Abstract
Description
- La présente invention concerne un dispositif ergonomique relatif aux lampes scialytiques d'éclairage utilisées dans le cadre du cabinet dentaire à partir de deux nouvelles options techniques: soit le pilotage à distance de l'orientation de la lampe par le chirurgien dentiste Soit la gestion automatisée de l'orientation de cette lampe.
- Dans l'une ou l'autre des options techniques, la mobilité de la lampe est assurée par deux micro moteurs qui lui sont intégrés .l'un des micro moteurs est dédié aux mouvements horizontaux de la lampe, le second micromoteur est dedié aux mouvements verticaux de cette lampe. Ces deux micro moteurs travaillant dans deux axes ont déjà été décrits dans le précédent brevet N°
2 606 915 déposé le 17/11/1986 - dans le présent développement,la poursuite à distance de la lampe ne présente pas ces inconvénients et laisse libre de toute contrainte le patient et l'opérateur.
- Pour les explications qui vont suivrent, on se reportera a la
figure 1 , qui est une vue générale de De l'espace professionnel dentaire avec la lampe scialytique 1,à interrupteur incorporé, munie d'une articulation 2 assurant sa mobilité horizontale et une articulation 3 assurant sa mobilité verticale . Cette lampe peut etre sur colonne ou suspendue a une suspension plafonnière et située devant le Fauteuil F , ou prend place le patient, représenté dans deux positions cliniques différentes. l'articulation 4 est dediée aux mouvements importants de la lampe en dehors des soins. Le bloc pédale 5 commande par un rhéostat 6, l'ensemble des instruments rotatifs 7. - En l'etat actuel de la technique, au départ et pendant l'acte dentaire, les réglages d'orientation de la Source lumineuse se font manuellement par le dentiste dans les plans vertical et horizontal.
- Le champ opératoire constitué par la bouche ouverte est de petite taille et la moindre variation de la tete du patient l'écarte de la zone éclairée...aussi le praticien est il contraint à de fréquents Réajustements de la source lumineuse pouvant entraîner une déconcentration ainsi qu'une fatigue Supplémentaire.. avec accumulation des « temps morts » entraînant une durée plus longue de l'intervention, de plus ces réglages manuels peuvent contaminer les mains de l'opérateur Même si ce dernier porte des gants protecteurs.
- Enfin ,quelque soit le type de lampe, le maximum d'intensité d'éclairage se situe toujours au centre du faisceau lumineux .
- Il faut donc bien cibler en permanence, la zone opératoire pour bénéficier de sa meilleure approche Visuelle, d'ou l'intéret d'une gestion a distance de la source lumineuse par asservissement
- Faisant l'objet du présent brevet ,après adoption d'un 1er positionnement manuel de la lampe. Paramètres à prendre en compte:
- La
figure 2 visualise l'angulation du faisceau lumineux 8 et la zone opératoire éclairée 9 Pour un patient en situation mi couché * avec schématisation en 10 d'une rotation pilotée De faible amplitude de la lampe 1 souhaitée dans le plan horizontal, Schématisation en 11 d'une rotation pilotée de faible amplitude souhaitée dans le plan vertical, afin de focaliser avec précision la zone opératoire 9 . *suivant l'acte clinique à réaliser ,on prendra pour principe que le praticien, aura adopter définitivement en début de phase opératoire, un positionnement déterminé du fauteuil dentaire F et situant la tete du patient dans la tetière T.Lafigure 3 donne la valeur moyenne de 5 cm pour balayer l'éclairage sur la totalité de l'arcade dentaire sup. 1 dans le plan vertical ,même valeur de 5cm. pour balayer dans le même plan ,l'arcade dentaire inf.2 . - On retiendra une course moyenne totale de 10 cm nécessaire en vertical, pour une ouverture de bouche souvent inférieure a 5 cm dans ce plan.... La
figure 4 donne la valeur moyenne de 6cm. pour balayer 1'éclairage sur la totalité de l'arcade Dentaire sup. 1 dans le plan horizontal, valeur de 6 cm pour l'arcade dentaire inf. 2 . - On retiendra une course moyenne de 10 cm. en horizontal.
- Ces valeurs réelles ne correspondent pas à l'échelle des figures, mais elles serviront à quantifier les amplitudes a retenir dans la gestion de l'orientation de la lampe.(elles ont été évaluées à partir d'arcades dentaires adultes schématisées ici,,elles seront donc à minorer chez l'enfant ,ce qui Ne remet pas en cause les dispositifs qui seront décrits plus loin.)
- A partir de ces mesures ,pour un patient adulte situé en général a moins de 1m. de la lampe, on retiendra une correction d'angulation du faisceau lumineux évalué à 10°maximum dans les deux axes d'orientation au niveau de la lampe à partir de la position préétablie en debut des soins.
- La
figure 5 montre que les arcades dentaires 1 et 2 peuvent etre divisées chacune en quatre quadrants à l'arcade sup.1 on décrit le quadrant sup.droit ant. (QSDA),le quadrant sup. droit post.(QSDP), le quadrant sup Gauche ant.(QSGA)et le quadrant sup. gauche post.(QSGP). A l'arcade inf.2,on décrit le quadrant inf.droit ant.(QIDA),le quadrant inf.droit post.(QIDP), le quadrant inf. Gauche ant.(QIGA)et le quadrant inf. gauche post.(QIGP). Au total un quadrant sur huit sera idéalement à focaliser en debut de phase opératoire suivant L'acte a réaliser en bouche. - Solutions techniques:
- Le but poursuivi est d'assurer une gestion pilotée a distance de la source lumineuse, cela suppose au départ un débrayage possible de ce système permettant en debut d'intervention, Un réglage manuel de la lampe par le dentiste sur le quadrant buccal souhaité.
