Abstract

Um eine Hüftpfanne (12; 212; 412), insbesondere für eine Revisionshüftgelenkendoprothese, umfassend eine Gelenkpfanne (16; 216; 416) zum Implantieren in den Beckenknochen (10) eines Patienten und mindestens eine plattenförmige Befestigungslasche (18, 20, 22; 218, 220, 222; 418, 420, 422), so zu verbessern, dass eine optimale Befestigung der Hüftpfanne am Patienten ermöglicht wird, wird vorgeschlagen, dass die mindestens eine Befestigungslasche mit der Gelenkpfanne kraft- und/oder formschlüssig koppelbar ist und dass die Hüftpfanne eine Kopplungseinrichtung (42; 242; 442) zum kraft- und/oder formschlüssigen Koppeln der mindestens einen Befestigungslasche mit der Gelenkpfanne in einer Kopplungsstellung umfasst.

Description

HÜFTGELENKENDOPROTHESE UND REVISIONSHÜFTGELENKENDOPROTHESE
Die vorliegende Erfindung betrifft eine Hüftpfanne, insbesondere für eine Revi sionshüftgelenkendoprothese, umfassend eine Gelenkpfanne zum Implantieren in den Beckenknochen eines Patienten und mindestens eine plattenförmige Befestigungslasche.
Ferner betrifft die vorliegende Erfindung eine Revisionshüftgelenkendoprothese umfassend eine Hüftpfanne, welche in einer Implantationsstellung einen Ge lenkpfanneneinsatz mit einer hohlkugeligen Gelenkkopfaufnahme aufnimmt, und einen Hüftschaft mit einem Gelenkkopf zur Ausbildung eines Kugelgelenks im Zusammenwirken mit der zum Gelenkkopf korrespondierend ausgebildeten Gelenkkopfaufnahme.
Hüftpfannen und Hüftgelenkendoprothesen der eingangs beschriebenen Art sind in verschiedenen Varianten bekannt. Sie kommen zum Einsatz, wenn bei einem Patienten eine Hüftgelenkendoprothese erstmalig implantiert oder aus getauscht wird. Insbesondere bei einer Revisionsoperation, bei der eine Hüft gelenkendoprothese ausgetauscht wird, sieht sich ein orthopädischer Chirurg verschiedenen Herausforderungen gegenüber. So liegen beispielsweise auf grund eines Versagens und/oder durch die Explantation, also den Ausbau der ursprünglich implantierten Hüftpfanne, Knochendefektsituationen vor, die re lativ Undefiniert sind, also eine hohe Varianz aufweisen. Dies erschwert insbe sondere die Befestigung einer nachfolgend zu implantierenden Hüftpfanne in Form einer Revisionspfanne. Sehr häufig sind nur sehr wenig Knochen und da mit kaum Verankerungsmöglichkeiten für die Revisionspfanne vorhanden, um eine reine Pressfit-Verankerung oder eine Verschraubung der Hüftpfanne zu realisieren. In diesen Fällen kommen sogenannte Revisionspfannen mit Befes tigungslaschen zum Einsatz. Allerdings passen diese nicht immer optimal auf die jeweilige Situation am Becken des Patienten. Es ist daher eine Aufgabe der vorliegenden Erfindung, eine Hüftpfanne und ei ne Hüftgelenkendoprothese der eingangs beschriebenen Art so zu verbessern, dass eine optimale Befestigung der Hüftpfanne am Patienten ermöglicht wird.
Diese Aufgabe wird bei einer Hüftpfanne der eingangs beschriebenen Art erfin dungsgemäß dadurch gelöst, dass die mindestens eine Befestigungslasche mit der Gelenkpfanne kraft- und/oder formschlüssig koppelbar ist und dass die Hüftpfanne eine Kopplungseinrichtung zum kraft- und/oder formschlüssigen Koppeln der mindestens einen Befestigungslasche mit der Gelenkpfanne in einer Kopplungsstellung umfasst.
Die vorgeschlagene Weiterbildung ermöglicht es insbesondere, eine Hüftpfan ne in Form einer Revisionspfanne in gewünschter Weise am Becken eines Pa tienten zu positionieren. Die mindestens eine Befestigungslasche kann dann, abhängig von der konkreten Situation beim Patienten, ebenfalls in gewünsch ter Weise positioniert und in einer optimalen Befestigungsstellung mit der Ge lenkpfanne kraft- und/oder formschlüssig gekoppelt werden, beispielsweise di rekt oder indirekt, wie nachfolgend anhand einschlägiger Ausführungsformen noch erläutert wird. Die Kopplungseinrichtung ermöglicht insbesondere eine Verbindung der mindestens einen Befestigungslasche mit der Gelenkpfanne auf einfache Weise. Anders als bei Hüftpfannen, bei denen Befestigungsla schen dauerhaft fest angeordnet sind, kann die wie vorgeschlagen weitergebil dete Hüftpfanne in unterschiedlichsten Situationen optimal implantiert werden. Eine indirekte Kopplung der mindestens einen Befestigungslasche mit der Ge lenkpfanne kann insbesondere dadurch realisiert werden, dass die mindestens eine Befestigungslasche an einem Träger oder einem Trägerelement angeord net oder ausgebildet ist, und dass der Träger oder das Trägerelement kraft- und/oder formschlüssig mit der Gelenkpfanne koppelbar sind. Die Kopplungs einrichtung ist dann ausgebildet zum Koppeln des Trägers oder des Träger elements mit der Gelenkpfanne. Ferner sei an dieser Stelle angemerkt, dass eine wie vorgeschlagen weitergebildete Hüftpfanne sowohl zur Ausbildung ei ner Hüftgelenkendoprothese, die zur Primärversorgung dient, als auch zur Ausbildung einer Revisionshüftgelenkendoprothese, die bei einer Prothesen revision genutzt wird, eingesetzt werden kann.
Um insbesondere eine hohe Stabilität und einen erleichterten chirurgischen Ablauf bei der Befestigung der Hüftpfanne am Knochen zu erhalten, ist es günstig, wenn an der Gelenkpfanne mindestens eine Befestigungslasche fest stehend angeordnet oder ausgebildet ist. Insbesondere kann diese einstückig, beispielsweise monolithisch, mit der Gelenkpfanne ausgebildet sein.
Um die Hüftpfanne beispielsweise mit Knochenschrauben an einem Knochen festlegen zu können, ist es günstig, wenn die mindestens eine Befestigungsla sche mindestens eine Befestigungselementdurchbrechung umfasst. Insbeson dere können zwei, drei, vier oder mehr Befestigungselementdurchbrechungen vorgesehen sein. Derartige Befestigungselementdurchbrechungen können ins besondere durch Bohrungen an der Befestigungslasche ausgebildet sein.
Vorteilhaft ist es, wenn die die mindestens eine Befestigungselementdurchbre chung eine konische oder hohlkugelige Innenfläche definiert. Insbesondere kann sich die mindestens eine Befestigungselementdurchbrechung von einer Oberseite zu einer Unterseite der mindestens einen Befestigungslasche koni sch verjüngen. Dies ermöglicht es insbesondere, Schrauben mit Senkköpfen einzusetzen, um die Befestigungslasche am Knochen zu fixieren. Dadurch können bei der Implantation Kanten und Vorsprünge, die mit Knochen und Weichteilgewebe in Kontakt kommen und diese schädigen könnten, minimiert werden. Hohlkugelige Innenflächen erlauben eine flächige Anlage von sphäri schen Schraubenköpfen.
Auf einfache Weise lässt sich die mindestens eine Befestigungslasche mit der Gelenkpfanne verbinden, wenn die Kopplungseinrichtung mindestens ein ers tes Kopplungselement und mindestens ein zweites Kopplungselement umfasst, die einerseits an der Gelenkpfanne und andererseits an der mindestens einen Befestigungslasche angeordnet oder ausgebildet oder dieser zugeordnet sind, und wenn das mindestens eine erste Kopplungselement und das mindestens eine zweite Kopplungselement in der Kopplungsstellung kraft- und/oder form schlüssig in Eingriff stehen. Eine Zuordnung eines der Kopplungselemente zur mindestens einen Befestigungslasche kann insbesondere dadurch erreicht wer den, dass das Kopplungselement am Träger oder Trägerelement angeordnet oder ausgebildet ist, an dem die mindestens eine Befestigungslasche angeord net ist.
Die Kopplungseinrichtung lässt sich auf einfache Weise ausbilden, wenn das mindestens eine erste Kopplungselement in Form eines Kopplungsvorsprungs ausgebildet ist und wenn das mindestens eine zweite Kopplungselement in Form einer Kopplungsaufnahme ausgebildet ist. Insbesondere kann die Kopp lungsaufnahme korrespondierend zum mindestens einen Kopplungsvorsprung ausgebildet sein. Grundsätzlich kann der Kopplungsvorsprung an der Gelenk pfanne oder am Befestigungselement oder an einem Träger oder Trägerele ment, an dem das Befestigungselement angeordnet oder ausgebildet ist, an geordnet sein. Alternativ kann statt dieses Kopplungsvorsprungs auch eine Kopplungsaufnahme vorgesehen sein, die dann mit einem Kopplungsvorsprung an der Gelenkpfanne in der Kopplungsstellung zusammenwirkt.
Für eine einfache Handhabung der Hüftpfanne ist es günstig, wenn die Kopp lungseinrichtung in Form einer Schraubverbindungseinrichtung, einer Klemmeinrichtung und/oder in Form einer Bajonettverbindungseinrichtung ausgebildet ist. So können die Gelenkpfanne und das mindestens eine Befesti gungselement oder ein Träger oder Trägerelement, an welchem das mindes tens eine Befestigungselement angeordnet ist, insbesondere schnell, einfach und zuverlässig miteinander gekoppelt werden.
