Prerequisites for creating the invention
A sore throat is characterized by pain or irritation in the throat or pharynx, usually caused by strep throat. A sore throat is most often caused by a viral infection. A sore throat can also be caused by streptococcal infections, tumors, gastroesophageal reflux disease, mononucleosis and allergies.
A sore throat can develop for many reasons, including a viral or bacterial infection or a general or seasonal allergy. Often associated with infection, common or seasonal allergies include some degree of nasal or sinus congestion. This congestion typically refers to the drainage of mucus from the nasopharynx, with mucus formed on the surface of the nasal mucosa or sinus lining draining into the upper part of the esophagus. The accumulation of nasal mucus in the upper esophagus also stimulates the swallowing reflex, often associated with a sore throat. Due to the swallowing reflex, acidic mucus moves and is in relatively constant contact with the throat area. Due to its acidic nature, the mucus in the sinus lining or nasal cavity erodes the epithelial tissue of the throat and thus the acidic mucus attacks the underlying tissue. When the nerve endings in the underlying tissue come into contact with the acidic mucus, a sensation identified as discomfort or pain associated with a sore throat occurs. The more inflamed the nasal mucosa or sinus lining is, the more acidic mucus is produced, the greater the erosion, and the greater the pain and discomfort associated with a sore throat.
A sore throat can be treated with a variety of dosage forms or agents. Common dosage forms include throat sprays, lozenges, and orally administered tablets or liquids, each of which may contain active ingredients. Sprays and lozenges usually contain topical analgesics or menthol to relieve sore throats. Tablets or liquids administered orally typically contain systemically active ingredients for pain, cough and/or colds; including, for example, acetaminophen, NSAIDs, decongestants, and/or cough suppressants. In some cases, these products contain cooling agents that also help relieve pain or create a sensation of less pain.
There remains a need for liquid compositions and methods that are safe and effective for treating, relieving, or reducing the severity of sore throat. Such a composition should produce a cooling effect, work quickly, and provide effective sore throat relief over an extended period of time.
Summary of the invention
The present invention relates to pharmaceutical liquid compositions that can be used to relieve sore throat. Compositions that provide a thin layer of coating on oral surfaces enhance sensation, such as cooling effect, compared to prior art compositions. In particular, the composition provides a longer-lasting, targeted cooling sensation in the throat area.
In a first aspect, the invention relates to a pharmaceutical liquid composition, wherein the liquid composition contains at least one cooling agent containing a phenyl ring containing a polar side group, at least one viscosity increasing agent and at least one surfactant. The unique composition with these particular types of cooling agent, viscosity-increasing agent, and surfactant provides the composition with improved oral feel compared to using a cooling agent having other structures in combination with a viscosity-increasing agent and surfactant. . There is a feeling of coolness on the back of the throat, and this feeling lasts longer compared to other compositions.
Additionally, it was found that the targeting and longer-lasting effects were absent in low-viscosity fluids even when a surfactant was included in the formulation. In the presence of only the viscosity modifier without the surfactant, the same flavor also produced a targeted cooling effect, but the magnitude and duration were less, demonstrating that it was the viscosity-increasing agent-surfactant mixture that produced the improved mouthfeel and its longer duration. and a specific group of cooling agents.
The invention also relates to a method for preparing such a pharmaceutical composition, as well as a method for alleviating a symptom selected from the group consisting of cough, nasal congestion and sore throat in a subject, comprising administering a pharmaceutical liquid composition.

To improve the feel of liquid cough and cold formulations in the oral cavity, various polymer additives have been used as rheology and texture modifiers. HPMC (hypromellose). With the additional inclusion of poloxamer in a composition containing the above rheology modifier and cooling flavor, a synergistic effect was observed. A feeling of coolness appeared on the back of the throat, and this sensation lasted longer (approximately 5-10 minutes or even more in some cases). To understand this effect, additional studies were conducted and it was found that the effect of targeted action and longer lasting sensation was absent in low-viscosity liquids, even when poloxamer was included in the composition. In the presence of only the viscosity modifier, without the poloxamer, the same flavor also gave a targeted cooling effect, but its severity and duration were less.
