Abstract

Im Rahmen Katheder-inverntioneller Therapiemaßnahmen, insbesondere bei der Durchführung von perkutanen transluminalen Ballonangioplastien (PTCA) sowie der Rotablation ergeben sich häufig Situationen, bei denen schwerwiegende Bradykardien eine passagere Elektrostimulation des Herzens erfordern.in the Framework catheter-non-therapeutic therapies, in particular when performing percutaneous transluminal balloon angioplasty (PTCA) as well as the rotablation often arise situations, where severe bradycardia is a transient electrical stimulation of the heart.

Hierbei besteht ein Prozedur-bedingtes Risiko einer schwerwiegenden, kreislaufwirksamen Bradykardie oder sogar einer Asystolie. Dieses Risiko besteht einerseits infolge der unmittelbar durch die Ballonkatheter-bedingten Okklusion und somit der konsekutiven Ischämie des Koronargefäßes sowie andererseits durch die Möglichkeit einer embolischen Verschleppung von Plaquematerial in diejenigen Anteile der rechten Koronararterie, die für die Blutversorgung der chronotropen Zentren, wie Sinus- und AV-Knoten, verantwortlich sind. Infolge einer Bradykardie kann es zu schwerwiegenden Blutdruckerniedrigungen bis hin zur Schocksymptomatik, im ungünstigsten Fall zu einem vollständigen Herzkreislaufstillstand kommen. Es gibt bisher keine gesicherten Erkenntnisse, bei welcher Intervention eine prophylaktische passagere Implantation einer rechtsventrikulären Stimulationssonde einzusetzen ist. Zu beachten ist dabei zusätzlich das Risiko begleitender Komplikationen, die durch den Einsatz von Schrittmachersonden bedingt sind. Von Bedeutung ist dabei das über 6-fache Risiko des Auftretens ventrikulärer Arrhythmien bei prophylaktischer Platzierung einer temporären Schrittmacherelektrode in den rechten Ventrikel im Rahmen diagnostischer Koronarangiographien. Eine Alternative zur kostenaufwändigen transvenösen Stimulation stellt die transkoronare Myokardstimulation dar. Dieses Stimulationsverfahren stellte sich sowohl am Tiermodell als auch beim Einsatz im Rahmen von Koronarinterventionen am Menschen als eine effektive Methode dar. Hierbei erfolgt die Stimulation des Herzens über den im Koronargefäß befindlichen Führungsdraht gegen eine unipolare Patch-Elektrode, welche am Patienten epikutan angebracht wird.
