Abstract
Die Erfindung betrifft die Bereitstellung einer Mikroemulsion enthaltend mindestens ein Allergen für die Hyposensibilisierung eines menschlichen oder tierischen Patienten gegen mindestens ein Allergen wie z.B. Bienengift, Wespengift, Pollenallergene, Allergene von Hausstaubmilben, Tierallergene oder Nahrungsmittelallergene mit einer Mikroemulsion, so dass eine dermale Applikation der Allergene möglich ist. Die Mikroemulsion umfasst PEG-8 caprylic/capric glyceride, Polyglyceryldioleat sowie eine Wasserphase, eine Ölphase und darin mindestens ein dispergiertes Allergen. In vivo-Experimente zeigen, dass mit dieser Mikroemulsion das Allergen das Stratum corneum durchdringt und in den menschlichen oder tierischen Körper gelangt. Allergene sind Substanzen, die im menschlichen oder tierischen Organismus Überempfindlichkeitsreaktionen des Immunsystems hervorrufen können, sogenannte allergische Reaktionen. Bei Allergenen handelt es sich üblicherweise um körperfremde Moleküle (meist Proteine), die im Rahmen einer normalen Immunantwort als harmlos erkannt werden und somit in einem gesunden Organismus keine allergische Reaktion auslösen. Im Gegensatz dazu wird das Allergen jedoch vom Immunsystem eines allergischen Organismus als gefährlich eingestuft und löst eine allergische Reaktion aus. Beim ersten Allergenkontakt kommt es zu einer Sensibilisierung, die noch weitgehend symptomlos verläuft. Ein wiederholter Kontakt mit dem gleichen Allergen führt zu einer krankmachenden allergischen Entzündungsreaktion, die je nach Art des Allergens und des Reaktionstyps die typischen Krankheitsbilder einer allergischen Reaktion wie Rhinokonjunktivitis allergica („Heuschnupfen“), Asthma bronchiale bis hin zu einem anaphylaktischen Schock hervorrufen kann.Allergens are substances that can cause hypersensitivity reactions of the immune system in the human or animal organism, so-called allergic reactions. Dazu gehören verschiedene Proteine wie Phospholipase A2 (Api m 1), Apamin, Hyaluronidase (Api m 2), Melittin (Api m 3), Mastzellen-degranulierendes Peptid, Tertiapin, Secamin und kleinere Moleküle wie Histamin, Dopamin und Noradrenalin. Wespengift enthält als Hauptallergen das Antigen 5 (Ves v 5), zudem Hyaluronidase, Phospholipase A sowie wespenspezifische Phospholipase B. Dabei wird durch die kontinuierliche Gabe kleinerer Allergenmengen versucht, eine Toleranz des Immunsystems gegenüber dem betreffenden Allergen zu induzieren, so dass ein erneuter Allergenkontakt nur noch geringe oder bestenfalls gar keine allergischen Beschwerden mehr verursacht. Umgangssprachlich wird daher auch gelegentlich von einer „Allergie-Impfung“ gesprochen. Im medizinischen Sprachgebrauch finden sich neben den oben erwähnten Bezeichnungen auch die Begriffe „spezifische Immuntherapie“, „Allergie-Immuntherapie“ oder „Desensibilisierung“. Der Begriff „Desensibilisierung“ ist allerdings nicht ganz exakt und gilt als veraltet, weil die Sensibilisierung für das betreffende Allergen durch die Therapie nicht aufgehoben wird. Sie bleibt nach wie vor erhalten, doch das Immunsystem reagiert auf das ursächliche Allergen nach einer erfolgreichen Hyposensibilisierung durch eine Allergenignoranz und/oder Anergie entweder gar nicht mehr allergisch oder nur noch bei sehr starken Allergenbelastungen. Die Hyposensibilisierung zielt darauf ab, den Körper allmählich an ein bestimmtes Allergen zu gewöhnen (Allergentoleranz), so dass es bei Kontakt mit diesem Allergen nicht mehr zu allergischen Beschwerden kommt. Somit nehmen im Laufe der Zeit die allergischen Beschwerden bei erfolgreicher Therapie spürbar ab und treten bestenfalls überhaupt nicht mehr auf. Die Dauer der Applikation im Rahmen einer allergenspezifischen Immuntherapie kann sich über mehrere Jahre erstrecken und ist davon abhängig, wie die Allergendosen verabreicht werden. Derzeit sind folgende Arten der allergenspezifischen Immuntherapie bekannt:
- Bei der subkutanen Immuntherapie (SCIT) erhält der Patient die Allergendosis subkutan, also unter die Haut injiziert wobei das Allergen entweder in einer wässrigen Lösung vorliegt oder an einen Depotträger gebunden ist. Sobald durch entsprechende Dosissteigerungen die individuelle Erhaltungsdosis erreicht ist, wird die Therapie in regelmäßigen Abständen von etwa vier bis sechs Wochen weitergeführt, um eine Toleranz des Immunsystems gegenüber dem Allergen zu erzielen.