To better understand the mechanism providing a targeted, longer-lasting cooling effect in samples containing a rheology modifier (Avicel, CMC), poloxamer and coolant No. 2, the following studies were carried out.
Zeta potential measurement.
Zeta potential (ζ potential) is the potential difference at the interface between solids and liquids. It is a measure of the electrical charge of particles suspended in a liquid. When a polymer solution is added to a mucin suspension, the change in the zeta potential of the mucin particle can characterize the polymer-mucin interaction (formation of an electrostatic double layer).
2.For mucoadhesion studies, mucin from the pig stomach was used. Briefly, the mucin was suspended in deionized water and stirred overnight using a magnetic stirrer to completely hydrate and disperse the mucin. CMC and poloxamer-188 were individually dissolved in deionized water to a level of 0.5%. Using a Malvern Zetasizer Nano, the zeta potential of 0.5 wt.% mucin was determined in triplicate. Initial experiments were carried out at different levels of mucin in the suspension (from 0.1 to 1.0 wt%), and a reproducible zeta potential value for a separately prepared mucin suspension was obtained at a level of 0.5 wt%. To measure the mucin-polymer interaction, different amounts of polymer solution (0.5 wt% in water) were added to the mucin suspension and the mixture was incubated at 37°C for 15 min before measuring the zeta potential. The measured zeta potential of a suspension of 0.5 wt.% porcine mucin in water was -4.5 mV. When a solution of CMC or poloxamer-188 was added to the mucin suspension, the zeta potential of the mucin particle changed to a more negative value, as shown in Table 1. 2.
188Zeta potential of a mucin particle in the presence of CMC and poloxamer-188
1).Varying amounts of CMC and poloxamer-188 solution were added to a known volume of mucin suspension, and it was observed that the zeta potential of the mucin-CMC and mucin-poloxamer system initially shifted to a higher negative value with increasing polymer concentration, reaching a plateau at a ratio of 1 :10 mucin:CMC and mucin:poloxamer-188. This shift of zeta potential to higher negative values indicates an interaction between the polymer and mucin. To see whether the combination of poloxamer and CMC produces a synergistic effect, solutions of poloxamer and CMC were simultaneously added to the mucin suspension to obtain a ratio of 1:10:20 mucin:CMC:poloxamer (the ratio of CMC to poloxamer in liquid cough and cold compositions is 1: 2). At the same time, a slight increase in zeta potential was observed from -39.47 mV (at a mucin:CMC ratio of 1:10) to 42.7 mV (Fig. 1).
CMC is an anionic polymer and, as with other anionic mucoadhesive polymers such as carbopol, a shift in the mucin zeta potential to higher negative values is expected. However, poloxamer-188 is a non-ionic polymer and electrostatic interactions are not expected to occur in this system. Poloxamers are block copolymers consisting of a central hydrophobic polyoxypropylene chain connected to two hydrophilic polyoxyethylene chains. The literature suggests that poloxamers have mucoadhesive properties due to their ability to form hydrogen bonds. Based
Благодаря такому δ-ve заряду на молекуле полоксамера в присутствии донора водородной связи полоксамер обладает способностью выступать в роли слабого анионного полимера и может вызывать сдвиг дзета-потенциала системы муцин-полоксамер в область более высокого значения -ve при их смешивании друг с другом.- 5 044547 structure, the poloxamer should not act as a hydrogen bond donor, since it does not have an OH group (a hydrogen atom covalently bonded to one of the electronegative oxygen atoms). However, due to the presence of 2 lone pairs of electrons on the oxygen atom in the oxyethylene part of the molecule, poloxamer molecules in an aqueous environment can receive a δ-ve charge and are able to form a hydrogen bond with a hydrogen bond donor. Due to this δ-ve charge on the poloxamer molecule in the presence of a hydrogen bond donor, the poloxamer has the ability to act as a weak anionic polymer and can cause a shift in the zeta potential of the mucin-poloxamer system to a higher -ve value when they are mixed with each other.
