Abstract

Zur besseren Haltbarkeit des implantierten Hüftendoprothesenschaftes bei Einbau einer Kunsthüfte ist von dem bisherigen Vorgehen, einen starren metallenen Schaft zu implantieren, abgesehen worden, da alle starren Prothesenschäfte, bedingt durch die Elastizität des menschlichen Knochens, mit seinen Bewegungen bei Last (Biegung, Torsion) zur Lockerung führen. DOLLAR A Der neuartige modulare elastische Hüftendoprothesenschaft ist dauerelastisch im menschlichen Oberschenkelknochen verankert und kann bei Bedarf leicht durch seine Modulartechnik bei Problemen modifiziert werden ohne komplette Erneuerung. DOLLAR A Der Schaft ist zementfrei eingebracht und nachspannbar bei auftretenden Lockerungen.

Description

  • Es ist heute üblich bei notwendigem Ersatz des menschlichen Hüftgelenkes (Verschleiß, Fehlform? Bruch etc.) künstliche Hüftgelenke zu implantieren, die aus einem Hüftpfannenersatz und einem Schenkelhalsersatz mit Oberschenkelkugel bestehen.
  • Der Schenkelhalsersatz mit künstlichem Hüftkopf muß dauerhaft im Oberschenkelknochen verankert werden. Hierzu wird überwiegend ein metallener Hüftprothesenschaft verwendet, der im Markraum des Oberschenkelknochens implantiert wird.
  • Hierzu gibt es eine unüberschaubare Anzahl von verschiedenen Bauformen des Prothesenschaftes und Verankerungstechniken (zementieren, zementfrei, oberflächenbeschichtet ganz oder teilweise etc.). Diese Vielzahl von verschiedenen starren Hüftendoprothesenschäften und die unterschiedlichen Arten der Befestigungen derselben läßt vermuten, dass es durch diese Techniken bisher noch nicht gelungen ist die grundsätzlichen Probleme (Lockerung der Schäfte und Knochenabbau) zu lösen.
  • Der im Patentanspruch 1 angegebenen Erfindung liegt das Problem zugrunde, dass alle bisher verwendeten Hüftendoprothesenschäfte innerhalb unterschiedlicher Standzeiten (abhängig vom Modell, Einbringtechnik, Knochenbeschaffenheit, Lebensalter des Patienten, Belastung durch den Patienten etc.) zu Lockerungen führen müssen, da ein fester unelastischer Hüftendoprothesenschaft auf Dauer in dem flexiblen menschlichen Oberschenkelknochen (Biegungen, Längendifferenzen dadurch, Torsionskräfte) diese Bewegungen nicht mitvollziehen kann. Dadurch entstehen Lockerungen mit Beschwerden, die zum Wechsel des Prothesenschaftes, manchmal mehrfach im Leben eines Patienten führen. Die Wechseloperationen sind aufwendig und für den Patienten oft sehr belastend.

