Definitions
- the present invention relates to a device for setting a ligament in surgery and a method for the surgical repair of a ligament. It also relates to tools for use with the device according to the invention.
- Ligaments also known as ligaments, are stretchy, fibrous connective tissue strands that flexibly connect moving parts of the bone skeleton, but restrict mobility to a functionally meaningful degree and stabilize them against non-physiological movements. If ligaments are stretched beyond their natural dimensions, ligament stretching and ligament tears can occur. Cruciate ligament tears are very common in the knee joint.
- a common way to replace the torn cruciate ligament is to use a ligament graft, especially an autologous graft from the patellar tendon or the semitendinosus tendon. In a known technique, the implant is removed from the tendon in such a way that one at the ends
- fix / fixation or fixation / fixation always mean the tensile and shear-resistant connection between two interfaces. It is important that the implant is drawn in and fixed in such a way that it is under a certain tension, which is required for the function of the joint, but on the other hand the tension is not so high in every joint position that the mobility is restricted .
- the screw in which the thread is also fixed by a so-called interference fit.
- the screw has a channel in which the thread is guided. In the channel, the thread can be pulled in the longitudinal direction by using a comparatively strong force. Nevertheless, the compression is strong enough to press the thread against the anchor element.
- US Pat. No. 6,517,579 B1 discloses an anchor device with which a ligament can be fixed directly.
- the device is in several parts.
- the ligament is guided between a sleeve and the drill channel in the bone.
- An element to be inserted inside the sleeve in the manner of a screw presses the sleeve with the ligament against the drilling channel, as a result of which the ligament is fixed.
- An additional fixation is also achieved by a clip-like cover, which presses the ligament into the bone in the manner of tearing.
- US 2004/037094 shows a bone anchoring element provided with eyelets, which is screwed into the bone by means of a screwdriver-like tool.
- the length of the thread drawn through the eyelets has to be set relatively elaborately with the help of an adjustable, buckle-like slider.
- US 2004/0098050 discloses an approximately can-shaped element which can be anchored in the bone and in which the adjustable length of the thread associated therewith can be fixed by latching the lid and thereby reshaping an elastic mass contained in the can.
- the devices known in the prior art have the disadvantage that the ligament is punctured very frequently for fixation, as a result of which it can tear. Furthermore, it is usually not possible to exactly determine the tension of the ligament adjust, because by setting with the known tools and devices a change in length very often occurs.
- the theoretical requirement to be put into practice is that the band must be fixed in such a way that neither the band is tightened further by the fastening, nor that the tension in the band is released. This is not guaranteed in most known devices.
- the band length is automatically changed when an interference screw is screwed in. The same applies if the threads are knotted and fixed under tension.
- the object of the present invention is therefore to provide a device for fixing a ligament, which makes it possible to fix the ligament without uncontrolled effects on the length and thus the tension of the ligament. Furthermore, damage to the ligament when fixing is to be avoided.
- the device according to the invention for fixing a ligament has a bone anchoring element and an adapter sleeve.
- Bone anchoring element has a proximal, i.e. H. closer to the patient's body, and a distal, d. H. end lying farther from the patient's body and has a longitudinal bore.
- the adapter sleeve has a proximal and a distal end. It can be received in the bore and it can be expanded radially. This radial expansion of the clamping sleeve allows the ligament to be fixed between the clamping sleeve and the bore of the bone anchoring element.
- the method according to the invention for the surgical repair of a ligament has the following steps after a replacement for the ligament to be repaired has been drawn in via at least one drill channel: a) insertion of a bone anchoring element (1) into the drill channel and anchoring in the latter, the ligament or at least one with this for fixing connected thread (50) in a bore (4) of the bone anchoring element, b) radial expansion of an adapter sleeve (10) accommodated in the bore, whereby the ligament between the adapter sleeve and the bore of the bone anchor element can be fixed.
- the ligament is drawn into the joint by making bone holes.
