Zusammenfassung:
Endoprothesen werden derzeit zum Beispiel im Bereich der Gefäße (arteriell und venös), der Gallenwege, der Luftwege sowie des Magen-Darm-Traktes eingesetzt. Die unterschiedlichen Indikationen haben zur Entwicklung verschiedener Typen von Endoprothesen hinsichtlich ihres Designs und ihrer Form geführt. Des weiteren werden die Endprothesen aus verschiedensten Materialien hergestellt. Insbesondere für die intravasale Anwendung von Endoprothesen - sogenannten Stents - hat sich bisher Metall als das am besten geeignete Material erwiesen. Dementsprechend werden die überwiegende Mehrzahl von Stents aus Metalllegierungen wie Edelstahl oder Nitinol hergestellt. Sind Endoprothesen aus nicht ferromagnetischen Metalllegierungen hergestellt, so können Patienten prinzipiell nach Plazierung von Endoprothesen auch in starken Magnetfeldern untersucht werden, da eine Bewegung der Endoprothesen durch Positionierung im MacTietfeld zum Beispiel eines Magnetresonanz-(MR)-tomographen nicht zu befürchten ist 1' 2. Diese Eigenschaft wird heute gerne als MR-Kompatibilität genauer als MR-Kompatibilität der ersten Art - bezeichnet 3. Hierbei wird allerdings nur die fehlende Gefährdung des Patienten berücksichtigt. Bisher verwandte Endoprothesen aus Metall, erzeugen Artefakte im Macfnetresonanztomographie-Bild, die insbesondere eine Beurteilung des Lumens einer Endoprothese mit Hilfe der Magnetresonanztomographie nicht erlauben 4' 5. Dies ist sowohl durch die unterschiedliche Magnetisierbarkeit der verwandten Metalllegierungen im Vergleich zu menschlichem Gewebe 6 als auch durch Wirbelströme beziehungsweise Radiofrequenzeffekte bedingt 7. Das hier vorgestellt Prinzip erlaubt es, Endoprothesen aus speziellen Metalllegierungen herzustellen, ohne dass diese Endoprothesen zu wesentlichen Bildartefakten im Rahmen der Magnetresonanztomographie führen. Zusammen mit den fehlenden ferromagnetischen Eigenschaften können diese Endoprothesen als „voll MR-kompatibel" bezeichnet werden, was der wissenschaftlich beschriebenen Anforderung der MR-Kompatibilität der zweiten Art entspricht 3. Im Gegensatz zu Ansätzen für MR-kompatible Stents dient bei dem hier vorgestellten Prinzip die Endoprothese nicht der aktiven Bildgebung 8 sondern stört aufgrund ihrer fehlenden Erzeugung von Artefakten das mit Standardtechniken aufgenommene Magnetresonanztomographie-Bild nicht. Während dieser Effekt mit Hilfe von nicht-metallischen Endoprothesen wahrscheinlich erzielt werden kann, so -zielt das vorliegende Prinzip auf metallische Endoprothesen ab, um so die guten Eigenschaften von Metallen und Metalllegierungen für Stents bei voller MR-Kompatibilität ausnutzen zu können.
Wirkungsweise:
Bei der Bilderzeugung in der Kernspintomographie werden Magnetfelder von 0.3 bis 2 Teslar benötigt. Gold und Kupfer weisen in einem starken Magnetfeld eine sehr niedrige, dem menschlichen Gewebe ähnliche Magnetisierbarkeit auf. Ma.gnetisierbare Gegenstände führen dagegen zu starken Bildauslöschungen. Gefäßendoprothesen aus dem hier beschriebene Implantatmaterial und zylindrischer Drahtstruktur können aufgrund der Materialeigenschaften in der Kernspintomographie eingesetzt werden, ohne dass störende Artefakte erzeugt W€;rden. . .
Insbesondere sind Implantate, mit diesen Eigenschaften geeignet für den Einsatz in menschlichen Gefäßen, Gefäßbypässen, Ureteren, intrahepatischen
Bypässen sowie für den Einsatz in sonstigen Hohlorganen. Bisher werden alle Verfahren zur Einbringung eines Implantates in ein Gefäß unter Durchleuchtung mit Röntgenstrahlen durchgeführt.
Hinsichtlich der Belastung der Patienten durch die ionisierende Strahlung und ihre Auswirkung haben in letzter Zeit jedoch Berichte über Hautverbrennungen nach durchleuchtungsgesteuerten Interventionen Aufsehen erregt. Ein Ersatz der Rönzgenverfahren durch kernspintomographische Verfahren ist wegen der fehlenden Strahlenbelastung der Kernspintomographie daher wünschenswert. Bisher ist der Ersatz der Röntgendurchleuchtungsverfahren durch die Kemspintomographie für Interventionen und Kontrolldarstellungen jedoch nicht möglich, da die heute allgemein verwendeten Implantate aus Edelstahl bzw. Nitinol im Magnetfeld kurzfristig magnetisiert werden und in der Kernspintomographie Artefakte in Form von Bildauslöschungen erzeugen, die ein sicheres Auswerten der Bilder fast unmöglich machen. Die hier beschriebenen Implantate aus einer Gold-Kupfer-Legierung weisen dagegen so gut wie keine Unterschiede der Magnetisierbarkeit zu menschlichem Gewebe im Magnetfeld auf und reduzieren Artefakte daher auf ein Minimum. Dadurch werden eine Implantation dieser Implantate und Kontrolluntersuchungen in der Kernspintomographie möglich.
Die gewählte Legierung besitzt außerdem eine höhere Zugfestigkeit als herkömmliches Gold bzw. Kupfer, was für den Einsatz unentbehrlich ist.
Beschreibung der Erfindung:
Es handelt sich um eine Gefäßendoprothese aus einem magnetresonanzkompatiblen Implantatmaterial aus einer speziellen goldkupferhaltigen Legierung, die sich dadurch auszeichnet, dass aufgrund von ähnlichen Magnetisierungseigenschaften wie menschliches Gewebe bei der Erzeugung von kernspintomographischen Aufnahmen (Magnetresonanz) keine wesentlichen Artefakte erzeugt werden und eine derartige Gefäßendoprothese geeignet ist für den Einsatz als Stent bzw. intraluminale Prothese.
Zur Reduzierung der Leitfähigkeit und dadurch Minimierung der Induktionsströme wird die Gefäßendoprothese nicht aus einem Rohr oder Flachblech gelasert, sondern besteht aus einer zylinderförmigen Drahtstruktur, welche aus einem oder mehreren Drähten gewickelt, gewebt bzw. geflochten ist. Um die Induktivität weiter zu reduzieren, können der Draht oder die Drähte noch zusätzlich durch Umhüllungen oder Beschichtungen bedeckt bzw. isoliert werden.
Außerdem können Medikamente zur Verbesserung der Biokompatibilität entweder unmittelbar auf Draht oder Drähte oder auf die umhüllten bzw. beschichteten Drähte aufgebracht werden.
