Beschreibungdescription
The invention relates to a method for DNA analysis of blood and a means for performing the method. The field of application of the invention is medical diagnostics.
Davon wären in Deutschland mehrere 100.000 Menschen betroffen. Der Gentest ist die genauesteIt is now certain that at least 5,000 diseases are genetic, that is, inheritable. Due to the decoding of the underlying genes and their disease-causing defects (gene mutations), approximately 450 hereditary diseases are accessible for diagnosis (JAMA: Statement on use of DNA testing for presymptomatic identification of cancer risk 271, 785 (1994)). Most of these inherited diseases are monogenic (synonymous with assigning a single gene to a specific disease) and include relatively rare diseases that occur in early childhood and are considered incurable. These include, for example, cystic fibrosis, muscular dystrophy or the Fragile (X) syndrome (BioEssays: Trinucleotide repeat expansion and human genetic diseases 16, 277 (1994)). In the past 5 years it has surprisingly turned out that a large number of so-called common diseases can also be inherited. These include such common diseases as hypertension and arteriosclerosis, breast cancer, colon cancer, diabetes, Alzheimer's, etc. Most of these diseases can be successfully treated with timely diagnosis. This has dramatically expanded the possible uses of genetic diagnostics. Familial hypercholesterolemia (FH), for example, is one of the most common autosomal dominant inherited diseases (about 1 in 500 people, arterosclerosis, thrombosis, and vascular biology: Vol. 15, 12 (1995)) with a high risk of heart attack. A genetic test for FH would make high-risk patients 2 Detect early and deliver targeted drug therapy with cholesterol-lowering drugs that significantly reduces the risk of heart attack. This would affect several 100,000 people in Germany. Similar considerations apply to the early detection of cancer risk patients. The genetic test is the most accurate

Die molekulargenetische Gendiagnostik wird in folgenden Schritten durchgeführt (Nature Medicine: Analysis of mismatch repair genes in hereditary non-polyposis colorectal cancer patients 2, 169 (1996)):Molecular genetic genetic diagnostics is carried out in the following steps (Nature Medicine: Analysis of mismatch repair genes in hereditary non-polyposis colorectal cancer patients 2, 169 (1996)):
1. Aus einer Blutprobe wird DNS isoliert;1. DNA is isolated from a blood sample;
2. Mittels Polymerasen-Ketten-Reaktion (PCR) wird ein die Mutation enthaltender DNS-Abschnitt amplifiziert;2. A DNA section containing the mutation is amplified by means of polymerase chain reaction (PCR);
The mutated and the normal DNA section are compared with one another by gel electrophoretic separation methods and / or Southern blots.
In special cases, the DNA section must be sequenced to identify an unknown mutation.
Whole blood is common as the starting material for the preparation of pure DNA in molecular biological research. A minimum amount of blood of 5-10 ml is necessary to purify such DNA.
- ca. 10 ml EDTA Blut werden im Puffer lysiert, der 1% Triton x-100 enthält;The DNA is usually purified according to the following scheme (Proc.Natl.Acad.Sc.: Analysis of human Y-chromosome-specific reiterated DNA in chromosome variants 74, 1245 (1977)), which is still verified differently by the laboratories: - About 10 ml of EDTA blood are lysed in the buffer containing 1% Triton x-100;
- 10 min Zentrifugation;- 10 min centrifugation;
- Aufnahme des Pellets in 1 ml 0,9% NaCl; - Lyse in 10% SDS plus EDTA;- Take up the pellet in 1 ml 0.9% NaCl; - lysis in 10% SDS plus EDTA;
- Zugabe von Na-perchlorat;- addition of Na perchlorate;
- Zugabe von Chloroform/Octanol zur Herausbildung von 2 Phasen;- Add chloroform / octanol to form 2 phases;
- 20 min Zentrifugation;- 20 min centrifugation;
- obere Phase plus 5ml Chloroform; - 5 min Zentrifugation;- upper phase plus 5ml chloroform; - 5 min centrifugation;
- obere Phase abnehmen;- remove the upper phase;
- mit Isopropanol die DNS fällen und mit Glasstab aufwickeln;- Precipitate the DNA with isopropanol and wind up with a glass rod;
- in kaltem Alkohol waschen;- wash in cold alcohol;
The disadvantage of this method, which was previously used in clinical diagnostics, is that generally 5-10 ml of EDTA blood from the patient (always venous blood!) Must be transported to the genetic laboratory in a cool place within 48 hours.
This usually requires express sample transport, preferably on ice, followed by 2 days of DNA isolation. The actual genetic test with the PCR does not begin until about 4 days after taking blood in the doctor's office. This means that, for example, the diagnosis of the cystic fibrosis genetic test can only be made after 14 working days at the earliest. If the doctor does not send the blood by express immediately after collection, the blood often clots, making proper DNA isolation impossible.
Es umfaßt ein Reinigungsreagenz, einen Kollektions- undGene Guard System for blood samples for DNA analysis using PCR known to be a complex system for the collection Transport that represents storage and purification of DNA. It includes a cleaning reagent, a collection and
Cleaning kit and a punch. However, this system is relatively expensive, since it requires this specially pretreated carrier for each patient.
The aim of the invention is therefore to improve the genetic test for medical diagnostics. It is based on the task of developing a fast, uncomplicated and cost-saving method for the collection of blood samples by the family doctor and self-sufficient patients and their further processing. Another object of the invention is to provide a means for transporting and storing blood samples.
Die Erfindung wird gemäß den Ansprüchen realisiert.The invention is implemented according to the claims.
The method for DNA analysis in the blood is based on the surprising finding that an existing amount of blood must not exceed 0.05 ml of blood per cm 3 of an area carrier, since otherwise the PCR is inhibited. This is due to the fact that blood proteins, such as hemoglobin, carry metal ions such as Fe or Zn, which inhibit the PCR.
According to the invention, the blood to be examined is applied to an absorbent, porous surface carrier for transport or storage, this surface carrier is incubated with water for analysis and the blood cell suspension obtained is concentrated. This is preferably done by rapid centrifugation, particularly preferably for 3 minutes at 14,000 x g. A solid pellet is obtained which is processed further; over 90% of the supernatant is discarded.
In einer bevorzugtenThe pellet is then mixed with a chelating agent and heated up to 100 ° C. In a preferred one
This heating is carried out in two stages; in a first step to 50 to 60 ° C and then to 100 ° C. According to the invention, the heating can also take place immediately at 100 ° C. Surprisingly, this heating to 100 ° C. after chelex chromatography produces an optimal DNA yield for the subsequent PCR.
The mixture is then centrifuged again and the DNA solution obtained as a supernatant is analyzed in a conventional manner, if appropriate after further storage at -20 ° C. The essential function of the chelating agent is that polyvalent metal ions are bound and the DNA is protected during the temperature treatment. This optimizes the subsequent PCR treatment. According to the invention, any commercially available chelating agent can be used.
Paper, cellulose, plastic films, cotton or linen are used as surface supports. Preferred is the use of filter paper, especially filter paper, as is used in routine clinical diagnostics, e.g. B. as Guthrie paper, is used. Whatman blotting paper as used in Southern, Northern and analog techniques is also preferred.
This paper is expediently designed in the form of a card which has several circular fields for taking the blood samples, contains instructions for using the card and fields for information about the patient, the doctor treating the patient and the date (Fig. 1).
The filter paper used as a surface support preferably has an absorption capacity of 0.03-0.05 ml blood / cm 2 .
The invention also relates to the means for transporting or storing blood for DNA analysis, comprising absorbent, porous surface supports such as paper, cellulose, plastic film, cotton or linen. The preferred medium is filter paper, especially filter paper with an absorption capacity of 0.03-0.05 ml blood / cm a .
The invention further relates to the use of blood applied to an absorbent, porous surface support for DNA analysis, preferably paper-dried blood.
The new method according to the invention offers the following advantages over the conventional method for obtaining DNA from fresh whole blood:
The patient samples can be sent in normal envelopes as paper-dried blood spots for the genetic test. Express delivery and sample cooling are no longer necessary. The genetic test can therefore be offered nationwide
2. Venenblut ist nicht mehr erforderlich. Der Patient kann selbst Fingerblut einsenden.2. Vein blood is no longer required. The patient can send in finger blood himself.
The unused blood spots on the marked paper circles can be kept for archival purposes.
The DNA extract can be obtained within 2 hours and can be used directly for the PCR.
The preparation of a genetic test diagnosis (mutation and deletion screening) is shortened from approx. 14 days with the start of taking blood in the doctor's office for conventional whole blood to approx. 7 days for paper-dried blood, i.e. by almost half.
einer DNS-Isolierung gemäß dem erfindungsgemäßen Verfahren um mehr als das 10-fache gesenkt werden kann (DNS-Isolierung nachCompared to other carriers (from Fitzco), costly pretreatment of the carrier according to the invention is not necessary. Surprisingly, the process can be carried out without special and expensive reagents (buffer systems) for paper treatment or blood extraction. Furthermore, no special punches for removing the blood spots and no special PCR tubes are required, which reduces the price a DNA isolation according to the method of the invention can be reduced by more than 10 times (DNA isolation after