Insbesondere für eine einfache Ausbildung einer Schraubverbindungseinrich tung ist es vorteilhaft, wenn die Kopplungsaufnahme ein Innengewinde um fasst und wenn der korrespondierende Kopplungsvorsprung ein zum Innenge winde korrespondierendes Außengewinde umfasst. Zudem kann so insbeson dere auch eine Handhabung der Hüftpfanne vereinfacht werden. Beispielswei se, kann die Komponente mit der Kopplungsaufnahme von außen auf den Kopplungsvorsprung aufgeschraubt werden. So lässt sich die Gelenkpfanne optional auch ohne zusätzliche Befestigungslaschen nutzen, beispielsweise für eine Erstimplantation. Bei Bedarf kann die dann noch während des chirurgi schen Eingriffs durch Aufschrauben eines Trägers oder Trägerelements mit der mindestens einen weiteren Befestigungslasche auf die Gelenkpfanne von au ßen eine oder mehrere weitere Befestigungsoptionen geschaffen werden. Eine Aufnahme für einen Gelenkpfanneneinsatz mit einer hohlkugeligen Gelenk kopfaufnahme bleibt dabei unverändert.
Um eine hohe Stabilität der Hüftpfanne in der Kopplungsstellung zu ermögli chen, ist es günstig, wenn das mindestens eine erste Kopplungselement oder das mindestens eine zweite Kopplungselement einstückig, insbesondere mono lithisch, an der mindestens einen Befestigungslasche angeordnet oder ausge bildet sind.
Gemäß einer vorteilhaften Ausführungsform der Erfindung kann vorgesehen sein, dass der Kopplungsvorsprung in Form eines Kopplungseinsatzes ausge bildet ist und dass die Gelenkpfanne die Kopplungsaufnahme für den Kopp lungseinsatz bildet. Insbesondere kann der Kopplungseinsatz hülsenförmig ausgebildet sein. Ein solcher Kopplungseinsatz wird also nicht außen an der Gelenkpfanne angeordnet, sondern in der Kopplungsaufnahme. Üblicherweise wird in die Gelenkpfanne ein Gelenkpfanneneinsatz eingesetzt mit einer hohl kugeligen Gelenkkopfaufnahme. Dies ist trotzdem möglich, denn der Gelenk pfanneneinsatz kann dann in den insbesondere hülsenförmigen Kopplungsein satz eingesetzt werden. Diese Ausgestaltung ermöglicht eine hohe Stabilität der Verbindung zwischen dem Kopplungseinsatz und der Gelenkpfanne. An dieser Stelle sei angemerkt, dass der Kopplungseinsatz, der insgesamt den Kopplungsvorsprung definiert, insbesondere einen Träger für die mindestens eine Befestigungslasche bildet.
Wie bereits erwähnt kann eine hohe Stabilität einer Verbindung des Kopp lungseinsatzes und der Gelenkpfanne in der Kopplungsstellung insbesondere dadurch erreicht werden, dass der Kopplungseinsatz in der Kopplungsstellung zwischen der Gelenkpfanne und einem in dieser aufgenommenen Gelenk pfanneneinsatz angeordnet ist. Beispielsweise kann hier eine doppelte Klemm verbindung ausgebildet werden, einerseits zwischen dem Kopplungseinsatz und der Gelenkpfanne und andererseits zwischen dem Gelenkpfanneneinsatz und dem Kopplungseinsatz. Alternativ kann auch vorgesehen sein, dass der Kopplungseinsatz eine Artikulationsfläche zur Ausbildung einer Dual-Mobility- Prothese aufweist, wobei die Artikulationsfläche mit einer korrespondierenden Artikulationsfläche eines Gelenkpfanneneinsatzes zusammenwirkt. Der Gelenk pfanneneinsatz weist in diesem Falle dann zusätzlich eine hohlkugelige Ge lenkkopfaufnahme (544) auf, um einen an einem Hüftschaft angeordneten oder ausgebildeten Gelenkkopf zur Ausbildung eines Kugelgelenks aufzuneh men.
Um insbesondere eine dauerhaft stabile Befestigung der Hüftpfanne am Kno chen des Patienten sicherstellen zu können, ist es günstig, wenn der Kopp lungseinsatz und die Gelenkpfanne kraft- und/oder formschlüssig in der Kopp lungsstellung in Eingriff stehen.
Vorteilhaft ist es, wenn der Kopplungseinsatz eine äußere erste konische Klemmfläche aufweist und wenn die erste Klemmfläche korrespondierend zu einer inneren zweiten konischen Klemmfläche der Gelenkpfanne ausgebildet ist. Der Kopplungseinsatz kann insbesondere hülsenförmig ausgebildet sein. Diese Ausgestaltung ermöglicht insbesondere die Ausbildung einer Klemmver bindung zwischen der ersten und der zweiten Klemmfläche und damit zwi schen dem Kopplungseinsatz und der Gelenkpfanne.
Um eine hohe Stabilität der Verbindung zwischen dem Kopplungseinsatz und der Gelenkpfanne erreichen zu können, ist es günstig, wenn die erste Klemm fläche und die zweite Klemmfläche in der Kopplungsstellung eine selbsthem mende Verbindung zwischen dem Kopplungseinsatz und der Gelenkpfanne ausbilden. Eine solche selbsthemmende Klemmverbindung kann insbesondere in Abhängigkeit des gewählten Materials oder der gewählten Materialien, aus denen der Kopplungseinsatz einerseits und die Gelenkpfanne andererseits aus gebildet sind, sowie unter Berücksichtigung eines Konuswinkels der ersten und zweiten Klemmfläche nach Bedarf vorgegeben werden. Auch eine Rauheit der Oberfläche kann bei einer solchen Verbindung als Parameter eine Rolle spie len. Alternativ oder zusätzlich könnte auch eine kontrollierte Kaltverschwei ßung zur Verbindung zwischen dem Kopplungseinsatz und der Gelenkpfanne dienen. So kann mit einem vorgegebenen Anzugsmoment beispielsweise eine zweiteilige Schraube gekoppelt werden, wobei bei einer solchen Kaltverschwei- ßung dann ein Lösemoment größer ist als das Anzugsmoment. Beispielsweise kann das Lösemoment mehr als 20 Nm betragen, das Anzugsmoment jedoch lediglich lONm. Dies kann insbesondere durch eine konstruktive Auslegung der Flächenpressung erreicht werden, die zur teilweisen Kaltverschweißung führt.
Ferner ist es vorteilhaft, wenn der Kopplungseinsatz eine innere dritte koni sche Klemmfläche aufweist und wenn die dritte Klemmfläche korrespondierend zu einer äußeren vierten konischen Klemmfläche des Gelenkpfanneneinsatzes ausgebildet ist. So kann auch zwischen der dritten und vierten Klemmfläche eine Konusverbindung ausgebildet werden, ähnlich wie zwischen der ersten und zweiten Klemmfläche.
Um insbesondere eine besonders stabile Verbindung zwischen dem Kopplungs einsatz und dem Gelenkpfanneneinsatz erreichen zu können, ist es günstig, wenn die dritte Klemmfläche und die vierte Klemmfläche in der Kopplungs stellung eine selbsthemmende Verbindung zwischen dem Kopplungseinsatz und dem Gelenkpfanneneinsatz ausbilden. Wie auch bei einer optional vorge sehen selbsthemmenden Verbindung zwischen der ersten und zweiten Klemm fläche kann eine solche selbsthemmende Verbindung zwischen der dritten und vierten Klemmfläche durch Erfüllen der für eine Selbsthemmung erforderlichen Bedingungen erreicht werden, also insbesondere ein entsprechender Konus winkel in Abhängigkeit der Materialpaarung Gelenkpfanneneinsatz einerseits und Kopplungseinsatz andererseits. Eine besonders hohe Stabilität bei der Verbindung zwischen Hüftpfanne und dem Becken des Patienten lässt sich beispielsweise dadurch erreichen, dass die mindestens eine Befestigungslasche einstückig, insbesondere monolithisch, mit dem Kopplungseinsatz ausgebildet ist.
Um eine hohe Variabilität bei der Befestigung der Hüftpfanne am Knochen des Patienten zu gestatten, ist es günstig, wenn der Kopplungseinsatz eine, zwei, drei oder mehr Befestigungslaschen trägt. Diese können einstückig mit dem Kopplungseinsatz ausgebildet sein oder wahlweise lösbar verbindbar, bei spielsweise mittels einer kraft- und/oder formschlüssigen Verbindung.
Gemäß einer weiteren bevorzugten Ausführungsform der Erfindung kann vor gesehen sein, dass der Kopplungsvorsprung in Form eines hülsenförmigen Pfannenabschnitts der Gelenkpfanne ausgebildet ist, dass die Kopplungsein richtung einen Kopplungsring umfasst, welcher die Kopplungsaufnahme defi niert, und dass der Kopplungsring in der Kopplungsstellung den hülsenförmi gen Pfannenabschnitt umgibt, insbesondere kraft- und/oder formschlüssig. Der Kopplungsring bildet in diesem Fall also insbesondere einen Träger bezie hungsweise ein Trägerelement, an dem die mindestens eine Befestigungsla sche angeordnet oder ausgebildet ist. Der Kopplungsring und die Gelenk pfanne sind wie beschrieben derart ausgebildet, dass sie in der Kopplungs stellung kraft- und/oder formschlüssig in Eingriff stehen. Dies ermöglicht eine hohe Stabilität der Hüftpfanne selbst und damit auch eine hohe Stabilität für die Befestigung der Hüftpfanne am Becken des Patienten.
