Abstract
L'invention a pour objet l'utilisation d'un adjuvant particulier, le DCchol pour la préparation de compositions vaccinales destinées à être administrées à des nouveau-nés.
Description
DCchol chez les nouveau-nés
La présente invention est relative à l'utilisation d'un adjuvant vaccinal pour la préparation d'un vaccin destiné à l'administration de mammifères nouveau-nés, ainsi qu'à une méthode d'immunisation de nouveau-nés. En particulier, l'invention est relative à l'utilisation de
DCchol pour la préparation d'un vaccin pour nouveau-nés.
La mise au point de vaccins est toujours difficile, mais elle est particulièrement compliquée lorsqu'il s'agit de vaccins destinés aux nouveau-nés; en effet, on a remarqué que lorsqu'on procédait à l'immunisation durant le 1er mois de la vie, on obtenait une moins grande quantité d'anticorps que lorsque l'immunisation était réalisée plus tard ; en outre, les anticorps obtenus étaient généralement d'une durée de vie plus courte que lorsque l'immunisation avait lieu plus tardivement.
D'autre part, il semblerait que les réponses des cellules T obtenues à un stade précoce de la vie, bien qu'étant des réponses qui maturent rapidement, soient des réponses différentes à la fois qualitativement et quantitativement de celles qui sont induites à un stade ultérieur de la vie.
Cependant, les nouveau-nés auraient besoin d'être très tôt armés pour pouvoir lutter contre un certain nombre de pathogènes, auxquels ils sont susceptibles d'être exposés très jeunes, en particulier le virus de l'Immunodéiicience Humaine, le Virus Respiratoire Syncytial, l'agent du paludisme, Heïicobacter pyîori, Haemophilus influenza type B, le virus de la polio, celui de l'hépatite B, les agents de la coqueluche, du tétanos, de la diphtérie, de la méningite, de la pneumonie.
En conséquence, il existe un besoin fort de pouvoir disposer de compositions vaccinales permettant d'immuniser efficacement des nouveau-nés. Pour répondre à ce besoin, la présente invention a pour objet l'utilisation de DCchol pour la préparation d'une composition vaccinale destinée à être administrée à des nouveau-nés.
Selon un mode particulier, la composition vaccinale comprend au moins un antigène contre les infections provoquées par Heïicobacter pyîori.
Selon un autre mode particulier, la composition vaccinale comprend au moins l'antigène TAT du virus du SIDA.
L'invention a également pour objet une méthode d'immunisation de nouveau-nés selon laquelle on administre aux nouveau-nés au moins un antigène vaccinal, dans laquelle le
perfectionnement consiste à administrer également au moins un adjuvant comprenant du DCchol.
De nombreux avantages de la présente invention apparaîtront à la lecture de la description détaillée qui va suivre, en référence à la Figure I qui illustre les résultats obtenus à l'exemple 1.
Aux fins de la présente description, on entend par nouveau-nés, des enfants âgés d'un mois au plus au moment où ils reçoivent la première injection.
Etant donné le fait qu'il est souhaitable de réduire autant que possible les essais cliniques menés chez les humains, et en particulier chez les enfants et les nouveau-nés, un modèle animal a été développé. En effet, bien qu'il soit difficile de comparer formellement la maturation immune d'une espèce à l'autre, de nombreuses expériences ont cependant permis de valider l'hypothèse que quelle que soit l'espèce, la maturation immune suit toujours les mêmes étapes, la différence entre les différentes espèces résidant dans la cinétique de passage d'une étape à l'autre. Aussi, les formulations vaccinales selon la présente invention, bien que destinées essentiellement à l'administration à des nouveau-nés humains, ont été mises au point sur la base de tests réalisés sur des souriceaux, les étapes de maturation immunitaire du souriceau à la souris adulte ayant été démontrée comme essentiellement les mêmes que les étapes de maturation immunitaire du nouveau-né humain à l'homme adulte. Dans ce modèle, les nouveau-nés humains d'un mois sont représentés par des souriceaux âgés d'une semaine.