- Ce réglage initial sera validé dans un microprocesseur ( avec interface de commande numérique de motorisation) dedié a la lampe munie de deux moteurs travaillant dans les deux axes vertical et Horizontal.Ce microprocesseur pouvant etre logé dans une console annexée à l'unit dentaire. la
figure 6 est une vue de façe de la lampe scialytique sur laquelle est localisé le micro moteur 12 dedié a la rotation horizontale pouvant etre associé ou intégré à l'articulation 2 qui assure déjà la même fonction en mode manuel(reportfig.1 ) et le micro moteur 13 dedié a la rotation verticale pouvant etre associé ou intégré à l'articulation 3 assurant la même fonction en mode manuel (fig.1A ce sujet ,on notera la poignée 14 utilisee couramment pour le réglage manuel de la lampe. ces deux servomoteurs vont donc gérer dans ces deux axes, le pivotement de la lampe sur des amplitudes réduites réservées à la phase soins, et quantifiées précédemment. L'originalité du présent brevet réside dans le fait que ces deux motorisations peuvent etre gérées Selon deux options de commande possible: - la gestion entièrement automatisée de la lampe.
- Sans intervention de l'opérateur ,via le microprocesseur au niveau duquel aura été validé
- Le 1er réglage manuel du faisceau lumineux sur le quadrant buccal intéressé par le soin,
- Une caméra située sur la lampe(en 20 sur la
figure 6 ) prend alors le relais : l'image correspondant à la zone opératoire va etre analysée et mise en mémoire à partir de qq.points caractéristiques buccaux faciaux elle sera réanalysée par le microprocesseur à chaque fois que la zone opératoire sort du champ lumineux avec une correction immédiate de la focalisation, par poursuite automatique de la commande des micro moteurs dans les deux axes directionnels. - La
figure 9 décrit l'ensemble du dispositif: - L'image de la zone ciblée par un 1er réglage manuel est transmise par la minicaméra 20
- Au microprocesseur 19 qui la met en mémoire, lorsque la minicaméra enregistre une variation de l'image, le microprocesseur transmet un signal de commande numérique aux deux micromoteurs 12 et 13 qui corrigent l'angulation de la lampe. le débrayage de ce dispositif qui est assuré par la touche pédale 16 peut etre complété par un interrupteur situé sur la mini caméra . seconde option: une gestion télécommandée de la lampe par le dentiste par le biais de la pédale 5 située au sol qui commande l'instrumentation générale (unit dentaire en
figure1 .) - La
figure 7 schématise le bloc pédale classique muni du rhéostat 6 d'activation et de contrôle des instruments rotatifs et d'une touche joystick 15 dédiée aux micro moteurs de la lampe. cette touche est présentée ici en position neutre, afin de permettre le réglage manuel de l'orientation du faisceau lumineux, en début d'intervention au niveau de cette lampe. La touche interrupteur 16,assure le débrayage de la commande automatisée , précédemment décrite en première option. - Les deux autres touches assurant des fonctions liées a l'unit dentaire ne seront pas décrites ici. Pour chaque correction de l'orientation du faisceau lumineux, l'opérateur envoie soit un signal électrique par l'intermédiaire du câble 17 ,soit un signal radio par technique sans fil 18.
- La
figure 8 décrit l'ensemble du dispositif: - A partir de la commande pédale joystick 15,le signal de l'opérateur est transmis via le câble Électrique17ou via deux platines blue tooth 18 (en pointillé sur la figure)vers le microprocesseur 19 Qui commande les micro moteurs 12 et 13 de la lampe 1 dans les deux axes directionnels.
- Ces deux dispositifs précédemment décrits selon l'invention sont particulièrement destinés au règlage motorisé a distance de l'orientation des lampes scialytiques au cabinet dentaire.
Claims (5)
- Dispositif de réglage à distance de l'orientation spatiale de la lampe scialytique Notamment pour cabinet dentaire à partir d'une position préréglée manuellement Comprenant deux micro moteurs dédiés respectivement aux mouvements vertical et horizontal de la lampe(1),comprenant des moyens de commande des moteurs,
par microprocesseur (19), pour assurer une poursuite automatisée de cette lampe, Relativement à la position préréglée, en fonction des informations en provenance d'au Moins une caméra (20),située en partie frontale de la lampe caractérisé en ce que les dits moyens de commande comprennent des moyens d'analyse de l'image provenant d'au moins une caméra isolant un ou plusieurs points de la zone bucco faciale du patient servant de référence pour la poursuite automatisée de cette lampe(1) Dans le but de la focaliser sur la zone opératoire. - dispositif selon la revendication 1,caractérisé en ce que les dits moyens de commande à Microprocesseur (19) comprennent des mémoires enregistrant la position de la lampe(1) Déterminée manuellement par l'opérateur pour réaliser une poursuite automatisée de la lampe.
- dispositif selon la revendication lou 2 caractérisé en ce qu'il comprend une pédale de commande délocalisée (5) dédiée à l'orientation de la lampe(touche 15)et/ou le Débrayage (touche 16 ) de la commande automatisée de la poursuite de cette lampe(1).
- dispositif selon revendication 3 caractérisé en ce que la dite pédale de commande (5), Comprend des moyens de commande par câble (17) pour commander la lampe(1).
- dispositif selon la revendication 3 caractérisé en ce que la dite pédale de commande(5) comprend des moyens de commande sans fil de type émetteur / récepteur (18) pour Commander la lampe (1).