Günstigerweise ist die mindestens eine Befestigungslasche einstückig, insbe sondere monolithisch, mit dem Kopplungsring ausgebildet. So können insbe sondere Relativbewegungen zwischen der Befestigungslasche und dem Kopp lungsring ausgeschlossen werden. Bei dieser Ausgestaltung bildet der Kopp lungsring eine Art Träger oder Trägerelement für die mindestens eine Befesti gungslasche. Um eine hohe Flexibilität bei der Befestigung der Gelenkpfanne am Knochen des Patienten zu ermöglichen, ist es günstig, wenn der Kopplungsring eine, zwei, drei oder mehr Befestigungslaschen trägt.
Um den Kopplungsring und die Gelenkpfanne insbesondere einfach und schnell miteinander koppeln zu können, ist es vorteilhaft, wenn der hülsenförmige Pfannenabschnitt und der Kopplungsring Komponenten einer Bajonettverbin- dungseinrichtung bilden. Bei Bajonettverbindungseinrichtungen ist es üblich, dass die miteinander zu koppelnden Komponenten zunächst aufeinander zu bewegt und dann relativ zueinander um eine Achse, die durch die Bewegungs richtung aufeinander zu definiert wird, verdreht werden. So lassen sich insbe sondere definierte Verbindungsstellungen oder Kopplungsstellungen erreichen. Alternativ können zur Befestigung des Kopplungsrings am hülsenförmige Pfan nenabschnitt über eine Schnappverbindung oder einen Presssitz miteinander gekoppelt werden.
Vorzugsweise überdeckt der Kopplungsring höchstens 5%, insbesondere höchstens 2%, einer äußeren Oberfläche der Gelenkpfanne. So ist die Gelenk pfanne also nahezu vollständig frei und kann mit dem Knochen des Patienten in Kontakt kommen. Insbesondere ermöglicht es diese Ausgestaltung, an der Gelenkpfanne optionale Befestigungsdurchbrechungen vorzusehen, die die Aufnahme beispielsweise von Befestigungselementen in Form von Knochen schrauben ermöglichen, um die Gelenkpfanne direkt am Knochen festzu schrauben.
Um den Kopplungsring an der Gelenkpfanne sicher und insbesondere in defi nierter Weise in der Kopplungsstellung halten zu können, ist es vorteilhaft, wenn die Gelenkpfanne eine von ihr nach außen weg weisend geöffnete Auf nahmenut aufweist und wenn der Kopplungsring in der Kopplungsstellung in der Aufnahmenut gehalten ist. Eine solche Ausgestaltung ermöglicht in der Kopplungsstellung allenfalls eine Rotation des Kopplungsrings und der Gelenk pfanne relativ zueinander um eine vom Kopplungsring definierte Längsachse. Die Kopplung zwischen der Gelenkpfanne und dem Kopplungsring kann auf einfache Weise insbesondere dadurch erreicht werden, dass die Aufnahmenut seitlich von einem ersten Ringflansch und einem zweiten Ringflansch begrenzt ist und dass die beiden Ringflansche parallel zueinander verlaufen. So kann sich der Kopplungsring beispielsweise in der Kopplungsstellung an beiden Ringflanschen abstützten.
Um eine optimale Abstützung des Kopplungsrings an der Aufnahmenut zu er möglichen, ist es vorteilhaft, wenn der von einem umlaufenden Rand der Ge lenkpfanne weiter entfernt angeordnete erste Ringflansch eine konstante oder im Wesentlichen konstante Breite aufweist. Beispielsweise dann, wenn der Kopplungsring keine konstante Breite aufweist, ist so sichergestellt, dass un abhängig von einer Rotationsstellung des Kopplungsrings relativ zur Gelenk pfanne, eine sichere Anlage des Kopplungsrings am ersten Ringflansch möglich ist. Insbesondere in einem Fall, wo der Kopplungsring Abschnitte in Umfangsrichtung aufweist, deren Breite geringer ist, als in Bereichen zwischen diesen Abschnitten, kann der Kopplungsring unabhänging von einer Dreh oder Rotationsstellung bezogen auf eine Längsachse des Kopplungsrings nicht über den ersten Ringflansch hinwegrutschen, wenn eine Breite des ersten Ringflansches konstant ist.
Vorteilhaft ist es, wenn der Kopplungsring eine Kopplungsringlängsachse defi niert und wenn eine Kopplungsringbreite des Kopplungsrings in radialer Rich tung bezogen auf die Kopplungsringlängsachse über einen Umfang des Kopp lungsrings variiert. Eine solche Ausgestaltung ermöglicht insbesondere auf ein fache Weise die Ausbildung einer Bajonettverbindungseinrichtung zwischen dem Kopplungsring und der Gelenkpfanne. Beispielsweise kann der Kopp lungsring komplementär zum zweiten Ringflansch ausgebildet werden, so dass dieser in einer Eingriffstellung über den zweiten Ringflansch geschoben und dann relativ zu diesem verdreht werden kann, um in der Kopplungsstellung zwischen den beiden Ringflanschen kraft- und/oder formschlüssig gehalten zu sein. Vorteilhaft ist es, wenn der vom umlaufenden Rand weniger weit entfernt an geordnete zweite Ringflansch eine Ringflanschbreite und eine Ringflan schlängsachse definiert und wenn die Ringflanschbreite in radialer Richtung bezogen auf die Ringflanschlängsachse über einen Umfang des Ringflansches variiert. Insbesondere kann eine Variation der Ringflanschbreite derart vorge sehen sein, dass diese komplementär zur Kopplungsringbreite ist. Dies ermög licht wie beschrieben insbesondere das Bewegen des Kopplungsrings in einer Eingriffstellung über den zweiten Ringflansch hinweg und das Überführen der Hüftpfanne von der Eingriffstellung in die Kopplungsstellung durch Rotation des Kopplungsrings um die Ringflanschlängsachse relativ zur Gelenkpfanne.
Für die Ausbildung einer Bajonettverbindungseinrichtung zum Koppeln der Ge lenkpfanne und des Kopplungsrings ist es insbesondere vorteilhaft, wenn die Kopplungsringbreite mindestens zwei, insbesondere drei oder vier, Maxima von sich über einen Teil des Umfangs des Kopplungsrings erstreckende Kopp lungsringbereiche aufweist und wenn der zweite Ringflansch eine entspre chende Anzahl von sich über einen Teil seines Umfangs erstreckende Ring flanschbereiche mit einer minimalen Ringflanschbreite aufweist und wenn sich die Kopplungsringbereiche und die Ringflanschbereiche über einander entspre chende Winkelbereiche bezogen auf die Ringflanschlängsachse und die mit die ser in der Kopplungsstellung zusammenfallende Kopplungsringlängsachse er strecken. Je mehr Maxima der Kopplungsringbreite und korrespondierende mi nimale Ringflanschbreiten vorgesehen sind, umso mehr unterschiedliche Stellungen zum in Eingriff Bringen des Kopplungsrings und der Gelenkpfanne können vorgesehen werden. Dies erhöht insbesondere eine Flexibilität bei der Kopplung zwischen dem Kopplungsring und der Gelenkpfanne, insbesondere intraoperativ für einen Operateur.
Günstig ist es, wenn der Kopplungsring von einer Lösestellung, in welcher er von der Gelenkpfanne getrennt ist, durch eine Bewegung parallel zur Ringflan schlängsachse in Richtung auf den ersten Ringflansch in eine Zwischenstellung überführbar ist, wenn der Kopplungsring von der Zwischenstellung durch eine Drehung um die Ringflanschlängsachse in die Kopplungsstellung überführbar ist und wenn der Kopplungsring in der Kopplungsstellung zwischen dem ersten Ringflansch und dem zweiten Ringflansch gehalten ist. In der beschriebenen Weise lässt sich insbesondere eine Bajonettverbindungseinrichtung zwischen der Gelenkpfanne und dem Kopplungsring auf einfache Weise ausbilden, die das Koppeln und gegebenenfalls Lösen der Teile voneinander, dann in umge kehrter Reihenfolge, ermöglicht.
Gemäß einer weiteren vorteilhaften Ausführungsform der Erfindung kann vor gesehen sein, dass der Kopplungsvorsprung in Form eines Befestigungsla schenvorsprungs und die Kopplungsaufnahme in Form einer korrespondieren den Befestigungslaschenaufnahme ausgebildet sind, dass der Befestigungsla schenvorsprung an der mindestens einen Befestigungslasche angeordnet oder ausgebildet ist, dass die Befestigungslaschenaufnahme an der Gelenkpfanne angeordnet oder ausgebildet ist und dass der Befestigungslaschenvorsprung und die Befestigungslasche in der Kopplungsstellung kraft- und/oder form schlüssig in Eingriff stehen. Die vorgeschlagene Weiterbildung ermöglicht ins besondere ein direktes Koppeln einer einzelnen Befestigungslasche mit der Gelenkpfanne. Die Befestigungslaschenaufnahme kann an der Gelenkpfanne an einem Flansch, beispielsweise einem umlaufenden Ringflansch, oder einem Flanschabschnitt angeordnet oder ausgebildet sein, die zum Beispiel im Be reich eines umlaufenden Rands der Gelenkpfanne angeordnet oder ausgebildet sind. So kann die Befestigungslasche bei Bedarf auch noch intraoperativ mit einer bereits temporär oder schon dauerhaft mit dem Becken des Patienten gekoppelten Gelenkpfanne in Eingriff gebracht werden. Insbesondere kann so intraoperativ eine optimale Befestigung der Gelenkpfanne am Beckenknochen erreicht werden, und zwar durch individuelle Anpassung und Positionierung der Befestigungslasche, die mit der Gelenkpfanne koppelbar ist.