  • Dieses Problem wird durch die im Patentanspruch 1–4 aufgeführten Patentmerkmale des neuartigen haltbaren modularen flexiblen Hüftendoprothesenschaft gelöst.
  • Die mit der Erfindungerzielten Vorteile bestehen insbesondere darin, dass der haltbare Hüftendoprothesenschaft eine Dauerelastizität innerhalb des Markraumes des menschlichen Oberschenkelknochens gewährleistet und somit den natürlichen Bewegungen des Oberschenkelknochens folgen kann ohne zu vorzeitigen Lockerungen und Knochenverlust und damit zu schmerzhaften Beschwerden zu führen.
  • Zusätzlich erlaubt die Modulartechnik die Anpassung an die unterschiedliche Anatomie unterschiedlicher Patienten sowie die leichte Korrigierbarkeit bei fehlerhafter Einbringung.
  • Der Eingriff ist leicht ernernbar, computergestützte Systeme sind nicht nötig.
  • Weiterhin besteht die Möglichkeit den haltbaren Hüftendoprothesenschaft bei auftretenen Lockerungen nachzuziehen, um eine aufwendige Wechseloperation zu vermeiden.
  • Eine vorteilhafte Ausgestaltung des modularen elastischen haltbaren Hüftendoprothesenschaftes ist in den Patentansprüchen 1.–4. angegeben. Es soll erreicht werden, dass der Hüftendoprothesenschaft in den oberen weichen spongiösen Knochenanteilen des Oberschenkelknochens einwächst, im mittleren festen kortikalen Knochenanteil elastisch fixiert anliegt und im unteren Bereich frei beweglich ist, um die bei Bewegung entstehenden Biege-Längen- und Torsionsbewegungen zu tolerieren, um keine Lockerungen und Belastungsschmerzen zu verursachen.
  • Ein Ausführungsbeispiel der Erfindung ist in der o.a. Zeichnung dargestellt und wird im Folgenden näher beschrieben.
  • Es zeigen in 1
    • 1 den oberen Modularanteil des Hüftendoprothesenschaftes mit Konus zur Aufnahme des künstlichen Hüftkopfes in bekannter Technik. Der obere Modularanteil beinhaltet eine glatte Bohrung zur Führung der Anzugschraube (2) der spannbaren elastischen Hüftendoprothese. Desweiteren beinhaltet der obere Modularanteil eine von der Seite unterhalb des Konusansatzes eingebrachte Gewindebohrung, die im Bereich der erstgenannten glatten Bohrung endet. Im unteren Anteil des oberen Modularanteiles sind kreisförmig um die glatte Bohrung für die Anzugschraube (2) mehere halbkugelförmige Vertiefungen eingearbeitet die zur eleastischen Aufnahme der Federstreben (4) dienen. Der obere Modularanteil sollte an der Oberfläche poröse Oberflächenstrukturen aufweisen, um ein Einwachsen des Knochens und damit eine bessere Verankerung zu ermöglichen.
    • 2 die Anzugschraube der elastischen spannbaren Endoprothese die durch Gewindeanteil den unteren Modularanteil (5) anzieht an den Rest der Prothese und dadurch die Federstreben (4) elastisch an den Oberschenkelknochen von innen im Markraum an den festen kortikalen Knochen drückt. Die Anzugschraube besitzt im oberen Anteil, wo sie im oberen Modularanteil (1) sitzt längsverlaufende Einfräsungen, damit sie nach Anspannen in dieser Position von der Sicherungsschraube (3) blockiert werden kann.
    • 3 die Sicherungsschraube, die zum einen einen festen Knochenanteil (Trochanter minor) durchschraubt, zum anderen hierbei weiter in die Gewindebohrung des oberen Modularanteiles (1) einschraubt bis zur Blockierung der Anzugschraube (2) dient damit erstens zur Sicherung gegen Verdrehung und Lösung des oberen Modularanteiles (1) im Oberschenkelknochen und zweitens zur Verdrehsicherung der gespannten Anzugschraube (2) nach Festziehen der Prothese. Da die Sicherungsschraube durch den Knochen hindurch genau ins Gewinde des oberen Modularanteiles (1) eingebracht werden muß, ist ein Zielgerät zum Vorbohren im Knochen sinnvoll, dass über definierte Punkte (z.B. Konus und glatte Bohrung im oberen Modularanteil (1) für die Anzugschraube (2)) angesetzt werden kann.
    • 4 die elastischen Federstreben, die durch Anziehen der Anzugschraube (2) gegen den Knochen von innen gedrückt werden und damit die elastische Verankerung der Prothese bewirken. Hierzu sind die Federstreben an beiden Enden kugelförmig abgerundet, um elastisch in die entsprechenden halbkugelförmigen Aufnahmen des oberen (1) und unteren Modularanteiles (5) aufgenommen zu werden.
    • 5 den unteren Modularanteil, der eine Gewindebohrung beinhaltet, die die Anzugschraube (2) aufnimmt und hierdurch ein Anspannen der Prothese ermöglicht. Die elastischen Federstreben (4) sitzen wie im oberen Modularanteil (1) in halbkugelförmigen Aufnahmen, damit diese elastisch gelagert sind. An der Außenseite befinden sich längsverlaufende erhabene Streben (6), die eine Verdrehsicherung des unteren Modularanteiles bewirken.
    • 6 die längsverlaufenden erhabenen Streben die ein vorzeitiges Mitdrehen des unteren Modularanteiles (5) beim Anziehen der Hüftendoprothese verhindern. Gleichzeitig ermöglichen die längsverlauf enden erhabenen Streben, dass bei stärkerem Anziehen der Prothese der untere Modularanteil (5) wieder ein Stück nach oben gezogen werden kann und damit die Flexibilität der elastischen Hüftendoprothese erhalten bleibt, und es vermieden wird einen ungünstigen Fixierungspunkt am Ende des Prothesenschaftes zu erzeugen, der zu ungünstigen Hebelkräften führen würde.

