Description
Forschungsergebnisse der letzten 10 bis 12 Jahre haben gezeigt, daß eine Reihe
verschiedener auf menschlichen Nerven vorkommender Kohlenhydrat-Immunepitope als
autoallergene Antigen-Kandidaten bei humanen peripheren Neuropathien involviert sind (für
Übersicht, siehe ¹). Im Jahre 1980 beschrieb Latov & al. ¹⁰ einen Patienten mit schwerer
demyelinisierender Neuropathie und hohem Titer an monoklonalem IgM kappa, das mit peripherem
Nerv reagierte. Immunsuppressive Therapie war begleitet von klinischer Besserung.
Es konnte später gezeigt werden, daß das betreffende IgM spezifisch an myelin-assoziiertes
Protein (MAG) und an sulphatiertes Glukuronylparaglobosid (SGPG) band ¹¹. Dabei
ist MAG und SGPG ein auf GlcAβ3Gal < befindliches Kohlenhydratepitop gemeinsam
4,13,16. Ein identisches Epitop kommt auf sauren Insekten-Glykosphingolipiden, Arthrosiden
vor ¹⁹. Etwa die Hälfte aller mit monoklonaler IgM-Gammopathie einhergehender
Neuropathien zeigen Antikörper, die an MAG oder SGPG binden (für Übersicht, siehe ⁵).
In den folgenden Jahren haben dann eine Reihe von Untersuchungen an Motorneuropathien
gezeigt, daß einige der Patienten - unter diesen Fälle mit den Anzeichen klassischer amyotropher
Lateralsklerose²² - hohe mono- oder polyklonale Antikörpertiter aufwiesen, die
gegen Ganglioside gerichtet waren (Gtet12.17; Gtet2a3,8,9,15,26; Gtet2b12,18; Gtet4b21,24).
Bei einigen Motorneuronerkrankungen kommen hochtitrige Antikörper vor, die
mehrere Ganglioside erkennen, nämlich solche, denen bestimmte Kohlenhydratepitope gemeinsam
sind6,23,25,26. Die Tatsachen, daß gegen Ganglioside gerichtete Antikörper zum
einen an Nervenzellen binden, zum anderen in vitro Zelltod hervorrufen14,22, und
darüber hinaus eine immunsuppressive Therapie benefizient ist, macht ihren Zusammenhang
mit der Aitiologie der in Frage stehenden Neuropathien deutlich.
Für die Diagnose der mit Gammopathien einhergehenden Neuropathien ist die Bestimmung
der gegen die entsprechenden Epitope gerichteten Antikörper entscheidend. Dies geschieht
im allgemeinen mittels ELISA. Eine Einschritt-Methode zur Charakterisierung
IgM-reaktiver Immunglobuline benutzt hochauflösende Agarose-Elektrophorese für die
Trennung der Immunglobuline. Transfer auf mit Glykolipiden beschichtete Nitrozellulose-
Folien, und Sichtbarmachung mit enzymgekuppelten anti-Immunglobulin Antikörpern⁷. Da
alle diese Verfahren vergleichsweise recht aufwendig sind, besteht ein dringender Bedarf
für einen schnellen halbquantitativen Test, mit dem ohne weitere Hilfsmittel, am Schreibtisch,
zusätzliche Information zur Diagnose in Frage kommender Krankheiten zu erhalten
ist. Aus diesem Grund haben wir den hier zur Anmeldung für ein Patent vorgelegten Teststreifen-
Ansatz entwickelt.