Um die Befestigungslasche insbesondere auf einfache Weise mit einer Befesti gungslaschenaufnahme in Eingriff zu bringen, die im Bereich eines am umlau fenden Rand der Gelenkpfanne angeordnet oder ausgebildeten Flanschs ange ordnet oder ausgebildet ist, ist es günstig, wenn der Befestigungslaschenvor- sprung von einer Unterseite der mindestens einen Befestigungslasche abste hend angeordnet oder ausgebildet ist. Dies ermöglicht insbesondere das in Eingriff Bringen der Befestigungslasche und der Gelenkpfanne von oben her kommend, also auf den Rand der Gelenkpfanne hin. Insbesondere ist eine int raoperative Kopplung der Befestigungslasche und der Gelenkpfanne so einfach und sicher realisierbar.
Um eine sichere Verbindung zwischen der Befestigungslasche und der Gelenk pfanne in der Kopplungsstellung zu ermöglichen, ist es vorteilhaft, wenn der Befestigungslaschenvorsprung mindestens eine Hinterschneidung definiert. Die Befestigungslaschenaufnahme kann insbesondere einen korrespondierenden Vorsprung umfassen, welcher in der Kopplungsstellung in die mindestens eine Hinterschneidung eingreift. Es können insbesondere eine, zwei oder drei Hin terschneidungen vorgesehen sein.
Einfach und sicher lassen sich die Gelenkpfanne und die Befestigungslasche in der Kopplungsstellung miteinander in Eingriff bringen, wenn der Befestigungs laschenvorsprung und die Befestigungslaschenaufnahme im Querschnitt T- för mig ausgebildet sind. Insbesondere kann bei entsprechender Dimensionierung eine Bewegung längs einer von der Befestigungslaschenaufnahme vorgegebe nen Bewegungsrichtung zwischen dem Befestigungslaschenvorsprung und der Befestigungslaschenaufnahme in der Kopplungsstellung in definierter Weise geführt werden. Die Befestigungslasche und die Gelenkpfanne können dann relativ zueinander noch bewegt werden, wenn sie die Kopplungsstellung ein nehmen. So kann ein Operateur die Befestigungslasche relativ zur Gelenk pfanne in eine optimale Befestigungsposition bringen, beispielsweise durch Verschieben der Befestigungslasche in der Befestigungselementaufnahme rela tiv zur Gelenkpfanne.
Auf einfache Weise lässt sich die Gelenkpfanne ausbilden, wenn die Befesti gungslaschenaufnahme in Form einer Befestigungslaschennut ausgebildet ist. Günstig ist es, wenn die die Befestigungslaschennut in einer Richtung parallel zu einer Gelenkpfannenlängsachse weisend geöffnet ist. Dies ermöglicht es insbesondere, den Befestigungslaschenvorsprung in einer Richtung parallel zur Gelenkpfannenlängsachse in die Befestigungslaschenaufnahme einzuführen.
Günstig ist es, wenn die Befestigungslaschennut einen Einführbereich und ei nen Kopplungsbereich definiert und wenn eine Befestigungslaschennutbreite im Einführbereich größer ist als im Kopplungsbereich. Diese Ausgestaltung er möglicht es insbesondere, den Befestigungslaschenvorsprung im Einführbe reich in die Befestigungslaschennut einzuführen und dann beispielsweise längs einer von der Befestigungslaschenaufnahme definierten Erstreckungsrichtung den Befestigungslaschenvorsprung vom Einführbereich in den Kopplungsbe reich zu bewegen, insbesondere zu verschieben. Insbesondere können so komplementäre Formen, beispielsweise Hinterschneidungen am Befestigungs laschenvorsprung und der Befestigungslaschennut, beim Überführen des Be festigungslaschenvorsprungs vom Einführbereich in den Kopplungsbereich in Eingriff gebracht werden.
Vorteilhaft ist es, wenn die Befestigungslaschenaufnahme bezogen auf die Ge lenkpfannenlängsachse konzentrisch ausgebildet ist und sich über einen Auf nahmeumfangswinkel von weniger als 180° erstreckt. Insbesondere kann der Aufnahmeumfangswinkel kleiner als 130° sein. Eine solche Ausgestaltung er möglicht es insbesondere, auch feststehende Befestigungslaschen an der Ge lenkpfanne anzuordnen, so dass eine zusätzliche, mit der Gelenkpfanne kop pelbare Befestigungslasche beispielsweise im angegebenen Winkelbereich an der Gelenkpfanne variabel positionierbar ist.
Günstig ist es, wenn der Befestigungslaschenvorsprung bezogen auf die Ge lenkpfannenlängsachse konzentrisch ausgebildet ist und sich über einen Vor sprungumfangswinkel erstreckt und wenn der Vorsprungumfangswinkel kleiner ist als der Aufnahmeumfangswinkel, insbesondere kleiner als 50%. Weitere insbesondere kann der Vorsprungumfangswinkel kleiner als 35% des Aufnah meumfangswinkels sein. Je kleiner der Vorsprungumfangswinkel ist, umso größer ist eine Variabilität bei der Positionierung der Befestigungslasche an der Gelenkpfanne.
Um den Befestigungslaschenvorsprung einfach und sicher in die Befestigungs laschenaufnahme einführen zu können, ist es vorteilhaft, wenn sich der Ein führbereich über einen Umfangswinkel erstreckt, welcher mindestens dem Vorsprungumfangswinkel entspricht.
Gemäß einer weiteren bevorzugten Ausführungsform der Erfindung kann vor gesehen sein, dass die mindestens eine Befestigungslasche eine Oberseite und eine Unterseite aufweist und dass die Unterseite in Form einer Knochenanlage fläche ausgebildet ist. Die Unterseite ist also derart ausgebildet, dass sie ge eignet ist, mit einem Knochen in Kontakt zu treten. Beispielsweise kann die Befestigungslasche aus einem biokompatiblen Material ausgebildet oder mit ei ner biokompatiblen Beschichtung, insbesondere die Unterseite, versehen sein.
Um eine optimale Anpassung der Befestigungslasche an den Knochen zu er möglichen, ist es günstig, wenn die Knochenanlagefläche rau ist.
Um insbesondere eine dauerhafte Befestigung der Gelenkpfanne am Knochen zu erreichen, ist es vorteilhaft, wenn die Knochenanlagefläche eine osteointe- grative Beschichtung aufweist.
Günstig ist es, wenn die Hüftpfanne, insbesondere die Gelenkpfanne und die mindestens eine Befestigungslasche, aus einem körperverträglichen Implan tatwerkstoff ausgebildet sind. Insbesondere kann der Implantatwerkstoff Titan oder eine Titanlegierung sein. So kann das Risiko einer Abstoßungsreaktion nach Implantation der Hüftpfanne minimiert werden.
Vorzugsweise ist die mindestens eine Befestigungslasche mindestens ab schnittsweise eben oder im Wesentlichen eben ausgebildet. So kann sie opti mal an ebene Knochenbereiche angepasst werden. Günstigerweise ist die mindestens eine Befestigungslasche mindestens ab schnittsweise gekrümmt. Insbesondere kann sie mit einem konstanten oder sich ändernden Krümmungsradius ausgebildet sein. Ein gekrümmter Abschnitt der mindestens einen Befestigungslasche kann insbesondere an gekrümmte Knochenabschnitte optimal angepasst werden, um eine flächige Anlage der Befestigungslasche am Knochen zu erreichen.
Um eine flächige Anlage der Befestigungslasche auch im Bereich konkav-kon vex gekrümmter Knochenflächen zu ermöglichen, ist es vorteilhaft wenn die mindestens eine Befestigungslasche mindestens abschnittsweise konkavo-kon- vex gekrümmt ausgebildet ist.
Vorzugsweise weist die mindestens eine Befestigungslasche eine konstante oder im Wesentlichen konstante Dicke auf. Ist die Dicke hinreichend gering, kann die mindestens eine Befestigungslasche optional intraoperativ an die Knochenkontur des Patienten beispielsweise durch Kaltverformen, insbeson dere Verbiegen, angepasst werden.
Gemäß einer weiteren bevorzugten Ausführungsform der Erfindung kann vor gesehen sein, dass die mindestens eine Befestigungslasche eine Befestigungs laschenlänge ausgehend vom mindestens einen ersten oder zweiten Kopp lungselement aufweist und dass die Befestigungslaschenlänge kleiner ist als ein maximaler Durchmesser des mindestens einen ersten oder zweiten Kopp lungselements. Insbesondere kann die Befestigungslänge kleiner sein als ein Durchmesser des Kopplungsrings oder ein Durchmesser des insbesondere hül senförmigen Kopplungseinsatzes. So kann insbesondere eine kompakte Hüft- panne für Revisionsoperationen bereitgestellt werden.
Um ein optimales Einwachsen der Gelenkpfanne in den Knochen zu ermögli chen, ist es günstig, wenn die Gelenkpfanne eine Außenfläche in Form einer Knochenanlagefläche aufweist und dass die Außenfläche eine osteointegrative Beschichtung aufweist. Die eingangs gestellte Aufgabe wird ferner bei einer Hüftgelenkendoprothese der eingangs beschriebenen Art erfindungsgemäß dadurch gelöst, dass die Hüftpfanne in Form einer der oben beschriebenen bevorzugten Ausführungs formen von Hüftpfannen ausgebildet ist.
Eine Hüftgelenkendoprothese mit einer solchen Hüftpfanne auszustatten hat insbesondere die oben im Zusammenhang mit bevorzugten Ausführungsfor men von Hüftpfannen beschriebenen Vorteile.
Eine besonders stabile Revisionshüftgelenkendoprothese kann insbesondere dadurch erreicht werden, wenn der Kopplungseinsatz der Hüftpfanne in der Kopplungsstellung zwischen der Gelenkpfanne und dem Gelenkpfanneneinsatz angeordnet ist. Der Kopplungseinsatz kann insbesondere hülsenförmig ausge bildet sein.
