Claims (11)

1. Optoelektronisk stimulerende anordning, der er konfigureret til at blive anvendt i en medicinsk behandling, der indebærer levering af en elektrisk strøm til et elektrisk exciterbart væv (Ζμ0) ved hjælp af to elektroder (3n, 3p), der er elektrisk koblet til vævet, hvilken optoelektroniske stimulerende anordning omfatter: (a) et hus (20), der indeholder en kilde (4) til elektriske impulser, som er elektrisk forbundet med (b) en lysemissionskilde (2) i optisk kommunikation med (c) en fotovoltaisk celle (1), der er elektrisk forbundet med to elektroder (3n, 3p) for at etablere to elektriske kontakter med vævet og derved danne et elektrisk stimulerende kredsløb, der fødes af den fotovoltaiske celle (1), som aktiveres af strålingen fra lysemissionskilden (2), hvori huset er fysisk adskilt fra elektroderne, kendetegnet ved, at anordningen desuden omfatter: (d) en kilde til kontrollysemission (13), der udsender et lys, når den aktiveres elektrisk, og (e) en lysemissionssensor (9), der er i optisk kommunikation med kilden (13) til kontrollysemission, og som er koblet til en processor, der er programmeret til at optage de pulser, som udsendes af kilden til kontrollysemission, hvilken processor fortrinsvis er en del af kilden til elektriske impulser (4), og hvori kilden til kontrollysemission (13) er placeret enten • i det elektrisk stimulerende kredsløb, i serie med elektroderne (3p, 3n) og den fotovoltaiske celle (1), eller • i et styrekredsløb, adskilt fra det elektrisk stimulerende kredsløb, i serie med en fotovoltaisk styrecelle (1C), som er i optisk kommunikation med den samme kilde (2) til lysemission som det stimulerende kredsløbs fotovoltaiske celle (1).
2. Optoelektronisk stimulerende anordning ifølge krav 1, hvori kilden til elektriske impulser (4) er egnet til at få lysemissionskilden til at udsende kalibrerede lysimpulser af en varighed på mellem 1 ps og 10 ms.
3. Optoelektronisk stimulerende anordning ifølge krav 1 eller 2, hvori den fotovoltaiske celle (1), når den aktiveres af lysemissionskilde (2), leverer ladninger på mellem 0,01 og 50 pC, fortrinsvis mellem 0,1 og 10 pC ved en højere spænding end den, der er tilgængelig fra kilden til elektriske impulser (4) .
4. Optoelektronisk stimulerende anordning ifølge et hvilket som helst af de foregående krav, der omfatter en af de følgende konfigurationer: (a) Huset (20) indeholder også lysemissionskilden (2), den fotovoltaiske celle (1) og omfatter elektriske forbindelsesstik (8), der er beregnet til elektrisk tilslutning af den fotovoltaiske celle (1) til de to elektroder (3p, 3n) for at danne det elektrisk stimulerende kredsløb, eller (b) huset (20) indeholder også lysemissionskilden (2) og er forbundet til et optisk fiber (7) til at overføre det lys, der udsendes af lysemissionskilden (2) , til den fotovoltaiske celle, der er anbragt inde i et hus (30), som er placeret adskilt fra huset (20) og er tilstødende elektroderne (3n, 3p), eller (c) indkapslingen (30) indeholder den fotovoltaiske celle (1) og lysemissionskilde (2), som er i elektrisk kontakt med den kilde til elektriske impulser, der er indeholdt i huset (20).
5. Optoelektronisk stimulerende anordning ifølge et hvilket som helst af de foregående krav, der desuden omfatter: • en genopretnings-lysemissionskilde (2R) i optisk kommunikation med • en fotovoltaisk genopretningscelle (IR), der er elektrisk forbundet med de to elektroder (3n, 3p) for at etablere to elektriske kontakter med vævet og derved danne et elektrisk ladningsgenopretningskredsløb parallelt med det elektrisk stimulerende kredsløb og fødet af den fotovoltaiske genopretningscelle (1), således at en strøm (I1R), når den fotovoltaiske genopretningscelle (IR) aktiveres af genopretnings lysemissionskilden (2R), løber gennem vævet i en retning modsat den strøm (II), der løber i det elektrisk stimulerende kredsløb, således at al elektrokemisk ladning, der induceres ved elektrode-vævs-grænsefladen, fjernes.
6. Optoelektronisk stimulerende anordning ifølge et hvilket som helst af de foregående krav, der desuden omfatter et DC-blokerende kredsløb, som omfatter en sikkerhedskapacitans (5) , der er placeret i serie med lysemissionskilden (2).

