Definitions
- the invention relates to a bone screw with a threaded screw shank and with a screw head, which has two opposite legs forming a receptacle between them, which are integrally connected to a tissue protection sleeve, the tissue protection sleeve extending from its one edge to the opposite edge extending slot.
- the invention further relates to a sleeve remover.
- Bone screws are widely used in medicine for surgical applications and are used in the form of pedicle screws to correct and stabilize the spine.
- the screw shaft designed as a threaded shaft is supplemented by a screw head designed as a tulip head with a receptacle between the legs, in which a rod can be placed and fixed from cranial to caudal, which connects adjacent vertebral bodies or entire groups of vertebrae.
- pedicle screws are known, for example, from EP 2452642 A2.
- the placement of the bone screw with the rod can be done in a conventional manner over an extensively exposed operating field, although the musculature must be detached from the spinous processes of the vertebrae and moved to the side in order to expose the pedicles.
- a minimally invasive approach in which a small skin incision is made over the area where the bone screw is to be placed, is therefore more advantageous.
- Using dilation sleeves a Operation channel created for setting the bone screw or pedicle screw.
- a tissue protection sleeve is also required, which is coupled to the bone screw and must be removed again after the rod has been placed and fixed.
- the bone screw mentioned at the outset is characterized in that the tissue protection sleeve is firmly connected to the screw head, namely both legs, so no measures are required to detachably couple the tissue protection sleeve to the bone screw before the operation and to release this coupling again intraoperatively.
- the bone screw can be handled as one part with the tissue protection sleeve, so that the tissue protection sleeve always and safely performs its protective function for the operating channel. Nevertheless, the tissue protection sleeve must be separated from the bone screw again after performing its function, for which purpose the tissue protection sleeve has a particularly suitable design with the slot extending over its entire length, which creates an access that can be used for the separation.
- the legs of the screw head are expediently connected to the tissue protection sleeve via a predetermined breaking point.
- This predetermined breaking point can be acted upon starting from the slit for separation. Because of the slit, the tissue protection sleeve is not closed in the circumferential direction, which limits its stability. In order to compensate or at least partially compensate for this, it is provided that the tissue protection sleeve has a stabilizing bracket on the edge opposite the legs.
- the tissue protection sleeve has, on the side opposite the slot, an opening extending from the edge facing the leg over part of the length of the tissue protection sleeve, through which the rod can be inserted and inserted into the receptacle, even if the tissue protection sleeve is still connected to the screw head.
- the tissue protection sleeve has a circular cross section and the slot extends over an arc of a circle with a length of between 15° and 90°, ie the arc of a circle covers an angular range which corresponds to a minimum of 15°.
- the usable interval for the size of the circular arc is up to 90°, i.e. a quarter of a circle, so that sufficient material remains for the purpose of the tissue protection sleeve.
- a sleeve remover that can be pushed over the tissue protection sleeve is provided for interacting with the bone screw, which sleeve remover, when pushed onto the tissue protection sleeve, has a separating member on its inner circumference at its end facing the legs, the extent of which in the circumferential direction is smaller than that circumferential extent of the slot.
- the separating member can thus be inserted into the slot and guided over the entire length of the tissue protection sleeve to its end facing the screw head, in order to act there close to the screw head, in particular on the predetermined breaking point, and to separate the tissue protection sleeve.
- the separating member is designed as a blade, which is guided along the predetermined breaking point by twisting the sleeve remover about its longitudinal axis and separating it.
- the blade ensures a clean cut with no splinters or chips and no burrs are formed.
- the cutting edge, particularly on the screw head, is not formed with sharp edges. It should also be noted that the blade acts on the predetermined breaking point in the direction of the longitudinal axis of the screw in the direction of the inside and thus prevents the legs from bending up.
- the sleeve remover has a drive opening in its circumference for the introduction of the torque for rotating the sleeve remover.
- a pin for the drive can be inserted into this drive opening.
- the drive opening is expediently arranged on the end pointing away from the legs and/or on the side opposite the separating member, since this improves handling.
- a thread is formed on the inside of the legs for receiving a fixing screw, which is below the predetermined breaking point.
- the fixing screw can be placed more easily if there is also a sleeve thread in the tissue protection sleeve adjoining the internal thread, at least over part of the longitudinal extension of the tissue protection sleeve.
- Fig. 1 is a side view of a bone screw, the two one
- FIG. 2 shows a perspective view of a fluff remover which has a separating member on its inner peripheral surface
- Fig. 3 shows a perspective view of the bone screw used as a pedicle screw, in the screw head between the two legs of which a rod is inserted and secured by a fixing screw, with the tissue protection sleeve connected to the screw head, for the removal of which the sleeve remover is provided
- Fig. 4 shows a side section of the pedicle screw from FIG. 3 with the tissue protection sleeve and the rod in a view of its end face
- FIG. 1 shows a bone screw 1 in the specific configuration as a pedicle screw.
- the bone screw 1 is formed with a screw shank 2 having a thread and with a screw head 3 which has two opposite legs 5 forming a receptacle 4 between them.
- a rod 6 can be inserted into this receptacle 4, which serves to correct and stabilize a spinal column by connecting adjacent vertebral bodies or groups of vertebrae with a corresponding number of pedicle screws using the rod 6.
- the legs 5 forming a tulip head are connected in one piece via a predetermined breaking point 7 to a tissue protection sleeve 8, which has a slot 9 extending from its one edge to the opposite edge, with the tissue protection sleeve 8 being circular in cross section and the Slot 9 extends over an arc of a circle with a length selected from an interval between 15° and 90°.
- the tissue protection sleeve 8 has a stabilizing bracket 10 on the edge opposite the legs 5 and an opening 11 extending from the edge facing the leg 5 over a part of the longitudinal extension of the tissue protection sleeve on the side opposite the slot 9 .
- the sleeve remover 12 thus has a body 15 designed as a hollow cylinder, which carries the separating member 14 at the end 13 on its inner circumference, which extends over part of the circumferential direction.
- the extension is dimensioned in such a way that the separating member 14 fits into the slot 9, ie can be easily inserted into it.
- the body 15 has a drive opening 16 in its circumference, with the drive opening 16 being arranged in the exemplary embodiment shown at the end pointing away from the legs 5 and on the side opposite the separating member 14.