Die nachfolgende Beschreibung bevorzugter Ausführungsformen der Erfindung dient im Zusammenhang mit den Zeichnungen der näheren Erläuterung. Es zeigen:

In Figur 1 ist schematisch ein menschlicher Beckenknochen 10 dargestellt, an welchem ein erstes Ausführungsbeispiel einer Hüftpfanne 12 einer Revisions hüftgelenkendoprothese 14 festgelegt ist.
Die Hüftpfanne 12 umfasst eine Gelenkpfanne 16, die in den Beckenknochen 10 implantierbar ist. Bei dem in den Figuren 1 bis 4 dargestellten Ausfüh rungsbeispiel der Hüftpfanne 12 sind insgesamt drei Befestigungslaschen 18, 20 und 22 angeordnet. Die Befestigungslaschen 18, 20 und 22 sind plattenför mig ausgebildet.
Die Gelenkpfanne 16 ist in Form einer halben Hohlkalotte 24 ausgebildet und definiert einen umlaufenden Rand 26. Der Rand 26 ist in Form eines Flansches 28 von der Hohlkalotte 24 in radialer Richtung bezogen auf eine Gelenkpfan nenlängsachse 30 ausgebildet.
Die beiden Befestigungslaschen 18 und 20 sind als Erweiterungen des Flan sches 28 in Form langgestreckter Plättchen 32 und 34 ausgebildet. Die beiden Plättchen 32 und 34 definieren jeweils eine Plättchenebene 36 beziehungs weise 38, die bezogen auf eine vom Rand 26 definierte Eingangsebene 40 leicht abgewinkelt sind. Die Befestigungslaschen 18 und 20 sind mithin also feststehend, nämlich ein stückig beziehungsweise monolithisch, an der Gelenkpfanne 16 angeordnet beziehungsweise ausgebildet.
Die Befestigungslasche 22 dagegen ist mit der Gelenkpfanne 16 direkt kraft- und/oder formschlüssig koppelbar ausgebildet.
Um die Befestigungslasche 22 mit der Gelenkpfanne 16 zu koppeln, umfasst die Hüftpfanne 12 eine Kopplungseinrichtung 42 zum kraft- und/oder form schlüssigen direkten Koppeln der Befestigungslasche 22 mit der Gelenkpfanne 16 in einer Kopplungsstellung, wie sie schematisch in den Figuren 1 bis 4 dar gestellt ist.
Die Kopplungseinrichtung 42 umfasst ein erstes Kopplungselement 44 und ein zweites Kopplungselement 46. Das erste Kopplungselement 44 ist an der Ge lenkpfanne 16 angeordnet beziehungsweise ausgebildet. Das zweite Kopp lungselement 46 ist an der Befestigungslasche 22 angeordnet beziehungs weise ausgebildet. In der Kopplungsstellung stehen das erste Kopplungsele ment 44 und das zweite Kopplungselement 46 kraft- und/oder formschlüssig in Eingriff.
Das erste Kopplungselement 44 ist bei dem in den Figuren 1 bis 4 dargestell ten Ausführungsbeispiel der Hüftpfanne 12 in Form eines Kopplungsvorsprungs 48 ausgebildet. Das zweite Kopplungselement 46 ist in Form einer Kopplungs aufnahme 50 ausgebildet, und zwar korrespondierend zum Kopplungsvor sprung 48.
Das erste Kopplungselement 44 ist einstückig, nämlich monolithisch, an der Befestigungslasche 22 angeordnet beziehungsweise ausgebildet.
Der Kopplungsvorsprung 48 ist in Form eines Befestigungslaschenvorsprungs 52 ausgebildet, die Kopplungsaufnahme 50 in Form einer zum Befestigungsla- schenvorsprung 52 korrespondierenden Befestigungslaschenaufnahme 54. Da mit ist der Befestigungslaschenvorsprung 52 direkt an der Befestigungslasche 22 angeordnet beziehungsweise ausgebildet. Die Befestigungslaschenauf nahme 54 ist direkt an der Gelenkpfanne 16 angeordnet oder ausgebildet.
In der Kopplungsstellung stehen der Befestigungslaschenvorsprung 52 und die Befestigungslaschenaufnahme 54 kraft- und/oder formschlüssig in Eingriff, wie dies schematisch in den Figuren 1 bis 4 dargestellt ist.
Die plättchenförmige Befestigungslasche 22 definiert eine Oberseite 56 und eine Unterseite 58. Der Befestigungslaschenvorsprung 52 ist an einem ersten Endbereich der Befestigungslasche 22 von der Unterseite 58 abstehend ange ordnet beziehungsweise ausgebildet.
Der Befestigungslaschenvorsprung 52 und die Befestigungslaschenaufnahme 54 sind im Querschnitt T-förmig ausgebildet, wie dies in Figur 4 im Schnitt gut zu erkennen ist. Durch diese besondere Querschnittsform definiert der Befesti gungslaschenvorsprung 52 zwei voneinander weg weisende Hinterschneidun gen 62.
Die Befestigungslaschenaufnahme 54 ist in Form einer Befestigungslaschennut 64 ausgebildet. Diese ist in einer Richtung parallel zur Gelenkpfannenlängs achse 30 weisend geöffnet.
Die Befestigungslaschennut 64 definiert einen Einführbereich 66 und einen Kopplungsbereich 68.
Eine Befestigungslaschennutbreite 70 im Einführbereich 66 ist größer als eine Befestigungslaschennutbreite 72 im Kopplungsbereich 68.
Die unterschiedlichen Befestigungslaschennutbreiten 70 und 72 ermöglichen es, dass der Befestigungslaschenvorsprung 52 im Einführbereich 66 in die Be festigungslaschennut 64 eingeführt werden kann. Die Befestigungslaschenaufnahme 54 ist bezogen auf die Gelenkpfannenlängs achse 30 konzentrisch ausgebildet und erstreckt sich über einen Aufnahmeum fangswinkel 74. Dieser ist bei dem in den Figuren 1 bis 4 dargestellten Ausfüh rungsbeispiel kleiner als 180°. Bei dem dargestellten Ausführungsbeispiel der Hüftpfanne 12 beträgt der Aufnahmeumfangswinkel 74 etwa 110°.
Der Befestigungslaschenvorsprung 52 ist bezogen auf die Gelenkpfannen längsachse 30 ebenfalls konzentrisch ausgebildet und erstreckt sich über einen Vorsprungumfangswinkel 76. Dieser ist kleiner als der Aufnahmeumfangswin kel 74. Bei dem in den Figuren 1 bis 4 dargestellten Ausführungsbeispiel der Hüftpfanne 12 ist der Vorsprungumfangswinkel 76 kleiner als 50% des Auf nahmeumfangwinkels 74, und zwar sogar kleiner als 35% des Aufnahmeum fangswinkels 74. Der Vorsprungumfangswinkel 76 beträgt bei dem dargestell ten Ausführungsbeispiel etwa 30°.
Der Einführbereich 66 erstreckt sich über einen Umfangswinkel 78, welcher mindestens dem Vorsprungumfangswinkel 76 entspricht. Dies ermöglicht es einem Anwender, den Befestigungslaschenvorsprung 52 parallel zur Gelenk pfannenlängsachse 30 in Richtung auf den Rand 26 hin zu bewegen und im Einführbereich 66 in die Befestigungslaschennut 64 einzuführen.
Nach dem Einführen des Befestigungslaschenvorsprungs 52 in die Befesti gungslaschennut 64 kann dann die Befestigungslasche 22 um die Gelenkpfan nenachse 30 in der Befestigungslaschennut 64 bewegt werden, und zwar in den Kopplungsbereich 68 hinein. Aufeinander zu weisende, die Befestigungsla schennut 64 teilweise begrenzende Randabschnitte 80 greifen dann in die Hin terschneidungen 62 ein. Eine Bewegung der Befestigungslasche 22 ist so formschlüssig in der Befestigungslaschennut 64 geführt, also dann, wenn der Befestigungslaschenvorsprung 52 und die Befestigungslaschennut 64 mindes tens teilweise in Eingriff stehen. Dabei überlappen der Befestigungslaschen vorsprung 52 und der Kopplungsbereich 68 mindestens teilweise über den vom Befestigungslaschenvorsprung 52 definierten Vorsprungumfangswinkel 76.
Da der Vorsprungumfangswinkel 76 deutlich kleiner ist als der durch den Kopplungsbereich 68 definierte Umfangswinkel 79, ist es möglich, die Befesti gungslasche 22 in unterschiedlichen Winkelstellungen bezogen auf die Gelenk pfannenlängsachse 30 in der Kopplungsstellung stufenlos zu verstellen und re lativ zur Gelenkpfanne 16 zu positionieren.
Die Befestigungslaschen 18, 20 und 22 stehen in radialer Richtung bezogen auf die Gelenkpfannenlängsachse 30 von der Gelenkpfanne 16 ab. Freie Enden 82, 84 und 86 der Befestigungslaschen 18, 20 und 22 sind abgerundet.
Die Befestigungslaschen 18 und 20 umfassen jeweils zwei Befestigungsele mentdurchbrechungen 88. Die Befestigungslasche 22 umfasst eine Befesti gungselementdurchbrechung 90.
Die Befestigungselementdurchbrechungen 88 und 90 sind in Form von Bohrun gen ausgebildet und definieren jeweils eine konische Innenfläche 92 bezie hungsweise 94, die sich von den Oberseiten 96 beziehungsweise 56 zu Unter seiten 98 beziehungsweise 58 der Befestigungslaschen 18 und 20 beziehungs weise 22 verjüngen.
