Description
The invention relates to a device for protecting the environment of wounds and wound edges against excessive moisture penetration and associated damage.
The device can be used in medical technology and can serve, for example, for fixing a medical device on the skin. It is particularly suitable for long-term use.
One indication of insufficient removal of excess moisture from adhesive pads for wound care is maceration, swelling and whitening of the skin. There are already devices for securing wound dressings and adhesive pads for wound care on the market, which are intended to regulate the water balance of the skin in the vicinity of wounds. These are z. For example, films with adhesive coating, which allow the transport of water vapor by migration (adsorption and desorption) within an airtight composite.
In particular, for the use of relatively heavy devices for the treatment of problem wounds, which are intended to achieve a comparatively long residence time on the wound, there is a need for fixings in the use of which tissue damage in the wound environment can be avoided. A particularly high risk of damage by moisture exists for the immediate edge of the wound. This is the case in particular in the case of wound dressings in which the wound surface is supplied with moisture. There is therefore a need for a device for protecting the wound environment, with which the immediate edge of the wound can be protected in the best possible manner by realizing a seal that is as precisely localized as possible against the moisture that crosses the wound edge from the wound surface. Only in this way can it be achieved that the climatic conditions on the wound surface and on the wound environment differ to the desired extent. The already known materials for attachment of devices for the treatment of wounds do not offer this possibility.
The aim of the invention is therefore to extend the uninterrupted life of a device for attachment of medical devices by adhesive contacts while avoiding tissue damage.
This is achieved by a device for protecting the environment of wounds, which can serve as a device for fastening medical devices by adhesive contacts, in which a spacer fabric is used for the air conditioning and moisture control of the wound environment, which on the one (facing the wound environment) outer surface at least is partially coated with an adhesive and on the second (facing away from the wound environment) outer surface is at least partially coated with a film or a membrane. Spacer knit fabrics are double-sided textiles in which the warp-knitted fabric surfaces are kept at a distance by spacer threads, so-called pile threads become. The film or membrane, which is located on the second outer surface of the spacer fabric, has on at least a part of its area facing away from the wound environment, an adhesive connection, which serves for fixing a medical device. Such material can be easily adapted to the particular real shape of the wound environment by transferring the real shape of the wound surface to the spacer fabric and cutting out an area of the appropriate size and shape from the spacer fabric. Thus, a device is obtained which is most suitable for covering the wound environment, including the immediate edge of the wound.
Particularly suitable for the device according to the invention are those spacer fabric configurations in which one outer surface has a relatively dense and homogeneously woven surface and the other outer surface has large, uniformly shaped openings with a diameter of up to several millimeters. The surface of the spacer fabric which has large openings is preferably used in the device according to the invention as the area facing the wound environment. This surface is provided with an adhesive in such a way that the adhesive is only on the edges of the openings. Thus, the adhesive forms a grid structure in which large adhesive-free areas are surrounded by relatively narrow, closed adhesive webs / adhesive webs. Over the narrow adhesive webs, the adhesive contact is made to the skin of the wound environment. Through the adhesive-free areas, moisture is transported from the skin into the inner volume of the spacer fabric. The resulting water vapor-air mixture is transported by convection through the inner volume of the spacer fabric and released to the environment. But it is also a forced ventilation of the wound environment possible, in which an air flow is advantageously performed in such a manner by the inner volume of the spacer fabric that an exchange of air takes place in the largest possible area covered with the spacer fabric wound environment. Feeding into the inner volume of the spacer fabric or sucking the air out of the inner volume of the spacer fabric may be via hose connectors attached to the spacer fabric, with tubing connected to the hose connectors which connect to an external device having the desired shape Air movement realized.
The spacer fabric is sealed in a suitable manner wherever the transport of gases or liquids must be interrupted in order to realize the ventilation and air conditioning of the skin of the wound environment in the desired manner. The sealing can be done for example by applying a film. This may advantageously be a flexible polymer film. For the device according to the invention, the product OPSITE FLEXIFIX from Smith & Nephew, for example, is well suited for this purpose. The film may also be a semi-permeable film. Another possibility for sealing the Spreizgewirks is the application of a crosslinkable polymer or a polymer-containing preparation, advantageously in pasty form. This may be, for example, a silicone-based material. Particularly advantageous is the combined use of the two variants mentioned, wherein one of the two outer surfaces or both outer surfaces of the spacer fabric are coated with film and the narrow edge surfaces (or side surfaces) are at least partially also provided either with foil and / or with a crosslinking sealant. The introduction or removal of liquids or gases into the inner volume of the spacer fabric can be done through the fabric layers of the outer surfaces. For some applications of the device according to the invention, it is advantageous if the supply or discharge of a liquid or a gas into the inner volume of the spacer fabric takes place through the cross section of the spacer fabric (through the narrow edge surfaces / side surfaces). In this case, fittings must be attached to the intended locations of the edge surfaces of the spacer fabric, through which hose connections to external sources of gases or liquids can be made. It has proved to be advantageous if, at the point provided for this purpose, the width of the spacer fabric is reduced to the inner diameter of the connecting piece and is covered with a thin, flexible polymer film. On the thus resulting extension of the Abstandsgewirks a connector can be fixed. The fixation can be done in different ways. For example, the connection piece may consist of an outer and an inner tubular part, wherein these two parts can be fixed to one another in a form-fitting manner by a snap closure, snap closure, bounce closure, screw cap, bayonet closure, clamp closure, crimp closure or snap fastener, and wherein the film has a jacket around the described one Extension of the spacer fabric forms, is clamped / squeezed in its entire circumference between the outer and the inner part of the connector. In this way, a functionally reliable input or output of the inner volume of the spacer fabric is created with little effort, which has a high mechanical stability. The fixation of the connection piece can also be non-positively or materially, for. B. by gluing done. It is but also a combination of these variants possible.
