Combinación de materiales para prótesis
articular.
La presente invención se refiere a una prótesis
articular ortopédica que comprende unos primero y segundo
componentes de articulación que tienen unas respectivas superficies
de apoyo en contacto mutuo.
Son conocidas las prótesis articulares
ortopédicas cuya finalidad es la sustitución de las articulaciones
naturales, como por ejemplo la articulación de una cadera, una
rodilla, un tobillo y un hombro. La sustitución articular
genéricamente implica la retirada de tejido de cada uno de los
huesos articulares de la articulación, y la implantación de los
componentes de la prótesis en cada uno de los huesos
articulares.
Es importante que el desgaste de los componentes
de una articulación artificial se reduzca al mínimo porque los
residuos de desgaste pueden originar reacciones fisiológicas
adversas. La selección de los materiales destinados a los
componentes articulares se realiza de tal forma que el desgaste se
reduzca al mínimo, y también para asegurar que la reacción
fisiológica de dichos residuos de desgaste originados se reduzca al
mínimo.
Es habitual en una prótesis articular
comprometer componentes cuyas superficies de articulación
coincidentes comprendan, respectivamente, un metal y un polímero
(articulación "metal sobre polímero"). Por ejemplo, la
superficie de apoyo sobre uno de los componentes podría comprender
una aleación de cromo-cobalto o a base de acero y
una correspondiente superficie de apoyo sobre el otro componente
podría comprender un polietileno de peso molecular alto. El acabado
de las superficies de ambos componentes se lleva a cabo para que
sean tersas. Durante la articulación de la junta, el componente
polimérico es sometido a un desgaste gradual, formando partículas
finas del polímero.
Se han llevado a cabo tentativas para formar
prótesis articulares en las cuales ambas superficies articulares son
fabricadas con metales (articulación "metal sobre metal").
Incluso con un acabado muy fino que produzca unas superficies de
apoyo lisas, puede no evitarse el desgaste de dichas prótesis. El
desgaste inevitablemente origina la formación de partículas de
metal. Las mismas partículas de metal producidas por el desgaste
tienden a presentar propiedades abrasivas de forma que, cuando
aparecen entre las superficies articulares, pueden originar un
desgaste ulterior. Así mismo, debido a la dureza similar de las
superficies articulares (debido a que ambas están formadas a partir
de metales), el desgaste tiende a producirse sobre ambas
superficies. Aunque los volúmenes de desgaste de una prótesis metal
sobre metal son menores que en una prótesis de metal sobre
polímero, las partículas de desgaste metálicas son muy pequeñas (por
ejemplo de aproximadamente 100 nm) y, en consecuencia, se presentan
en un número elevado. Los efectos fisiológicos de las partículas de
desgaste metálicas no son bien conocidos. Sin embargo, persiste el
deseo de reducir el volumen y el número de las partículas de
desgaste en comparación con algunos de los sistemas existentes.
La presente invención proporciona una prótesis
articular ortopédica en la cual el material de una de las
superficies de apoyo comprende un material metálico y el material de
la otra superficie de apoyo comprende un material cerámico.
La Patente estadounidense nº 5,641,323 divulga
una prótesis articular de acuerdo con el preámbulo de la
reivindicación 1.
De acuerdo con ello, en un aspecto, la invención
proporciona una prótesis articular ortopédica que comprende un
primero y un segundo componentes articulares que tienen unas
respectivas superficies de apoyo en contacto mutuo, comprendiendo
el material de la primera superficie de apoyo un material metálico y
comprendiendo el material de la segunda superficie de apoyo un
material cerámico, siendo la dureza del material metálico de al
menos aproximadamente 2.500 MPa, y siendo el material metálico al
menos de aproximadamente 2.500 MPa, y siendo la dureza del material
cerámico mayor que la del material metálico en al menos
aproximadamente 4.000 MPa.
El uso de las superficies de apoyo formadas
mediante materiales metálicos y cerámicos proporciona ventajas
significativas. Los materiales posibilitan la formación de unas
superficies de apoyo muy lisas con una geometría muy precisamente
controlada, de forma que las superficies pueden encajarse con
precisión. Los materiales de las dos superficies pueden
seleccionarse con unas durezas que sean mayores que las de los otros
sistemas articulares, de forma que la tendencia al desgaste de
aquellos durante la articulación se reduce, y con una dureza
diferencial que asegura que una de las superficies es genéricamente
susceptible de permanecer lisa durante la articulación. Esto, a su
vez, puede producir un desgaste inferior de la superficie
opuesta.
