Description
The invention relates to an attachment for attaching to the drill side of the handpiece of a drilling device for the explantation and transformation of bone.
The explantation and transformation of human bone material is primarily used for the purpose of transplantation. Implantations and transplantations of bone material are carried out primarily in otorhinolaryngology, oral, maxillofacial and neurosurgery, as well as in plastic, general and dental surgery. Bone parts are removed from various parts of the body, preferably from the iliac crest and skull region, and transplanted. Depending on the surgical need, the bone fragments removed "in one piece" are re-implanted in their original form as a whole or as any part of it, or after being crushed in a bone mill as granules. In practice, different types of bone granules are known for use in dental and plastic surgery as well as maxillofacial surgery, which are provided by so-called bone banks. A distinction is made between homologous granules that contain bones from other people, heterologous granules that contain bones from animals, and mixtures of these two types of granules. When using such granules, there is a risk - albeit a very small one - of transmitting pathogens that cause infectious diseases such as HIV, hepatitis, Jakob-Creuzfeld syndrome and BSE. The use of artificial bone replacement materials in their pure form, such as hydroxyapatite, is also problematic, as foreign body reactions often occur that prevent such materials from completely healing.
Zur-Minimierung der mit derartigen Im-Particularly in the dental field, it is often only necessary to implant very small amounts of bone, such as when filling bone pockets in the alveolar bone in the area of the tooth roots. To minimize the costs associated with such implants,
Given the risks associated with implantations, namely the risk of infection on the one hand and the risk of a foreign body reaction on the other, it is advisable to take the bone material from a suitable bone donor site from the patient on whom the implantation is to be carried out.
The invention is based on the task of creating a device that is suitable for removing small amounts of bone granulate with little equipment outlay. The device should therefore not only be used for removing - explanting - human bone, but also for crushing - transforming - and producing bone meal granulate for immediate or later re-implantation.
The attachment according to the invention for a drilling device solves the above problem by means of the features of claim 1. According to this, the attachment mentioned at the beginning has a receiving channel for the handpiece and a suction channel, wherein the receiving channel and the suction channel open into a suction bell that is open at the end on the drill side.
Der Aufsatz wirdAccording to the invention, it was first recognized that the drills known from the surgical or dental field are also suitable as an abrasive instrument for removing and crushing bone material. It has been recognized as advantageous in this context that such drilling devices are usually already available in practices and hospitals where bone material transplants are usually carried out. However, it has also been recognized that the known drilling device alone is not sufficient, since the removed and crushed bone material must also be collected. According to the invention, an attachment for a known drilling device is therefore proposed, with the aid of which the removed bone meal granulate can be sucked out. The attachment is
Dies stellt eine enorme Zeitersparnis dar, da das so gewonnene Knochenmehlgranulat sofort für eine Transplantation zur Verfügung steht.virtually slipped over the drill and attached to the handpiece of the drill. The attachment therefore includes a receiving channel for the handpiece. In addition, a suction channel is provided through which the dissolved bone meal granulate can be sucked out. According to the invention, it has been recognized that the bone meal granulate can be sucked out particularly efficiently if the bone donor site is shielded in a suitable manner. The front, drill-side area of the attachment according to the invention is therefore designed as a suction bell, so that the attachment is open like a bell on the drill side and can be positioned over the bone donor site. The processes of drilling, crushing, sucking and collecting therefore take place virtually simultaneously in a bell-like, enclosed space. This represents an enormous time saving, since the bone meal granulate obtained in this way is immediately available for transplantation.
In an advantageous design of the attachment according to the invention from a manufacturing point of view, the receiving channel and the suction channel are guided essentially parallel to the longitudinal axis of the attachment. A liquid is generally used to cool and also to transport the drilled-out bone meal granulate. It is therefore advantageous if a liquid supply is provided that leads directly into the suction bell. The liquid supply can be implemented, for example, in the form of a line that runs outside the handpiece of the drilling device. In this case, it is advantageous if the attachment has at least a third channel for this line as a liquid supply, which is also guided essentially parallel to the longitudinal axis of the attachment directly into the suction bell. Another possibility is to use internally cooled drills without a separate liquid supply. In this case, the liquid is supplied via the handpiece of the drilling device.

richtung in die Absaugglocke des erfindungsgemäßen Aufsatzes geführt.direction into the suction bell of the attachment according to the invention.
