarse con un acetábulo (43) y para alojar el primer
elemento (14) estructural de implante.
10. Implante según cualquiera de las
reivindicaciones 1 a 8, que comprende además un segundo elemento
estructural de implante adaptado para alojarse en un acetábulo
(43).
11. Implante según la reivindicación 10, en el
que el segundo elemento estructural de implante comprende una
cavidad (40) adaptada para alojar el primer elemento estructural de
implante en la forma de revestimiento (38) y que incluye al menos
un orificio en una superficie exterior del segundo elemento (46)
estructural de implante que proporciona acceso a la cavidad
(40).
12. Implante según la reivindicación 10 u 11,
en el que el segundo elemento (46) estructural de implante se
compone de cerámica.
13. Implante según la reivindicación 10 u 11,
en el que el segundo elemento (46) estructural de implante se
compone de metal.
14. Implante según la reivindicación 10 u 11,
en el que el segundo elemento (46) estructural de implante se
compone de plástico.
15. Implante según cualquier reivindicación
anterior, en el que el primer elemento (14) estructural de implante
se compone de cerámica.
16. Implante según una cualquiera de las
reivindicaciones 1 a 14, en el que el primer elemento (14)
estructural de implante se compone de metal.
17. Implante según una cualquiera de las
reivindicaciones 1 a 14, en el que el primer elemento (14)
estructural de implante se compone de plástico.
18. Implante según cualquier reivindicación
anterior, en el que la cabeza (12) de vástago de implante se
compone de cerámica.
19. Implante según una cualquiera de las
reivindicaciones 1 a 17, en el que la cabeza (12) de vástago de
implante se compone de metal.
