Description
Die Erfindung betrifft ein Verfahren zur Herstellung von calciumoxidarmer
Spongiosa-Knochenkeramik.
Es ist seit längerem bekannt, daß sich mineralisierter und zur Keramik
gesinterter Knochen vorzüglich als Knochenersatzwerkstoff eignet. Bei
Knochenkeramik liegt naturgemäß größte Übereinstimmung von chemi
scher Zusammensetzung, strukturellem Aufbau und mechanischer Festig
keit mit dem natürlichen Knochen vor. Spongiosa-Knochenkeramik erweist
sich darüber hinaus als in besonderem Maße osteokonduktiv. Die offene,
interkonnektierende, trabekuläre Struktur von spongiöser Knochenkeramik
fördert das Auf- und Einwachsen neuer Knochenmatrix, so daß sich im
Verlaufe der Einheilung eine intensive Durchbauung und damit Integration
des Keramikimplantates ergibt. Knochenkeramiken hergestellt aus
Knochen tierischer Herkunft werden daher vermehrt als Knochenersatz
materialien bei der Osteosynthese und bei der Rekonstitution von
krankheits- oder unfallbedingten Knochendefekten eingesetzt.
Die Herstellung von Knochenkeramik aus tierischem Knochen erfolgt in aller
Regel in der Weise, daß man ausgewählte Knochen bzw. Knochenstücke
zunächst mechanisch von allen anhaftenden Weichteilen befreit, diese
anschließend durch Sägen zu Stücken geeigneter Form und Größe grob
zurichtet, und diese dann durch Entfernung aller weiteren organischen
Anteile mineralisiert. Der Mineralisierungsvorgang beginnt zunächst mit
mehrmaligem Auskochen in Wasser. Die weitere Behandlung kann etwa mit
fett- bzw. eiweißlösenden Lösungsmitteln und/oder mit Hilfe von Wasser
stoffperoxid erfolgen, wie beispielsweise in EP 0 141 004 beschrieben. Als
besonders einfach und effektiv haben sich pyrolytische Mineralisierungs
verfahren erwiesen. Hierbei wird durch Hitzeeinwirkung der organische
Anteil des Knochens zersetzt und der resultierende Kohlenstoff anschlie
ßend im Sauerstoffüberschuß ausgebrannt. Für die Knochenpyrolyse sind
Temperaturen zwischen 500 und 1000°C üblich. Nach der Mineralisierung
des Knochens erfolgt die Sinterung zur Keramik, wobei Temperaturen
zwischen 800 und 1400°C gebräuchlich sind. Durch die Sinterung erhält der
Werkstoff erst die gewünschte endgültige Festigkeit. Bei den genannten
Vorgängen ist besonders darauf zu achten, daß die poröse Struktur des
ursprünglichen Knochens soweit als möglich erhalten bleibt. Bei der Um
wandlung von spongiösem Knochenmaterial zu Spongiosa-Knochenkeramik
kann vorzugsweise nach einem Verfahren gemäß DE 37 27 606 vorge
gangen werden, in dem eine spezielle Führung von Temperatur und reduk
tivem bzw. oxidativem Charakter der Atmosphäre eine besonders schonende
Pyrolyse ermöglicht.
Es hat sich gezeigt,daß Knochenkeramik zu unkontrollierter Instabilität
neigt, wenn neben der Hauptphase Hydroxylapatit nennenswerte Anteile
an Calciumoxid vorliegen, welche etwa röntgenographisch nachgewiesen
werden können. Dies ist der Fall bei Gehalten an Calciumoxid von mehr
als 5%. Bei Zutritt von Wasser, etwa in Form von Luftfeuchtigkeit bei
ungeschützter Lagerung oder im Organismus nach der Implantation, tritt
eine sukzessive Umwandlung von Calciumoxid in Calciumhydroxid ein, mit
der eine erhebliche Volumenzunahme einhergeht. Je nach mengen
mäßigem Anteil kann es dabei bis hin zum vollständigen Zerfall der
Keramik kommen. Der Anteil an Calciumoxid in der Knochenkeramik ist
naturbedingt und kann nach Tierrasse und individueller Haltung
schwanken. Das Calciumoxid entsteht dadurch, daß bei Pyrolyse und
Sinterung das im Knochen natürlich vorhandene Calciumcarbonat in
Calciumoxid umgewandelt wird.