Im Unterschied zur SCIT muss die Einnahme der Allergene bei der SLIT zwar täglich erfolgen, kann dafür aber nach einer anfänglichen Einweisung durch den Arzt auch vom Patienten allein weitergeführt werden. Im Normalfall sind die Nebenwirkungen generell gering und klingen schnell wieder ab. In schwerwiegenden Fällen kann es jedoch zum starken Anschwellen des kompletten Gesichts- und/oder Rachenbereichs kommen, so dass ein Larynxödem unbehandelt zum Tod durch Ersticken führen kann. Darüber hinaus kann die Gabe der Allergendosen zusätzlich eine Belastung für das Immunsystem darstellen, in dem sich an den Behandlungstagen ein Gefühl der Abgeschlagenheit bei den Patienten bemerkbar macht. Als weitere Nebenwirkung, wenn auch in seltenen Fällen, kann es im Rahmen der subkutanen Hyposensibilisierung zu einem allergischen Schock kommen, wie beschrieben mit potentiell lebensbedrohlichem Krankheitsverlauf. Dabei ist das Polyglyceryldioleat ausgewählt aus der Gruppe Polyglyceryl-6 dioleat oder Polyglyceryl-3 dioleat. Die Wasserphase umfasst Polyol, Glycerol und PBS, wobei Polyol ausgewählt ist aus der Gruppe Butylenglycol oder Propylenglykol. Die Wasserphase kann alternativ zusätzlich ein Konservierungsmittel vorzugsweise Phenoxyethanol umfassen z.B. mit einem Anteil von 1%. Die Ölphase umfasst Cetearylethylhexanoat und Isopropyl-palmitat. Das mindestens eine Allergen ist ausgewählt aus der Gruppe der rekombinant hergestellten oder aufgereinigten Einzelallergene bzw. aus Kombinationen dieser Einzelallergene, aus aufgereinigten Gesamtallergenextrakte sowie aus der Gruppe der immunogenen Peptide. Das mindestens eine Allergen ist das Hauptallergen des Bienengifts (Api m 1) oder das Hauptallergen des Wespengifts (Ves v 5). Alternativ ist das mindestens eine Allergen ausgewählt aus der Gruppe Pollenallergen, Allergen von Hausstaubmilben, Tierallergen oder Nahrungsmittelallergen. Kombinationen von Einzelallergenen z.B. das Hauptallergen des Bienengifts mit mindestens einem weiteren Pollenallergen, Tierallergen, Nahrungsmittelallergen oder mindestens einem Allergen von Hausstaubmilben sind möglich und können in der Mikroemulsion solvatisiert und einem Patienten zur dermalen Immuntherapie verabreicht werden. Das mindestens eine Allergen bzw. die Kombination von Allergenen wird in einer Konzentration von 0,05 mg/ml bis 1 mg/ml eingesetzt. Alternativ umfasst die erfindungsgemäße Mikroemulsion mindestens einen lipophilen, hydrophilen oder amphiphilen Gelbildner. In einer Ausführungsform umfasst die erfindungsgemäße Mikroemulsion mindestens ein Adjuvans aus der Gruppe Aluminiumhydroxid oder Monophosphoryl-Lipid-A. Alternativ umfasst die erfindungsgemäße Mikroemulsion mindestens einen Duft- oder Geschmacksstoff, z.B. einen Bitterstoff, wenn die Mikroemulsion bei Tieren verwendet werden soll oder einen angenehm riechenden Duftstoff, wenn die Mikroemulsion beim Menschen verwendet werden soll. Diese Verfahren sind dem Fachmann sehr gut bekannt und wurden auch nach Standardprotokoll durchgeführt. Anhand dieser Versuche kann gezeigt werden, dass die dermale Applikation der erfindungsgemäßen, allergenhaltigen Mikroemulsion im Mausmodell zur Induktion einer klinischen Toleranz gegenüber dem entsprechenden Allergen führt. Dieses Ergebnis wird durch immunologische Versuche untermauert. So kommt es zur Induktion protektiver, allergenspezifischer IgG-Antikörper und zu einer Reduktion des Gesamt-lgE.
