Description
The invention relates to an arrangement for monitoring a fetus during childbirth.
In clinical obstetrics, it is currently the standard procedure to find the fetus by means of a cardiotocogram, hereinafter abbreviated as CTG. The Heart rate via a scalpel electrode, via an insertion mechanism, as in DE 42 44 691 is described, applied, derived directly from the fetus and parallel to the Contraction pressure recorded. The course of fetal heart rate and labor pressure allows however, no reliable diagnostic information about the actual oxygen supply of the fetus.
Therefore, efforts are being made to measure the oxygen saturation of fetuses sub partu. In the 44 07 541 becomes e.g. B. a probe held with negative pressure on the scalp rind described, in which by means of photodiodes emitting at two different wavelengths and a photo receiver outside the vacuum range the ratio of Oxyhemoglobin is determined as the sum of oxyhemoglobin and deoxyhemoglobin. The The probe also contains an electrode for taking the fetal EKG and is over Connection lines connected to a pulse oximeter. However, as from specialist articles (Knitza, R. Hypoxic threat to the fetus sub partu, published by Steinkopff, Darmstadt 1994) is the measurement of lethal oxygen saturation is relatively inaccurate.
If there is a conspicuous CTG course, additional information about the Metabolic state of the fetus required.
A common method is to take and analyze blood from the fetus, which is one invasive intervention requires and is complex.
On the other hand, there is already a method and a device for determining the oxidative Metabolic state before and during childbirth have been proposed, see DD 283 218. This is done by means of a light guide probe attached to the preceding part of the fetus fluorescence spectroscopic the characteristic of the respiratory chain activity NADH (nicotinamide adenine dinucleotide) recorded and evaluated. The fiber optic probe will by means of a bell-shaped fixable holder by negative pressure at the measuring point attached.
The advantage of this method is that there is no invasive intervention and a more precise one It is possible to make a statement about the current metabolic state.
However, the CTG and NADH methods have not yet been successful to apply. A major problem is the parallel attachment of two measuring heads at the fetus.
The invention has for its object an arrangement for monitoring a fetus during childbirth to create both the electronic signals for the Record the fetal heartbeat in the cardiotocogram as well as the optical signals for the characterization of the oxidative energy metabolic state can be obtained.
Diese Aufgabe wird gemäß dem Patentanspruch 1 gelöst. Die Unteransprüche haben vorteilhafte Weiterbildungen zum Gegenstand. This object is achieved according to claim 1. Have the subclaims advantageous developments on the subject.
Due to the integral arrangement of an electrical and optical sensor in one Common housing is the space requirement for the simultaneous extraction of meaningful measurement parameters about the fetal heart rate and the oxygen supply of the fetus so small that the measuring probe is placed even with a small opening of the cervix can be. An invasive fetal blood analysis is no longer necessary.
The arrangement according to the invention allows existing measuring heads to be used Retrofit scalpel electrodes with an optical sensor. On the other hand, it is possible existing measuring heads with suction cup and optical fibers with an electrical sensor retrofit.
The combination of CTG detection and detection of the fetal oxidative combination according to the invention The state of energy metabolism results in a synergetic effect through the statements About the health status of the fetus are possible, the statements from the CTG alone Far exceed accuracy and make diagnostics much more reliable.
Because of the common flexible hose and its dimensioning, a simple one Handling and good sterilization properties achieved as well as the individual Connection lines protected against damage and dirt. A removable one Inserter improves the cleaning options.
The invention is explained in more detail below using an exemplary embodiment:

For the sake of clarity and ease of illustration, the drawings are partial carried out schematically.
Referring to FIG. 1, an arrangement according to the invention with scalp electrodes 21 is placed on a fetus. 7 The arrangement consists of a housing 1 , an inserter 5 and a flexible hose 6. The housing 1 is designed as a cylindrical body made of a tissue-compatible, electrically insulating material, such as polyethylene. The housing 1 contains an electrical sensor 2 and an optical sensor 3. The attachment electrodes 21 protrude from the part 11 of the housing 1 facing the fetus 7 in the form of spiral scale electrodes.
The optical sensor is designed as an optical fiber bundle 32 , the end face 31 of which is flush with the end face 111 of the part 11 . On the outer surface of the housing 1 , comparison electrodes 22 are attached, which provide the reference potential for the top electrodes 21 .
The inserter can be pushed onto the part 12 of the housing 1 facing away from the fetus. The connection between the two is designed in a manner not shown, but in a manner known per se so that, on the one hand, the inserter can be easily removed from the housing 1 and, on the other hand, a certain threshold torque, which corresponds to the required insertion torque for the scalpel electrodes, is not exceeded. In this way, injuries to the fetus 7 are prevented. The part of the optical fiber bundle 32 leading away from the fetus and the connecting lines 23 to the electrodes 21 , 22 are guided in the flexible hose 6 in a manner protected against contamination and damage. The hose 6 preferably consists of a tissue-compatible, elastic, chemically resistant and mechanically resistant material. At the ends of the optical fibers 32 and the connecting lines 23 , connecting pieces 33 , 24 are attached, which serve as connections to subsequent evaluation devices.
Wie Fig. 2 zu entnehmen ist, umfassen die spiralförmigen Aufsatzelektroden das aus mehreren Lichtleitfasern bestehende LichtleitfaserbündelThe flexible hose 6 is dimensioned such that the inserter can be easily moved on it. As can be seen in FIG. 2, the helical attachment electrodes comprise the optical fiber bundle consisting of several optical fibers
FIG. 3 shows an embodiment of the arrangement according to the invention which does not require an invasive intervention.
The part 11 of the housing 1 facing the fetus is designed at its end as a suction cup. The attachment electrodes 21 are applied to the end face 111 of the part 11 as a conductive metal coating. The suction cup is placed on the fetus 7 by means of an inserter 5 and a negative pressure is generated in it via an evacuation line 4 .
Thus, the optical fiber bundle 32 lies with its end face 31 with an attached suction cup to the surface of the fetus or the tissue, the optical fiber bundle 32 in the housing 1 is displaceable and pressure-tight suitably stored.
The evacuation line 4 can be formed within the housing 1 as a rigid tube onto which a hose 41 is pushed on the outside. It would also be conceivable to run the hose 41 into the bell-shaped part 11 and to fix it in a suitable manner within the housing 1 .