Die besondere Ausgestaltung der Befestigungselementdurchbrechungen 88 und 90 ermöglicht es, die Hüftpfanne 12 mit Knochenschrauben 100, deren Gewindeabschnitt 102 die Befestigungsdurchbrechungen 88 beziehungsweise 90 durchsetzt, am Beckenknochen 10 festzulegen. Durch die konischen Innen flächen 92 beziehungsweise 94 können Knochenschrauben 100 mit Senkköp fen 104 genutzt werden, so dass deren Kopfflächen 106, die vom Gewindeab schnitt 102 weg weisen, flächenbündig mit den Oberseiten 96 beziehungs weise 56 der Befestigungslaschen 18, 20 beziehungsweise 22 angeordnet sind, wenn die Hüftpfanne 12 im Beckenknochen 10 verankert ist. Die Gelenkpfanne 16 weist weitere im Querschnitt kreisförmige Durchbrechun gen 108 sowie zwei langlochartige Durchbrechungen 110 auf, die eine direkte Befestigung der Gelenkpfanne 16 am Beckenknochen 10 mit weiteren, in den Figuren nicht dargestellten Knochenschrauben ermöglichen.
Ferner ist koaxial zur Gelenkpfannenlängsachse 30 an der Gelenkpfanne 16 eine Durchbrechung 112 angeordnet, die zur Aufnahme eines Kopplungsvor sprungs, welcher an einem in den Figuren nicht dargestellten Gelenkpfannen einsatz angeordnet oder ausgebildet ist, dient.
In den Figuren 5 bis 7 ist ein weiteres Ausführungsbeispiel einer insgesamt mit dem Bezugszeichen 212 bezeichneten Hüftpfanne für eine Revisionshüftge lenkendoprothese 214 schematisch dargestellt. Es sei angemerkt, dass die Revisionshüftgelenkendoprothese 214 auch zur Primärversorgung, also beim erstmaligen Ersatz des natürlichen Hüftgelenks eines Patienten durch eine Pro these, eingesetzt werden kann. Dies gilt entsprechend auch für alle nachfol gend noch beschriebenen Ausführungsbeispiele von Revisionshüftgelenkendo prothesen.
Die Hüftpfanne 212 umfasst eine Gelenkpfanne 216 die in Form einer Hohlka lotte 224 ausgebildet ist.
Die Hüftpfanne 212 umfasst insgesamt drei Befestigungslaschen 218, 220 und 222, die einstückig, nämlich monolithisch mit einem Kopplungsring 314 ausge bildet sind. Der Kopplungsring 314 bildet ein Trägerelement 316, welches die Befestigungslaschen 218, 220 und 222 trägt.
Bei der Hüftpfanne 212 sind die Befestigungslaschen 218, 220 und 222 indi rekt kraft- und/oder formschlüssig, nämlich mittels des Trägerelements 316, mit der Gelenkpfanne 216 koppelbar. Dies wird ermöglicht durch eine Kopp lungseinrichtung 242 zum kraft- und/oder formschlüssigen indirekten Koppeln der Befestigungslaschen 218, 220 und 222 mit der Gelenkpfanne 16 in einer Kopplungsstellung, die schematisch in den Figuren 6 und 7 dargestellt ist. Die Kopplungseinrichtung 242 umfasst ein erstes Kopplungselement 244 und ein zweites Kopplungselement 246, die einerseits an der Gelenkpfanne 216 und andererseits am Kopplungsring 314 und damit indirekt an den Befesti gungslaschen 218, 220 und 222 angeordnet beziehungsweise ausgebildet sind. In der Kopplungsstellung stehen das erste Kopplungselement 244 und das zweite Kopplungselement 246 kraft- und/oder formschlüssig in Eingriff.
Das erste Kopplungselement 244 ist in Form eines Kopplungsvorsprungs 248 ausgebildet, das zweite Kopplungselement 246 in Form einer Kopplungsauf nahme 250, die korrespondierend zum Kopplungsvorsprung 248 ausgebildet ist.
Die Kopplungseinrichtung 242 ist in Form einer Bajonettverbindungseinrich tung 318 ausgebildet.
Die Befestigungslaschen 218, 220 und 222 sind einstückig, nämlich monoli thisch, mit dem Kopplungsring 314 ausgebildet, so dass auch das zweite Kopplungselement 246 einstückig, nämlich monolithisch, mit den Befesti gungslaschen 218, 220 und 222 ausgebildet ist.
Bei der Hüftpfanne 212 ist der Kopplungsvorsprung 248 in Form eines hülsen förmigen Pfannenabschnitts 320 der Gelenkpfanne 216 ausgebildet. Der Kopp lungsring 314 definiert die Kopplungsaufnahme 250. In der Kopplungsstellung umgibt der Kopplungsring 314 den hülsenförmigen Pfannenabschnitt 320, und zwar kraft- und/oder formschlüssig. Dabei überdeckt der Kopplungsring 314 nicht mehr als 5% einer äußeren Oberfläche der Gelenkpfanne 216.
Die Bajonettverbindungseinrichtung 318 umfasst den hülsenförmigen Pfannen abschnitt 320 und den Kopplungsring 314. An der Gelenkpfanne 216 ist bezogen auf eine Gelenkpfannenlängsachse 230 eine radial nach außen weg weisend geöffnete Aufnahmenut 322 ausgebildet, in der der Kopplungsring 314 in der Kopplungsstellung gehalten ist.
Von einem umlaufenden Rand 226 der Gelenkpfanne 216 steht in radialer Richtung bezogen auf die Gelenkpfannenlängsachse 230 ein Ringflansch 326 ab. Die Aufnahmenut 322 ist vom Ringflansch 326 sowie einem parallel zu die sem verlaufenden Ringflansch 324 begrenzt.
Der vom Rand 226 weiter entfernt angeordnete erste Ringflansch 324 weist eine konstante Breite 328 auf. Der zweite Ringflansch 326 weist eine Ring flanschbreite 330 auf und definiert eine Ringflanschlängsachse 332.
Die Ringflanschbreite 330 variiert über einen Umfang des Ringflanschs 326 be zogen auf die Ringflanschlängsachse 332. Insgesamt sind drei sich jeweils über ein Teil des Umfangs erstreckende Ringflanschbereiche 334 ausgebildet, die eine minimale Ringflanschbreite 330 aufweisen. Bei dem in den Figuren 5 bis 7 dargestellten Ausführungsbeispiel wird die minimale Ringflanschbreite 330 dadurch realisiert, dass der Ringflansch 326 in diesen Ringflanschberei chen 334 unterbrochen ist.
Der Kopplungsring 314 definiert eine Kopplungsringlängsachse 336. Eine Kopplungsringbreite 338 des Kopplungsrings 314 in radialer Richtung bezogen auf die Kopplungsringlängsachse 336 variiert über einen Umfang des Kopp lungsrings 314.
Bei dem in den Figuren 5 bis 7 dargestellten Ausführungsbeispiel der Hüft- pfanne 212 weist die Kopplungsringbreite 338 drei Maxima in Form von sich über einen Teil des Umfangs des Kopplungsrings 314 erstreckende Kopplungs ringbereiche 340 auf. Die Anzahl der Kopplungsringbereiche 340 entspricht so mit den Ringflanschbereichen 334. Die beschriebenen Kopplungsringbereiche 340 und die Ringflanschbereiche 334 erstrecken sich über einander entsprechende Winkelbereiche bezogen auf die Ringflanschlängsachse 332. Diese fällt in der Kopplungsstellung mit der Kopplungsringlängsachse 336 zusammen.
Die Ringflanschbereiche 334 und die Kopplungsringbereiche 338 sind derart angeordnet und ausgebildet, dass der Kopplungsring 314 von einer Löse stellung, die schematisch in Figur 5 dargestellt ist, in welcher der Kopplungs ring 314 also von der Gelenkpfanne 216 getrennt ist, durch eine Bewegung in einer Richtung parallel zur Ringflanschlängsachse 332 über den zweiten Ring flansch 326 geschoben werden kann, bis er am ersten Ringflansch 324 an schlägt. Der Kopplungsring 314 befindet sich dann in einer Zwischenstellung an der Gelenkpfanne 216.
Ausgehend von dieser Zwischenstellung kann der Kopplungsring 314 durch eine Drehung um die Ringflanschlängsachse 332 in die Kopplungsstellung überführt werden. In der Kopplungsstellung ist der Kopplungsring 314 zwi schen dem ersten Ringflansch 324 und dem zweiten Ringflansch 326 gehalten, wie dies schematisch in Figuren 6 und 7 dargestellt ist.
Die Befestigungslaschen 218 und 220 sind in Form flacher Plättchen 232 und 234 ausgebildet.
Die Befestigungslaschen 218 und 220 weisen analog den Befestigungslaschen 18 und 20 Befestigungselementdurchbrechungen 288 auf. An der Befesti gungslasche 222 ist eine Befestigungselementdurchbrechung 290 ausgebildet.
Die Befestigungselementdurchbrechungen 288 und 290 weisen sich konisch von der jeweiligen Oberseite 256 beziehungsweise 296 zur jeweiligen Unter seite 258 beziehungsweise 298 der Befestigungslaschen 222 beziehungsweise 218 und 220 verjüngende Innenflächen 294 beziehungsweise 292 auf. An der Gelenkpfanne 216 sind in analoger Weise wie bei der Gelenkpfanne 16 drei im Querschnitt kreisförmige Durchbrechungen 308 sowie zwei langlochar tige Durchbrechungen 310 ausgebildet, die das Festlegen der Gelenkpfanne 216 am Beckenknochen des Patienten mit Knochenschrauben ermöglichen. Die Anordnung und Ausbildung der Durchbrechungen 308 und 310 ermöglicht eine hohe Variabilität bei der Befestigung der Gelenkpfanne 216 am Beckenkno chen.