- the bone anchoring element is introduced into this bone hole, the ligament or the thread or the threads which are connected to the ligament being in the hole of the
- Bone anchoring element is picked up and passed through.
- the adapter sleeve is now inserted into the bore, which is designed in such a way that sufficient play remains in the ligament or thread.
- the necessary tension is applied to the ligament using a spring balance, and the ligament or the threads are fixed.
- the clamping sleeve is spread radially so that the ligament or the thread is clamped between the clamping sleeve and the bone anchoring element and is thus fixed.
- both the ligament itself and at least one thread connected to the ligament can be fixed.
- the number of threads is not critical, so that in the following the terms “one thread” and “several threads" are used interchangeably.
- a mandrel which is connected to the clamping sleeve, is provided in particular for spreading the clamping sleeve.
- the mandrel in turn has a proximal and a distal end. It is passed through the adapter sleeve. The diameter of the proximal end is larger than the inner diameter of the adapter sleeve.
- the clamping sleeve is radially expanded by pulling the mandrel out of the clamping sleeve, as a result of which the clamping sleeve is expanded due to the difference in size of the proximal end of the dome and the inside diameter of the clamping sleeve.
- the advantage of this design is that the expansion sleeve is spread from the proximal end to the distal end. This means that the thread or the ligament is first fixed near the body and only then is the further part of the thread or the ligament subsequently fixed by the further spreading. This has the advantage that the tension is already established with the first fixation at the first point of the bone anchoring element. It cannot change due to the further fixing of the ligament or the thread between the tensioning sleeve and the bone anchoring element.
- the radial expansion can also be carried out using grub screws or similar elements.
- An alternative design would also be the insertion of a rivet.
- the bone anchoring element is preferably designed in the manner of a hollow bone screw. It is preferably made of metal with a suitable roughening (e.g. roughly blasted), but can also be made of bovine or human bone material that has been appropriately pretreated for implantation.
- the roughening can e.g. B. done by coarse corundum blasting.
- An advantage of the roughening of the surface on the bone side is that the secondary fixation, i. H. the bone growth is improved.
- the bone anchoring element can also be made from resorbable material such as polylactide or similar resorbable materials as they are for the so-called resorbable
- Interference screws are used. It is inserted into the bone canal by screwing it in.
- a specially designed screwdriver is provided for screwing in, which is preferably cannulated, so that the tension threads can be passed through it.
- the threads serve as a guide when introducing the bone anchoring element into the drill channel. This is extremely advantageous since the position of the bone anchoring element is predetermined by the exit point of the solid fixation threads from the bone hole.
- the bone anchoring element can hereby be placed on the screwdriver outside the body and only then can it be guided through the soft tissue channel to the drilling channel through the threads.
- the clamping sleeve is preferably profiled on the outside, in particular ribbed, edged or nubbed, as a result of which the ligament is particularly well secured.
- the profile can be transverse, oblique, diamond-shaped, helical or in one comparable pattern with periodic alternation of depths and majesties.
- the adapter sleeve is preferably made of metal.
- the clamping sleeve and / or the bone anchoring element are made of a shape memory alloy, which is also known under the name "MemorymetaH".
- a specially designed pair of pliers is provided for pulling the dome out of the clamping sleeve and thus its radial expansion. She is one
- Pop rivet pliers similar. In addition, it allows the threads to be tightened with the help of additionally attached tensioning devices with a defined, possibly also measurable tensile force, and thus to press the ligament graft into the bone.
- the guide sleeve of the screwdriver After inserting the bone anchoring element with the special screwdriver, the guide sleeve of the screwdriver remains on the head of the bone anchoring element and thus forms a fixed working channel through the so-called soft tissue jacket.
- the mandrel of the pop rivet pliers with the attached clamping sleeve (10) is then inserted through the working channel and, after the clamping sleeve has been spread, the threads are cut off with a conventional thread cutter.