In DE 40 28 683 ist vorgeschlagen worden, das mineralisierte Knochen
material vor der Sinterung einer Behandlung mit einer wäßrigen Lösung
einer organischen Säure, insbesondere Zitronensäure, zu unterziehen.
Durch diesen Schritt gelingt es Calciumoxidanteile aus dem Material
herauszulösen, so daß man nach der Sinterung eine Knochenkeramik
erhält, die röntgenographisch zu mehr als 99% aus Hydroxylapatit
besteht.
Obwohl diese Vorgehensweise sehr effektiv ist und zu einer gebrauchs
fähigen stabilen Knochenkeramik führt, sind mit ihr doch Nachteile verbun
den. Nachteile liegen zum einen im operativen Bereich der Verfahrens
führung. So muß insbesondere bei pyrolytischer Mineralisierung das
Material zunächst wieder auf Raumtemperatur abgekühlt werden, um die
Säurebehandlung und die notwendigen Spülvorgänge durchführen zu
können. Danach muß zur Sinterung von neuem von Grund auf aufgeheizt
werden. Diese Vorgänge sind zeit- und energiezehrend. Darüber hinaus
verteuern Herstellung und Verbrauch der Säurelösung sowie der Ver
brauch an Spülwasser zum neutralwaschen des Materials die Herstell
kosten. Zum anderen gestaltet sich die Steuerung und Überwachung des
Schrittes der Säurebehandlung als nicht einfach. Einerseits muß hierbei
gewährleistet sein, daß praktisch alles Calciumoxid herausgelöst wird,
andererseits darf die Hydroxylapatit-Matrix nicht oder nur wenig durch die
Säure angegriffen werden. Die spongiöse Feinstruktur ist an diesem
Verfahrenspunkt noch nicht ausreichend verfestigt und daher sehr
empfindlich.
Der vorliegenden Erfindung lag daher die Aufgabe zugrunde,
die genannten Nachteile der bisherigen Verfahrensweise zu mindern.
Überraschend wurde nun gefunden, daß sich in mineralisierter Knochen
matrix enthaltene Calciumoxidanteile ohne weiteres entfernen lassen,
wenn man die Knochenmatrix einem Auswaschungsvorgang mit deminera
lisiertem Wasser bei einer Temperatur von 10 bis 80°C und über einen
Zeitraum von 4 Std. bis 7 Tage unterzieht. Das Herauslösen von Calcium
oxid mittels Säure, wie insbesondere mit wäßriger Zitronensäurelösung,
hat sich somit als nicht erforderlich erwiesen.
Gegenstand der Erfindung ist somit ein Verfahren zur Herstellung von
calciumoxidarmer Spongiosa-Knochenkeramik, bei dem man in Stücken
zugerichteten spongiösen Knochen durch Entfernung aller organischen
Anteile mineralisiert und dann die mineralische Knochenmatrix zur
Keramik sintert, das dadurch gekennzeichnet ist, daß man zur Entfernung
von Calciumoxidanteilen aus der mineralisierten Knochenmatrix diese
einer Auswaschung mit demineralisiertem Wasser bei einer Temperatur
von 10 bis 80°C über einen Zeitraum von 4 Std. bis 7 Tage unterzieht.
Das erfindungsgemäße Verfahren wird praktisch in völliger Analogie zu
dem aus DE 40 28 683 bekannten durchgeführt. Es wird hier lediglich der
zwischen der Mineralisierung des Knochenmaterials und der Sinterung zur
Keramik vorgesehene Schritt der Säurebehandlung durch eine Behand
lung mit demineralisiertem Wasser ersetzt. Demineralisiertes Wasser ist
als solches bekannt und ohne weiteres verfügbar. Es läßt sich nach
allgemein bekannten Verfahren in beliebiger Menge aus natürlichem
Wasser herstellen, beispielsweise durch ein- oder mehrfache Destillation
und/oder Entzug der gelösten mineralischen Salze mittels Ionenaus
tauschern. Demineralisiertes Wasser zeigt in aller Regel einen leicht
sauren pH-Wert, der bei etwa 6,0 liegt. Es wird angenommen, daß dieser
leicht saure Charakter des demineralisierten Wassers die Auswaschung
von Calciumoxid aus der mineralisierten Knochenmatrix begünstigt.
