Description

Die Erfindung betrifft eine Hüftgelenkendoprothese nach dem Oberbegriff des An­ spruchs 1.
Stand der Technik ist der modulare Aufbau einer Hüftgelenkendoprothese mit einem Prothesenschaft, der im Oberschenkelknochen implantiert ist. Auf dem Prothesen­ schaft ist eine Kugel befestigt, die in einem Pfanneneinsatz gelagert ist. Aus der EP 0 856 199 A1, EP 0 649 641 A1 oder DE 44 02 675 A1 sind Konstruktionen bekannt, bei denen konisch geformte Pfanneneinsätze in Metallschalen eingepaßt sind, wobei die konische Klemmung ausgenutzt wird, um eine sichere Fixation des Pfanneneinsatzes in der Metallschale zu gewährleisten. Auch sind Konstruktionen bekannt, die durch sphärische Außenkonturen eine Verringerung der Baugröße des Pfanneneinsatzes gestatten.
Aufgrund der Problematik Osteolyse ist bei Hüftgelenkprothesen die Paarung PE (Polyethylen) des Pfanneneinsatzes gegen Keramik oder Metall der Schale umstritten. Es wird über alternative Paarungen im Bereich hart/hart diskutiert.
Die Anmelderin bietet hierzu seit vielen Jahren die Paarung Keramik/Keramik an, d. h. keramischer Kugelkopf und keramischer Pfanneneinsatz. Im Laufe dieser Zeit wur­ de von Keramikpfannen auf Keramikpfanneneinsätze umgestellt, wodurch kleinere Metallschalen mit dieser Gleitpaarung realisiert werden konnten. In den letzten Jahren wurden die Pfanneneinsätze mittels konischer Klemmung im Metallgehäuse verankert.
Durch diese konische Klemmung sind die keramischen Pfanneneinsätze unter Bela­ stungssituationen unterschiedlichen Spannungsfeldern (Zug/Druck) ausgesetzt. Da­ durch können Pfanneneinsätze aufgrund der notwendigen Bauteilsicherheit nur bis zu einer gewissen Wandstärke hergestellt werden.
Der Erfindung liegt die Aufgabe zugrunde, durch optimale Kontaktflächen der Pfan­ neneinsätze mit der Schale, die Spannungen zu minimieren und dadurch auch klein­ ste Größen von Hüftgelenkendoprothesen mit der Paarung hart/hart auszustatten.
Erfindungsgemäß wird diese Aufgabe dadurch gelöst,
  • - daß die Metallschale aus einem Oberteil und einem Unterteil besteht, wobei zwi­ schen dem Oberteil und dem Unterteil der Pfanneneinsatz eingebaut ist und
  • - daß der Pfanneneinsatz mit mindestens zwei Flächen vollflächig an der Innenseite der Metallschale anliegt.
In bevorzugter Ausführungsform liegen die gesamten Außenflächen des Pfannenein­ satzes an der Innenseite der Metallschale an.
Mit all diesen Maßnahmen sind Spannungen bzw. Spannungsspitzen im keramischen Pfanneneinsatz vermieden, so daß auch kleinste Größen der Metallschalen mit kera­ mischen Pfanneneinsätzen bestückt werden können. Die geteilte Metallschale um­ schließt den Pfanneneinsatz bis auf die Gleitfläche, so daß der Pfanneneinsatz rund­ herum geschützt ist.
Weitere Merkmale der Erfindung ergeben sich aus den Figuren, die nachfolgend be­ schrieben sind. Es zeigt:
Fig. 1 im Schnitt eine erfindungsgemäße Hüftgelenkendoprothese mit eingesetztem Pfanneneinsatz mit drei Kontaktflächen und
Fig. 2 eine geteilte Metallschale mit eingesetztem Pfanneneinsatz mit zwei Kontakt­ flächen.
Fig. 1 zeigt im Schnitt eine erfindungsgemäße Hüftgelenkendoprothese mit einem Prothesenschaft 1, der im Oberschenkelknochen verankert ist. An diesem Prothesen­ schaft 1 aus Metall ist ein keramischer Kugelkopf 2 befestigt, der in einem Pfannen­ einsatz 3 gelagert ist. Der Pfanneneinsatz 3 hat im Inneren eine Gleitfläche 5 für den Kugelkopf 2 und ist in eine Metallschale 4 eingesetzt. Die Metallschale 4 besteht aus einem Oberteil 4a und einem Unterteil 4b, die durch bekannte Maßnahmen fest mit­ einander verbunden sind.
Der keramische Pfanneneinsatz 3 weist drei äußere Flächen A, B, C auf, die vollflä­ chig, d. h. mit ihrer gesamten Fläche an der Innenseite der Metallschale 4 anliegen. Die Fläche A ist dabei parallel zur Symmetrieachse 6 angeordnet. Die Fläche C am Boden des Pfanneneinsatzes 3 ist senkrecht zur Symmetrieachse 6 angeordnet. Die dazwischen liegende Fläche B befindet sich in einem spitzen Winkel zur Symmetrie­ achse 6. Auf der Außenseite der Metallschale 4, d. h. dem Oberteil 4a, ist ein Schneidgewinde 7 angeordnet, mit dem die Metallschale 4 in den Beckenknochen eingedreht wird. Das Oberteil 4a liegt auf der Oberkante des Pfanneneinsatzes 3 mit einem Überhang 8 auf, so daß die Oberkante des Pfanneneinsatzes 3 geschützt ist.
Fig. 2 zeigt nur die geteilte Metallschale 4 mit Oberteil 4a und Unterteil 4b und einge­ setztem Pfanneneinsatz 3. Der keramische Pfanneneinsatz 3 weist hier nur zwei äu­ ßere Flächen A, B auf, die an der Metallschale 4 anliegen. Die Fläche A ist wie in Fig. 1 parallel zur Symmetrieachse 6 angeordnet. Die Fläche B ist sphärisch ausgebildet und stellt den Boden des Pfanneneinsatzes 3 dar.
Die Metallschale 4 weist in diesem Fall kein äußeres Schneidgewinde auf, da die Metallschale 4 in eine äußere Schale (nicht gezeigt) eingelassen wird, die dann im Beckenknochen verankert wird.

Claims (2)

1. Hüftgelenkendoprothese mit einem Prothesenschaft (1), der im Oberschenkel­ knochen verankert ist, einem am Prothesenschaft (1) befestigten Kugelkopf (2), der in einem keramischen Pfanneneinsatz (3) gelagert ist, der in eine Metallscha­ le (4) eingelassen ist, die direkt oder über eine Außenschale im Beckenknochen implantiert ist, dadurch gekennzeichnet,
  • - daß die Metallschale (4) aus einem Oberteil (4a) und einem Unterteil (4b) be­ steht, wobei zwischen dem Oberteil (4a) und dem Unterteil (4b) der Pfannen­ einsatz (3) eingebaut ist und
  • - daß der Pfanneneinsatz (3) mit mindestens zwei Flächen (A, B, C) vollflächig an der Innenseite der Metallschale (4) anliegt.
2. Hüftgelenkendoprothese nach Anspruch 1, dadurch gekennzeichnet, daß die gesamten Außenflächen (A, B, C) des Pfanneneinsatzes (3) an der Innenseite der Metallschale (4) anliegen.