- FIG. 1 shows a section through a device according to the invention in connection with a corresponding tool
- FIG. 2 the section corresponding to FIG. 1, the device being shown in use according to the invention (with thread);
- FIG. 3 two exemplary embodiments of a bone anchoring element according to the invention; each in perspective view (A, C) and in sectional view (B, D)
- FIG. 4 a section through an adapter sleeve according to the invention
- FIG. 5 a side view of a dome according to the invention
- Figure 6 a screwdriver according to the invention in a partially broken side view (A, C with guide sleeve) and in section (B without guide sleeve).
- FIGS. 1 and 2 each show a device according to the invention in section, as it is assembled ready for use, a thread 50 connected to a ligament (not shown) additionally being entered in FIG. 2.
- the device according to the invention for fixing a ligament has a bone anchoring element 1 which has a proximal end 2 and a distal end 3.
- the bone anchoring element 1 has a bore 4 which runs along the longitudinal axis 5 of the bone anchoring element or the entire arrangement.
- the bone anchoring element 1 has a thread on its outside, e.g. B. a bone thread, for example in a sawtooth shape.
- an engagement opening 7 is provided, through which the bone anchoring element 1 by means of a
- the bore 4 preferably has two areas, an area 4a and an area 4b, the area 4b being enlarged in diameter compared to the area 4a. This creates an axial stop 8.
- a clamping sleeve 10 is introduced into the interior of the bone anchoring element 1.
- the adapter sleeve 10 also has a proximal end 11 and a distal end 12 (FIG. 4).
- the adapter sleeve 10 is a hollow cylinder.
- the outside diameter D2 of the clamping sleeve is matched to the inside diameter D1 of the bore 4 of the bone anchoring element 1 in such a way that the clamping sleeve 10 can be inserted into this bore 4.
- the clamping sleeve 10 has a profiling 15 on its outside. This profile 15 is a preferred embodiment - the clamping sleeve 10 can also be smooth. Profiling advantageously offers a certain increased static friction.
- the clamping sleeve 10 is connected to a mandrel 20.
- This mandrel 20 is passed through the interior of the clamping sleeve 10.
- the proximal end 21 of the dome 20 is expanded in comparison to the distal end 22 of the dome 20.
- FIG. 5 shows the corresponding diameter D3 of the proximal end 21 in comparison to the diameter D4 of the distal end.
- the inner diameter D5 of the clamping sleeve 10 is smaller than the outer diameter D3 of the dome 20, but larger than the outer diameter D4 of the dome 20.
- the mandrel 20 can be passed through the clamping sleeve 10 from the proximal end 11.
- both the outside diameter D3 of the dome 20 and the outside diameter D2 of the clamping sleeve 10 are smaller than the inside diameter D1 of the bone anchoring element 1. Collet 10 and mandrel 20 are consequently inserted into the bore 4, more precisely in the area 4b of the bore 4.
- the ligament is fixed in that the clamping sleeve 10, as indicated by the arrows 13 in FIG. 4, is expanded radially by pulling out the dome 20.
- the movement in the distal direction as indicated by arrow 34 (see FIGS. 1 and 2), expands the clamping sleeve 10 not only in the inside diameter D5 but also in the outside diameter D2.
- the wall 14 is pressed against the inner surface 9 of the bone anchoring element 1 and the thread 50 is thereby fixed. It is, so to speak, between the bone anchoring element 1 and the clamping sleeve 10 fixed by jamming.
- FIGS. 1 As can be clearly seen from FIGS.
- the proximal end 11 of the clamping sleeve 10 is first pressed against the bone anchoring element 1, more precisely against its inner surface 9. Only with continued distal movement of the dome 20 is the thread 50 or the ligament fixed along the further regions of the tensioning sleeve 10. The thread 50 and thus the ligament are consequently first fixed at the proximal end of the fixation. This has the advantage that shortening or other length changes are no longer possible due to the further determination process. The thread 50 and thus the ligament is already fixed with the first contact of the wall 14 of the clamping sleeve 10 with the bone anchoring element 1.