Mit dem von uns erfundenen und ausgearbeiteten Verfahren (im folgenden kurz "PIG-
stictest" [pathological immunoglubilin stick test] bezeichnet) ist es möglich ohne Anwendung
weiteren Instrumentars, außer einer Routine-Pipette, innerhalb von zwei Stunden, die
Gegenwart anomal erhöhter Antikörpertiter gegen die hier in Frage stehenden wesentlichen
Oligosaccharid-Epitope nachzuweisen. Die verwendeten Trägersubstanzen der Antigen-
Epitope sind SGPG (wahlwese Arthrosid oder MAG), Gtet1, Getet2a und Gtet2b
(eventuell zusätzlich Gtet4b). Die Teststreifen bestehen aus mit Polyisobutylmethacrylat
stabilisiertem Kieselgel beschichteter Plastikfolie, auf die an 5 Stellen Glykolipide (SGPG,
2×; Gtet1, 1×; Gtet2a; 1×; Gtet2b, 1×) punktförmig aufgetragen sind. Während die Ganglioside
in jeweils nur einer Konzentration appliziert sind, ist SGPG zweimal, aber in verschiedener
Menge aufgegeben. Da alle Menschen geringe Titer mit dem SGPG-Epitop reagierender
Antikörper im Serum aufweisen, wurde hier einmal eine vergleichsweise größere
Menge als "interne" Positivkontrolle, ein andermal eine nur mit pathologisch hohen Titern
reagierende geringere, aufgetragen.
Der PIG-stictest wird zur Analyse von 10 Serumproben konfektioniert (siehe Abb. 1).
Seine Haltbarkeit erstreckt sich auf mindestens ein halbes Jahr bei trockner Lagerung bei 4-
6°C. Ein dem Pig-stictest vergleichbarer Test ist uns nicht bekannt.
Zur Durchführung des PIG-stictests werden die Streifen 10 Min. in Röhrchen 1
(Blockierungsmedium; siehe Abb. 1) vorinkubiert. Sodann werden im gleichen Röhrchen
direkt 20 µl des zu analysierenden Serums zugegeben, und nach 30minütiger Inkubation der
Teststreifen (siehe Abb. 1) dreimal mit 2 ml Lösung 5 je ca. 1 Min. in Röhrchen 2 gewaschen.
Nach weiterer Inkubation in Röhrchen 3 (anti-human Ig Antikörper; siehe Abb. 1)
werden die Teststreifen wiederum in Röhrchen 2 dreimal mit 2 ml Lösung 5 je 1 Min. gewaschen.
Anschließend werden die Streifen in Röhrchen 2, das man mit 2.9 ml Chromogen-
Lösung (2 ml Lösung 5, 80 µl Chromogen, 10 µl H₂O₂) für maximal 5 Min. angefärbt.
Durch Eingeben der Streifen in Wasser kann die Farbreaktion stark angehalten werden.

Claims (3)
1. Herstellung und Anwendung von Teststreifen mit multiplem Auftrag von Oligosaccharidepitop-
Trägern zur immunologischen Erfassung und Charakterisierung anomal hoher Antikörpertiter
im Körperflüssigkeiten. Oligosaccharidepitopträger sind: SGPG (wahlweise
MAG oder Arthrosid), Gtet1, Gtet2a, Gtet2b, Gtet4b.
2. Bei den unter 1. genannten Teststreifen, die Anwendung einer internen Positivkontrolle
in Form einer vergleichsweise größeren Auftragsmenge von SGPG oder einem heterophilen
Antigen, e. g. Arthropentaosylceramid oder Forssman-Glykolipid, welche mit allen menschlichen
Seren unter den gegebenen Bedingungen Farbbildung erfolgen läßt.
3. Die Anbietung des Tests unter Anwendung gefriergetrockneter Reagentien:
Röhrchen 1, Rinderserumalbumin/Phosphatpuffer/Natriumchlorid;
Röhrchen 3, Rinderserumalbumin/Phosphatpuffer/Natriumchlorid/POD-markierte anti-human Immunglobulin Antikörper.
Röhrchen 1, Rinderserumalbumin/Phosphatpuffer/Natriumchlorid;
Röhrchen 3, Rinderserumalbumin/Phosphatpuffer/Natriumchlorid/POD-markierte anti-human Immunglobulin Antikörper.