El material cerámico de la segunda superficie de
apoyo es más duro que el material metálico de la primera superficie
de apoyo. Por ejemplo, puede ser preferente, en la relación de la
dureza del material cerámico de la segunda superficie de apoyo con
respecto a la del material metálico de la primera superficie de
apoyo, que esta relación sea de al menos aproximadamente dos, más
preferente de al menos aproximadamente tres. Puede ser preferente
que la diferencia entre la dureza del material cerámico de la
segunda superficie de apoyo y la del material metálico de la primera
superficie de apoyo sea de aproximadamente 8.000 MPa, más preferente
de al menos aproximadamente 10.000 MPa. La diferencia a menudo no
será mayor de aproximadamente 30.000 MPa, por ejemplo no mayor de
aproximadamente 20.000 MPa o de 15.000 MPa. Las durezas se miden de
acuerdo con el baremo DIN50359.
El uso de un material cerámico que es
significativamente más duro que el material metálico tiene la
ventaja de que la tendencia del material cerámico al desgaste
durante la articulación se reduce al mínimo. Así mismo, la acción
de la superficie cerámica contra la superficie metálica puede pulir
la superficie metálica, de forma que el desgaste adicional de las
superficies de articulación es mantenida al mínimo.
Preferentemente, la dureza del material cerámico
de la segunda superficie de apoyo es de al menos aproximadamente
10.000 MPa, más preferentemente de al menos aproximadamente 14.000
MPa, por ejemplo de al menos preferentemente 15.000 MPa, o de al
menos aproximadamente 20.000 MPa o mayor.
Preferentemente, la dureza del material metálico
de la primera superficie de apoyo es de al menos aproximadamente
3.500 MPa. La dureza del material metálico a menudo será inferior a
aproximadamente 8.000 MPa, especialmente inferior a aproximadamente
7.000 MPa, por ejemplo entre 4.000 y 5.000 MPa.
El material cerámico genéricamente comprenderá
un material no metálico, cristalino, duro, especialmente un
material inorgánico. Un material de apoyo podría estar formado con
un material de este tipo mediante un proceso que incluya, por
ejemplo, la exposición a unas temperaturas y presión altas. Ejemplos
de materiales cerámicos incluyen óxidos duros. Ejemplos de
materiales de óxido duro apropiados incluyen óxido de aluminio
(alumina) y el óxido de circonio (circonia). El óxido de circonio se
utilizará preferentemente en mezclas con otros materiales, como por
ejemplo uno o más de entre el óxido de aluminio y el óxido de itrio.
Otros materiales cerámicos incluyen determinados carburos y
nitruros, como por ejemplo carburos y nitruros de titanio, cromo,
silicona, aluminio y circonio.
Son conocidos determinados metales que pueden
utilizarse para formar prótesis articulares ortopédicas y que
pueden utilizarse en la prótesis articular de la presente invención.
Ejemplos de dichos metales incluyen aleaciones de
cromo-cobalto, aleaciones de titanio, y determinados
aceros inoxidables.
Preferentemente la rugosidad superficial de la
superficie de apoyo no es mayor de aproximadamente 0,015
\mumR_{a}, más preferentemente no mayor de aproximadamente 0,01
\mumR_{a}, especialmente no mayor de aproximadamente 0,008
\mumR_{a}, no mayor de aproximadamente 0,005 \mumR_{a},
medidas utilizando aparatos rugosímetros de superficie
convencionales. Es una ventaja del empleo de materiales cerámicos en
la prótesis articular de la presente invención que pueden ser
sometidos a un acabado pertinente obteniendo dichas propiedades
superficiales lisas mediante técnicas de acabado superficial
accesibles utilizando equipos comercialmente disponibles.
Preferentemente, la rugosidad superficial de
cada una de las superficies de apoyo no es mayor de aproximadamente
0,05 \mumR_{a}, más preferentemente, no mayor de aproximadamente
0,03 \mumR_{a}, por ejemplo, no mayor de aproximadamente 0,01
\mumR_{a}.
