Description

The invention relates to an implant for medialization of the Vocal cords according to the preamble of claim 1, as is known for example from US-A-5 197 982. These implants are used for paralysis of the vocal fold narrow the glottis and the vocal folds out to bring them back to the midline in their lateral position. For this purpose, an opening is made in the thyroid cartilage, through which the implant is inserted and thereby the vocal fold is medialized. A disadvantage of the so far mainly The implants used is that there are certain objections to this used material exist. These implants must be dated Surgeons are cut out of a silicone block. The Application is therefore time-consuming and the design is successful not always optimal. In addition, the fixation of the implants difficult, so that a shift is possible. Such Displacement of the implants z. B. inside the larynx can have life-threatening consequences as it causes breathing is relocated.

The object of the present invention is an implant to create that with a standardized technique during surgery under local anesthesia of the patient can be used quickly and safely, the degree of Medialization is individually adjustable.

The task is carried out with an implant of the type mentioned at the beginning Art solved according to the invention in that between the middle part and at least one of the hook-shaped End regions a bend of the metal strip is arranged, that after inserting the implant between the thyroid cartilage and the vocal fold lies. By changing the radius the contact pressure of the middle part can be countered by this bend the vocal fold vary. The contact pressure can also be used also vary in a smaller range in that the entire implant changed in its distance from the vocal fold becomes. About this type of setting the contact pressure of the implant to enable at least one of the hook-shaped End areas using a set screw on the thyroid cartilage be attachable. To avoid rejection reactions of the The implant can either be made from a body-compatible body Made of metal and / or with a body-compatible Be covered. Coming as material for the implant for example titanium sheet or also gold or stainless steel sheet in question.

A particular advantage of the present invention is that The implant is secure after insertion into the thyroid cartilage can be positioned so that a displacement of the implant, possibly even in the airways with life-threatening Complications, is not possible. This is due to the special Shaping the implant and bending the metal ends achieved, and can be achieved by fixing the implant further secured with a set screw on the thyroid cartilage become.

A preferred embodiment of a implant according to the invention explained in more detail with reference to the drawing.

The only figure shows a partial section through the human Larynx. It is the one located in the anterior larynx area To recognize thyroid cartilage 10 from which itself Vocal folds 11 to cartilage 20 not shown extend. There is an opening in the side of the thyroid cartilage 10 12 has been introduced through which an implant 13 for one of the two vocal folds 11 is inserted. The implant 13 is indicated in the figure by a thick black line and bent from a metal strip. It points a middle part 14 which bears against the vocal fold 11. The end regions 15 and 16 of the implant 13 are hook-shaped trained and embrace the edges of the opening 12 in the thyroid cartilage 10. Between the middle part 14 and the lower hook-shaped End region 16 of implant 13 is a bend 17 of the metal strip arranged. This bend 17 is between the thyroid cartilage 10 and the vocal fold 11 and generated thus a bias of the middle part 14 against the vocal fold 11. The degree of preload can be changed by changing the Radius of the bend 17 can be varied, indicated by a double arrow 18 is indicated. Also a shift in the middle section 14 in the direction of the vocal fold 11 leads to a Increasing the preload of the middle part 14. This shift can be made by means of a set screw 19, with which the end region 15 of the implant 13 with the thyroid cartilage 10 is connected. In the implant shown thus the bias of the middle part 14 against the vocal fold 11 either by bending or compressing the bend 17 or by adjusting the screw 19 during the Operation of the patient under local anesthesia become. According to the result of being constantly with the patient feasible voice tests can thus be an optimal Preloading the implant 13 and thus optimal support achieve the vocal fold 11.