Par amélioration de la réponse immune selon l'invention, on entend l'obtention chez le nouveau-né d'une réponse immune se rapprochant de celle obtenue chez l'adulte; il peut notamment s'agir d'une réponse en anticorps ou en cellules T qui serait quantitativement augmentée par rapport à celle généralement obtenue avec les vaccins de l'art antérieur, ou encore d'une modification qualitative de la réponse obtenue; les caractéristiques qualitatives des réponses immunes pouvant s'exprimer par la nature des anticorps produits, ainsi que par la nature et la quantité des cytokines induites, ce qui permet en particulier de déterminer l'orientation ThI ou Th2 de la réponse. Chez les souris, lorsqu'on s'intéresse à la réponse humorale, c'est-à-dire aux immunoglobulines produites, on considère que les IgG2a traduisent une orientation de la réponse dans le sens ThI5 et que les IgGl traduisent une orientation de la réponse vers le type Th2. Lorsqu'on regarde les cytokines produites, on considère que PInterféron γ est un
indicateur d'une orientation ThI, alors que les Interleukines 4 et 5 sont des indicateurs d'une orientation Tn2.
Par DCchol au sens de la présente invention, on entend du 3β pST-( TSf', N'- dimethylaminoethane) carbamoyl] cholestérol, qui peut être obtenu à partir de cholesteryl chloroformate et de N, N-dimethylethylenediamine, selon la méthode décrite dans le brevet USP 5,283,185 ou encore selon la méthode décrite à l'exemple 8 du brevet USP 5,753,262. De façon préférentielle, on utilise la forme saline du DCchol, en particulier le chlorure.
Par composition vaccinale, on entend une composition comprenant au moins un antigène vaccinal d'intérêt pour l' immunisation des nouveau-nés. Il peut s'agir d'une composition vaccinale monovalente, c'est-à-dire comprenant un seul type d'antigène, ou d'une combinaison vaccinale comprenant plusieurs antigènes permettant d'immuniser simultanément contre une ou plusieurs maladies. Les antigènes susceptibles d'être adjuvés grâce à la présente invention peuvent être de nature diverse; il peut s'agir de protéines, de peptides, d'oligo- ou poly-saccharides (conjugués ou non), de virus ou portions de virus,...etc., plus généralement de tout antigène intéressant à introduire dans une composition vaccinale pédiatrique. La présente invention ne concerne pas l'utilisation du DCchol pour adjuver des compositions vaccinales qui, au lieu de comprendre les antigènes eux-mêmes, comprendraient les antigènes sous forme dé leur « précurseur » qu'est l'ADN. La présente invention présente un intérêt particulier dans le cas de certaines maladies susceptibles d'affecter un nouveau-né dès les premiers instants de sa vie; il s'agit par exemple du SIDA, du paludisme, ...
Selon l'invention, la composition vaccinale peut être administrée par toutes les voies habituellement utilisées pour l'administration de vaccins, et notamment par voie sous cutanée, intramusculaire, transcutanée et intradermique. Le schéma d'administration est avantageusement un schéma prévoyant au moins 2 administrations successives : une lère administration suivie d'un rappel quelque temps après, la lère administration étant effectuée dans le 1er mois de vie.
Avantageusement, la composition vaccinale préparée selon l'invention présente une immunogénicité accrue sans pour autant connaître une réactogénicité augmentée. La réactogénicité peut être appréciée de différentes manières; chez la souris, on effectue les surveillances suivantes : - réactogénicité locale : au site d'injection vérification de la présence ou non d'un granulome inflammatoire avec présence ou non d'une cicatrice empêchant la pousse des poils, - réactogénicité systémique : pesée hebdomadaire permettant de vérifier si la prise de poids est normale ou pas. De façon classique, les compositions vaccinales selon l'invention peuvent également comprendre tous les éléments parfois présents dans les compositions selon l'art antérieur, tels que conservateurs, stabilisants ou autres.
Les exemples qui suivent illustrent quelques modes de réalisation de la présente invention.