The invention thus provides a simple, quick and safe method for producing a DNA extract from paper-dried blood for carrying out genetic tests. Critical amounts of DNA can also be obtained with the method according to the invention. Since no special sample transport is required and the diagnosis can be made in the doctor's office about 7 days after the blood has been drawn, there is an essential prerequisite for being able to offer the genetic test according to the invention, in particular, to the general practitioner, in whose hands the responsibility for early detection and prevention lies. The first step, the blood sample preparation for the genetic test by the "family doctor", is uncomplicated, safe, quick and inexpensive, and can be carried out by the patient himself at home.
The invention will be explained in more detail below using an exemplary embodiment.
Anschließend werden dieSimple filter paper is provided with circles with a diameter of 1 cm. The circles are wetted with approx. 0.04 ml of blood, which can be taken from the finger, vein, heel or ear. Venous blood is not required. The paper must be such that it can absorb about 0.03-0.05 ml of blood per cm 2 . 3-4 mm strips of paper are cut out of the blood spot and placed in a small Eppendor tube (for comparison purposes, 3-10 microliters of fresh whole blood can be pipetted directly into an Eppendor tube). Then l ml of water are added and incubated on a rocker for 20 min by gently swirling the tube. Then the

Added shaking. Then shake at 56 ° C for 20 min on a vortex shaker, then at 100 ° C for 8 min. The suspension is then centrifuged at 14,000 x g for 3 min. The supernatant is carefully removed and stored at -20 ° C. 1-15 microliters per 25 microliter batch volume are pipetted directly from this solution into the PCR mixture. For comparison, the procedure was carried out as described above with 3-10 microliters of whole blood. For comparison purposes, a mutation screening for cystic fibrosis, a deletion screening for muscular dystrophy type Duchenne / Becker and a mutation screening for familial hypercholesterolemia were carried out in parallel with paper-dried or whole blood from test subjects. The results are identical.