La prótesis articular de la presente invención
puede comprender unos primero y segundo componentes de los cuales
uno tiene una superficie de apoyo convexa genéricamente redondeada,
y el otro es cóncavo y puede recibir dentro del componente con la
superficie de apoyo convexa. Alguno de dichos componentes cóncavos
son a veces designados como "componentes de copa". Ejemplos de
prótesis articulares con estas características incluyen las prótesis
articulares de cadera y hombro. En muchos productos, las superficies
de apoyo redondeadas serán sustancialmente esféricas, aunque debe
entenderse que la superficie tendrá genéricamente la configuración
de solo una parte de una esfera. Preferentemente, el componente con
la superficie de apoyo convexa es el segundo componente y tiene una
superficie de apoyo cerámica, teniendo el componente cóncavo una
superficie de apoyo metálica, aunque pueden utilizarse
construcciones alternativas en las cuales el componente con la
superficie de apoyo convexa sea el primer componente con una
superficie de apoyo metálica.
Cuando uno de los componentes tiene una
superficie de apoyo redondeada (especialmente esférica), su diámetro
puede ser de hasta 60 mm o más. Sin embargo puede ser preferente que
el diámetro sea no mayor de aproximadamente 40 mm, más
preferentemente no mayor de aproximadamente 35 mm (por ejemplo,
aproximadamente 28 mm), y puede ser no mayor de aproximadamente 20
mm, por ejemplo no mayor de 15 mm, e incluso tan reducido como de
aproximadamente 10 mm. Es una ventaja del uso de la combinación de
materiales duros utilizada en la prótesis de la presente invención,
y de su resistencia combinada al desgaste, que posibilitan que las
superficies de apoyo se creen con unas dimensiones transversales
más pequeñas (diámetro en el caso de una superficie de apoyo
esférica) de las que se han encontrado como necesarias cuando se
utilizan otros materiales superficiales de apoyo.
Cuando las superficies de apoyo son esféricas es
preferible que sus diámetros sean controlados de forma que las
variaciones no excedan de aproximadamente 0,02 mm, más
preferentemente de aproximadamente 0,015 mm, especialmente de
aproximadamente 0,005 mm. En el caso de al menos un componente
cerámico, es posible que las variaciones de diámetro se mantengan
por debajo de aproximadamente 0,002 mm, por ejemplo por debajo de
aproximadamente 0,001 mm.
Un ejemplo de una técnica que puede utilizarse
para obtener un componente con una superficie de apoyo que sea
esférica y pulimentada implica la conformación del componente
alrededor de su eje geométrico contra una piedra abrasiva
cilíndrica, girando ésta última alrededor de su eje. El componente y
la piedra abrasiva están dispuestos formando un ángulo entre sus
ejes de aproximadamente 45º, y de forma que sus ejes se entrecrucen
en el centro de la esfera sobre el componente. La rotación combinada
del componente y de la piedra abrasiva crea una superficie esférica
con tal de que el círculo de contacto cubra tanto el polo como el
ecuador de la esfera. Puede incluirse una etapa de pulimentado
final para optimizar el acabado de la superficie.
El componente de copa puede tener una superficie
de apoyo interna cóncava con una configuración genéricamente
esférica. Es sabido que dichas superficies de apoyo tienen unas
configuraciones que se desvían de la exacta esfericidad, y dichas
desviaciones podrían incorporarse en la prótesis articular de la
presente invención.
Los primero y segundo componentes de la prótesis
de la invención pueden estar hechos enteramente de materiales
metálicos y cerámicos que proporcionen las superficies de apoyo.
Frecuentemente, sin embargo, será preferente que los materiales de
las superficies de apoyo proporcionen sólo una parte de los
componentes. Por ejemplo, en el caso del componente femoral de una
prótesis articular de cadera, la cabeza esférica (que se aloja en
el componente de copa cotiloideo) podría estar formada como un
bloque sólido de material cerámico, con un hueco ahusado en su
interior el cual puede recibir un pasador ahusado sobre la parte
principal del cuerpo del componente femoral. La parte principal,
con el vástago que se extiende por el interior de la cavidad medular
del fémur, puede estar hecha, como es sabido, de un material
metálico.
