Abstract
Description
Gegenstand der vorliegenden Erfindung ist eine neue Formulierung für Prostaglandin E1.The present invention relates to a new formulation for prostaglandin E 1 .
Prostaglandin E1(= PGE1) ist ein bekannter Wirkstoff mit u. a. vasodi latierender Wirkung. Da der Wirkstoff nicht sehr stabil ist, muß er stabilisiert werden.Prostaglandin E 1 (= PGE 1 ) is a well-known active ingredient with a vasodilating effect. US 43 70 349 sowie US 42 29 360). Das Lyophilisat muß dann kurz vor der Verwendung mit Wasser rehydratisiert werden. Lyophilisationsverfahren sind allerdings aufwendig und koste nintensiv, insbesondere bei der Herstellung von Arzneimitteln, die für eine parenterale Applikation vorgesehen sind (Keimfreiheit).The liposomal formulation of hydrolysis-sensitive active ingredients, such as. Vereinzelt wurde auch, insbesondere bei Verwendung synthetischer Lipide zur Bildung der Liposomen von Präparationen berichtet, die über lange Zeiträume bei Raumtemperatur stabil sind. Die Lagerstabilität von Liposomenlösungen ist daher oft, insbesondere bei hydrolyseempfindlichen Stoffen wie PGE, oder Peptiden/Proteinen einzig durch den Wirkstoff limitiert: Emulsionen, die für parenterale Applikationen vorgesehen sind, müssen nach der Herstellung autoklaviert werden (Keimfreiheit) PGE1-haltige Emulsionen verlieren dabei allerdings bekanntermaßen einen deutlichen Anteil des Wirkstoffs infolge der hohen Temperaturbelastung (110 bis 120°C). Es wäre daher zur Erreichung maximaler Lagerstabilität wün schenswert, den Wirkstoff erst kurz vor der Applikation der Liposomen lösungen oder der Emulsion zuzusetzen, um ihn so kurz wie möglich dem wäßrigen Milieu auszusetzen. Große Anstrengungen sind unternommen worden, um Verfahren zu finden, Wirkstoffe in den Innenraum von Liposomen zu verkapseln. Bei die sem Verfahren wird die wirkstoffhaltige Liposomenlösung mit Hilfe ei ner sauren Pufferlösung von pH 7 auf pH 4 acidifiziert, durch den da durch aufgebauten Ionengradienten erfolgt eine effiziente Verkapselung des PGE1 in den Liposomen. Diese Verkapselung bleibt aber nur aufrecht erhalten, wenn der pH-Wert der Liposomenlösung im Sauren belassen wird, da letztendlich nur der pH-Gradient (Innenraum der Liposomen pH 7, äußere Wasserphase pH 4) die Verkapselung bewirkt. Wird der pH Wert wieder neutral gestellt (pH 7), fließt der Wirkstoff wieder aus den Liposomen heraus. Genau dies geschieht bei der Verabreichung der artiger Präparationen, beispielsweise bei der intravenösen Applikation durch die Verdünnung mit Blut (pH des Vollblutes ca. 7,2-7,4). Es ist allerdings theoretisch auch möglich, den transmembranen pH-Gradienten durch Acidifizieren einer bei alkalischen pH-Werten (z. B. 9-10) herge stellten Liposomlösung auch auf pH 7 herzustellen. Damit wäre die Ver kapselungsstabilität auch im biologischen Milieu (z. B. Vollblut) ge währleistet. Allerdings sind bekanntermaßen weder die eingesetzten Liposomen bildenden Hilfsstoffe (z. B. Phospholipide) noch PGE, bei derart alkalischen pH-Werten stabil. Diese Methode ist daher nicht an wendbar. Durch Einbau des Wirkstoffs in Lipidformulierungen konnte die Wirksamkeit deutlich verbessert werden (siehe EP 1 50 732). Diese Voraussetzung ist bereits durch einfaches Mischen des Wirkstoffs mit einer wäßrigen Lipidformu lierung (z. B. Der erste Teil besteht aus der "lee ren" Lipidformulierung (Liposomenlösung bzw. Emulsion), der zweite Teil aus dem Wirkstoff, z. B. in pulverisierter Form oder in Form einer konzentrierten Lösung in z. B. Ethanol.
