Description
Die Erfindung bezieht sich auf eine Hüftgelenkprothese mit einer Gelcnkkugel und einer Gelenkpfanne.
Seit dem Jahre 1948 sind Prothesen für den Ersatz des Hüftgelenks bekannt. Vorerst wurde nur die
Kugel am Schenkelhals ersetzt, später folgten dann beide Hüftgelenksteile, d.h. Pfanne im Becken und
Kugel am Schenkelhals. Operationen, die nur den Ersatz eines Gelenkteiles verlangen, können mit den
bisherigen Materialien und Prothesenkonstruktionen zur Zufriedenheit durchgeführt werden. Wenn aber
aus pathologischen oder unfallbedingten Gründen beide Gelenkteile ersetzt wreden müssen, entstehen
neue Probleme, welche bis heute noch nicht zur Zufriedenheit gelöst werden konnten.
Durch die großen Krafteinwirkungen, die bei der Belastung der Gelenke auftreten, werden die ursprünglichen
Gelenkflächen der eingesetzten Prothesen deformiert oder neigen hauptsächlich infolge der
gleitenden Reibung zum Anfressen. Die so entstehenden schlechten Gelenk- bzw. Reibungseigenschaften
verursachen verschlechterte Beweglichkeit sowie Schmelzen. Außerdem gelangen die entstehenden
Abriebnradukte in das benachbarte Gewebe und können zu entsprechenden Reaktionen führen. Durch
Verwendung unterschiedlicher Materialien für Pfanne und Kopf konnten zum Teil bessere Resultate
erzielt werden. Dabei werden für die Pfanne Kunststoffe und für die Kugel Metalle verwendet. Um die
Gleitreibung weiter zu verringern, wurde eine Metallpfanne mit einer 3-Punkt-Auflage aus Kunststoff
(schweizerische Patentschrift 449 173) versehen. Die drei Kunststoff auflagen sind jedoch der großen spezi-
fischen Belastung auf längere Zeit nicht gewachsen, und die Folge ist wieder Reibung von Metall auf Metall.
Aufgabe der Erfindung ist es, eine Prothese für den Ersatz des Hüftgelenks zu schaffen, bei der die
Reibung zwiochen der künstlichen Gelenkpfanne und
der künstlichen Gelenkkugel des Schenkelhalses auf einen Geringstwert gebracht wird und keine Abriebprodukte
entstehen.
Die Erfindung löst diese Aufgabe bei einer Hüft-
gelenkprothese mit einer Gelenkkugel und einer Gelenkpfanne dadurch, daß zwischen Gelenkkugel und
Gelenkpfanne Kugeln als Wälzkörper vorgesehen sind, die fast den ganzen Raum zwischen Gelenkpfanne
und Gelenkkugel ausfüllen, und daß der
as Raum zwischen Gelenkpfanne und Gelenkkugel auch
zur Verhinderung des Austrittes von Abriebprodukten oder Schmiermitteln seitlich abgedichtet ist. Der
erreichte Vorteil besteht darin, daß die Lebensdauer des künstlichen Hüftgelenks auf ein Mehrfaches der
bekannten Ausführungsformen verlängert wird, so daß auch jüngere Menschen die neue Prothese erhalten
können, ohne daß diese erneuert werden muß.
Weitere Einzelheiten ergeben sich aus der Beschreibung einer beispielsweisen Ausführungsfonn
der Erfindung an Hand der Zeichnung, die einen Mittenschnitt durch die Hüftgelenkprothese zeigt.
Eine auf einen im Femurschaft verankerten Stiel steckbare Gelenkkugell sitzt in einer Gelenkpfanne
2, wobei diese an ihrer offenen Seite von
einem ringförmigen Deckel 3 begrenzt und die Gclenkkugel
gehalten wird. Der Raum zwischen Gelenkpfanne und Gelenkkugel ist mit einer ausreichenden
Anzahl kleiner Kügelchen 4 ausgefüllt. Der ringförmige Deckel ist mittels einem Gewinde mit der
Gelenkpfanne verbunden und weist eine Kugel- und eine Pfannendichtung in Form je eines Ringes 5 und
6 auf. Das Gelenk ist auf Lebenszeit geschmiert, z. B. mit Fett oder mit Teflonpulver. Die beiden Ringe bestehen
aus einem körperverträglichen, alterungsbcständigen und chemisch resistenten Material, wie
z.B. Silikonkautschuk. Der Querschnitt muß aber nicht unbedingt kreisförmig sein, sondern kann jede
andere zweckmäßige Form erhalten. Die Gelenkpfanne kann mit unterschiedlicher Wandstärke ausgeführt
werden, um dadurch die Lastverteilung im Lager beeinflussen zu können. Bei einer beispielsweisen
Ausführungsform nimmt die Wandstärke der Gelenkpfanne 2 vom Scheitelpunkt bis zum Rand kontinuierlich
ab.
Als Materialien können vorzugsweise für Gelenkpfanne und -kugel korrosionsbeständige und körperverträgliche
Cr-Co-Mo-Legierungen verwendet werden. Vorteilhafterweise können die Lagerkügelchen
aus einem festeren Material als Pfanne und Kugel bestehen.
Als wesentlicher Vorteil dieser Gelenkprothese ist anzusehen, daß sie in zwei Teilen eingebaut werden
kann: der fertige Gelenkteil wird im Becken und der
Stiel für die Kugel (nicht gezeichnet) wird im Femurschaft
eingebaut und mittels eines Konus od. dgl. mit ihr zusammengebaut. Außerdem ist ein sehr feines
Justieren der Beweglichkeit des Gelenkes mittels _
des mit einem Gewinde versehenen Deckelringes 3 5 prothesen notwendig ist

Claims (6)
1. Hüftgelenkprothese mit einer Gelenkkugel und einer Gelenkpfanne, dadurch gekennzeichnet,
daß zwischen Gelenkkugel (1) und Gelenkpfanne (2) Kugeln (4) als Wälzkörper vorgesehen
sind, die fast den ganzen Raum zwischen Gelenkpfanne und Gelenkkugel ausfüllen, und
daß der Raum zwischen Gelenkpfanne und Gelenkkugel auch zur Verhinderung des Austrittes
von Abriebprodukten oder Schmiermitteln seitlich abgedichtet ist.
2. Hüftgelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß die Abdichtung durch
zwei Ringe (5 und 6) aus Silikonkautschuk bewirkt wird.
3. Hüftgelenkprothese nach Anspruch 2, dadurch gekennzeichnet, daß die beiden Abdichtringe
(5 und 6) in einen Metallring (3) eingesetzt sind, welcher auf die Pfanne aufschraubbar ist
und zur Halterung der Gelenkkugel sowie zur Begrenzung der seitlichen Bewegung der Kugel
dient.
4. Hüftgelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß die Gelenkpfanne eine
vom Scheitel bis zu ihrem Rand sich kontinuierlich vermindernde Wandstärke aufweist.
5. Hüftgelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß Gelenkpfanne und
Gelenkkugel aus einer Cr-Co-Mo-Legierung bestehen.
6. Hüftgelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß die Walzkörper (4)
aus einem härteren Material als Gelenkpfanne (2) und Gelenkkugel (1) bestehen.