Al menos uno de los componentes de la prótesis
podría estar provisto de un material de apoyo suministrado como capa
superficial. Por ejemplo, el segundo componente podría estar
genéricamente constituido a partir de uno o más materiales
metálicos, con una capa superficial de un material cerámico el cual
está dispuesto para suministrar la superficie de apoyo. La capa
superficial será preferentemente de al menos aproximadamente 0,005
mm, más preferentemente al menos de aproximadamente 0,01 mm,
especialmente de al menos aproximadamente 0,05 mm, por ejemplo de
al menos de aproximadamente 0,1mm, 0,5 mm, 1,0 mm, 1,5 mm o 2,0 mm o
más. Debe entenderse que el revestimiento superficial del material
cerámico es tal que tiene que ser específicamente creado sobre el
componente, frente a los revestimientos superficiales que se
conforman mediante procesos tales como la oxidación debida a la
exposición a los agentes oxidativos ambientales. La dureza de una
capa superficial se mide de acuerdo con el baremo DIN50359.
Uno o ambos componentes articulares pueden
fabricarse como piezas separadas a partir de las cuales puedan
montarse a continuación los componentes. Por ejemplo, cuando la
prótesis se utiliza en la cadera de un paciente, el componente
femoral puede fabricarse como piezas de vástago y cabeza separadas,
proporcionando la cabeza la superficie de apoyo. De modo similar, el
componente cotiloideo puede fabricarse como piezas de cubierta y de
inserto (o de revestimiento) proporcionando el inserto la
correspondiente superficie de apoyo. Las piezas separadas entre los
componentes pueden fijarse entre sí utilizando técnicas conocidas,
incluyendo técnicas de fijación mecánicas (basadas especialmente en
fuerzas friccionables entre las correspondientes superficies
ahusadas).
A continuación se describirá una forma de
realización de una prótesis articular de acuerdo con la presente
invención, a modo de ejemplo y con referencia al dibujo que se
acompaña, el cual es una vista lateral, parcialmente en sección, a
través de la prótesis.
Con referencia al dibujo, una prótesis articular
de cadera 1 comprende un componente cotiloideo 2 y un componente
femoral 4. El componente femoral incluye una cabeza 6 que tiene una
superficie de apoyo convexa esférica. El componente cotiloideo
comprende una cubierta metálica 8 con unos orificios 10 que se
extienden a través de ella para ajustar unos tornillos mediante los
cuales la cubierta puede fijarse a la pelvis de un paciente.
El componente cotiloideo contiene un inserto 12
que tiene una superficie de apoyo cóncava esférica 14. El inserto y
la cubierta tienen unas superficies ahusadas correspondientes 16 que
encajan entre sí cuando el inserto es insertado dentro de la
cubierta. El inserto queda retenido en posición dentro de la
cubierta mediante fuerzas friccionales entre las superficies
ahusadas.
La cabeza 6 del componente femoral está hecha de
una alumina de calidad médica con una dureza de aproximadamente
16.000 MPa. La rugosidad superficial (R_{a}) de la cabeza es
inferior a aproximadamente 0,01 \mum. El inserto está hecho de
una aleación de calidad médica de cromo-cobalto de
alto contenido en carbono con una rugosidad superficial similar y
una dureza de aproximadamente 5.000 MPa. Los diámetros de la cabeza
del componente femoral y del inserto son de aproximadamente 28 mm,
con una tolerancia en el diámetro de 60 \mum. La dureza de los
materiales se midió de acuerdo con el baremo DIN50359 utilizando un
sistema Fischer 4100.
Una prótesis articular de acuerdo con lo
anteriormente descrito fue comparada con una prótesis en la cual la
cabeza del componente femoral estaba hecha con una aleación de
cromo-cobalto de bajo contenido en carbono en lugar
de alumina para posibilitar la realización de comparaciones con
respecto a las tasas de desgaste. Se probaron las prótesis en una
posición anatómica adecuada en un simulador de cadera con dos ejes
de movimiento y un eje de carga, tal como se describe por Barbor
et al en el Proc Instn Mech Engnrs, Part H. Vol. 213 (1999).