Optoelektronisk stimulerende anordning ifølge et hvilket som helst af de foregående krav, hvori den fotovoltaiske celle (1) omfatter et enkelt fotovoltaisk diodekredsløb (10) eller alternativt omfatter N fotovoltaiske diodekredsløb (10), der er placeret i serie, for at styre den fotovoltaiske celles (1) spænding, hvor N ^ 2.
8. Optoelektronisk stimulerende anordning ifølge et hvilket som helst af de foregående krav i form af et implantat, der er udformet således, at i det mindste elektroderne (3n, 3p) kan implanteres i vævet for at blive stimuleret elektrisk, et hus (20), der i det mindste indeholder kilden til elektriske impulser (4), kan implanteres på et sted fjernt fra det væv, der skal stimuleres elektrisk, og hvori et hus (30), der i det mindste indeholder den fotovoltaiske celle (1), kan implanteres tilstødende elektroderne og det væv, der skal stimuleres elektrisk.
9. Optoelektronisk stimulerende anordning ifølge et hvilket som helst af de foregående krav, hvor det elektrisk exciterbare væv (Zbio) , som skal fødes med elektrisk strøm, er valgt fra (a) neurale væv og (b) muskelvæv.

Optoelektronisk stimulerende anordning ifølge krav 1, der er konfigureret til at blive anvendt som en i det mindste delvist implanterbar anordning i en eller flere af de følgende stimuleringer: (a9) Deep Brain Stimulation til behandling af Parkinsons sygdom, epilepsi, depression, obsessiv-kompulsiv tilstand, dystoni, essentiel tremor, smerte eller blindhed (alO) cortikal hjernestimulering til behandling af døvhed, blindhed eller epilepsi (all) rygmarvsstimulering til behandling af smerte, paraplegi, spasticitet, Parkinsons sygdom (al2) spinalrodstimulering til behandling af smerte, inkontinens (al3) sensoriske nervers stimuleringer, såsom stimulering af nervus cochlearis ved døvhed eller stimulering af synsnerve ved blindhed (al4) stimulering af motorisk nerve til behandling af lammelser af central oprindelse, såsom dropfodsyndrom, stimulering af nervus phrenicus ved luftvejssygdomme, stimulering af nervus hypoglossus ved søvnapnøsyndrom (al5) stimulering af sensoriske nerver til behandling af smerte eller for at tilvejebringe haptisk feedback eller anden sensorisk modalitetsinformation hos patienter med en protese (A16) stimulering af nervus vagus til behandling af epilepsi, depression, smerte, inflammatorisk syndrom, fedme, hjertesvigt (b3) muskelstimulering til behandling af skoliose, forskellige former for lammelse af perifer oprindelse, respirationsinsufficiens, søvnapnøsyndrom, eller (b4) hjertestimulering til behandling af hjertearytmi.

Optoelektronisk stimulerende anordning ifølge krav 1, der er konfigureret til at blive anvendt som en ekstrakorporal anordning ved en eller flere af de følgende stimuleringer: (a) diagnosticering af nerve- og muskelsygdomme ved måling af reaktionslatenser, ledningshastigheder, responsamplituder, stimuleringstærskler, rekrutteringskurver for direkte responser og refleksresponser i de følgende strukturer: (i) test af motorisk nerve, hvorved stimulatoren aktiverer motoriske eller blandede nerver gennem huden, alt imens den tilsvarende mekaniske aktivitet eller elektriske muskelaktivitet registreres (ii) stimulering af sensoriske nerver, hvorved stimulatoren aktiverer sensoriske nerver gennem huden, alt imens det resulterende samlede nerveaktionspotentiale registreres (iii) transkutan stimulering af sensoriske eller blandede nerver, alt mens de fremkaldte potentialer på forskellige punkter langs den centrale vej op til hjernen registreres (iv) direkte transkutan muskelstimulering, alt imens den tilsvarende mekaniske eller elektriske muskelaktivitet registreres (b) terapeutisk behandling ved transkutan stimulering af motoriske nerver eller muskler: (v) ved prostetisk behandling af lammelser eller pareser af central oprindelse, for eksempel behandling af et "dropfodsyndrom" (vi) for at bevare tilstanden af perifere organer, mens lammede centrale neurale strukturer regenererer og for at undgå spasticitet, liggesår og andre komplikationer af immobiliseringen (vii) for at forbedre muskelstyrken i tilfælde af lokal svaghed eller asymmetri, såsom ved scoliose (viii) ved bodybuildings- og sportstræningsanvendelser (c) terapeutisk behandling ved transkutan stimulering af sensoriske nerver eller følsomme områder (iv) for at reducere smerte (v) for at give en blind person en mønstret hudstimulering i et sensorisk substitutionsapparat (vi) for at tilvejebringe et advarselssignal eller et afferent signal i enheder til bekæmpelse af søvnighed.