- the dome 20 can be pulled out to fix the thread 50 using, for example, a tool in the manner of pop rivet pliers.
- pliers 30 are shown schematically and in sections in FIGS. 1 and 2. These pliers have a front end 31 through which the mandrel 20 is inserted. The front end 31 of the forceps 30 is axially supported on the distal end 3 of the bone anchoring element 1 and the distal end 12 of the clamping sleeve 10. The mandrel 20 is engaged with the engagement device 33. When the pliers 30 are actuated, the mandrel 20 is moved out of the clamping sleeve 10 in the distal direction, as indicated by the arrow 34.
- a thread guide can also be provided at the front end 31 (not shown) through which the thread 50 is guided. This has the advantage that the thread always has enough play and is not inadvertently clamped between the front end 31 of the forceps 30 and the distal end 3 of the bone anchoring element.
- Figure 6 shows a screwdriver 40 according to the invention in side view (A; C: partially broken view) and in section (B). It has a handle 41 and a shaft 42.
- the engagement area 44 is designed in such a way that the engagement opening 7 of the bone anchoring element 1 can thus be positively engaged.
- the screwdriver 40 is cannulated so that the thread 50 can be passed through it.
- the channel 43 can be seen in FIG. 6B.
- a guide sleeve 60 is pushed over the shaft 42 and is preferably clamped thereon with a friction fit.
- the guide sleeve 60 is removable (see FIG. 6C).
- the guide sleeve 60 serves to hold the opening through the skin and so-called soft parts, and as a guide when inserting the clamping sleeve 10 connected to the mandrel 20.
- the guide sleeve 60 is removed and the clamping sleeve 10 is spread radially by pulling out the mandrel 20 using the pliers described above.
- FIG. 3 shows two exemplary embodiments of a bone anchoring element 1, 1 'according to the invention.
- the bone anchoring element 1' At its proximal end 2 ', the bone anchoring element 1' has a cylindrical guide section 16 which fits into the bone bore and which has been found to be advantageous when the bone anchoring element 1 'is introduced into the predrilled bone. Furthermore, the engagement opening T is designed as a hexagon socket or Torx (not shown).
- the surgeon proceeds as follows after pulling in the ligament: First, the bone anchoring element 1 is inserted into the Hole or inserted into the drill channel in the bone. For this, the screwdriver 40 is used, which engages the bone anchoring element 1.
- the thread 50 is first passed through the bore 4 of the bone anchoring element 1 and is then passed through the channel 43 of the cannulated screwdriver 40 with the aid of a thread insertion aid, not shown.
- the surgeon now has adequate guidance, which is provided by the thread 50.
- the cannulated screwdriver 40 and the bore 4 of the bone anchoring element 1 ensure that the thread 50 of the ligament is not damaged when the bone anchoring element 1 is screwed in and is also not jammed between the screw and the drilling channel.
- the screwdriver 40 is pulled out.
- the guide sleeve 60 which has been inserted together with the screwdriver 40, remains on the bone anchoring element 1.
- the thread now runs in the bone anchoring element 1 and is guided outwards through the guide sleeve 60.
- the surgeon then introduces the clamping sleeve 10 of the bone anchoring element together with the mandrel 20 through the guide sleeve 60 into the bone anchoring element 1, so that the thread 50 is correctly guided between the inner surface 9 of the bone anchoring element 1 and the wall 14 of the clamping sleeve 10, as in FIG 2 shown. Subsequent to
- the surgeon adjusts the tension of the ligament, as is known in the art, for example also with the aid of a spring balance.
- the mandrel 20 is pulled out of the clamping sleeve 10, as described above, for example with the aid of the pliers 30.
- the clamping sleeve 10 has been expanded radially, that is to say the mandrel 20 has been pulled out of the device the pliers 30 are removed.
- the bone anchoring element 1 is set and the ligament is fixed.