Example 2. Rheology studies.
Rheology studies of mucin, polymer, and their mixtures were conducted to study synergism and mucoadhesion. Solutions of 8% poloxamer-188 and 4% CMC were prepared by separately dissolving poloxamer-188 and CMC in purified water. A 4% mucin suspension was prepared by mixing the mucin in purified water overnight using an overhead mixer.
Rheological data (stationary shear sweep curves) were measured for an 8% poloxamer-188 solution, a 4% CMC solution, and a 4% mucin suspension. To study the interaction, a known amount of poloxamer was dissolved in a suspension of 4% mucin to obtain 8% poloxamer-188 in a suspension of 4% mucin. The CMC-mucin mixture was prepared in a similar manner by dissolving 4% CMC in a suspension of 4% mucin. Steady-state shear sweep curves for poloxamer-188 solution, CMC solution, mucin suspension, and mucin-poloxamer, mucin-CMC, and mucin-poloxamer-CMC combinations at 25°C are shown in FIG. 2. These curves were analyzed using a TA Instruments AR 2000 rheometer used in a parallel plate configuration.
For this study, high levels of polymer (poloxamer-188 and CMC) were used to detect the interaction effect between mucin and the polymers (poloxamer-188 and CMC). According to FIG. 2 poloxamer-188 has the lowest viscosity (approximately 0.0027 Pa-s at a shear rate of 1.00 1/s), at the same shear rate the viscosity of the mucin suspension is approximately 0.06113 Pa-s. However, a significant drop in viscosity can be observed for a mixture of mucin and poloxamer-188 at the same level of both components. This reduction in viscosity has not previously been observed or described.
The viscosity of a 4% CMC solution at a shear rate of 1.00 1/s is approximately 2.545 Pa-s, and the mucin-CMC combination system at the same shear rate has a viscosity of 7.274 Pa-s, mucin-CMC poloxamer-188-23.1 Pa -With. When mixing 2 non-reacting polymers, the resulting viscosity is expected to be a combination of the viscosities for the dissolved polymers, with the viscosity being primarily determined by the component with the highest viscosity. However, in the presence of synergistic interactions, the resulting viscosity can be significantly higher or lower, as observed with the mucin-CMC, mucin-poloxamer, and mucin-CMC-poloxamer systems.
Because significantly higher concentrations of poloxamer-188 and CMC were used, the study was repeated with lower concentrations of poloxamer-188 and CMC. Rheological measurements were carried out at both 25°C and 37°C. In fig. 3 and 4 show the rheological profile for 2% poloxamer-188, 1% CMC, 2% mucin and their combinations.
According to FIG. 3 and 4 at both 25°C and 37°C, a similar trend in the viscosity profile can be observed, reflected in the higher viscosity of the mucin-CMC and mucin-CMC-poloxamer systems compared to the additive viscosities of the components.
As discussed previously, synergism and no significant change in viscosity are expected when a non-interacting polymer is mixed with mucin. To evaluate this behavior, studies were also carried out using a nonionic polymer (HPMC) instead of CMC. The results of the rheology study for the combination of 2% poloxamer-188, 1% HPMC E4M and 2% mucin at 37°C are presented in FIG. 5. The graph below clearly shows the lack of interaction between HPMC, mucin and poloxamer, since the curve for HPMC practically overlaps with the HPMC + mucin + poloxamer curve and the HPMC + mucin curve. Since the measurements were carried out at 37°C, the degree of decrease in the viscosity of the mucin-poloxamer system was negligible, as previously observed.
Example 3. Chemical interaction between refrigerant and polymer as determined by FTIR and 1 H NMR.
