Abstract

Werkzeug zum Entfernen konisch geklemmter Keramik-Pfanneneinsätze (1) aus der Metallschale (4) einer Hüftgelenkendoprothese, mit verspannbaren Elementen, die von einer axial bewegbaren Spannvorrichtung radial nach außen bewegbar und dadurch mit dem Pfanneneinsatz (1) verspannbar sind, dadurch gekennzeichnet, dass die Spannvorrichtung gegeneinander verspannbare und damit radial ausdehnbare Tellerfedern (2) aufweist, durch die ein Gewindestab (3) hindurchgeht, und dass der Gewindestab (3) drehbar in einer Gewindebohrung (7) einer Positionierplatte (5) sitzt und eine Auskragung (6) zum Abstützen der Tellerfedern (2) aufweist.

Description

  • Die Erfindung betrifft ein Werkzeug zum Entfernen konisch geklemmter Keramik-Pfanneneinsätze aus der Metallschale einer Hüftgelenkendoprothese.
  • Hüftgelenkendoprothesen bestehen aus einem Prothesenschaft, der im Oberschenkelknochen verankert ist und einem auf den Prothesenschaft aufgesetzten Kugelkopf. Der Kugelkopf ist in einem Pfanneneinsatz gelagert, der wiederum in einer Metallschale eingesetzt ist, die direkt oder über eine Außenschale im Beckenknochen verankert ist.
  • Die Fixation von Keramik-Pfanneneinsätzen in einer Metallschale einer Hüftgelenkendoprothese mit Hilfe einer konischen Klemmung ist mittlerweile bewährter Stand der Technik. Dabei ist die Extraktionstechnik zum intra- oder postoperativen Extrahieren eines einmal fixierten Pfanneneinsatzes bisher noch unbefriedigend gelöst. Das zerstörungsfreie Entfernen des Pfanneneinsatzes mit Hilfe von Greifwerkzeugen erfordert einen unvertretbar hohen konstruktiven Aufwand, während die Entfernung durch Zerstören des Pfanneneinsatzes durch die auftretenden Keramiksplitter bzw. toxischen Schleifschlämme eine medizinisch nicht akzeptable Lösung darstellt.
  • Ein Werkzeug, von dem der Oberbegriff des Patentanspruch 1 ausgeht, ist beschrieben in DE 44 29 026 A1 . Dieses Werkzeug weist eine Spannvorrichtung aus mehreren Segmenten auf, die von einem runden Keilstück auseinander getrieben werden. Die Segmente graben sich mit radial abstehenden Schneiden in den Keramikeinsatz der Gelenkprothese ein. Das Entfernen des Pfanneneinsatzes aus der Metallschale erfolgt durch Hammerschläge, die in Rückwärtsrichtung auf ein Hammerteil ausgeübt werden, welches an der Spannvorrichtung angebracht ist.

  • Der Erfindung liegt die Aufgabe zugrunde, ein Werkzeug zum Entfernen konisch geklemmter Keramik-Pfanneneinsätze aus der Metallschale einer Hüftgelenkendoprothese zu schaffen, das keine segmentierte Spannvorrichtung benötigt.
  • Das erfindungsgemäße Werkzeug ist durch den Patentanspruch 1 definiert. Hiernach ist vorgesehen, dass die Spannvorrichtung gegeneinander verspannbare und damit radial ausdehnbare Tellerfedern aufweist, durch die ein Gewindestab drehbar hindurchgeht, und dass der Gewindestab in einer Gewindebohrung einer Positionierplatte sitzt und eine Auskragung zum Abstützen der Tellerfedern aufweist.
  • Gemäß einer Weiterbildung der Erfindung ist vorgesehen, dass die Positionierplatte die Tellerfedern in radialer Richtung überragt und sich am Ende des Pfanneneinsatzes abstützen kann. Durch die Abstützung der Positionierplatte an dem Pfanneineinsatz wird die Positionierplatte gegen Drehung gesichert. Außerdem wird sichergestellt, dass die Tellerfedern in der vorgesehenen Höhe am Pfanneneinsatz angreifen.
  • Ein Vorteil der Erfindung besteht darin, dass das Werkzeug unabhängig vom verwendeten Prothesensystem und nur abhängig vom Innendurchmesser des Pfanneneinsatzes universell einsetzbar ist. Der Klemmmechanismus besteht aus zwei Tellerfedern, deren Außenkontur der Innenform des Pfanneneinsatzes an der Klemmfläche entspricht. Durch den Gewindestab werden die Tellerfedern so gegeneinander verspannt, dass sie in der Kalotte des Pfanneneinsatzes verspannt sind. Nach der Fixation des Werkzeuges im Inneren des Pfanneneinsatzes kann durch einen Schlagimpuls in Ausziehrichtung der Pfanneneinsatz gelockert und aus der Metallschale entnommen werden.
  • In bevorzugter Ausführungsform entspricht die Außenkontur der Tellerfedern der Innenform des Pfanneneinsatzes, wodurch sich die Tellerfedern leichter verspannen lassen. Dies wird noch verbessert, wenn die Außenkontur der Tellerfedern mit einer Diamantkörnung besetzt ist. Die Diamantkörnung kann z.B. durch einen galvanischen Prozess aufgebracht werden.