Claims (4)

  1. Haltbarer Hüftendoprothesenschaft zur Implantation beim Menschen bei Erkrankungen des Hüftgelenkes wie z.B. Verschleiß (Arthrose), Fehlform (Dysplasie) oder Bruch (Fraktur) dadurch gekennzeichnet, dass der haltbare Hüftendoprothesenschaft aus einem Modularsystem besteht das flexible Anteile beinhaltet wie Federstreben oder Lamellen u.ä. aus Stahl, Titan oder Biokompatiplen Kunststoffen, um nach Implantation in den menschlichen Oberschenkelknochen durch geeignete Vorrichtungen wie z.B. eine Gewindestange, Federn oder Hydraulik u.ä. unter Spannung gesetzt werden zu können innerhalb des Markraumes des Oberschenkelknochens, um einerseits die Hüftendoprothese im Markraum zu halten aber andererseits die natürlichen Bewegungen des Oberschenkelknochens (Biegung in alle Richtungen einschließlich Torsion) zuzulassen, um vorzeitige Lockerungen des Hüftendoprothesenschaftes und Knochenverlust zu vermindern.
  2. Haltbarer Hüftendoprothesenschaft nach Patentanspruch 1, dadurch gekennzeichnet, dass es ein Modularsystem ist, das es ermöglicht die verschiedenen Formen des menschlichen Schenkelhalses (Winkelgrade, Längen) sowie des Markraumes des Oberschenkelknochens (Durchmesser, Biegungen) nachzuahmen. Eine evtl. nötige Korrektur des Hüftendoprothesenschaftes bei Fehleinbringungen (z.B. Luxationsneigung der Hüfte, falsche Beinlänge und oder Beindrehung) ist durch das Modularsystem jederzeit während oder nach der Operation durch erneuten Eingriff leicht möglich ohne den gesamten Schaft wechseln zu müssen und damit den Oberschenkelknochen unnötig zu beschädigen.
  3. Haltbarer Hüftendoprothesenschaft nach Patentanspruch 1, dadurch gekennzeichnet, dass der obere Modularanteil des Hüftendoprothesenschaftes eine Bohrung beinhaltet zur Aufnahme der Gewindestange zum Festziehen der Hüftendoprothese, die durch geeignete Vorrichtungen (z.B. Schraube durch den Oberschenkelknochen und den oberen Modularanteil des Hüftendoprothesenschaftes hindurch bis zur Gewindestange) die Gewindestange nach Festziehen blockieren kann. Eine Schraube zur Blockierung der Gewindestange, die durch den Oberschenkelknochen hindurch eingebracht würde, könnte gleichzeitig den oberen Modularanteil der Hüftendoprothese gegen Verdrehung und Lockerung sichern. Um eine zusätzliche Festigung des oberen Modularanteiles der Hüftendoprothese im Oberschenkelknochen zu erreichen ist es ggf. sinnvoll den Modularanteil mit porösen Oberflächenstrukturen zu versehen, die das Einwachsen des Knochens gestatten.
  4. Haltbarer Hüftendoprothesenschaft nach Patentanspruch 1, dadurch gekennzeichnet, dass der untere Modularanteil des Hüftendoprothesenschaftes eine Bohrung mit Gewinde beinhaltet zum Anziehen der Gewindestange und damit zum Festziehen der flexiblen mittleren Anteile des Hüftendoprothesenschaftes zur inneren Verklemmung desselben im Markraum des Oberschenkelknochens bei erhaltener Flexibilität. Der untere Modularanteil weist Verdrehsicherungen auf (z.B. Längsrillen vorstehend), die diesen zum einen während der Anzugphase im Schaft sichern zum anderen aber nach vollendeter Anzugphase des mittleren elastischen Anteiles des Hüftendoprothesenschaftes es noch ermöglichen dass das untere Modularstück sich in Längsrichtung des Oberschenkelknochens noch wieder etwas lösen kann, um in diesem Bereich keinen festen Haltepunkt zu verursachen. Damit hebelt der Hüftendoprothesenschaft nicht über einen feststehenden unteren Punkt, sondern ist frei in allen Bewegungsrichtungen (Biegung, Längendifferenz, Torsion) des menschlichen Oberschenkelknochens. Dadurch werden Lockerungen und Knochenabbau vermindert. Bei evtl. späteren Lockerungen des Hüftendoprothesenschaftes ist es nach Entriegelung der Verdrehsicherung im oberen Modularanteil der Prothese möglich diese nachzuziehen, um aufwendige, für den Patienten belastende Wechseloperationen zu vermeiden.
