Neben genetischen Faktoren spielen bei der Pathogenese von Herz-Kreislauferkrankungen vor allem Fehlernährung und Übergewicht, Alkohol- und Nikotinabusus sowie ein Mangel an körperlicher Bewegung eine Rolle.
Ein häufiges Krankheitsbild ist die koronare Herzkrankheit, unter der die Angina pectoris und der Herzinfarkt zusammengefaßt werden. Die Angina pectoris ist ein multifaktorielles Krankheitsbild, das durch eine Atherosklerose der Koronararterien verursacht wird. Durch flußlimitierende Koronarstenosen kommt es zu einer Minderdurchblutung der Herzmuskulatur in Form einer stabilen oder instabilen Angina pectoris, einer stummen Myokardischämie, ischämisch bedingter Herzinsuffizienz, Herzrhythmusstörungen oder einem akuten Myokardinfarkt. Der Herzinfarkt wird durch den Verschluß einer Koronararterie mit einem Thrombus (Blutpfropf) hervorgerufen. Der Thrombus bleibt meistens an einer Verengung der Herzkranzgefäße stecken. Die dahinter liegenden Regionen des Herzmuskels werden dann nicht mehr mit Blut versorgt. Je nach Ort des Infarkts können große oder weniger große Bereiche betroffen sein. Die Basistherapie besteht in der Elimination der bekannten Risikofaktoren und einer medikamentösen Thrombozytenaggregationshemmung mittels Acetylsalizylsäure oder Ticlopidin. Zur Behandlung der Angina pectoris-Anfälle werden Gefäßdilatatoren wie Nitrate, Betarezeptorenblocker oder Calziumantagonisten verwendet, die jedoch unerwünschte Wirkungen wie Hypotonie, Blutumverteilung (steal-Phänomen) oder cardiodepressive Nebenwirkungen haben können.