If there is a chemical interaction, such as hydrogen bonding, between the cooling flavor (cooler #2) and the mucoadhesive polymer (poloxamer-188 and/or CMC), the cooling sensation from the coolant will be longer lasting because the cooling flavor will be preferentially distributed in the polymer phase, combining with mucous membrane and create a feeling of coolness. To identify the presence of hydrogen bonds between coolant No. 2 and poloxamer-188 and/or CMC, the FTIR spectra of a mixture of coolant No. 2 with poloxamer-188 and CMC and the spectra of individual components were compared, and no peak shifts or changes in their shape were observed, which means that there are no chemical interactions between them.
Proton NMR spectra of poloxamer-188, coolant no. 2, and a mixture of coolant no. 2 with poloxamer-188 were recorded by dissolving them in deuterated DMSO. In fig. 6-8 show the spectra

1-3.Example 4. Results and summary of examples 1-3.
Based on the available rheology and zeta potential measurements, it is clear that there is some degree of interaction between mucin and CMC. This interaction may be attributed to electrostatic interaction and/or chain entanglement between mucin and CMC, similar to other anionic mucoadhesive polymers such as carbopols. In the presence of a nonionic polymer such as HPMC, no interactions were observed.
Благодаря такому увеличению вязкости время нахождения жидкости на ткани увеличивается, создавая эффект покрытия в области горла и обеспечивая более продолжительное ощущение прохлады в области горла благодаря наличию охладителя № 2.The results of rheology and zeta potential measurements also show the presence of an interaction between mucin and poloxamer-188. The presence of poloxamer led to a decrease in the viscosity of the mucin suspension. Zeta potential measurements also indicate the presence of interaction, but to a lesser extent compared to CMC. Rheological data for mucin-CMC-poloxamer combinations at low and high concentrations and temperatures showed a significant increase in viscosity, significantly exceeding the additive effect. Based on available data, it can be assumed that the viscosity of liquid cough and cold compositions increases significantly upon contact with mucous tissues in the oral cavity (if swallowed). This increase in viscosity increases the residence time of the liquid on the fabric, creating a coating effect in the throat area and providing a longer-lasting cool sensation in the throat area due to the presence of the No. 2 cooler.
The lack of peak shift in the FTIR spectra can be attributed to the absence of a liquid (aqueous) medium that could facilitate the formation of hydrogen bonds. It should be noted that if these 2 components are mixed in an aqueous medium, the peaks from hydrogen bonds will be completely masked/covered by the peaks of water, and therefore FTIR spectroscopy in this case will not be the best tool for analysis. In the 1H NMR data there is no indication of the existence of a chemical interaction such as hydrogen bonds between the poloxamer and coolant no. 2. However, it should be noted that the measurements were carried out in d-DMSO (coolant no. 2 is insoluble in water, so DMSO was used), and under these conditions hydrogen connections may be missing.
Example 5. Evaluation of taste testing results.
Various polymer additives were added to the base formulation of liquid cough and cold formulations to determine interactions, which included, among other things, various types of Avicel®, CMC and HPMC. The basic composition of the composition is presented in table. 6. With the additional inclusion of poloxamer in a composition containing the above rheology modifier and cooling flavor, a synergistic effect was observed. A sensation of coolness was perceived at the back of the throat, and this sensation lasted longer (approximately 5-10 minutes or even more in some cases). Additional studies showed that the targeted effect and longer-lasting sensation were absent in low-viscosity liquids, even when poloxamer was included in the composition. In the presence of only the viscosity modifier, without the poloxamer, the same flavor also gave a targeted cooling effect, but its severity and duration were less.
Additional studies have been conducted to understand the cooling flavor/cooling agent effect in formulations containing both poloxamer and viscosity modifiers.
