[0231] Further genome-wide genotyping of Icelandic patients diagnosed with coronary artery disease and/or myocardial infarction was performed. Recruitment and phenotyping was described above under Example, and all methods involved were as described in the above.
[0232] Descriptions of the additional replication cohorts are as follows:
Cohort from Italy: The subjects from Verona were enrolled into the Verona Heart Project, which is an ongoing study aimed at identifying new risk factors for coronary artery disease (CAD) and myocardial infarction (Ml) in a population of subjects with angiographic documentation of their coronary vessels. In brief, the CAD group had angiographically documented severe coronary atherosclerosis, the majority of them being candidates for coronary artery bypass grafting or percutaneous coronary intervention. Control subjects were selected such that they had normal coronary arteries, being submitted to coronary angiography for reasons other than CAD. Controls with history or clinical evidence of atherosclerosis in vascular districts beyond the coronary bed were excluded. Information on Ml diagnoses was gathered through medical records showing diagnostic electrocardiogram and enzyme changes, and/or the typical sequelae of Ml on ventricular angiography. The study was approved by local Ethical Committee. Informed consent was obtained from all the patients after a full explanation of the study.
Cohort from Johns Hopkins: The study group from Baltimore is comprised of The Johns Hopkins University Sibling and Family Heart Study, G-Cal, and GeneSTAR studies. The subjects consisted of families identified from 1983-2006 from probands with premature coronary disease event (before 60 years of age). Probands with documented CAD were identified at the time of hospitalization in any of the Baltimore area hospitals. Their apparently healthy siblings without CAD were recruited as well as adult offspring and spouses. Of probands, generally 24% had an Ml as their index event, 65% had a PCI and/or CABG, 8.4% had angina with angiography documented CAD and were treated medically, 1.7% had sudden cardiac death, and 1.7% had angiographic documented coronary stenosis with an abnormal stress test and no symptoms.
Cohort from WTCCC: This study cohort has been described previously (Nature 447:661-78 (2007)). Summary statistics from a genome-wide association study performed on these samples is publicly available at http://www.wtccc.org.uk/info/summary_stats.shtml.
[0233] Reanalysis of the markers presented in Table 1 was performed using the expanded dataset of 3,047 individuals diagnosed with coronary artery disease, 2,395 diagnosed with myocardial infarction, and 28,321 population controls. The results are presented in Table 3.
[0234] The top 100 markers in the scan were explored further by replication analysis in six cohorts from Durham (North Carolina, US), Philadelphia (Pennsylvania, US), Atlanta (Georgia, US), John Hopkins University (Maryland, US), Italy, and in a dataset from the Wellcom Trust Case Control Consortium (WTCCC; England). The replication cohorts combined comprise 6,108 cases and 19,396 controls. The results of the replication analysis is shown in Table 4A - 4H, both separately for each cohort and a combined analysis for all the replication cohorts, as well as a combined for all the cohorts, including the Icelandic discovery cohort. Many of the variants discovered in the Icelandic discovery cohort show overall significant association in the combined replication groups. The effects are modest, which explains why some cohorts do not indicate significant association, while other cohorts do so.
[0235] In the combined analysis of all the groups (9,155 cases; 47,717 controls; Table 4H), marker rs11751605 stands out as the most significant, with a combined P-value of 1.16 x 10'8. This is a highly significant number, and in fact represents statistical significance at the genome-wide level, after applying the conservative Bonferroni correction for the number of statistical tests performed (approx. 300,000 in the combined analysis). It should be noted that the number of tests performed in the replication effort is much smaller; consequently, the correction to be applied to correct for the number of tests is smaller, or 100 (giving P-values of 0.05/100, or about 0.0005 as significant after correction). Several markers meet this threshold of significance.

1. 2. Fremgangsmåde ifølge krav 1, hvor tilbøjeligheden, som skyldes tilstedeværelsen af den i det mindste ene allel, er øget modtagelighed.

Fremgangsmåde ifølge et hvilket som helst af de foregående krav, som yderligere omfatter, at mindst én biomarkør i en prøve fra individet vurderes, hvor biomarkøren er en hjertemarkør eller en inflammatorisk markør valgt blandt kreatinkinase, troponin, glykogenphosphorylase, C-reaktivt protein (CRP), serum amyloid A, fibrinogen, interleukin-6, vævsnekrosefaktor-alfa, opløselige vaskulære celleadhæsionsmolekyler (sVCAM), opløselige intervaskulære adhæsionsmolekyler (sICAM), E-selektin, matrixmetalloprotease type 1, matrixmetalloprotease type 2 , matrixmetalloprotease type 3, matrixmetalloprotease type 9, serum sCD40L, leukotriener, leukotrienmetabolitter, interleukin-6, vævsnekrosefaktor-alfa, myeloperoxidase (MPO) og N-tyrosin.
6. Fremgangsmåde ifølge krav 5, hvor leukotrienet er valgt blandt LTB4, LTC4, LTD4 and LTE4.
7. Fremgangsmåde ifølge et hvilket som helst af de foregående krav, som yderligere omfatter, at ikke-genetisk information analyseres for at foretage risikovurdering, diagnose eller prognose af individet, hvor den ikke-genetiske information er valgt blandt alder, køn, socioøkonomisk status, tidligere sygdomsdiagnose, individets sygdomshistorik, familiehistorik med hensyn til koronararteriesygdom eller myokardieinfarkt, biokemiske målinger og kliniske målinger.
8. Apparat til bestemmelse af en genetisk indikator for koronararteriesygdom eller myokardieinfarkt i et menneskeligt individ fra en kaukasisk population, hvilket apparat omfatter: en computerlæsbar hukommelse; og en rutine lagret i den computerlæsbare hukommelse; hvor rutinen er tilpasset til at kunne eksekveres på en processor for at analysere markør-og/eller haplotypeinformation for i det mindste ét menneskeligt individ med hensyn til i det mindste én polymorf markør, som er valgt fra markøren rsll751605 samt markører i koblingsuligevægt med denne valgt fra gruppen bestående af markørerne angivet i tabel 5, og generere et output baseret på markør- eller haplotypeinformationen, hvor outputtet omfatter et risikomål for den i det mindste ene markør eller haplotype som en genetisk indikator for koronararteriesygdom eller myokardieinfarkt hos det menneskelige individ.