Las pautas de articulación y carga se encontraban en línea con lo
descrito por Paul en Proc Instn Mech Engnrs, Vol. 181 Part 3J,
pp8-15 (1967). Las prótesis se lubricaron utilizando
suero bovino (25%). Las prótesis se probaron durante cinco millones
de ciclos. La articulación fue interrumpida de vez en cuando para
recoger el lubricante y cualquier residuo de desgaste con fines de
análisis.
Se encontró que los residuos de desgaste de la
prótesis de cerámica sobre metal de acuerdo con la presente
invención resultaba significativamente menos cuantioso que en la
prótesis con superficies de apoyo de metal sobre metal. La prótesis
de metal sobre metal mostró una elevada tasa de desgaste "de
asiento" (ciclos de 3,12 \pm 0,45 mm^{3}/10^{6}) durante
aproximadamente el primer millón de ciclos, la cual se redujo a
continuación hasta un estado de tasa de desgaste uniforme inferior
(ciclos de 1,56 \pm 0,78 mm^{3}/10^{6}). Se detectó un
desgaste muy bajo en los componentes de cerámica sobre metal,
ascendiendo hasta aproximadamente 0,01 \pm 0,45 mm^{3}/10^{6}
ciclos, a lo largo del curso del entero test de tres millones de
ciclos, aunque la cantidad precisa de desgaste no pudo ser medida
con precisión utilizando los aparatos de pesar disponibles. Así
mismo, sustancialmente todo el residuo de desgaste de los
componentes de cerámica sobre metal fue metálico.
Los tamaños medios de las partículas de los
residuos de desgaste fueron de 18 \pm 1,37 mm para los componentes
de cerámica sobre metal y, de 30 \pm 0,25 mm para los componentes
de metal sobre metal.
Claims (11)
1. Prótesis articular ortopédica que comprende
unos primero y segundo componentes de articulación (2,4) que tienen
unas superficies de apoyo respectivas en contacto mutuo,
comprendiendo el material de la primera superficie de apoyo un
material metálico y comprendiendo el material de la segunda
superficie de apoyo (14) un material cerámico, siendo la dureza del
material metálico de al menos aproximadamente 2.500 MPa, y siendo la
dureza del material cerámico mayor que la del material metálico en
al menos aproximadamente 4.000 MPa, caracterizada porque las
primera y segunda superficies de apoyo son ambas esféricas e
íntimamente coincidentes, siendo la variación del radio de cada una
de las superficies de apoyo no mayor de 0,01 mm y siendo la
rugosidad superficial de cada una de las superficies de apoyo no
mayor de 0,05 \mum R_{a}.
2. Prótesis de acuerdo con la reivindicación 1,
en la cual la variación de radio de cada una de las superficies de
apoyo no es mayor de 0,0075 mm.
3. Prótesis de acuerdo con la reivindicación
1, en la que la relación de la dureza del material cerámico de la
segunda superficie de apoyo con respecto a la del metal de la
primera superficie de apoyo es al menos de aproximadamente 2.
4. Prótesis de acuerdo con la reivindicación 3,
en la que dicha relación de dureza es de al menos aproximadamente
3.
5. Prótesis de acuerdo con la reivindicación
1, en la que la diferencia entre la dureza del material cerámico de
la segunda superficie de apoyo y la del metal de la primera
superficie de apoyo no es mayor de aproximadamente 30.000 MPa.
6. Prótesis de acuerdo con la reivindicación
1, en la que la dureza del material cerámico de la segunda
superficie de apoyo es de al menos aproximadamente 10.000 MPa.
7. Prótesis de acuerdo con la reivindicación
1, en la que el material cerámico comprende un óxido duro.
8. Prótesis de acuerdo con la reivindicación
1, en la que uno de los componentes tiene una superficie de apoyo
sustancialmente convexa, y el otro de los componentes es cóncavo y
puede recibir el componente con la superficie de apoyo convexa
dentro de éste.
9. Prótesis de acuerdo con la reivindicación
8, en la que el segundo componente tiene la superficie de apoyo
convexa y el primer componente es cóncavo.
10. Prótesis de acuerdo con la reivindicación 1,
en la que el material de la superficie de apoyo de al menos uno de
los componentes es suministrado por una capa superficial.
11. Prótesis de acuerdo con la reivindicación
10, en la que el grosor de la capa superficial es de al menos
aproximadamente 50 \mum.