1. Composition vaccinale comprenant la protéine TAT.
1.1 Préparation des compositions vaccinales.
On prépare des compositions vaccinales comprenant comme antigène une protéine TAT III B détoxifiée. La protéine TAT a été détoxifiée par une réaction d'alkylation en milieu alcalin par utilisation d'iodoacétamide dans les conditions suivantes : nombre de micromoles d'iodoacétamide = 200 X nombre de micromoles de TAT + nombre de micromoles de DTT. Cette protéine détoxifiée et son procédé de préparation sont décrits en détails dans la demande WO99/33346 où elle est identifiée sous le terme de TAT carboxyméthylée.
Cet antigène TAT recombinante est présent en solution à 2g/l.
Les compositions vaccinales sont préparées soit avec un adjuvant selon l'art antérieur, c'est-à-dire de l'oxyhydroxyde d'aluminium ou une émulsion à base de squalène, soit selon l'invention avec du DCchol; en outre, afin de pouvoir mettre en évidence le pouvoir adjuvant des différents produits testés, on prépare également une composition comprenant uniquement l'antigène en solution saline.
La composition comprenant de l'oxyhydroxyde d'aluminium est préparée à partir d'une suspension à 8g/l, fournie par la Société Superfos Biosector.
L'émulsion est préparée en mélangeant à rUltraturrax pendant quelques secondes 0,18g de Tween™ 80 (fourni par Merck) 0,16g de trioléate de sorbitan (fourni par la Société Seppic sous le nom de Montane VG™85) et 1,29 g de squalène (fourni par la Société Fluka) dans de l'eau en quantité suffisante pour 30 ml. On obtient alors une pré-émulsion, qui est homogénéisée au Microfluidiseur MI lO (Microfluidics, Newton MA ) par 20 passages à une pression moteur de 60 psi donner une émulsion mono disperse avec des gouttelettes d'une taille moyenne de 120nm.
La composition liposomale de DCchol est préparée en dispersant 1,3 g de poudre de
DCchol (fourni par la Société Avanti® Polar Lipids Inc) dans 65ml d'eau distillée; la suspension est agitée pendant 15 min à température ambiante (22°C) et 1 heure à 650C. Après refroidissement autour de 39-4O0C, la suspension est filtrée à travers un filtre hydrophile Durapore™ (Millipore) de porosité 0,45 μm et sous pression à travers un filtre
Anotop™(Whatmann) de porosité 0,2 μm.
On obtient alors une suspension de liposomes dont la taille moyenne est de 176nm, et dont la concentration est de 20μg/ml.
Chacune des compositions préparées est diluée dans une solution saline qui contient au final 50mmol/l de Tris(hydroxymethyl)aminométhane et 100 mmol/1 de chlorure de sodium, le pH étant ajusté à 7,4.
On obtient alors des doses vaccinantes ayant la composition suivante :

1.2 Immunisation des souris
On dispose de souris Balb/c adultes et de souris « nouveau-nées » âgées de 1 semaine réparties en groupes de 6 à 8 (groupes de souriceaux) ou de 5 à 6 (groupes de souris adultes). On réalise des expériences avec 2 injections sous-cutanées de la même formulation vaccinale à JO et à J21. On effectue des prélèvements sanguins après l'injection de rappel. Les immunoglobulines spécifiques de la TAT sont dosées par technique ELISA. Un total de 10 à 40 souris par formulation a été testé dans plusieurs expériences indépendantes : pour le DC-Chol : 27 adultes, 38 nouveau-nés, pour l'oxyhydroxyde d'aluminium : 18 adultes, 22 nouveau-nés; pour l'émulsion : 10 adultes et 16 nouveau-nés et en solution saline : 10 adultes et 15 nouveau-nés. Les résultats obtenus sont indiqués sur la Figure I qui indique, pour chacune des formulations testées, la quantité totale d'anticorps IgG exprimée en 1Og10 des titres arbitraires ELISA
Ces résultats montrent que l'oxyrrydroxyde d'aluminium et l'émulsion qui sont de bons adjuvants chez les souris adultes, ne donnent pas de résultats satisfaisants chez les nouveau- nés; contrairement à eux, le DCchol est capable d'augmenter la réponse irnmune induite par une composition vaccinale lorsqu'elle est administrée à des adultes, mais également lorsqu'elle est administrée à des nouveau-nés.