Bei einer in den Figuren nicht dargestellten Variante der Hüftpfanne 212 ist le diglich der Ringflansch 324 vorgesehen. Der Kopplungsring 314 weist in die sem Fall eine konstante Breite 328 über den gesamten Umfang auf. Eine in Richtung auf die Gelenkpfannenlängsachse 230 hin gerichtete Innenfläche des Kopplungsrings 314 ist mit einem Innengewinde versehen, der Pfannenab schnitt 320, der sich zwischen dem Rand 226 und dem Ringflansch 324 er streckt, mit einem korrespondierenden Außengewinde. Diese Ausgestaltung ermöglicht es, den Kopplungsring und die Gelenkpfanne miteinander zu ver schrauben. Statt der Bajonettverbindungseinrichtung 318 bei der Hüftpfanne 212 ist hier dann also eine Kopplungseinrichtung in Form einer Schraubverbin dungseinrichtung vorgesehen.
Ein weiteres Ausführungsbeispiel einer Revisionshüftgelenkendoprothese 414 ist schematisch in Figur 8 dargestellt. Sie umfasst eine Hüftpfanne 412, die in einer Implantationsstellung einen Gelenkpfanneneinsatz 542 mit einer hohlku geligen Gelenkkopfaufnahme 544 aufnimmt. Ferner umfasst die Revisionshüft gelenkendoprothese 414 einen Hüftschaft 546 mit einem Gelenkkopf 548, wel cher im Zusammenwirken mit der korrespondierend ausgebildeten Gelenk kopfaufnahme 544 ein Kugelgelenk 550 ausbildet.
Die Hüftpfannen 12 und 212 können in analoger Weise eine entsprechend dem Gelenkpfanneneinsatz 542 ausgebildeten, in den Figuren nicht dargestellten Gelenkpfanneneinsatz aufnehmen, um zusammen mit einem Hüftschaft,, der in einer Femurkavität eines Femurs des Patienten verankert wird, ein Kugelge lenk mit dem am Hüftschaft gehaltenen Gelenkkopf ausbilden. Die Gelenkpfanne 416 ist in ihrem grundsätzlichen Aufbau insbesondere der Gelenkpfanne 216 ähnlich. Sie weist jedoch keine Ringflansche 324 und 326 auf. An der Gelenkpfanne 416 sind jedoch auch im Querschnitt kreisförmige Durchbrechungen 508 und langlochartige Durchbrechungen 510 zum Festle gen der Gelenkpfanne 416 am Beckenknochen des Patienten ausgebildet.
Die Hüftpfanne 412 umfasst ebenfalls drei Befestigungslaschen 418, 420 und 422, die mit der Gelenkpfanne 416 in einer Kopplungsstellung koppelbar sind. Zu diesem Zweck umfasst die Hüftpfanne 412 ebenfalls eine Kopplungseinrich tung 442 mit einem ersten Kopplungselements 444 in Form eines Kopplungs vorsprungs 448 und einem zweiten Kopplungselement 446 in Form einer Kopplungsaufnahme 450.
Der Kopplungsvorsprung 448 ist in Form eines hülsenförmigen Kopplungsein satzes 552 ausgebildet, welcher ein Trägerelement 516 für die Befestigungsla schen 418, 420 und 422 bildet. Diese sind einstückig, insbesondere monoli thisch, mit dem Kopplungseinsatz 552 ausgebildet. Der Kopplungseinsatz 552 trägt somit drei Befestigungslaschen 418, 420 und 422.
Die Befestigungslaschen 418 und 420 sind plättchenförmig ausgebildet und konkavo- konvex gekrümmt. Die Befestigungslaschen 418, 420 und 422 wei sen jeweils mehrere Befestigungselementdurchbrechungen 488 beziehungs weise 490 auf, die analog zu den Befestigungslaschen 18 und 20 beziehungs weise 22 konische Innenflächen 492 beziehungsweise 494 aufweisen.
Bei der Hüftpfanne 412 bildet die Gelenkpfanne 416 die Kopplungsaufnahme 450 für den hülsenförmigen Kopplungseinsatz 552.
In der Kopplungsstellung ist der Kopplungseinsatz 552 zwischen der Gelenk pfanne 416 und dem in dieser aufgenommenen Gelenkpfanneneinsatz 542 an geordnet. Dabei stehen der Kopplungseinsatz 552 und die Gelenkpfanne 416 kraft- und/oder formschlüssig in Eingriff. Der hülsenförmige Kopplungseinsatz 552 weist eine äußere erste konische Klemmfläche 554 auf. Diese ist korrespondierend zu einer inneren zweiten ko nischen Klemmfläche 556 der Gelenkpfanne 416 ausgebildet. Die Klemmflä chen 554 und 556 bilden in der Kopplungsstellung eine selbsthemmende Ver bindung zwischen dem Kopplungseinsatz 552 und der Gelenkpfanne 416 aus.
Weiterhin weist der Kopplungseinsatz 552 eine innere dritte konische Klemm fläche 558 auf. Diese ist korrespondierend zu einer äußeren vierten konischen Klemmfläche 560 des Gelenkpfanneneinsatzes 542 ausgebildet. Zwischen den Klemmflächen 558 und 560 ist in der Kopplungsstellung ebenfalls eine selbst hemmende Verbindung ausgebildet, und zwar zwischen dem Kopplungseinsatz 552 und dem Gelenkpfanneneinsatz 542.
Der Kopplungseinsatz 552 kann in unterschiedlichen Ausrichtungen relativ zur Gelenkpfanne 416 bezogen auf die Gelenkpfannenlängsachse 430 mit der Ge lenkpfanne 416 gekoppelt werden. Auf diese Weise ergeben sich nahezu belie big viele Möglichkeiten, die Durchbrechungen 508 und 510 relativ zu den Be festigungselementdurchbrechungen 488 und 490 anzuordnen und auszurich ten. Dies gestattet eine hochflexible Befestigung der Hüftpfanne 412 am Be ckenknochen den Patienten.
Bei allen beschriebenen Ausführungsbeispielen von Hüftpfannen weisen die Befestigungslaschen 18, 20, 22, 218, 220, 222 beziehungsweise 418, 420 und 422 eine Oberseite 56, 96, 256, 296 beziehungsweise 456, 496 und eine Un terseite 58, 98, 258, 298 beziehungsweise 458, 498 auf. Die Unterseiten 58, 98, 258, 298 beziehungsweise 458, 498 sind in Form einer Knochenanlageflä che ausgebildet. Diese ist rau und weist optional eine osteointegrative Be schichtung 162, 362 beziehungsweise 562 auf.
Die Hüftpfannen 12, 212 und 412, und zwar insbesondere die Gelenkpfannen 16, 216 und 416 sowie die Befestigungslaschen 18, 20, 22, 218, 220 und 222 sowie 418, 420 und 422, sind aus einem körperverträglichen Implantatwerk stoff ausgebildet. Dieser ist bei den beschriebenen Ausführungsbeispielen Ti tan. Alternativ kann er bei anderen Ausführungsbeispielen auch eine Titanle gierung sein.
Die Befestigungslaschen 18, 20, 22, 218, 220, 222, 418, 420 und 422 sind ab schnittsweise eben und/oder abschnittsweise gekrümmt ausgebildet. Bei un terschiedlichen Ausführungsbeispielen sind konstante oder sich ändernde Krümmungsradien vorgesehen.
Die Befestigungslaschen 18, 20, 22, 218, 220, 222, 418, 420 und 422 weisen eine konstante oder im Wesentlichen konstante Dicke auf.
Die Befestigungslaschen 18, 20, 22, 218, 220, 222, 418, 420, 422 weisen je weils eine Befestigungslaschenlänge auf, die kleiner ist als ein Durchmesser des Kopplungsrings 314 beziehungsweise des Kopplungseinsatzes 552.
Die Gelenkpfannen 16, 216 und 416 weisen jeweils eine Außenfläche 170, 370 beziehungsweise 570 auf, die optional bei bestimmten Ausführungsbeispielen eine osteointegrative Beschichtung trägt.
Alle oben beschriebenen Ausführungsbeispiele von Hüftpfannen 12, 212 und 412 ermöglichen eine individuelle Befestigung am Beckenknochen eines Pati enten, unabhängig von einer konkreten Knochensituation, die insbesondere nach der Explantation einer zuvor eingesetzten Gelenkpfanne sich oftmals erst im Verlauf der Explantation genau ergibt. Ein Operateur hat dann die Möglich keit, durch entsprechende Wahl der Hüftpfannen 12, 212 beziehungsweise 412 sowie die variable Anordnung der jeweiligen Befestigungslaschen relativ zur jeweiligen Gelenkpfanne eine optimale Befestigung der Hüftpfannen am Be ckenknochen des Patienten zu realisieren. Sämtliche beschriebenen Ausführungsbeispiele von Hüftpfannen 12, 212 und 412 können insbesondere als Teil von Hüftgelenkendoprothesen für einen erst maligen Austausch eines natürlichen Hüftgelenks sowie als Teil von Revisions hüftgelenkendoprothesen zum Einsatz kommen.

Claims

Patentansprüche
1. Hüftpfanne (12; 212; 412), insbesondere für eine Revisionshüftgelenk endoprothese (14; 214; 414), umfassend eine Gelenkpfanne (16; 216; 416) zum Implantieren in den Beckenknochen (10) eines Patienten und mindestens eine plattenförmige Befestigungslasche (18, 20, 22; 218, 220, 222; 418, 420, 422), dadurch gekennzeichnet, dass die mindestens eine Befestigungslasche (22; 218, 220, 222; 418, 420, 422) mit der Ge lenkpfanne (16; 216; 416) kraft- und/oder formschlüssig koppelbar ist und dass die Hüftpfanne (12; 212; 412) eine Kopplungseinrichtung (42; 242; 442) zum kraft- und/oder formschlüssigen Koppeln der mindestens einen Befestigungslasche (22; 218, 220, 222; 418, 420, 422) mit der Gelenkpfanne (16; 216; 416) in einer Kopplungsstellung umfasst.