Für den Auswaschungsvorgang genügt es, das stückig zugerichtete,
mineralisierte Knochenmaterial in ein entsprechend dimensioniertes Bad
von demineralisiertem Wasser zu verbringen und dort für eine als aus
reichend und zweckmäßig erscheinende Zeit und bei entsprechender
Temperatur zu belassen. Eine Umwälzung bzw. Bewegung des Wasser
bades kann der Herauslösung der löslichen Bestandteile förderlich sein.
Als ausreichend und zweckmäßig haben sich ein Verhältnis von 10 l
Wasser pro 1 kg Knochenmaterial und eine Behandlungsdauer von 4 Std.
bis 7 Tage bei einer Badtemperatur zwischen 10 und 80 °C erwiesen. Ein
ein- oder mehrfacher Austausch des Waschwassers bzw. eine kontinuier
liche Durchspülung mit frischem demineralisierten Wasser begünstigt das
Ergebnis und kann die Behandlungszeit verkürzen. Durch die angegebene
Vorgehensweise läßt sich praktisch der gesamte in der Knochenmatrix
vorhandene bzw. erwartbare Anteil an Calciumoxid im wesentlichen rest
los entfernen. In aller Regel bewegt sich der natürliche Anteil an Calcium
oxid im Knochen zwischen 5 und 10%. Durch röntgenographische Unter
suchung an der zum Schluß erhaltenen Keramik läßt sich feststellen, daß
das Material zu mehr als 95% und in aller Regel zu 97-99% aus Hydro
xylapatit besteht. Calciumoxid läßt sich, falls überhaupt, nur noch in
Spuren nachweisen.
Durch das erfindungsgemäße Verfahren lassen sich nicht unerhebliche
Mengen an bislang für ein vergleichbares Ergebnis benötigter Säure
einsparen. Darüber hinaus fallen vor allem Nachspülvorgänge, um das
Material neutral zu waschen, weg, so daß auch hierdurch Kosten und Zeit
gespart werden können. Weiterhin ist das erfindungsgemäße Verfahren
wesentlich schonender, da die Hydroxylapatit-Matrix durch die Behand
lung mit demineralisiertem Wasser nicht angegriffen wird.
Nach Abschluß des Auswaschvorganges werden die Stücke getrocknet
und dann in üblicher Weise zur Keramik gesintert.
Rohe, von Weichteilen befreite Gelenkköpfe frisch geschlachteter Rinder
werden mittels Säge in quaderförmige Stücke der Abmessungen von grob
30 × 30 × 100 mm zerteilt und diese dreimal ca. 1 Std. lang mit Wasser
ausgekocht.
Die Knochenstücke werden dann 6 Std. lang bei 110°C getrocknet.
Danach werden diese unter Stickstoffatmosphäre innerhalb von 9 Std. auf
450°C erhitzt. Während einer anschließenden 20stündigen Aufheizperio
de von 450°C auf 600°C wird die Atmosphäre linear auf Luftsauerstoff
umgestellt und danach innerhalb von 5 Std. auf 900°C erhitzt. Nach dem
Abkühlen werden die Stücke in ein Bad von demineralisiertem Wasser
(10 l pro 1 kg Knochenmaterial) verbracht und in dem bewegten Bad bei
einer Temperatur von 20°C für einen Zeitraum von 48 Std. behandelt.
Nach dieser Behandlung werden die Stücke 3 mal mit demineralisiertem
Wasser nachgewaschen.
Zur Endsinterung werden die Stücke innerhalb von 21 Std. auf 1250°C
erhitzt, 3 Std. auf dieser Temperatur gehalten und dann frei abgekühlt.
Die erhaltenen Knochenkeramikstücke zeigen die unveränderte poröse
Struktur des ursprünglichen spongiösen Knochens. Röntgenographisch
besteht die Keramik zu etwa 99% aus Hydroxylapatit.

- Verfahren zur Herstellung von calciumoxidarmer Spongiosa-Knochen keramik, bei dem man in Stücken zugerichteten spongiösen Knochen durch Entfernung aller organischen Anteile mineralisiert und dann die mineralisierte Knochenmatrix zur Keramik sintert, dadurch gekenn zeichnet, daß man zur Entfernung von Calciumoxidanteilen aus der mineralisierten Knochenmatrix, diese einer Auswaschung mit deminerali siertem Wasser bei Temperaturen von 10 bis 80°C über einen Zeitraum von 4 Std. bis 7 Tage unterzieht.