3).Samples of the cooling agent were requested from various flavor suppliers and included in the manufacturer's recommended amount and rated by different individuals regarding their effectiveness/intensity of the cooling effect, the area within the mouth where the cooling effect was felt, the time of onset of the cooling effect, and the duration of the cooling effect. effect. It was interesting to see that all the cooling agents produced different degrees of cooling effect, and the areas within the mouth where the cooling effect was felt were different. Most coolers produced a cooling effect in the mouth, tongue, or roof of the mouth, with little or no cooling effect on the back of the throat, with the exception of Symcool® NAT Cooler No. 2, WS-3, and WS-5. Coolant #2 did not produce a cooling effect in the mouth at all, and all the cooling effect was concentrated on the back of the throat, however, the other 3 agents produced a cooling effect in the mouth and on the tongue, along with some cooling effect on the back of the throat. Below is a summary of the results of the taste evaluation of various refrigerant systems (also presented in Table 3).

Mixing procedure.
To mix those indicated in the table. 3 materials used the following procedure. The composition was prepared by adding purified water to a suitable container. Subsequently, Avicel RC591 was added while mixing using a high shear mixer until dissolved, followed by the addition of propylene glycol while mixing using a paddle mixer at about 500-1000 rpm. The remaining ingredients were added and mixed at about 50-100 rpm for at least 30 minutes. After complete dissolution, the solution was heated in a water bath to a temperature of 140 to 170°C for 15 min with continuous stirring. The solution was cooled to room temperature, after which propylene glycol was added with stirring. The remaining ingredients were added and mixed for at least 30 minutes.

Example 6: Electrical conductivity measurement.
Measurement of electrical conductivity of solutions.
9.The conductivity of solutions containing poloxamer-188 and coolant No. 2 in purified water at various concentrations was measured using a MultiLab 4010-2 conductivity meter with a TDS 4320 conductivity and temperature sensor. The results of the conductivity measurements are presented graphically in FIG. 9.
The electrical conductivity of solutions increases with increasing concentration of poloxamer-188 from 0.2 to 2.0%. The addition of 0.15% coolant No. 2 has a significant effect on the electrical conductivity of the solution at low concentrations of poloxamer-188, but at high concentrations of poloxamer-188 the two curves practically converge.
Because Coolant No. 2 is insoluble in water, a water-5% propylene glycol solution was used to prepare solutions containing varying amounts of Coolant No. 2 and the poloxamer concentration was maintained at 0.4%. The electrical conductivity of solutions increases significantly from about 7.4 MK S/cm to about 35 MK S/cm with the addition of only 0.05% coolant No. 2. Increasing the level of coolant No. 2 from 0.05 to 0.45% gives an almost linear increase in electrical conductivity from about 35 MC S/cm to about 55.3 MC S/cm (Fig. 2). It is important to note that when coolant No. 2 was added, the solutions became cloudy, indicating the formation of an emulsion. With an increase in the content of coolant No. 2, the turbidity of the solutions practically did not change.
10.An increase in the electrical conductivity of solutions may indicate an increase in the ionic character of the solution, and may affect ionic attraction and binding to a charged mucosal surface. The results are shown in Fig. 10.
Electrical conductivity of solutions containing viscosity modifiers (CMC and HPMC).
5.In addition, the electrical conductivity of water-PEG solutions containing 0.35% CMC and 0.35% HPMC was measured in water and in combination with coolant no. 2 and poloxamer. The results of electrical conductivity measurements are presented in table below. 5.
Since CMC is an ionic polymer, the electrical conductivity of a 0.35% CMC solution is very high both in water and in the presence of poloxamer and coolant No. 2, however, the addition of a nonionic polymer viscosity modifier HPMC E4M practically does not change the electrical conductivity of the solution. The electrical conductivity of a solution containing HPMC, poloxamer 188 and coolant No. 2 is almost the same as the electrical conductivity of a solution with the same levels of poloxamer 188 and coolant No. 2, but without HPMC. The decrease in electrical conductivity of a solution containing HPMC indicates less ionic character and negligible ionic attraction and binding to the charged mucosal surface. Similar trends were observed when measuring rheology, and when mixed with a mucin suspension, the change in solution viscosity for HPMC was less than when using CMC. The expected interaction with the mucous surface (throat) may be greater when using CMC due to the significant increase in electrical conductivity.