There are now various possibilities for designing the attachment according to the invention. In the simplest variant, the suction channel and the channel for supplying liquid are realized with a round cross-section. Depending on the manufacturing process of the attachment, however, other shapes of the cross-sectional area of these channels are also conceivable. It is also possible to design the suction channel and, if applicable, the channel for supplying liquid as a ring channel, whereby the suction channel would surround the receiving channel and, if applicable, the channel for supplying liquid.
Since dental and surgical instruments are only used in sterile conditions, the attachment according to the invention could advantageously be designed as a disposable part.
In addition to the attachment itself, a device for explanting and transforming bones is also proposed, with a drill, a handpiece for the drill, a drive unit for the drill and an attachment according to the invention, as described above. A particular advantage of such a device is that the penetration depth of the drill can be easily specified, namely by defining it by the protrusion of the drill from the suction cup of the attachment. By appropriately specifying the penetration depth of the drill into the bone, the type of bone to be removed - corticalis/spongiosa - can be determined. Corticalis contains a protein that induces bone growth and is therefore particularly suitable for transplantation purposes.
WesentlichIn principle, very different drills can be used in conjunction with the attachment according to the invention.
Als Antrieb für den Bohrer kann bspw. eine Luftturbine oder ein Micromotor dienen.is only that the receiving channel for the handpiece is dimensioned according to the drill and the handpiece. When using an internally cooled drill, as already mentioned, no separate liquid supply, i.e. no corresponding channel, is required in the attachment. An air turbine or a micromotor can be used to drive the drill, for example.
In an advantageous embodiment, the device for explanting and transforming bones comprises a vacuum hose system, which is connected to the suction channel of the attachment on the handpiece side. The liquid-bone particle mixture is sucked out during the drilling process via this vacuum hose system. In addition to the suction hose connected to the suction channel, such a vacuum hose system also comprises a suction device in the form of a vacuum pump. In an advantageous embodiment of the device according to the invention, this vacuum pump is preceded by a filter system or a centrifuge device with which the bone particles are separated from the liquid components of the sucked-out mixture. The separation can also take place in a sedimentation process.
Als Einsatzmöglichkeiten seien hier Verfahren in der zahnärztlichen Chirurgie und der Kieferchirurgie genannt, speziell in der Implan-It is important that the attachment according to the invention can be used in conjunction with known dental and surgical units that are part of the standard equipment in practices and hospitals, so that the equipment required for the explantation and transformation of bone is minimized. By dispensing with the use of homologous and heterologous bone granules from bone banks, the risks of unwanted, accidental disease transmission are eliminated. In principle, however, such bone materials can also be obtained using the attachment according to the invention. Possible applications include procedures in dental surgery and jaw surgery, especially in implantation.

Fig. 3 shows an overall functional diagram of a device for explanting and transforming bones with an attachment according to the invention.
Fig. 1 shows a drilling device 1 for explanting and transforming bones, which comprises a drill 2 and a handpiece 3 for the drill 2. Furthermore, an attachment 4 is shown - once independent of the drilling device 1 and once attached to the drilling device 1 or the handpiece 3.
According to the invention, the attachment 4 has a receiving channel 5 for the handpiece 3 and a suction channel 6. Both channels 5 and 6 open into a suction bell 7 formed at the drill-side end of the attachment 4.
Both the intake channel 5 and the suction channel 6 extend essentially parallel to the longitudinal axis through the attachment 4.
From the illustration of the drilling device 1 with the attached attachment 4, it is clear that the drill 2 protrudes from the attachment 4 or over the edge of the suction bell 7. This protrusion defines the penetration depth of the drill 2.
The attachment 4 shown in Fig. 1 has a third channel 8 which runs essentially parallel to the longitudinal axis of the attachment 4 and opens into the suction bell 7 and serves as a liquid supply directly into the suction bell 7. The drilling device 1 shown comprises a line 9 for a cooling medium which runs outside the handpiece 3. When the attachment 4 is placed on the drilling device 1, this line 9 is led through the channel 8 into the suction bell 7.
The arrangement and geometry of these channels 5, 6 and 8 is shown for the embodiment described here in Fig. 2, which shows a top view of the attachment 4 from the drill side, i.e. a view into the suction bell 7.