2. Hüftpfanne nach Anspruch 1, dadurch gekennzeichnet, dass die mindes tens eine Befestigungslasche (18, 20, 22; 218, 220, 222; 418, 420, 422) mindestens eine Befestigungselementdurchbrechung (88, 90; 288, 290; 488, 490) umfasst, insbesondere zwei, drei, vier oder mehr Befesti gungselementdurchbrechungen (88, 90; 288, 290; 488, 490), wobei insbesondere die mindestens eine Befestigungselementdurchbre chung (88, 90; 288, 290; 488, 490) eine konische Innenfläche (92, 94; 292, 294; 492, 494) definiert, insbesondere sich von einer Oberseite (56, 96; 256, 296; 456, 496) zu einer Unterseite (58, 98; 258, 298;
458, 498) der mindestens einen Befestigungslasche (18, 20, 22; 218, 220, 222) verjüngend.
3. Hüftpfanne nach einem der voranstehenden Ansprüche, dadurch gekenn zeichnet, dass die Kopplungseinrichtung (42; 242; 442) mindestens ein erstes Kopplungselement (44; 244; 444) und mindestens ein zweites Kopplungselement ( 46; 246; 446) umfasst, die einerseits an der Ge lenkpfanne (16; 216; 416) und andererseits an der mindestens einen Befestigungslasche ( 22; 218, 220, 222; 418, 420, 422) angeordnet oder ausgebildet oder dieser zugeordnet sind, und dass das mindestens eine erste Kopplungselement (44; 244; 444) und das mindestens eine zweite Kopplungselement ( 46; 246; 446) in der Kopplungsstellung kraft- und/oder formschlüssig in Eingriff stehen, wobei insbesondere das mindestens eine erste Kopplungselement (44; 244; 444) in Form eines Kopplungsvorsprungs (48; 248; 448) ausgebil det ist und dass das mindestens eine zweite Kopplungselement (46;
246; 446) in Form einer Kopplungsaufnahme (50; 250; 450) ausgebildet ist, insbesondere korrespondierend zum mindestens einen Kopplungsvor sprung (48; 248; 448).
4. Hüftpfanne nach Anspruch 3, dadurch gekennzeichnet, dass der Kopp lungsvorsprung (448) in Form eines Kopplungseinsatzes (552) ausgebil det ist und dass die Gelenkpfanne (416) die Kopplungsaufnahme (450) für den Kopplungseinsatz (552) bildet.
5. Hüftpfanne nach Anspruch 4, dadurch gekennzeichnet, dass der Kopp lungseinsatz (552) in der Kopplungsstellung zwischen der Gelenkpfanne (416) und einem in dieser aufgenommenen Gelenkpfanneneinsatz (542) angeordnet ist.
6. Hüftpfanne nach Anspruch 4 oder 5, dadurch gekennzeichnet, dass der Kopplungseinsatz (552) eine äußere erste konische Klemmfläche (554) aufweist und dass die erste Klemmfläche (554) korrespondierend zu einer inneren zweiten konischen Klemmfläche (556) der Gelenkpfanne (416) ausgebildet ist, wobei insbesondere die erste Klemmfläche (554) und die zweite Klemm fläche (556) in der Kopplungsstellung eine selbsthemmende Verbindung zwischen dem Kopplungseinsatz (552) und der Gelenkpfanne (416) aus bilden.
7. Hüftpfanne nach einem der Ansprüche 4 bis 6, dadurch gekennzeichnet, dass die mindestens eine Befestigungslasche (418, 420, 422) einstückig, insbesondere monolithisch, mit dem Kopplungseinsatz (552) ausgebildet ist.
8. Hüftpfanne nach einem der Ansprüche 3 bis 7, dadurch gekennzeichnet, dass a) die Kopplungsaufnahme ein Innengewinde umfasst und dass der korrespondierende Kopplungsvorsprung ein zum Innengewinde korrespondierendes Außengewinde umfasst und/oder b) das mindestens eine erste Kopplungselement (44; 444) oder das mindestens eine zweite Kopplungselement (246) einstückig, insbe sondere monolithisch, an der mindestens einen Befestigungslasche (22; 218, 220, 222; 418, 420, 422) angeordnet oder ausgebildet sind.
9. Hüftpfanne nach einem der voranstehenden Ansprüche, dadurch gekenn zeichnet, dass an der Gelenkpfanne (16) mindestens eine Befestigungs lasche (18, 20) feststehend, insbesondere einstückig, weiter insbesonde re monolithisch, angeordnet oder ausgebildet ist.
10. Hüftpfanne nach einem der Ansprüche 2 bis 9, dadurch gekennzeichnet, dass der Kopplungsvorsprung (248) in Form eines hülsenförmigen Pfan nenabschnitts (320) der Gelenkpfanne (216) ausgebildet ist, dass die Kopplungseinrichtung (242) einen Kopplungsring (314) umfasst, welcher die Kopplungsaufnahme (250) definiert, und dass der Kopplungsring (314) in der Kopplungsstellung den hülsenförmigen Pfannenabschnitt (320) umgibt, insbesondere kraft- und/oder formschlüssig.
11. Hüftpfanne nach Anspruch 10, dadurch gekennzeichnet, dass a) die mindestens eine Befestigungslasche (218, 220, 222) einstückig, insbesondere monolithisch, mit dem Kopplungsring (314) ausgebil det ist und/oder b) der Kopplungsring (314) höchstens 5%, insbesondere höchstens 2%, einer äußeren Oberfläche der Gelenkpfanne (216) überdeckt.
12. Hüftpfanne nach Anspruch 10 oder 11, dadurch gekennzeichnet, dass die Gelenkpfanne (216) eine von ihr nach außen weg weisend geöffnete Auf nahmenut (322) aufweist und dass der Kopplungsring (314) in der Kopp lungsstellung in der Aufnahmenut (322) gehalten ist.
13. Hüftpfanne nach Anspruch 12, dadurch gekennzeichnet, dass die Aufnah menut (322) seitlich von einem ersten Ringflansch (324) und einem zweiten Ringflansch (326) begrenzt ist, dass die beiden Ringflansche (324, 326) parallel zueinander verlaufen, dass der Kopplungsring (314) von einer Lösestellung, in welcher er von der Gelenkpfanne (216) ge trennt ist, durch eine Bewegung parallel zur Ringflanschlängsachse (332) in Richtung auf den ersten Ringflansch (324) in eine Zwischenstellung überführbar ist, dass der Kopplungsring (314) von der Zwischenstellung durch eine Drehung um die Ringflanschlängsachse (332) in die Kopp lungsstellung überführbar ist und dass der Kopplungsring (314) in der Kopplungsstellung zwischen dem ersten Ringflansch (324) und dem zweiten Ringflansch (326) gehalten ist.
14. Hüftpfanne nach einem der Ansprüche 3 bis 13, dadurch gekennzeichnet, dass die Kopplungseinrichtung (242) in Form einer Schraubverbindungs einrichtung, einer Klemmeinrichtung und/oder in Form einer Bajonettver bindungseinrichtung (318) ausgebildet ist.
15. Hüftpfanne nach einem der Ansprüche 2 bis 14, dadurch gekennzeichnet, dass der Kopplungsvorsprung (48) in Form eines Befestigungslaschen- vorsprungs (52) und die Kopplungsaufnahme (50) in Form einer korres pondierenden Befestigungslaschenaufnahme (54) ausgebildet sind, dass der Befestigungslaschenvorsprung (52) an der mindestens einen Befesti gungslasche (22) angeordnet oder ausgebildet ist, dass die Befesti gungslaschenaufnahme (54) an der Gelenkpfanne (16) angeordnet oder ausgebildet ist und dass der Befestigungslaschenvorsprung (52) und die Befestigungslaschenaufnahme (54) in der Kopplungsstellung kraft- und/ oder formschlüssig in Eingriff stehen.
16. Hüftpfanne nach Anspruch 15, dadurch gekennzeichnet, dass die Befesti gungslaschenaufnahme (54) in Form einer Befestigungslaschennut (64) ausgebildet ist.
17. Hüftpfanne nach Anspruch 15 oder 16, dadurch gekennzeichnet, dass die Befestigungslaschenaufnahme (54) bezogen auf die Gelenkpfannenlängs achse (30) konzentrisch ausgebildet ist und sich über einen Aufnahme umfangswinkel (74) von weniger als 180° erstreckt, wobei insbesondere der Befestigungslaschenvorsprung (52) bezogen auf die Gelenkpfannenlängsachse (30) konzentrisch ausgebildet ist und sich über einen Vorsprungumfangswinkel (76) erstreckt und dass der Vor sprungumfangswinkel (76) kleiner ist als der Aufnahmeumfangswinkel (74), insbesondere kleiner als 50%, weiter insbesondere kleiner als 35%, wobei weiter insbesondere sich der Einführbereich (66) über einen Um fangswinkel (78) erstreckt, welcher mindestens dem Vorsprungumfangs winkel (76) entspricht.
18. Hüftgelenkendoprothese (14; 214; 414), insbesondere Revisionshüftge lenkendoprothese (14; 214; 414), umfassend eine Hüftpfanne (12; 212; 412), welche in einer Implantationsstellung einen Gelenkpfanneneinsatz (542) mit einer hohlkugeligen Gelenkkopfaufnahme (544) aufnimmt, und einen Hüftschaft (546) mit einem Gelenkkopf (548) zur Ausbildung eines Kugelgelenks (550) im Zusammenwirken mit der zum Gelenkkopf (548) korrespondierend ausgebildeten Gelenkkopfaufnahme (544), dadurch gekennzeichnet, dass die Hüftpfanne (12; 212; 412) in Form einer Hüft- pfanne (12; 212; 412) nach einem der voranstehenden Ansprüche aus gebildet ist, wobei insbesondere der Kopplungseinsatz (552) in der Kopplungsstellung zwischen der Gelenkpfanne (416) und dem Gelenkpfanneneinsatz (542) angeordnet ist.