2. Composition vaccinale comprenant l'uréase de Heîicobacter pylori.
2.1. Préparation des compositions vaccinales.
On prépare des compositions vaccinales ayant comme antigène l'uréase recombinante de Heîicobacter pylori. Cette uréase a déjà été décrite comme un candidat potentiel pour la fabrication d'un vaccin contre les ulcères de l'estomac induits par la bactérie Heîicobacter pylori. Un procédé pour sa préparation est décrit dans la publication suivante : Journal of

Les compositions vaccinantes sont préparées de la même manière que décrit à l'exemple précédent, sauf à remplacer au moment du mélange final, la suspension comprenant l'antigène Tat, par une suspension comprenant de l'uréase.
On obtient des doses vaccinantes de 50μl comprenant à chaque fois lOμg d'uréase et soit 200 μg de DCchol, soit 200 μg d' AlOOH.
2.2 Immunisation des souris.
On dispose de souris Balb/c adultes et de souris âgées de 1 semaine, réparties en groupes de
5 à 8 (groupes de souris adultes, ou groupes de souriceaux).
On réalise des expériences avec 2 injections sous-cutanées de la même formulation vaccinale à JO et à J21 (expériences 1 et 2), ou avec 3 injections à JO, J21 et J36 (expériences 3 et 4).
On effectue des prélèvements sanguins après la lere injection, après le 1er rappel et éventuellement après le 2nd rappel (dans le cas des expériences 3 et 4).
Les immunoglobulines spécifiques de l'uréase sont dosées par technique ELISA.
Les résultats relatifs aux titres mesurés, exprimés en log10 des titres ELISA, relativement aux quantités d'IgGl et d'IgG2a produites chez les souris nouveau-nés sont indiqués dans le tableau ci-après :
Les dosages des quantités d'anticorps présents chez les souriceaux avant la lere injection ne peuvent pas être réalisés sans sacrifier les animaux.
Aussi, il n'est pas possible d'avoir pour un même souriceau, son taux d'anticorps avant immunisation et après immunisation; cependant, un certain nombre de dosages ont quand même été effectués, et bien qu'ils varient en fonction des anticorps maternels, on a pu observer qu'ils étaient en moyenne bien inférieurs à ceux obtenus après la lere injection, ce qui permet bien de valider l'effet de l'immunisation.
Les résultats obtenus montrent que le DCchol permet d'induire chez les nouveau-nés une plus grande quantité d'anticorps que l'oxyhydroxyde d'aluminium.
Dans chacune des expériences réalisées, bien que cela ne soit pas décrit en détail ici, on a également immunisé un même nombre de souris adultes; les résultats obtenus montrent que les titres en anticorps chez les souris adultes sont légèrement plus élevés que chez les souriceaux; par contre, on voit dans la population des adultes que si le DCchol est un aussi bon adjuvant que l'oxyhydroxyde d'aluminium, il ne lui est pas supérieur en ce qui concerne la production d'anticorps.
D'autre part, les critères permettant d'apprécier la réactogénicité des compositions vaccinales sont satisfaisants quelque soit l'adjuvant utilisé.
Claims
1. Utilisation de DC chol pour la préparation de compositions vaccinales destinées à être administrées à des nouveau-nés.
2. Utilisation selon la revendication 1, caractérisée en ce que la composition vaccinale comprend au moins un antigène contre les infections provoquées par Helicobacter pylori
3. Utilisation selon la revendication 1, caractérisée en ce que la composition vaccinale comprend au moins l'antigène TAT du virus du SIDA.
4. Méthode d'immunisation de nouveau-nés, selon laquelle on administre aux nouveau-nés au moins un antigène vaccinal, dans laquelle le perfectionnement consiste à administrer également au moins un adjuvant comprenant du DCchol.