  • Weitere Merkmale der Erfindung ergeben sich aus der Zeichnung, die nachfolgend beschrieben wird.
  • Mit dem Bezugszeichen 4 ist eine Metallschale gezeigt, die direkt oder über eine äußere Schale im Beckenknochen verankert ist. In dieser Metallschale ist über eine konische Klemmverbindung 8 ein Keramik-Pfanneneinsatz 1 eingesetzt. Durch die konische Klemmverbindung ist der Pfanneneinsatz 1 fest in der Metallschale 4 verankert.
  • Muss z.B. in einer Operation der Pfanneneinsatz 1 ausgewechselt werden, so wird der Oberschenkelknochen mit dem implantierten Prothesenschaft und dem aufgesetzten Kugelkopf aus dem Pfanneneinsatz 1 gezogen, so dass der Pfanneneinsatz 1 freiliegt, wie in der Figur gezeigt. Anschließend wird das erfindungsgemäße Werkzeug aufgesetzt.
  • Das Werkzeug besteht aus einer Positionierplatte 5, die auf den Pfanneneinsatz 1 aufgelegt wird. Mittig in der Positionierplatte 5 ist eine Gewindebohrung 7 angeordnet, in der ein Gewindestab 3 durch Drehen vertikal verschiebbar ist. Dieser Gewindestab 3 hat an seinem Ende einen Drehgriff (nicht gezeigt) und an seinem in den Pfanneneinsatz ragenden Ende eine Auskragung 6. Zwischen der Positionierplatte 5 und der Auskragung 6 sind zwei gegeneinander angeordnete Tellerfedern 2 eingesetzt. Wird der Gewindestab 3 verdreht, so werden die Tellerfedern gespannt oder entspannt, je nachdem, ob der Abstand zwischen Auskragung 6 und der Positionierplatte 5 verkleinert oder vergrößert wird. Werden die Tellerfedern 2 gespannt, so vergrößert sich die radiale Ausdehnung der Tellerfedern 2. Hierdurch lassen sich die Tellerfedern 2 im Pfanneineinsatz 1 bzw. in der Kalotte verspannen und damit fixieren.
  • Zur besseren Fixation entspricht die Außenkontur der Tellerfedern 2 der Innenform des Pfanneneinsatzes 1. Außerdem ist die Außenkontur der Tellerfedern mit einer Diamantkörnung versehen.
  • Nach der Fixation des Werkzeugs im Inneren des Pfanneneinsatzes kann durch einen Schlagimpuls in Ausziehrichtung der Pfanneneinsatz 1 gelockert und aus der Metallschale 4 entnommen werden. Der Schlagimpulse kann z.B. durch einen Schlag auf den Drehgriff in Ausziehrichtung erfolgen.

Claims (4)

  1. Werkzeug zum Entfernen konisch geklemmter Keramik-Pfanneneinsätze (1) aus der Metallschale (4) einer Hüftgelenkendoprothese, mit verspannbaren Elementen, die von einer axial bewegbaren Spannvorrichtung radial nach außen bewegbar und dadurch mit dem Pfanneneinsatz (1) verspannbar sind, dadurch gekennzeichnet, dass die Spannvorrichtung gegeneinander verspannbare und damit radial ausdehnbare Tellerfedern (2) aufweist, durch die ein Gewindestab (3) hindurchgeht, und dass der Gewindestab (3) drehbar in einer Gewindebohrung (7) einer Positionierplatte (5) sitzt und eine Auskragung (6) zum Abstützen der Tellerfedern (2) aufweist.
  2. Werkzeug nach Anspruch 1, dadurch gekennzeichnet, dass die . Positionierplatte (5) die Tellerfedern (2) in radialer Richtung überragt und sich am Ende des Pfanneneinsatzes (1) abstützen kann.
  3. Werkzeug nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Außenkontur der Tellerfedern (2) der Innenform des Pfanneneinsatzes (1) entspricht.
  4. Werkzeug nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die Außenkontur der Tellerfedern (2) mit einer Diamantkörnung besetzt ist.
